3 Stark County 4-H Youth Program GOLD MEDAL AWARD GUIDELINES 1) The Gold Medal Awards will be used to recognize members who have shown outstanding effort, and personal increase of knowledge and skill, as well as those of outstanding achievement, in their category. 2) Gold Medal nomination forms will be sent directly to all organizational advisors (usually in late August or early September). Nomination forms must be returned to the 4-H Office or postmarked by October 15. 3) Any advisor may initiate the nomination. Advisors may nominate any 4-H member. The 4-H member does not have to be a member of the advisors own club. 4) 4-H Members may only be nominated on one project area, and members must have carried the project for which they are being nominated during the current year to be eligible for a Gold Medal Award. 5) Up to three 4-H members in each category may be selected for recognition. The top member in the category will receive a Gold Medal pin and certificate. The two runners-up will receive a certificate denoting honorable mention. 6) The Awards Committee will make the determinations using the nomination form and a point system based on four areas. The areas will be weighted as follows: -Citizenship/Leadership -Personal Initiative -Club Participation -Growth in Subject Area up to 40 points Total Possible Points = 100 7) It is not necessary for more than one advisor to nominate a member. When more than one advisor feels a member should be nominated, it is suggested that those advisors collaborate in completing the nomination form.

Return To: OSU Extension, Stark County 2650 Richville Drive S.E., Suite 100, Massillon, Ohio Nominations must be postmarked by October 15 to be eligible. All 4-H Members are Eligible for County Medals. Medals in each of the areas listed below may be given to 4-Hers who have shown considerable growth and progress in 4-H this past year. Any 4-H advisor may initiate the nomination for a Gold Medal Award. Members must have carried a project in the category for which they are nominated after 2 or more years and the project area must have been carried during the current and previous years. Members may only be nominated in one (1) CATEGORY annually. Gold medal winners cannot repeat in same project area, but can be nominated for excellence in another project category. Please follow all directions carefully. It is important that you make answers clear and legible. 1.
