2 Page 2 of Service Coordination/ICDM survey The Primary Care Partnerships (PCP) are committed to continuously improving the quality of practice to support improved coordination of services and management of chronic disease (ICDM). To support this work and as part of the PCP planning and reporting requirements, the Department of Health (DoH) is conducting a survey to measure service coordination and ICDM practice. We encourage your organisation to participate in this survey. This is an opportunity to get a picture of how programs within your organisation are implementing practice standards to improve care for your clients. The survey will assist the future work of the PCPs and DoH by identifying areas that require greater focus. Following the survey a report will be provided by the DoH, including local and state-wide feedback. The survey is delivered within the context of a continuous quality improvement process. This minimises the burden on organisations for data reporting and produces evidence for existing organisation and program specific quality assurance systems and accreditation processes. A link to the survey and supporting resources are available on Sexual and Reproductive Health Strategy The Sexual and Reproductive Health Strategy Reference Group Meeting scheduled for October 21 st had to be postponed and has been rescheduled to Wednesday November 18 th. The draft strategy will be discussed at the meeting. New Sexual & Reproductive Health Resources & Projects Parents, Families and Friends of Lesbians and Gays (PFLAG) have poster available for schools. Contact: contactus@pflagvictoria.org.au TESTme Project - Outreach STI service for country youth Family Planning Victoria has developed two new resources to aid in the teaching of contraception. One is a quick guide, the other is a comprehensive kit with samples. Contact jjones@fpv.org.au

The coaches are using the skills and tools with clients, and have found the course useful. A follow-up session will be planned so that the coaches can get together and discuss how the coaching process is working for them and for their clients. CDM Network Meeting The next meeting will be held on Tuesday 1 December from 2:30pm-4:30pm in the Lecture Hall at CGHS, Palmerston St, Sale. This meeting will include planning for 2010 meetings. New members welcome, please encourage staff interested/working with chronic disease clients to attend. Please RSVP by Friday 27 November. Integrated Chronic Disease Management (ICDM) Online Clearinghouse The Clearinghouse is a resource to support the work of all Victorian clinicians and services pursuing integrated chronic disease management, including: Early Intervention in Chronic Disease initiatives Community health services HARP-CDM programs Primary Care Partnership staff and member agencies Divisions of General Practice. The ICDM online clearinghouse will allow practical resources and tools, plus brief explanations of their development and use, to be uploaded to a website by their authors where others can then search and access them. It is intended to reduce the duplication of and increase and expediate access to practical unpublished tools and resources for all health services, as well as reduce the growing burden upon those health services running well-established and innovative Victorian chronic disease initiatives by providing an efficient mechanism for the sharing learnings and resources. The Information Centre builds workforce capacity and contributes to partnerships and information sharing at all levels. High quality information is continually gathered from a diverse range of reputable sources on a wide range of topics. Information is made available to practitioners in

4 Page 4 of 11 the field through an interpretative clearinghouse system using this website, direct , forums and circulation of hard copy publications. The Information Centre also delivers a series of forums throughout Victoria on topics relating to physical activity program development. The forums include content that addresses marginalised population groups including CALD, indigenous and people with a disability. A draft of the Wellington Health Promotion Catchment Plan has been put together from the consultations that were conducted in April/May and from

