1 Category: Affiliate 1. Jeff Beck, J.D., CCGC I am deeply committed to the efforts of the National Council on Problem Gambling. I applaud the accomplishments of the Executive Director and the staff. I believe my involvement in various capacities within the National Council reflects my strong support for its efforts. I bring to the table my experience (recovering compulsive gambler, last bet 1/12/97), my clinical skills, my legal background and my passion for the compulsive gambling field. I do in excess of 60 presentations a year to a vast array of audiences, educating people about compulsive gambling. I think this is an integral part of the NCPG mission. As gambling continues to expand I feel there is a need for professionals to lead the National Counsel, professionals who reflect a commitment to providing help to compulsive gamblers and their loved ones. I believe the National Council needs to be an active participant and in the forefront of the dialogue concerning gambling expansion, research, education and awareness. I have had the honor of serving on the Board of Directors of the National Council on Problem Gambling for the last three years and as Vice President the last year. I have been Chairman of the Affiliate Committee the last two years and Chairman of National Problem Gambling Awareness Week Committee the last four years. I also serve on the Education Committee, Helpline Committee, and Prevention Committee. I am the Assistant Director for Clinical Services, Treatment and Research at the Council on Compulsive Gambling of New Jersey. I am a certified compulsive gambling counselor, a licensed professional counselor and an attorney. I currently serve as President of American Compulsive Gambling Certification Board and as Vice President of Association of Professional Trainers.
2 Category: Affiliate 2. Susan Edgar, Ph.D., R.N. I m interested in working with the board of NCPG to help identify problems and to share ideas for solutions. Delaware is currently planning to expand gambling to the Internet, with the concerns that arise whenever gambling is expanded. In this case, for example, there are significant concerns about minors having access. Surrounding states are also showing an interest in this new source of revenue. The more that affiliates can collaborate on solutions and approaches, the more consistent and effective policies can be. Susan has a Ph.D in Psychoeducational Processes, is a registered nurse and a Certified Addictions Counselor. She has worked in the addictions field for over 35 years, having worked for the Delaware Council on Gambling Problems for the last 7 years where she is currently the Deputy Director. Previously, she provided training for the PA Department of Health, Bureau of Drug Abuse Programs, for many years. She has also trained for PA Dept. of Welfare, PA Board of Probation and Parole and PA Dept. of Corrections. She has been counselor and/or instructor for the Wilmington Department of Police since 1993 and has taught at Penn State, Wilmington College School of Nursing and Lincoln University, Master of Human Services and Master of Science in Administration Programs.

Rose Gruber As Executive Director of the Wisconsin Council on Problem Gambling, I have provided trainings, workshops and exhibits throughout the state and was instrumental in the development of the original treatment provider training program developed through the WCPG. I have presented to service clubs, DOC, Gamblers Anonymous meetings, EAP groups, AODA, mental health, and professional organizations. I continue to work with schools throughout Wisconsin to implement an educational curriculum for middle and high school students that helps to increase awareness and aids in prevention efforts. I have worked with correctional facilities throughout Wisconsin providing resource materials for their libraries, making connections to GA and working to implement educational programming into the correctional system. My role as executive director allows me to carry out the WCPG s goals of promoting public education and awareness, maintaining a toll-free statewide Helpline, advocating quality assistance for all individuals affected by gambling problems, identifying high risk individuals and groups, and providing expert training to professional counselors. The strengths I can bring to the NCPG Board of Directors are my knowledge of problem gambling disorders, 15 years of experience as executive director, and my compassion for individuals and families affected by gambling disorders. Rose Gruber graduated from the University of Wisconsin-Green Bay as a returning adult student in 1989, with a double major in Psychology and Human Development. From , she was employed through Help of Door County in Sturgeon Bay as the Domestic Violence Services Coordinator. Since 1997, she has been the Executive Director for the Wisconsin Council on Problem Gambling. She was a member of the National Council on Problem Gambling Board of Directors from She previously served as the chairperson for the National Helpline Committee. She has attended a variety of training sessions and workshops, which include over 200 hours of Gambling specific training. Rose developed and implemented the WCPG s public awareness programs. Rose provides a variety of presentations throughout the state. In the past year she has done over 150 media interviews for television and radio programs. Rose participated in the development sessions of the Wisconsin Recovery Community Organization (WIRCO) and was elected as an officer on its Board of Directors in She has also served on the boards of the Legal Aid Society of Door County, Women s Employment Project of Door County and Habitat for Humanity.

