VALUES Collaboration Empowerment and Self-Determination Diversity and Social Justice Accountability Integrity and Transparency Innovation About Our Programs District Mental Health Services for Older Adults Program About the Program: The program offers specialized community based geriatric services in the Kenora and Rainy River Districts. Who is Eligible: Seniors that are 60 years of age and older with a dementia or other serious mental illness, such as: Clinical Depression Mood Disorder Anxiety Disorder Schizophrenia Vascular Dementia Alzheimer s Disease or Related Dementia The target population includes older adults living in community and facility-based environments including personal residences, supportive housing, chronic and acute care facilities, LTC Facilities. Supports, services, and education are also available to the caregivers and care providers of these individuals. The Program Provides: Assessment and Cognitive Screening Counselling or Therapy Cognitive-Behavioural Interventions Care and Treatment Planning Referral and Advocacy Monitoring Education and Support to Caregivers Community Outreach and Presentations Geriatric Psychiatry Specialty Services: A geriatric psychiatrist is available via tele-health on a weekly rotating basis in each community for assessment, case consultation and education. The program also has access to a number of other specialists in the field of geriatric care to assist in meeting the needs of the older adult population. 3 About Our Programs District Peer Support Program About the Program: Consumers and family members have identified the need for and benefit of services provided by a peer; someone who has experienced a mental illness and is knowledgeable of mental health and social services. This is a consumer driven initiative that provides support to individuals and families who are affected by the stigma and reality of mental illness throughout the Kenora and Rainy River Districts. Our staff has a blend of education, training and personal experience with mental illness. Through the combination of these three components, staff promote recovery; providing a supportive and safe environment where people can talk about their mental health issues without judgement and with confidentiality. Who is Eligible: Individuals 18 years of age and older that have a personal mental health issue, or have had one in the past. Family members of individuals with a mental health issue. The Program Provides: Peer, Family and Forensic Peer Support Peer Support Drop-In Centres Educational Training such as Mental Health First Aid, Wellness Recovery Action Plan (WRAP), Hearing Voices that are Distressing and Anti-Stigma Education Regional Consumer and Family Conferences Eight Session Family Information Support Program Referral and Advocacy Services PEER SUPPORT: Peer support is staffed by those who have direct personal experience receiving mental health services, helping and supporting other adults with mental illness to work on their own recovery. FAMILY SUPPORT: When someone is diagnosed with a mental illness it can affect the whole family including parents, children, spouses and siblings. We can help support family members and caregivers through this difficult time with education, information, individual support and more. FORENSIC PEER SUPPORT: Forensic Peer Support provides support to individuals who have been, are in, or are at risk of being in the criminal justice system. Forensic Peer Support Workers provide advocacy and support as individuals focus on their paths to recovery. We provide a supportive, confidential and safe place to assist individuals with their recovery. Who is Eligible: Individuals 18 years of age and older that have a personal mental health issue, or have had one in the past. Family members of individuals with a mental health issue. 4 About Our Programs Psychogeriatric Resource Program About the Program: This program assists in the care of those individuals with complex care needs and responsive behaviours in Long Term Care Facilities and Community Support Agencies. The Psychogeriatric Resource Program consists of the Psychogeriatric Resource Consultants (PRC) and Outreach Personal Support Workers (Outreach PSW). Who is Eligible: Long Term Care Facilities and Community Support Agencies who serve people with dementia, mental illness or other complex needs. The Program Provides: The Role of the PRC Consultation Services Assist in Developing Interventions for Persons with Responsive Behaviours Assist and Advise in Care Plan Development Education and Training to Staff Act as an Advisor, Educator, Facilitator and Network Builder Identify Gaps and Integrate Services The Role of the Outreach PSW Under the direction of the PRC, provides oneto-one support of residents with responsive behaviours Provides hands-on practical strategies and support to LTC staff Implements best practices in behaviour management Shares knowledge using a peer-support model to mentor and coach LTC staff Certified Training Offered: P.I.E.C.E.S. U-First Gentle Persuasive Approaches in Dementia Care: Responding