1 Arrow. 2nd VICE PRESIDENT: Michael Hyland, PT, DPT, CEEAA Michael grew up in Pensacola, FL and earned a BS in Biology from Spring Hill College in Mobile, AL and then graduated from The University of St. Augustine for Health Sciences in St. Augustine, FL with a Doctorate in Physical Therapy. After finishing the PT doctoral program he sought employment in Oklahoma with his then fiancé. Michael s first position was in acute care rehab at Oklahoma State University Medical Center. Ten months later he transitioned to the Outpatient Department where he worked from March of 2013 through March of 2016, and also continued to assist as needed on the acute care side with orthopedics, med/surg, and ICU. In April of 2016 he accepted a new position as PT with Encompass Home Health and Hospice and this is my current employer. My wife Julie and I have two beautiful little girls, aged one and two, and for the time being we are settled in Broken During the 5 years that I have practiced as a physical therapist I have been active in the APTA, including membership in the OPTA. I have also been a section member, first in Acute Care Section, and then in the Academy of Geriatric Physical Therapy. For the last three years I have served as the State Advocate for the Academy of Geriatric Physical Therapy and am halfway through the second two year term. This leadership position is voluntary and was inspired by networking with like-minded therapists at CSM in Since that time I have attended two additional CSM conferences and my passion continues to grow, not only for this profession, but also for the field of geriatric physical therapy. As an AGPT member I am also signed up for the Balance and Falls SIG, Health Promotion and Wellness SIG, Bone Health SIG, and have attended some of these meetings as able while at CSM. In 2013 I became certified in LSVT BIG therapy for Parkinson s rehab and last year I earned the CEEAA title, which stands for Certified Exercise Expert for the Aging Adult. I am planning to take the GCS exam in the next 1-2 years. I feel that these attributes demonstrate that I am a leader in the profession and have the experience and knowledge to serve in an officer s position in the OPTA. Thank you for your consideration of my candidacy. TREASURER: Keith Khoo, PT Keith Khoo has been a clinician since 1985 after graduating from the University of Oklahoma. He has held a variety of offices from nominations chairperson to a two term presidency within the Oklahoma Physical Therapy Association (OPTA) for 10 years. He was recognized with the Chapter Founder s Award in He currently is the OPTA Treasurer and nominated to run for another term. His practice has been varied from Regional Director of Operations responsible for several states to being a treating therapist in hospitals. He has evaluated and treated pediatrics to geriatrics. He has practiced in varied environments, from long term care, outpatient sports medicine, industrial medicine departments to his present practice at the Scholl Center where he evaluates and subsequently treats primarily balance and vestibular deficient patients. His responsibilities will also include treating patients with neuropathies, orthopedic limitations, headaches, and movement disorder patients like Parkinson and Multiple Sclerosis. He has completed the Vestibular certification course at Emory University in 2006, has been a certified Kinesio Taping Practitioner since 2010 and Balance-Base Torso- Weighting Certified Therapist since Keith has taught courses not only on balance and vestibular rehabilitation, but also on Kinesio taping, wound care, debridement certification, incontinence management, management of the Parkinson and Multiple Sclerosis patient. He has also provided continuing education courses on modality applications (electrical stimulation, ultrasound, laser and light) for varied neuro-musculoskeletal dysfunctions both here in the United States and abroad. He is currently a Clinical Specialist with Richmar and is an instructor for LiteGait II unweighting dynamic suspension system for use with Neurologic, Orthopedic and Balance challenged patients to correct gait deviations and initiate gait in early stages of rehabilitation. Presently holding the position of Oklahoma Physical Therapy Association (OPTA) treasurer, prior OPTA board of directors experience as Eastern District and OPTA President, Eastern District and OPTA Vice President, Membership Chairperson, Nominating Committee and Chairperson, nominated as a candidate to the American Physical Therapy Association (APTA) Board of Directors and not in the least, being an OPTA member and practicing Physical Therapist for the past 32 years.