5 Page 5 of 11 individual meetings with agencies in September. As it is a catchment plan, it is important that the current and proposed work of agencies is reflected within the plan and that agencies are identified next to specific tasks. The draft has been circulated for agencies to review the tasks and responsibilities included in the plan and to add in specific strategies/tasks from their own plans where appropriate. Feedback on the draft plan would be appreciated by Tuesday 10th November. A revised version will be circulated from the feedback prior to the plan being submitted to DoH on 30 November. Wellington Health Promotion Network Meeting Twelve people attended a meeting on Tuesday 27 th October to discuss the formation of a Wellington Health Promotion Network. This meeting was called in response to the results of a survey which was conducted in September to gauge the interest and purpose of a network. Survey results showed that: 80% are already involved in a community health type network Most relevant priority areas for a health promoting network include mental wellbeing 70% and Nutrition and Physical Activity 80%. 70% of respondents said they would like to meet every second month. Respondents indicated that they would like a network to include the following: opportunity to network 90%, project planning/support 90%, training and development 70% and funding opportunities 70%. The results formed the basis of the discussion at the meeting and it was decided that a network would be supported and welcomed by agencies. A bimonthly meeting on a Tuesday morning was proposed. The TOR will be developed for the network at the next meeting. The meeting also provided an opportunity to discuss the Wellington Health Promotion Catchment Plan An overview of the Catchment Planning process was provided by Debbie Mitchell, Department of Health and a draft of the Wellington Health Promotion Catchment plan was circulated and discussed. The next meeting will be on Tuesday 15 December at 9.30am in the Conference Room in the Community Services building at Central Gippsland Health Service. New members are welcome to attend. Cert IV TAA 3 day Introduction to Health Promotion facilitator training - Gippsland region Twelve senior health promotion practitioners and managers from across Gippsland will participate in a 3 day Introduction to Health Promotion Facilitator Training course in Sale on 25 & 26 November and 2 December. The course has been organised by the Department of Health Gippsland region and is being run by Box Hill TAFE. The training includes three modules from the Cert IV Training and Assessment and will focus on the delivery and facilitation of training. At the end of the training, participants will have the knowledge and skills to effectively plan, organise and deliver the four-hour Introduction to Health Promotion training workshops to a

6 Page 6 of 11 broad range of health and community services such as their own workplace, local government, community health services, aged care services, mental health services, school nursing, government departments etc. For further information about the training please contact Debbie Mitchell, Department of Health, Gippsland HP Conference The Gippsland Health Promotion Conference was held on Tuesday 20 October at Monash Univerisity, Churchill. 115 people from a range of sectors including community health, local government and education attended the conference. Berni Murphy, Deakin University facilitated the conference and discussed the importance of evaluation and encouraged the participants to reflect on the emphasis that is currently put on evaluation. The keynote speakers, Dr Therese Riley, University of Melbourne & Ms Melanie Nichols, Deakin University discussed the evaluation methods and tools used in a selection of mental health promotion and childhood obesity prevention projects. The conference theme Learning and Sharing through Evaluation provided an opportunity for nine Gippsland presenters to discuss local programs and, in particular, the methods and instruments used for evaluation and the results. 16 posters were also displayed around the auditorium. In the afternoon attendees participated in concurrent workshops on conducting focus groups, developing surveys and using log books and diaries for evaluation. In the evening 45 people attended the conference dinner at the Waratah Restaurant. The dinner featured a presentation by Jim Hyde, Director Public Health, Department of Health on the current status and future direction of prevention in Victoria. The conference was organised by the Gippsland Health Promotion Task Group Research Sub-committee. The conference was made possible through contributions by VicHealth, Gippsland Women s Health Service, the four Gippsland PCPs, Department of Health, PHCRED and Monash University GMS and MUDRIH. 7 Page 7 of 11 Lap the Lakes Patrick Kilday, GippSport For the 69 teams that took part in this year s Go for your life Lap the Lakes event, the day provided all with the opportunity to walk, jog, stroll and roll around the stunning surrounds of Lake Guthridge and Lake Guyatt. The event saw in excess of 280 people decked out in green Go for your life t-shirts make their way around the 3.3km circuit on Wednesday 21st October. Participants came from a range of groups across the local community, including Primary & Secondary Schools, businesses, disability groups, amateur running groups, local government, families, as well as pole walkers and those with four-legged counterparts. Particularly impressive was the support from local volunteers and helpers from the Salvation Army, CFA, SES, CAVA, St John s Ambulance and the Lions Club. The contribution from the volunteers was vital in staging this community event. In addition, one quarter of the proceeds from the event will be donated to this year s selected charity, The Salvation Army. For more information on Lap the Lakes, contact GippSport on White Ribbon Day 25 November Jan Tracey, Gippsland Women s Health Service The White Ribbon Foundation of Australia aims to eliminate violence against women by promoting culture-change around the issue.