Wiley Harwell, D.Div., NCGC II My involvement with the National Council on Problem Gambling is through my role as an Affiliate from the Oklahoma Association on Problem and Compulsive Gambling and as a member of the Education Committee. Last year in Boston, I helped facilitate a strategic planning and focus session for the Affiliates committee, using the Ideal Speaking situation format. Last November, I helped facilitate a similar process used for the Board members of the NCPG. If elected as a board member, I hope to help NCPG reach a solvent financial goal and to help raise money for the upcoming years. My strength in being a board member is to help with strategic planning and to help develop a vision for the future. At the same time it is important for all board members to help with the financial future of the organization and to help with fund raising. It is my hope that NCPG can maintain its position as the leading organization in the country for the needs of the problem gambler and their family members. Wiley D. Harwell is the executive director of the Oklahoma Association for Problem and Compulsive Gambling. Wiley has a Doctor of Ministry degree from Southern Methodist University, a Masters of Divinity from Southern Seminary in Louisville, Kentucky and a bachelor s degree from Wayland Baptist University. Wily is a licensed professional counselor, a certified employee assistance professional, advanced certification in hypnotherapy, and is a National Certified Gambling Counselor-II. Wily has served as the director of employee assistance programs for twenty years and has provided counseling to employees and family members for over fifty companies. During that time he has provided training programs to these companies and currently leads trainings for tribal casinos as well as continuing education for mental health professionals on problem gambling. OAPCG is the official affiliate of the National Council on Problem Gambling. As the affiliate, Wiley administers the Oklahoma Helpline, leads training for certification to become a certified gambling counselor, works with tribes to promote responsible gambling, promotes awareness for problem gambling to the public and government officials, and works in cooperation with the Oklahoma Department of Mental Health as an advocate for all aspects of problem gambling issues.

Janet Miller, M.S., CCGC Having been a member of the National Council for years, I have observed the board meetings at all annual conferences, reviewed different policies and procedures of the board, assisted with involvement to numerous committees, and supported the National Council staff in words and deeds. I would appreciate the honor of serving on the board for the National Council due to its over-reaching service it provides for people from all over the country. I believe I would offer the clinical and budgetary insights from a non-profit perspective that could benefit the board. I have a 27 year history of counseling and administrative skills to assist with my decisions to best server this board. I would be greatly appreciative to serve on the National Council board to provide my services to an organization that has given so much to help Louisiana. I have a master s degree in counseling, and have been certified as a licensed addiction counselor since I also have certifications in co-occurring disorders, and an advanced certification due to tenure in the counseling field. I have been the Program Director for Center of Recovery-CORE, an adult residential facility in Louisiana treating pathological gambling problems since I have been in a counseling private practice since 1987, and have worked both impatient and outpatient programs within a profit organizational setting. Since working with the Louisiana Association on Problem Gambling, I have overseen the administration of the non-profit programs and the clinical aspects of both CORE and our previous treatment center in New Orleans, CORE-South. I have provided trainings for various states and conferences for other people seeking counseling skills with various substance dependence and gambling problems.
6 Category: Organizational 1. Ray Pineault, J.D. Mohegan Sun recognizes that for most people gambling is a social or recreational activity. For others, gambling causes problems and becomes uncontrollable and is no longer a choice. As Executive Vice President and Chief Operating Officer at Mohegan Sun, I am committed to ensuring our responsibility to be sensitive to our guests and our host community by proactively addressing problem gambling. I am deeply committed to the efforts of the National Council on Problem Gambling and for the past few years have enjoyed serving on NCPG s Development Committee. Along with my involvement at the national level, I and Mohegan Sun are committed to working closely with the Connecticut and Massachusetts Councils on Problem Gambling. The Connecticut Council is also represented on our internal Responsible Gambling Committee. As legalized gambling continues to grow, I feel it is important for the gaming industry to continue to be actively involved in discussions surrounding problem gambling. Ray Pineault brings more than 21 years of legal and business expertise to his position with Mohegan Sun. As Executive Vice President and Chief Operating Officer at Mohegan Sun, Pineault manages and oversees all property business operations. Mr. Pineault has served for more than six years as chair/co-chair of the company s Responsible Gambling committee, is a member of the CT Coalition on Responsible Gambling, and has served for the past two years as a member of the NCPG Development Committee. As an advocate of responsible gambling Mr. Pineault has supported and worked closely with the Connecticut and Massachusetts Councils on Problem Gambling, Hamilton-Madison House, a NYC-based nonprofit dedicated to improving the quality of life of the Asian community, and Mark Potenza, Yale Center of Excellence in Gambling Research on numerous proposals and projects to improve our understanding of problem gambling. Pineault has been with the Mohegan Tribe and Mohegan Sun since March Pineault currently serves as a member of the Anthem Connecticut s Advisory Board and as an adjunct professor at Mitchell College teaching a class on Business and Hospitality Law. Pineault holds a Bachelor of Arts in Psychology from the University of Connecticut as well as a law degree from Quinnipiac College School of Law. He resides in Glastonbury, Connecticut with his wife and three children.