2 DELEGATE: Peggy DeCelle Newman, PT, MHR Peggy has practiced as a physical therapist for more than thirty years in a variety of settings including acute care, outpatient orthopedics, institutional long-term care and home health. Additionally, she has managed allied health professionals in all of these settings. Peggy s teaching experience began as Academic Coordinator of Clinical Education (ACCE) at the University of Oklahoma from 1988 through She then served as the PTA Program Director at Oklahoma City Community College (OCCC) for eleven years from After leaving OCCC, she practiced clinically for a year returning to the University of Oklahoma Department of Rehabilitation Sciences faculty in 2007 as the Director of Clinical Education and Assistant Professor. Peggy returned to OCCC and PTA education in January Peggy has served the Oklahoma Physical Therapy Association in many roles including Chapter President and Chief Delegate. She completed two consecutive terms as Chief Delegate in She was selected as a 2010 APTA Lucy Blair Service Award recipient. She was elected in June 2013 to the APTA Nominating Committee (NC) and appointed as the NC Liaison to the APTA Leadership Development Committee in Peggy was appointed by the APTA Board of Directors to the National Awards Committee - Subcommittee on Lectures and the APTA NEXT Planning Committee beginning July 2016 through June She was appointed to the Oversight Panel for the Analysis of Practice for the PT and PTA Licensure Examination by the Federation of State Boards of Physical Therapy She was appointed by the Federation as a Core Member Continued Competency Certification Reviewer in 2011 and subsequently invited/appointed to serve as a Final Reviewer for the Continued Competency Certification in Peggy has presented locally and nationally on topics including Vision 2020, "Using Support Personnel Effectively", "The Guide to PT Practice: An Introduction" and "Ethical and Legal Problems for Rehabilitation Therapists: Ethical and Legal Responsibilities in Today's Health Care Environment", Let s Talk Ethics ; Clinical Practice Questions Explored and Moral Courage in a Culture of Moral Distress. She presented Enhancing the PT : PTA Partnership at the APTA Annual Conference in Boston in June 2010 ; The Teachable Moment - Explicitly Modeling Clinical Reasoning Skills in Clinical Education at APTA Section for Education Educational Leadership Conference in Kansas City in October 2014; and most recently on the APTA Leadership Development Committee s CSM 2015, 2016 & APTA NEXT Peggy is co-author of PTA Handbook: Keys to Success in School & Career, 2 nd edition published in November She is a contributing author to Geriatrics for the PTA by Jennifer Bottomly, PhD (Slack 2010) and Physical Therapist Assistant s Examination Chapter 8 Standards of Care (Learning Express, LLC 2010). She authored an article in Perspectives Magazine, Build Strong Teams. (APTA 2011). She is currently working on two chapters as an invited contributor to a Clinical Reasoning textbook due out in the summer I want to thank the Nominating Committee for the honor of being slated for the position of Delegate. As I reviewed my CV in order to update my BIO, it dawned on me that I have been actively involved in our professional association in one way or another since I was a physical therapy student in While on one hand that makes me quite a seasoned therapist, it also affords me a great deal of appreciation for all that has been accomplished as our profession continues to evolve and mature. As is evident in my Bio, I have practiced and had managerial responsibilities for therapists and therapist assistants in a variety of clinical settings; taught and been involved in the administration of physical therapist assistant education as a program director; as well as, taught and been involved in the administration of clinical education in both PT and PTA education. I have had the privilege of serving the Oklahoma Chapter throughout my career on committees, task forces and in elected positions. More recently I have had the opportunity to serve at the national level through Board of Director appointments (APTA & Federation of State Boards of PT) and was elected to the APTA Nominating Committee. Many of the individuals that I ve worked alongside have become close colleagues and are cherished friends. Through these various endeavors, I have been excited, challenged, perplexed at times; forever changed, in that my lens has been enriched and widened. The attributes I bring to this position include my passion for our profession, for the Oklahoma Chapter and, as Delegate, my unwavering commitment to speak forth during Caucus & Town Hall meetings, as well as, on the floor of the House of Delegates in order to contribute an Oklahoma perspective to the dialogue and do my utmost to accurately represent my PT & PTA colleagues back home. What I uniquely offer on this 2017 ballot is the rich network of colleagues and friends across the country that I ve made throughout my years of service. This is often important in keeping our small chapter in the know during the House of Delegates. Additionally, I am a PTA educator with experience in PT education as well and, I am a practicing clinician working with patients in an orthopedic setting each week. 3 DELEGATE: Chris Juneau, PT, DPT, ATC, MTC, MBA Chris Juneau has lead a distinguished career as a physical therapist for nearly 15 years, and certified athletic trainer for over 16 years including service as the Rehab Coordinator for