2 Message from the CEO & Board Chairman We have capped another year of serving people in our community and helping them achieve wellness, happiness and recovery. We open our door each day to the possibility of hope and healing. Integrating the services we offer was and always is our goal. We are not only concerned with the mental health needs of our clients, we are also here for them by coordinating physical health care and substance use care. Our interdisciplinary teams consist of case managers, counselors, psychiatrists, physicians, physician assistants and Advanced Practice Registered Nurses. The strength and support of this team approach is what sets us apart and gives us the tools to advance the health of our community and advance the lives of our clients. Because of your support in 2016 we ve been able to conquer crisis, heal minds and treat bodies in more ways than we ever have before. From specialized kids programs, intensive adult programs and to tele-psychiatry and trauma-informed care, our array of services positively impacts the health and wellness of our community. But amid the triumphs, we look toward the challenges of the future with an ever-changing world of health care complexities. We are continually looking for ways to interact and respond to the people in the Dayton area that need us the most. We are only a single part of the recovery of a client. It takes a team, a community and optimism. We are glad you are here to help us in integrating, partnering and serving others. 3 Mental Health South Community is a local leader in care with an array of programs to help meet our clients needs that arise throughout the lifespan. Diagnostic Assessment Psychiatric Services Individual, family and group counseling Assertive Community Treatment (ACT) for adults Integrated Dual Disorder Treatment (IDDT) Community Support for children and adults Partial Hospitalization for youth Home based family therapy services for families involved in Juvenile Court Foster Care School-based counseling programs Certified in Employment Services Employee Assistance Program (EAP) and Drug Free Workplace Training Caring Hands Food Pantry Primary Care We provide clients the ability to receive both their mental and physical healthcare in a coordinated manner and with a positive, personal approach. Acute care office visits Physicals for school/work/sports Most laboratory services Pharmacy on site Family planning services Health Screenings and care coordination Most Immunizations STD screening EKG testing TB testing Referrals for specialty care Substance Abuse South Community strives to instill hope, value and worth for our clients on their recovery journey. Trained in Integrated Dual Disorder Treatment (IDDT), an evidence-based model for adults who have co-occurring issues of substance abuse and serious mental illness. Dually certified for alcohol and drug and mental health care for adults, adolescents and youth. INTEGRATE

4 COMMUNITY PARTNERSHIPS Specialty Court Programs: Mental Health Court and Drug Courts are treatment based alternatives to prisons, youth detention facilities, jails and Probation. They are a problem-solving approach to help offenders into long term recovery. South Community partners with the community to give treatment services, comprehensive supervision and drug testing to Juvenile Drug Court, Adult Drug Court and Women s Court. School-based Programs: South Community partners with local school districts within their elementary, middle and high schools to contractually provide on-site mental health services to area students. As part of this program, students have access to mental health therapy, case management and psychiatric services (with certain programs). Training, consultation and crisis intervention is also provided. This year we counseled children from Centerville, DPS, Eaton, Huber Heights, Kettering, MCESC Learning Centers East and West, New Lebanon, Northmont, Northridge, Oakwood and School of Advertising Art as well as providing counseling intervention to those students involved in Miamisburg Satellite Diversion Court.

6 SERVE Alcohol and other Drug (AoD) Expansion: South Community has been highly responsive to the needs of the community and stand ready to face the Opioid epidemic as well as the sensitive needs of our adult clients with alcohol and other drug related issues. We ve expanded our service offerings and currently provide all of the following services to help fight these disorders. Open Access: scheduling that allows for more options and same-day appointments In-House Pharmacy: medications right at appointment site NARCAN Response: education and information on accessibility Primary Care on Site: coordinating healthcare in one place Genetic Testing: enabling for more accurate prescription therapy Trauma Informed Care: All of our therapists are learning promising practices designed for all types of trauma, and behavioral health manifestations. Traumatic stress brings about changes in the brain. Research shows that witnessing traumatic events like domestic violence can impact the physical development of the brain potentially leading to lifelong health and social issues. But, we can assist to reverse the effects offering healing and thriving.

7 This is the best place I have ever received help. My therapist is great and so is my doctor. I am so lucky to be able to come here. All of South Community providers are very caring and respectable. I would be lost if not for South Community. South Community has changed my life. I don t know where or how I would be able to survive without the resources. Love it..