to Persons with Challenging Behaviours (GPA) Interactive Workshops: Virtual Dementia Tour Mealtime Matters Unfair Hearing Test Presentations Offered: Alzheimer s Disease Communication Strategies Caring for Individuals with Dementia Activation and Dementia The Importance of Activity Intimacy and Sexuality Responsive Behaviours The Program is Offered: Throughout the Kenora and Rainy River Districts For More Information Contact: PSYCHOGERIATRIC RESOURCE PROGRAM Rainy River District Kenora District Case Management Program About the Program: The Case Management Program is designed to facilitate recovery and maintain a quality of life directed by the individual. Community Support Workers will develop an individualized service plan to assist persons in developing the skills and supports necessary to maintain a level of independence that allows them to live and thrive in the community. Supports are provided in the community setting or within the person s home environment according to the individual s choice. Who is Eligible: This program is designed to assist and support individuals 18 years of age and older who have a serious mental illness. The Program Provides: Assistance with Daily Living and Meeting Basic Needs (i.e. Housing, Financial and Advocacy) Support in Employment / Vocational / Social Activities Life and Social Skills Development Assistance Navigating the Mental Health System and Other Relevant Networks i.e. Ow, Odsp Assistance with Accessing Psychiatric and Medical Services including Symptom and Medication Management Assistance for Individuals in Identifying Strengths and Determining Areas of Need and Developing Support Plans using a Recovery Philosophy Crisis Prevention, Intervention and Management Health Promotion and Education with a Focus on Recovery and Wellness Referrals to Other Community Resources and the Coordination of a Support Network Case Management Services can be a brief or longterm support. 5 About Our Programs Education Program About the Program: CMHA is committed to public awareness efforts that result in reduced stigma of mental illness. Through education and training activities the Education Program assists services and groups in our community to understand the issues that people with mental illness face and to learn ways to support those individuals. Understanding each other builds an inclusionary community. Education services are provided through the activities and services listed below: Resources for Public Education: Presentations, Workshops and Seminars Information Displays and Health Fairs Public Education Campaigns Resource Distribution such as Fact Sheets, Pamphlets and Information Packages Media Promotion and Advertising Publications such as CMHA Northern Regional Branch Newsletter Mental Health Matters Examples of Presentations Offered: Mental Health Overview Eating Disorders and Psychotic Disorders Deliberate Self Injury and Suicide Seasonal Affective Disorders (SAD) Stigma and Mental Health What is Mental Health? Stress and Uncertainty in the Workplace Concurrent Disorders Language Hurts Life of Stress Healthy Relationships Making Healthy Choices Body Image and Health Eating (Presentations can be developed to meet the needs and interest of the group requesting it.) Who is Eligible: Individuals 18 years and older considered to have a Serious Mental Illness, Developmental Disability, Fetal Alcohol Spectrum Disorder, Concurrent Diagnosis, Dual Diagnosis or Acquired Brain Injury and who have come in contact with the Criminal Justice System. The Program Provides: Assistance Accessing Duty Counsel and/or Legal Aid Mental Health Diversion Planning Assistance Retaining Legal Representation Case Management Custody Release Planning Probation/Ontario Review Board Support Services Advocacy and Referral Community Outreach The Program is Offered: Throughout the Rainy River District For More Information Contact: Court Diversion and Support Program Housing Program About the Program: Our Housing Program provides a limited number of rent supplements to individuals who are homeless, at risk of being homeless or who are living in inadequate housing. Who is Eligible: Individuals 18 years and older who experience an ongoing mental health disability or are assessed as in recovery from an addiction. The benefit is restricted to Employment and Income Assistance participants. e.g. Ontario Works, Ontario Disability Support Program, (ODSP) or otherwise qualify for RGI (Rent Geared to Income). The Program Provides: Funding for Improved Housing Case Management Services Landlord/Tenant Advocacy Home Management Support A Recovery Based Focus The Program is Offered: Within the Fort Frances Area For More Information Contact: Housing Program
6 Relaxation: Deep relaxation, like meditation, when practiced regularly, not only relieves stress and anxiety, but also is shown to improve mood. Deep relaxation has many other potential benefits as well it can decrease blood pressure, relieve pain, and improve your immune and cardiovascular systems. Making time to find enjoyment is also an important element of relaxation.