the Atlanta Falcons in He is the leading authority in rehabilitation of industrial injuries, sports injuries and orthopedic lesions. He has been and continues to be a practicing physical therapist, clinical instructor, researcher and educator. Chris recently joined Valir Physical Therapy, serving as the Director of Clinical Operations for over 17 outpatient centers across Oklahoma. In addition, he is the CCCE and lead therapist for the PT and PTA student internship program. Dr. Juneau is currently serving as a teaching assistant at Langston University School of Physical Therapy assisting with Human Interaction & Ethics, Procedures & Modalities, Physiology & Anatomy and Advanced Biomechanics curriculum courses. He is the former Regional Therapy Director for Concentra, managing 8 dynamic centers across New Mexico and West Texas. Within Concentra's national organization and leader in Occupational Medicine, Dr. Juneau, has been honored to serve as Chair for the Physical Therapy Medical Expert Panel; researching, developing, and implementing the latest evidence based practices and trends within the realm of physical therapy. Chris formerly was the Center Therapy Director for Concentra in New Orleans, LA, managing two centers, and serving as the Area Lead throughout the Louisiana market and Regional CCCE for the TX/LA area. From , he was appointed and served as the Government Affairs Chair for the Louisiana Physical Therapy Association state chapter. Chris served as a Gratis Faculty member for Louisiana State University Health Science Center, in New Orleans, assisting with orthopedic courses including Movement Science I: Biomechanics and Anatomy, Movement Science II: Emergency Sports Assessment, and is a sponsor and mentor for Student Business Plan Projects. In addition, he is the author and facilitator of Core Strengthening: an Evidence Based Approach, continuing education course recognized and approved by the Louisiana and Texas Physical and Occupational Therapy state boards. More recently, he has facilitated Best-in-Class Practice lectures separately for the treatment of the Shoulder, Knee, and Lumbar Spine. Each course has been reviewed and approved for 2.0 CME credits in Texas and New Mexico. Chris received his Bachelors of Science Degree from Louisiana State University (LSU), Executive Masters of Business Administration Degree from Centenary College of Louisiana and Doctorate of Physical Therapy Degree from the University of St. Augustine for Health Sciences (USA). He recently earned his Manual Therapy Certification endorsed through USA. Chris began his career as a scholar student athletic trainer at LSU, assigned to designing conditioning programs, as well as, injury prevention and rehabilitation for a variety of sports including Football, Baseball, Women s Gymnastics, and Men s Tennis. Following his commencement from LSU and USA, Chris was selected to be an assistant athletic trainer intern in the NFL Europe League. Under the direction and mentorship of Mayfield Armstrong ATC, Dr. Kevin Wilk PT, and Dr. Lawrence Lemak MD; he assisted in player injury management and rehab, which is the foundation of his early intervention philosophy and sports medicine emphasis. Following the end of the season, Chris became the Outpatient Physical and Occupational Therapy Team Leader for CHRISTUS Schumpert Health System in Shreveport, LA. Being very active within the community in Shreveport, Chris served as an Honorary Guest Speaker and Board Member of Think First National Injury Prevention Foundation, presenting to adolescents regarding human anatomy, rehabilitation and injury prevention. In 2008, Chris became the Coordinator of Rehabilitation and Assistant Athletic Trainer for the NFL s Atlanta Falcons Football Club, LLC. Responsibilities included designing player rehab programs and coordinating player status with the athletic performance team, as well as, directly reporting injury status to the NFL. He directly assisted in the recovery, as well as, in the decisions of returning players to practice and game participation. In addition, Chris received advanced training in Selective Functional Movement Assessment and Functional Movement Screening, which is a specialized skill set for identifying the sources of dysfunction and designing individualized corrective exercise programs, applicable for high demand occupations. I am a clinician, researcher and educator and I am generally regarded as one of the leading occupational medicine and physical therapy subject matter experts in the evaluation and treatment of spinal and extremity injuries. I demonstrated servant leadership with my team, patients and clients alike throughout New Mexico and West Texas, and now Oklahoma. I continue to advocate for the APTA, ethically engaging in patient care, practicing evidenced based research and cultivating education. I have a strong work ethic, solution oriented thinking and dedication to develop the business knowledge and clinical skills in physical therapists to safely assume leadership in the non-operative management of neuro-musckuloskeletal disorders. With a healing focus, I continue to refine my clinical skills, investing in our future physical therapists, developing our established physical therapists, and inspiring health. From , I was appointed and served as the Government Affairs Chair for the Louisiana Physical Therapy Association state chapter. It would be an honor and privilege to serve as the Delegate for the Oklahoma Physical Therapy Association. Thank you for your consideration.

