It is characterized by anovulation, infertility, and hyperandrogenism, with clinical manifestations of irregular menstrual cycles, hirsutism and acne.. Drugs such as metformin are used in the clinical side to treat PCOS. Plant derived natural compounds used in our traditional Indian medicine have been showing better efficacy in the treatment of PCOS. The primary medicine for PCOS followed in siddha treatment is the bark of ashoka tree given in a powdered form. The presence of catechin, kaempferol, leucopelargonidin, epicatechin and leucocyanidin were founsd in the bark and flowers of ashoka tree. The two target proteins taken were follistatin and activin. Docking is done to prove the efficacy of these Indian medicines against PCOS. The most popular software used to predict protein-ligand docking is Accelrys Discovery Studio. The best docking score was noted for 5 structures against 2 target proteins-activin and follistatin. Out of the 5 drugs epicatechin gave the highest docking score against activin and leucocyanidin gave the highest docking score against follistatin for polycystic ovarian syndrome. 2 INTRODUCTION PCOS Polycystic ovarian syndrome (PCOS) is a condition where a hormonal imbalance affects follicular growth during the ovarian cycle causing the affected follicles to remain in the ovary. The retained follicle forms into a cyst & with each ovarian cycle a new cyst is formed leading to multiple ovarian cysts. Polycystic ovary syndrome is characterised by anovulation, infertility, and hyperandrogenism, with clinical manifestations of irregular menstrual cycles, hirsutism, and acne. The condition affects an estimated 5-10% of women of reproductive age, although this varies depending on the diagnostic criteria used.[1-5] One of the commonest presenting complaints of women with polycystic ovary syndrome is anovulatory infertility. They also have increased prevalence of cardiovascular risk factors similar to that seen in the metabolic syndrome (which, like polycystic ovary syndrome, is characterised by insulin resistance). By the age of 40, up to 40% will have type 2 diabetes or impaired glucose tolerance.polycystic ovary syndrome is therefore an important health concern and may represent a major health issue affecting young women today. 3 One of the key factors now being realized in modern medicine in relation to PCOS is hyperinsulinemia increased levels of insulin in the blood. This indicates a direct link between obesity and PCOS as an increased level of insulin in the blood is due to de-sensitivity of cells to insulin, a factor present in diabetes mellitus type II. [6-11]Increased insulin in the blood stimulates androgen secretion by the ovarian stroma the connective tissue of the ovary and reduces serum sex hormone binding globin (SHBG) causing increased levels of free testosterone. Due to the presence of increased androgen in the ovary, the follicle undergoing maturation in the ovary cycle is affected causing anovulation of that particular follicle. The presence of insulin also impacts apoptosis natural occurring death of a defective cell; of the follicle causing the follicle to continue to survive whereas under normal circumstance it would have perished. The ovarian cycle is governed by a hormonal feedback system moderated by the hypothalamus thus it requires constant feedback of hormonal levels for it to properly regulate the release of follicle stimulating hormone (FSH) and lutenizing hormone (LH) from the anterior pituitary gland. Both of these hormones play crucial parts in the development of the follicle during the ovarian cycle. As the hormonal levels in the body are imbalanced and the ovum is not released by the ovary the positive feedback mechanism to the hypothalamus to suppress FSH and LH release from the pituitary gland is impeded.[12-16] However the hypothalamus is receiving positive and negative feedback from hormones but not due to the normal occurrence of the ovarian cycle. The hormonal feedback that the hypothalamus is receiving is due to high levels of estrogen that has been formed from free androgens in the peripheral tissue. 2307
