It was my honor to welcome over 100 members, resident trainees and guests to our last meeting. Dr. Runowicz reviewed the science behind and shared her personal perspectives regarding cancer screening. Afterwards Dr. Runowicz recounted that our society was the most engaging audience to whom she has lectured. I enjoyed the synergy of a world-renowned speaker interacting with a first-class audience. This week a pregnant patient asked me about the Ebola virus. The current outbreak of Ebola, the fifth major and the most widespread, has evoked fear because of its high mortality rate and the fact that we do not yet have a cure or preventative vaccine. In West Africa, 10,000 people have been infected and 5,000 people have perished. The human suffering and loss of life is tragic. In the United States, we must prepare for potential Ebola virus cases in our cities, our hospitals, and our practices. In late October as I write my president s message, the media is abuzz with talk of healthcare workers who have become infected while caring for patients with Ebola. Education, coordinated preparation, and adherence to screening guidelines should minimize further spread. I used my patient s worries about Ebola as a segue way to talk about a virus much more likely to affect her here in Philadelphia. In contrast to Ebola, influenza will infect hundreds of millions of people this year (5-15% of the world s population), killing 250,000 to 500,000 worldwide. This year in the U.S., tens of thousands of people will die from influenza. Our pregnant patients are very susceptible to influenza and, if infected, they are more likely to get seriously ill or die from it. In the last few years, our conversations have shifted from discussing the safety of influenza vaccination in each trimester, to discussing the protective effect that antenatal vaccination can provide to newborns. One of the most important things that we can do is to recommend influenza vaccinations to our pregnant patients. As a condition of employment in healthcare, most of us are required to get annual influenza vaccinations. We can also make our patients safer by listening to their fears, providing reassurance when appropriate, and advising them on evidence-based medical care. Our relationships with our patients are just as important as the medical care we provide. At the November meeting of the OB Society, the focus will be on relationships. I encourage you to invite your colleagues, office staff, and spouse to what promises to be an incredible experience. They will thank you for the gift. November s meeting will be my third opportunity to hear Dr. Granai speak. I was lucky enough to experience his moving presentation when Dr. Granai was a keynote speaker at the 1998 ACOG annual clinical meeting and then again when Dr. Jackie Guttman invited him to speak to the Obstetrical Society. On both occasions, I was moved to laughter and tears. Local fourth year medical students on the pathway to Ob/ Gyn residency are welcome to attend our November meeting free of charge. His lecture, if one could call it a lecture, was riveting and brought me to tears (ok, I cry when watching Modern Family). Shortly thereafter, I was in the position to select speakers for my term as Obstetrical Society president. As I wanted to start with a bang, I chose Skip Granai to be my first speaker. Michelle Battistini, MD was a 1983 graduate of the Medical College of Pennsylvania, where she remained for her Ob/ Gyn residency. From early in her career, Dr. Battistini was a remarkably dynamic, competent and committed physician. Michelle s passion was to provide the highest quality of health care and service to women. To honor Dr. Michelle Battistini, the Obstetrical Society of Philadelphia presents this upcoming lecture. Cornelius O. Skip Granai III, MD, FACOG, FACS, is an Associate Professor at Brown University and the Director of the Program in Women s Oncology. Dr. Granai heads up a team that includes residents, three gynecologic oncology fellows, four certified oncology nurses, three nurse practitioners and the multi disciplinary support of many others, including medical students from Brown and outside institutions. Dr. Granai received a Master s Degree in Cell Biology from the University of Vermont in He received his Medical Degree from the University of Vermont in Dr. Granai completed his residency in Obstetrics and Gynecology at Tufts University School of Medicine in 1981 and his Fellowship in Gynecologic Oncology at New England Medical Center. Prior to joining Women & Infants Hospital of Rhode Island and Brown University, Dr. Granai was on the faculty of Tufts and Harvard Universities and on the staffs of New England Medical Center and Massachusetts General Hospital. Dr. Granai is board certified in Obstetrics and Gynecology and Gynecologic Oncology. He is a Fellow of the American College of Surgeons, the Society of Surgical Oncologists, the Society of Gynecologic Oncology, the New England Association of Gynecologic Oncologists, and the American Society of Clinical Oncology. In addition to serving as a member of the Executive Committee of the Brown/Tufts Cancer program, Dr. Granai was selected to be a participant of the Fetzer Institute Relationship-Centered Care Group. He serves as Co-Chair of Care New England/Lifespan Oncology Service Line Committee and was elected to the Information Committee of the Society of Gynecologic Oncologists. Mindfulness Based Childbirth and Parenting Reducing Anxiety During Pregnancy Diane J. Abatemarco, PhD, MSW Associate Professor of Pediatrics Director of Pediatric Population Health Research Alfred I DuPont Hospital for Children Nemours/Thomas Jefferson University Meghan Gannon, MPH Clinical Research Coordinator Department of Pediatric Population Health Alfred I DuPont Hospital for Children Nemours/Thomas Jefferson University Mindfulness Based Childbirth and Parenting (MBCP) will be offered to patients through the Department of Obstetrics and Gynecology at Jefferson University Hospital. Dr. Jason Baxter, the department s Director of Research, Dr. Diane Abatemarco, Chief of Pediatric Population Health, Nemours/Thomas Jefferson University and Dr. Janet Catov, Associate Professor, University of Pittsburgh, obtained funding from the Pennsylvania chapter of the March of Dimes and from Keystone First to provide MBCP to inner city patients at Jefferson s prenatal clinic. In 2011 the rate of preterm birth (PTB) in the United States was nearly 12% across all races. Although it has decreased since 2006, the current rate of PTB continues to be higher than it was in the 1980s and 1990s, and is more than double the rate in other developed countries. Socioeconomic and racial disparities continue to exist; Non- Hispanic African-American women have the highest rates of PTB (17%) 1,2. Overall preterm births account for 69% of perinatal mortality and a substantial proportion of longterm morbidity. Reasons for PTB remain unclear, although spontaneous PTBs have been associated with inflammation, infection, and maternal vascular disturbances. Maternal smoking, nulliparity, low socioeconomic status, and lean maternal body mass index are also risk factors for PTB 3,4. 