4 John was born and raised in California. Having family ties in Oklahoma, he attended the University of Oklahoma Health Sciences Center, earning his Bachelor s of Science in Physical Therapy in He received his Doctorate in Physical Therapy and completed the fellowship program in Orthopedic Manual Therapy at EIM Health Institute in 2011, and is a recognized Fellow in the American Academy of Orthopedic Manual Physical Therapist (AAOMPT). John has also received his Board Certification as an Orthopedic Clinical Specialist in He has also completed two year-long programs in Orthopedic Manual Therapy. He is a member of the APTA, OPTA, AAOMPT, the APTA s Orthopedic Section and the Foot and Ankle Special Interest Group. John s professional interests are teaching and mentoring fellow physical therapists particularly in the clinical setting, and integrating evidence based practice with clinical decision making. He has taught a number of continuing education courses/workshops over the years at the local, state and national level. John s primary interest is the evaluation and treatment strategies for shoulder and hip conditions and orthopedic differential diagnosis. In his spare time, John enjoys experimenting with different ways to perform rehabilitation exercises and participating in ultra-endurance activities. Over the years he has completed a number of marathons and ultra-marathons; both he and his wife Patricia enjoy hiking, especially at high altitudes. John is the owner/clinical manager of Physical Therapy Central Bethany and currently resides in Oklahoma City. I have been a physical therapist for quite some time and have seen the profession grow by leaps and bounds over the years. What I have observed is the quality of young therapists entering the profession each year, where there are some really outstanding clinician-researchers, clinicians who are just interested in sharing research with their colleagues and young therapists who demonstrate a strong interest and talent in becoming future leaders in Oklahoma and nationally. I believe the role of the nominating committee is to identify those individuals who may be interested in serving, and to provide the mentorship to be successful. I further believe that it is important to recruit potential leaders as many of our present leaders who have served year after year, may coming to the end of the careers, opt to pursue other interests or have family commitments. With the ever changing health care environment, the need for strong leadership is utmost as we grow and move forward as a profession. That being said, if selected to the nominating committee, I look forward to serving. NOMINATIONS COMMITTEE: LeRae Scroggins, PT, MBA Le Rae obtained her BS from the University of Iowa in 1980 prior to graduating with a degree in Physical Therapy from the Mayo Clinic School of Health Sciences in She was on Orthopedic Certified Specialist from , and graduated from Northwestern Missouri State University with her MBA in LeRae worked in Joplin, MO at St. John s Regional Medical Center as a supervisor and PT for Acute Care Department from She was the Director of Rehab for Freeman Health Systems from , then became the owner of Focus on Physical Therapy, INC in Joplin and Neosho MO from LeRae made the move back to St John s Regional Medical Center in 2004 as the Administrative Director of Rehab Services, Occupational Medicine and Sport Medicine. In 2008, she became a traveling PT for Med Travelers and then began practicing in the Home Health setting in the Joplin, MO area. LeRae worked as the Rehab Specialty Director for Amedysis from , Healthback HH from , before making the move in 2015 to Oklahoma where she is currently the Director of Rehabilitation for Carter Healthcare. LeRae holds a PT license in Oklahoma, Missouri, and Kansas. She is also a Certified Dementia Practitioner. LeRae is a member of the American College of Health Care Executives, Past President of the Joplin Rotary Club, past President of the Neosho Exchange Club, Past Board Member of the Joplin Area Chamber of Commerce, President of the Mayo Clinic Physical Therapy Alumni Association, and a member of the APTA Home Health Section. In her spare time, LeRae likes to travel to beach destinations, gardening, boating, and spending time with friends and family. I have been an officer in the Mayo Clinic Physical Therapy Alumni Association for the last 25 years and currently serve as the President. I also was active as a director of the Missouri Physical Therapy association and have been an active member of the APTA for 35 years. I value ethical and professional practice of our profession. I continue to gain education to keep current on issues that affect the practice of physical therapy. I have experience as a PT and in management in multiple settings. I will promote the OPTA by seeking qualified and energized individuals to serve the organization professionally.