4 Ovaries affected by PCOS TARGET PREPARATION PDB The Protein Data Bank (PDB) is a repository for the three-dimensional structural data of large biological molecules, such as proteins and nucleic acids.[17-19] The data, typically obtained by X-ray crystallography or NMR spectroscopy and submitted by biologists and biochemists from around the world, are freely accessible on the Internet via the websites of its member organisations (PDBe, PDBj, and RCSB). The PDB is overseen by an organization called the Worldwide Protein Data Bank, wwpdb. NCBI NCBI is directed by David Lipman, one of the original authors of the BLAST sequence alignment program and a widely respected figure in bioinformatics. He also leads an intramural research program, including groups led by Stephen Altschul (another BLAST co-author), [20-23]David Landsman, Eugene Koonin (a prolific author on comparative genomics), John Wilbur, Teresa Przytycka, and Zhiyong Lu. SWISS-MODEL SWISS-MODEL is a structural bioinformatics web-server dedicated to homology modeling of protein 3D structures. Homology modeling is currently the most accurate method to generate reliable three-dimensional protein structure models and is routinely used in many practical applications.[24-29] Homology (or comparative) modelling methods make use of experimental protein structures ("templates") to build models for evolutionary related proteins ("targets"). PUBMED.PubMed is a free resource that is developed and maintained by the National Center for Biotechnology Information (NCBI), at the U.S. National Library of Medicine (NLM), located at the National Institutes of Health (NIH).Publishers of journals can submit their citations to NCBI and then provide access to the fulltext of articles at journal web sites using LinkOut. 2308

TARGETED PROTEIN FOLLISTATIN Follistatin also known as activin-binding protein is a protein that in humans is encoded by the FST gene.follistatin is an autocrine glycoprotein that is expressed in nearly all tissues of higher animals.[37-39] It is a single chain glycosylated polypeptide that can bind to activin. Its primary function is the binding and bioneutralization of members of the TGF-β superfamily, with a particular focus on activin, a paracrine hormone. ACTIVIN Activin is a dimer composed of two identical or very similar beta subunits. Inhibin is also a dimer wherein the first component is a beta subunit similar or identical to the beta subunit in activin 2309
6 RESULTS FOLLISTATIN Follistatin also known as activin-binding protein is a protein that in humans is encoded by the FST gene.follistatin is a single-chain gonadal protein that specifically inhibits folliclestimulating hormone release. 16 LEUCOCYANIDIN CATECHIN EPICATECHIN KAEMPFEROL 0 ACTIVIN LEUCOPELARGONIDIN LEUCOCYANIDIN CATECHIN DISCUSSION Docking was performed for proteins follistatin and activin with natural drugs using the software Accelrys Discovery Studio. [23-32]These proteins were docked with 5 drugs against pcos derived from the bark and flowers of ashoka tree. Based on energy minimization value among the 5 ligands which has the highest score is said to be the best docked molecule. The best dockind score was noted for these structures and table was created. Out of these drugs, leucyanidin gives the best docking score with follistatin and epicatechin gives the best docking score with activin for Polycystic ovarian syndrome.follistatin also known as activin-binding protein is a protein that in humans is encoded by the FST gene.follistatin is an autocrine glycoprotein that is expressed in nearly all tissues of higher animals. [31-39]It is a single chain glycosylated polypeptide that can bind to activin. When follistatin binds to activin it suppresses 2320
17 the role of activin to stimulate the secretion of Follicle Stimulating Hormone (FSH). FSH plays an important role in folliculogenesis and decrease in FSH level may arrest follicular development. Activin is a dimer composed of two identical or very similar beta subunits. In the ovarian follicle, activin increases FSH binding and FSH-induced aromatization. It participates in androgen synthesis enhancing LH action in the ovary and testis. In the male, activin enhances spermatogenesis.[40-45] CONCLUSION Natural products discovered from medicinal plants have played an important role in the treatment of Polycystic Ovarian Syndrome. Plant-derived compounds have played an important role in the development of several useful drugs against Polycystic Ovarian Syndrome. These include epicatechin, kaempferol, leucopelargonidin, leucocyanidin and catechin. There are more than 270,000 higher plants existing on this planet and many diseases are treated with drugs that come from natural product sources. For thousands of years, natural products have played an important role throughout the world in treating and preventing human diseases. Natural product medicines have come from various source materials including terrestrial plants, terrestrial microorganisms, marine organisms and terrestrial vertebrates and invertebrates. In this study, leucocyanidin and epicatechin have found to have significant role against Polycystic Ovarian Syndrome, because throughout the interaction studies both these drugs shown to have the best docking score for Polycystic Ovarian Syndrome. Hence leucocyanidin and epicatechin may be used as drug against Polycystic Ovarian Syndrome.