8 Page 8 of 11 The major strategies to achieve this are a national media campaign as well as education and leadership programs aimed at men and boys around Australia. White Ribbon Day was created by a handful of Canadian men in 1991 on the second anniversary of one man's massacre of fourteen women in Montreal. They began the White Ribbon Campaign to urge men to speak out against violence against women. From 2000, the Commonwealth Government Office for Women ran awareness activities on the International Day, and, in 2003, the Australian branch of the United Nations Development Fund for Women, UNIFEM, began a partnership with men and men's organisations to make this a national campaign. Ten thousand white ribbons were distributed in Today hundreds of thousands of white ribbons are worn by men and women across Australia - men in workplaces; men and women in all Australian police forces; men in national and local sporting organisations; men in the media; men and women in politics; men in the defence forces; men and women in capital cities and in rural and regional Australia. What the White Ribbon Represents Wearing a White Ribbon is not a badge of purity or a badge of perfection. It does not mean that the wearer has perfect relationships. It means that this man believes that violence towards women is unacceptable. It is a visible sign that the wearer does not support or condone the use of violence against women. Everyone can show their commitment to ending violence against women by wearing a white ribbon. Proceeds from purchasing a White Ribbon go towards changing community attitudes through high impact awareness campaigns. For further info on events across Victoria visit: Let s GET Connected Wellington Shire Council The Let s GET Connected Gippsland East Transport Project was established to work with local communities and transport operators to coordinate and make the best possible use of existing transport resources. The project has established local partnerships across the region with community groups, agencies and transport operators. A number of initiatives are now well underway due to the success of this project, including: Better Access to School Buses to provide improved transport options for postsecondary school students and rural and remote communities Working with the Health Issues Centre in Melbourne to develop a resource kit to support the journey of rural patients travelling to Melbourne Hospitals Developing a Melbourne Medical Map showing public transport access options to Melbourne Hospitals Working with Australian Red Cross re coordination of Red Cross Patient Transport Services across Gippsland Trialling a number of new bus services in small communities such as Maffra, Boisdale, Briagolong and Stratford

Client Service Models Medicare and Community Health Service Department of Human Services This is advanced notice of an upcoming forum which will explore how community health services are implementing models that support effective care of chronic disease and complex conditions using the Medicare Benefits Schedule (MBS). A number of MBS item numbers have the potential to lead to more integrated local approaches to health care provision by providing remuneration for GPs, practice nurses and allied health professionals to work together. Work by community health services to implement new models of care that involve general practice and incorporate appropriate MBS item numbers can also enhance client access to public and private services. Taking Action on Diabetes is a Victorian Government funded type 2 diabetes prevention program targeting Victorians 50 years and over, and Indigenous Victorians over 18, who are identified as being at high risk of developing type 2 diabetes. Diabetes Australia Vic is leading the Life! Taking Action on Diabetes program. The Greater Green Triangle University Department of Rural Health is conducting the training of program facilitators and is taking a lead role in evaluation and development of the Life! Taking Action on Diabetes program. The next Life! program facilitator training will be held on 3 and 4 of March Your insight on whether or not there are enough Life! facilitators in your area will be appreciated. This will enable us to target areas where more Life! facilitators are needed. Additionally, if you know of someone who is interested and suitable to become a Life! facilitator please ask them to send expression of interest form by COB 6 November. Bring a pan of salted water to boil. 2. Combine all ingredients (except gyoza wrappers) in a large bowl. Mix well. 3. Place a teaspoonful of meat mixture at the centre of a wrapper. 4. Lightly moisten the edges with a little water. Fold wrapper over in half and roll up into a finger. Press ends firmly to seal. 5. Place in boiling water and cook for 10 minutes. Drain well and serve.