5 ETHICS COMMITTEE: Denise G. Bender, PT, JD, MEd Denise earned her physical therapy degree from St. Louis University, her Master in Education degree from Wayne State University, and her law degree with honors from University of Maryland, Baltimore School of Law. She presently serves as the Physical Therapy program director and Associate Professor in the Department of Rehabilitation Sciences at the University of Oklahoma Health Sciences Center (OUHSC) since 2008 and was named as the Jill Pitman Jones Professor of Physical Therapy in As a physical therapist with over 35 years of experience, she has been an APTA member in the geriatric physical therapy and education sections. She practiced full-time in clinical settings including the US Navy for ten years, part time in a faculty practice for 11 years, and taught as core faculty in two academic institutions for 25 years. Currently, she provides interprofessional supervision to a student team at the Good Shepherd Pro Bono Clinic. While on faculty at University of Maryland, Baltimore, she participated for three years on the Maryland Board of Physical Therapy Examiners sub-committee that managed the impaired providers program for the physical therapy practice community. In addition to licensure as a physical therapist in Oklahoma, she is a licensed attorney admitted to the Oklahoma state bar and served as a member of the Ruth Bader Ginsberg Inn of Courts for four years. She is a lifetime member of the Phi Delta Phi International Legal Honor Society. Her legal education provides her with an additional frame of reference through which to analyze the issues that arise in her roles as clinician and educator. In 2012, she completed the inaugural class of the APTA s Educational Leadership institute (ELI) Fellowship program. In 2016 she completed the Medical Education Research Certification (MERC). She is her institution s representative to ACAPT and serves on their Education and Pedagogy Consortium. She also serves as core faculty on the campus-wide interprofessional didactic and clinical program EPIC since I view our profession s core ethical values as the essential framework against which all potential clinical, administrative, and professional actions must be evaluated. For the past twenty five years, I have served as physical therapy faculty at two large public institutions. My teaching responsibilities have always included coursework related to administrative and ethical aspects of clinical practice. For the past nine years, I have served as program director for the Doctor of Physical Therapy program and am currently also functioning as part of the executive committee that provides departmental leadership during the search to fill the existing vacancy in the department chair position. My role as program director offers multiple opportunities to guide both faculty and students as they deal with ethical, moral and legal issues related to clinical practice and daily life situations. It is my responsibility to help them clearly examine the details of the situation in the context of an ethical framework created by the code of ethics. My educational background is unique in that it includes a law degree earned with honors at the University of Maryland Baltimore. Despite the many jokes about lawyers, legal education is grounded in the principle of using ethical and rational reasoning to analyze and develop an action plan for ethical issues. The ethics committee serves an important if often unnoticed role for our association. It is not easy to function as a health professional in an environment that can seem so adversarial. Just as any other physical therapist, I have experienced many times when doing the right thing was neither convenient nor easy. There was a constant struggle between the course of action that I could follow and the one that I knew I must follow. These experiences taught me the importance of following the established standards when it would have been so much easier and sometimes more comfortable to convince myself that the specific details in a situation would justify a different response. As a member of the Ethics Committee, I am prepared to apply this hard won knowledge and expertise to serve my physical therapy colleagues.