7 Category: Individuals 1. Seth Eisenberg, M.D. I seek the privilege of being able to continue service as a member of the NCPG Board of Directors. I have been on the Board for twelve years, starting as an Affiliate President, and I supported and remain committed to the Board reorganization process that has changed our Council culture dramatically, but has strengthened us for the future. I have been an active member on six different NCPG committees and would like to complete the work we have done this year on the Communications and Self Exclusion Committees. I don t want to be on the Board forever, but I would like stay on the Board for now because I think I can provide important leadership as our organization and field continue to expand and become more complex. I believe my training as an addiction psychiatrist has fostered my interest in, and allows me a comprehensive perspective of problem gambling. I have been and will remain committed to attendance and active participation at the regularly scheduled Board meetings as well as the fall Strategic Planning Meeting and the Annual Meeting at the conference. I ask for your election support and hope to see you in Milwaukee. I began my career 30 years ago as a child psychiatrist working with mentally ill adolescents who also had drug and alcohol problems. Since then I have developed treatment programs and provided direct psychiatric care to adults, adolescents and children in California, Hawaii, Indiana, Illinois and Georgia, specializing in the treatment of co-occurring psychiatric and substance use disorders. In the early 1990s the Illinois Council on Problem Gambling was comprised of an impressive group of professional and recovering individuals who mentored me and helped me integrate my dual diagnosis experience into the problem gambling field. I gave the first workshop ever presented on problem gambling to the American Academy of Addiction Psychiatrists in 1997 and became president of ICPG in My professional commitment to the problem gambling field remains strong and active. I was instrumental in developing problem gambling treatment in Illinois, continue to provide technical assistance to those programs and completed a study on adolescent gambling. I was the training director for Addiction Psychiatrists at Northwestern Medical School, and built problem gambling into the curriculum. I also train extensively on the Recovery Oriented System of Care, Brief Interventions, medication treatment for behavioral health care and motivational interviewing.

Jeff Marotta, Ph.D., NCGC II I have served as an advocate for problem gamblers and their family members as well as a provider of problem gambling services since I began to specialize in problem gambling treatment in During my post-doctorate training I had the opportunity to work with the gaming industry and began to develop an interest in large-system approaches toward mitigating gambling related harm. Shortly thereafter I was hired to develop a statewide problem gambling service system and helped pioneer the practical application of a public health approach toward problem gambling. This background has led me to believe there is common ground between industry, government, and socially minded health advocates. This common ground can serve as a foundation to develop collaborative approaches and solutions to challenging issues. I believe gambling is an activity that is here to stay and one that involves substantial risks to individuals, families, businesses, and society. Our task is to work together to create an environment where the risks associated with gambling are minimized. To effectively approach this task, we need to continually pursue greater knowledge, advocate for healthy public policy, adapt to changing conditions, and utilize lessons learned from global experiences and the broad field of public health. Jeffrey Marotta provides training and consultation services through his company Problem Gambling Solutions while concurrently serving as Clinical Associate Professor at Oregon Health & Science University. Dr. Marotta has a doctoral degree in Clinical Psychology from the University of Nevada, Reno and certifications as an advanced Alcohol & Drug Addiction Counselor, Nationally Certified Gambling Counselor, and Board Approved Clinical Consultant. As Director of Oregon Problem Gambling Services from 2000 to 2007, he developed the State of Oregon s highly acclaimed problem gambling treatment and prevention system and served the Oregon Lottery and Oregon Legislature as their chief advisor on matters relating to problem gambling. Since beginning his consulting business, he has assisted with the development of programs and policies to address problem gambling in eleven states and three countries. He has produced over 100 professional presentations and publications including two national surveys of publicly funded problem gambling services, statewide strategic plans, and responsible gambling materials for lotteries and casinos. When not working, Dr. Marotta enjoys his volunteer work as a Board Member on two community councils and spending time with his wife, 8-year old son, and 5-year old daughter.
9 Category: Individuals 3. Marc Potenza, M.D., Ph.D. I would be happy to serve as member of the NCPG Board. I have for more than a decade focused my career activities on advancing strategies for preventing and treating problem gambling, with recent work with the NCPG and its CT state affiliate in policy efforts. If I am selected to be a member of the Board, I would aim to continue and increase these and other prevention, treatment and policy efforts related to problem gambling. A specific aim would be to increase ties and coordinate efforts with other problem-gambling-related entities including the National Center for Responsible Gaming (with which I interact on the Conference Program Committee and as the Director of the NCRG-supported Center of Excellence in Gambling Research at Yale). Additionally, I would aim to bring my research experience to the Board, with a hope of bringing to the Board recent advances related to empirically validated approaches to problem gambling prevention and treatment. I am a board-certified psychiatrist with subspecialty certification in addiction psychiatry. I am a Professor of Psychiatry, Neurobiology and Child Study at Yale University. I am the founding director of the Yale Problem Gambling Clinic and the Director of the Center of Excellence in Gambling Research at Yale. I have consulted to the Connecticut Problem Gambling Services for over a decade in a clinical capacity (treating people and also recently as Medical Director). I have published over 200 research articles with a majority relating to gambling. Our group has published on multiple aspects of gambling and gambling problems (employing epidemiological, clinical, treatment development, genetic, brain imaging, and cognitive measurements and approaches). I have been involved in DSM-5 research workgroups focused on pathological gambling, generating articles on the topic that have contributed to the proposal to move pathological gambling together with substance use disorders in DSM-5. I have edited books on pathological gambling and other impulse control disorders. I have worked closely with multiple health agencies (APA, WHO) on issues relating to problem gambling. I have interacted with the media and policy makers locally, nationally and internationally on issues related to regulation of gambling and proposed changes to such regulations.