4 (Continued from page 3) There is increasing evidence that levels of stress-related emotions, such as anxiety, may also be related to the risk of adverse pregnancy outcomes, including low birth weight or preterm delivery. Catov, Abatemarco et al. addressed the impact of anxiety on gestational age in both African-American and Caucasian women. Results demonstrated that trait anxiety assessed at 18 weeks gestation was associated with a reduction in gestational age at birth among African-American women but not among Caucasian women. In addition, nonpreeclamptic, African- American women with higher anxiety appeared to have an increased risk for PTB 3. More recently we found that African-American pregnant women with high anxiety had lower concentrations of pro-inflammatory markers compared to those without high anxiety, raising the possibility that chronic anxiety may blunt the necessary gestational inflammatory response among African-American women 5. The above findings led investigators to develop the Mindfulness Based Childbirth and Parenting (MBCP) intervention to reduce stress and anxiety in pregnant, African-American women on Medicaid. This nine-week program consists of two-hour sessions and a one-day retreat, all led by MBCP expert, Kathleen O Malley RN, BSN,CCRP. Ms O Malley is a clinical research nurse coordinator in Jefferson s Ob/Gyn department. The nine-week program culminates with a celebration to honor the participating women and reinforce their ability to practice mindfulness in order to improve the quality of their lives. The sessions are to be held in Jefferson s prenatal clinic at 833 Chestnut Street. Women will receive instruction, support, course materials, MP3 players and incentives for their commitment to the program. This innovative project includes validated measures to determine the effect of MBCP on pregnancy associated stress, depression, anxiety, optimism, and adherence to care. Patients will be followed up to monitor postpartum contraception behavior and adherence to postpartum and pediatric well-child visits. Prior research has demonstrated positive effects of mindfulness based interventions, including improved health, reduction in the impact of stress, improved psychological well-being, reduced anxiety and depression, enhanced ability to cope with stress, and an increased satisfaction with parenting 6,7. Although mindfulness based interventions implemented during pregnancy are in their early stages, research suggests that they may help expectant parents to better cope adamrat freedigitalphotos.net stockimagesat freedigitalphotos.net with the stress associated with transition to parenthood. Mindfulness Based Childbirth and Parenting (MBCP), which is an adaptation of the Mindfulness Based Stress Reduction program 6, 7 has been shown to significantly increase mindfulness and positive affect, and decrease anxiety, depression, and negative affect in pregnancy 8. In a more recent randomized control trial, pregnant women in a mindfulness intervention program experienced larger decreases in pregnancy-specific and pregnancyrelated anxiety than participants in a reading-only control group, when both groups were tested before and after their respective treatment 9. Previous research by Abatemarco and Catov showed a relationship between anxiety and gestational age. An intervention based on MBCP would provide an opportunity to reduce trait anxiety in pregnant African- American women and would be a critical first step in determining whether reducing anxiety may be related to increased gestational age and a decreased risk of PTB. MBCP intervention is also likely to reduce parental stress during pregnancy, as well as enhance preparedness and parenting skills once the baby is born. Dr. Abatemarco currently has funding from the Children s Bureau, Abandoned Infants Assistance, Administration for Children and Families, and DHS to develop and implement mindfulness based parenting curricula for pregnant women and new mothers who are in drug treatment. The proposed project at Jefferson will build on this work. Similar to the MBSR program, the MBCP program relies on systematic instruction in mindfulness meditation to develop a self-regulatory approach for enhancing and eventually maintaining physical and emotional health and well-being. MBCP allows parents-to-be to use the time during pregnancy and childbirth to learn the life skill of mindfulness to deal with stress, pain and fear. A goal of MBCP is to have the skills of mindfulness in place at birth. Expectant parents find that as they practice being in the present moment, they are able to transition to parenthood with greater confidence, awareness, and connectedness. Mindfulness has been shown to reduce negative physical and psychological responses to stress in the general population as well as in pregnant women 10, 11, 12, 13. MBCP aims to provide pregnant women with skills geared toward reducing their stress and anxiety, which may lead to a reduced rate of PTB among this population. Although MBCP research has shown significant improvements in women to date, the intervention has not been implemented with African-American women who may also be socioeconomically disadvantaged 8. 5 (Continued from page 4) Nevertheless, Hughes, Williams, Bardacke, Duncan, Dimidjian, & Goodman (2009) and Duncan and Bardacke (2010) have previously demonstrated that when women s responses are compared from pre-intervention to post-intervention, MBCP correlates with increases in mindfulness and positive affect and decreases in pregnancy related anxiety, depression, and negative affect 14, 8. Furthermore, participants in MBCP reported experiencing perceived benefits of using mindfulness practices during the perinatal and early parenting periods. In an ongoing study, Catov and Abatemarco found that among disadvantaged pregnant women, 65% of whom are African-American, stress reduction and parenting are among the top programs participants felt they needed. The study aims to contribute to social and public health changes that will protect the youngest and most vulnerable children within their families. Investigators have targeted nulliparous, African-American women for several reasons: rates of PTB are highest among this group; PTBs recur in about 25% of women with a prior PTB (thus preventing the first PTB is key); reasons for a first PTB are largely unknown, although stress is recognized as a causal factor. The project is currently in the early stages of planning and development.