6 ETHICS COMMITTEE: Kelli Trobaugh, PTA Kelli has been a PTA for 8 years. She has worked for two different home health agencies, the most recent being Encompass Home Health for the past 6 years. Kelli received Physical Therapist Assistant of the year in 2013 from the Oklahoma Association for Home Care and Hospice. In 2014, she received her Advanced Proficiency in Geriatrics through the APTA. Kelli also became LSVT Big certified in She has served as the president of the PTA SIG for the past two years. At Encompass Home Health, Kelli is a certified preceptor and has trained employees in Oklahoma, Kansas, Florida and Arizona. This year Kelli was awarded Preceptor of the year for the Encompass Tulsa office. I have a good understanding of ethical values with implementing policies at work with new hires. I have performed numerous chart audits and assist new hires with adhering to ethics with proper documentation and patient care. ETHICS COMMITTEE: Brandon Scharrer, PT, DPT Brandon graduated from the University of Oklahoma Health Sciences Center in 2013 as a Doctor of Physical Therapy. Prior to that, he graduated from Oklahoma Christian University with a B.S. in Biology with honors. He has worked at Valir Physical Therapy since graduation, first in Midwest City and now managing the Piedmont clinic. He enjoys interacting with patients of all ages and injury types, focusing on injury prevention and screening to ensure people are able to stay as mobile as possible. He specializes in orthopedic therapy and balance/vestibular disorders. He is involved in the Oklahoma Physical Therapy Association and the American Physical Therapy Association. Brandon and his wife have three children that keep them busy! In his spare time Brandon enjoys spending time with his family and playing outside, going on walks with his daughters, and camping. My desire to become a member of the Ethics Committee stems from me wanting to become more actively involved in our state association, and take ownership of our profession as opposed to relying on others to complete these essential tasks. The attributes that can be helpful for me to complete the tasks of being included on the Ethics Committee include maintaining integrity, empathy, and holding myself to high standards not only professionally but personally. I strive to be consistent in decisions based on the right choice and treating others with respect while also empathizing with their current state so as not to pigeon-hole a specific situation or circumstance. Too often it is easy to become entrenched in one stance with an inability to see another side or angle, which is why I attempt to remain unbiased while making decisions until a thorough understanding of facts and circumstances are understood. My experience through working in biological research prior to physical therapy, as well as managing a clinic has helped me to understand the importance of ethical behavior and how it can affect not only a specific person but an entire subset of individuals. Through multiple supervisory roles I have maintained at a variety of jobs, I have had to work within a committee of individuals in determining the correct action needed for preventing poor choices or correcting them once an event has occurred. 7 PT ADVISORY COMMITTEE PT POSITION: Mitsy Martin-Davis PT, MS, CWS Mitsy Martin-Davis is a 1983 graduate of the University of Oklahoma Physical Therapy Program. She has practiced full-time in the OKC area since 1983.She received her Master s degree in 1990 from OU and her Certification as a wound specialist in Her professional practice has been centered in acute care and specifically wound care. She is currently the Director of Rehab Services at OU Medical Center and The Children s Hospital. She has experience in outpatient, home health and school based therapy. Mitsy has been involved in the education of students and peers in wound care through OUHSC and OCCC as well as the Don Waits Memorial Lecture Series. Outside of the world of Physical Therapy, Mitsy enjoys serving her church, travels with family, running, biking and spending time at Grand Lake. I have 3 years experience with serving on the Physical Therapy Advisory Board Committee. I attend the Federation of State boards of Physical Therapy Annual Meeting, and I serve the Federation of State Board of Physical Therapy Board Member Regulatory Training. I have 34 years of practice in Oklahoma as a Physical Therapist in Acute Care, as well as experience in Outpatient, and Home Health settings. PT ADVISORY COMMITTEE PT POSITION: Andrew Ward, PT, COMT Andy is a co-owner of Redbud Physical Therapy which was established in He attended Oklahoma City University and received his BS in Biology in Andy is a former US Peace Corps Volunteer stationed in the Philippines Andy attended physical therapy school at Langston University and graduated in He is currently seeking his Doctor of Science degree in physical therapy from Texas Tech University, which he hopes to complete by December of Andy has worked in the orthopedic environment for the last 24 years. He enjoys working with all types and ages of patients and specializes in the treatment of the spine and chronic pain. Andy has been married for 25+ years to his wife Erin. They have been blessed with 4 wonderful children that keep them very active. Candidate question: What attributes and experiences do you bring to the table to fulfill this position? I have been practicing physical therapy for 25 years and I have been privileged to work with many outstanding PT s in Oklahoma during that time. For the past 20 years I have been a PT in private practice and have faced many issues that directly affected me as a PT and as a practice owner. I believe my years of experience would be put to good use if I am elected to serve on the Oklahoma PT Advisory board. Thank you in advance for your kind consideration.

8 PT ADVISORY COMMITTEE PT POSITION: Scott Erbst, PT Scott graduated from the University of Santa Clara in California and pursued a commercial real estate career in the heart of the burgeoning Silicon Valley. Despite a moderate amount of success, Scott felt unfulfilled so he left his suit and tie behind and embarked on another career journey. Scott received his degree in physical therapy from Langston University in 1992 then moved to California to manage a clinic in Palo Alto. Scott returned to Oklahoma for good in Upon returning, Scott worked in multiple sites throughout the state while employed by a registry company. While this experience was a fantastic opportunity to broaden a skill set and sculpt an eclectic approach to care, Scott s greatest interest remained in outpatient orthopedics. Scott met his wife, Angie in PT school and they have two children who consume Scott's time not spent in the clinic. Thank you for considering me as a candidate for a position on the PT Advisory Committee. I believe a person s life experiences determine his attributes to carry out any particular task successfully since we cannot successfully operate outside the framework of our own personality. That being said, I have played on team oriented sports the majority of my life so working with a group of quality individuals is a task I embrace. Physical therapy is a second career choice for me and it gives me great satisfaction to have the opportunity to bring a measure of health and healing to others. I want to do what I can to ensure that the profession endures for others to have my experience. Lastly, my experience of having brain surgery and coming to accept my son's diagnosis with Duchenne muscular dystrophy reinforced the reality that life is fragile and it also bolstered my sense of humility. PT ADVISORY COMMITTEE PT POSITION: Deb Mason, PT Deb Mason, MS, PT, is a graduate of the Physical Therapy Program at the University of Osteopathic Medicine and Health Sciences (now Des Moines University), in addition to her undergraduate B.S. degree in general biology from the University of Illinois. Deb is the Director of Physical Therapy at Orthopaedic and Sports Medicine Center where she has been employed for 25 years. She has a strong affinity for orthopedics and sports medicine due to her athletic background, with a special interest in manual therapy. She was a three time NCAA All-American in the shot-put and went on to compete in the 1992 Olympic Trials in this event. Deb has been an active member of the OPTA and has previously served on the Ethics Committee and has assisted in the continuing Education Approval process. I have been an active member of the OPTA for many years, having previously served on the Ethics Committee and volunteering to assist in the CEU approval process. Over the years I have had the opportunity to attend several OK PT State Board Advisory Meetings and feel as though this position is very important to our PT licensure and practice oversight. As a departmental manager I have a strong administrative and leadership background. I feel that I would bring these attributes to the Board. I am eager to become more involved in leadership roles within the Physical Therapy profession and I would be honored if you, as my peers, would give me this opportunity

Hinkle, MS, PTA Carla received a Bachelor s degree in Education from New Mexico State University in 1979, served 3 years active duty with the U.S. Army, received an Associate degree as a PTA from Tulsa Junior College in 1991, a Master s degree in Occupational and Adult Education in 2004 and has continued to pursue further graduate hours. She has been a full-time faculty member in the Tulsa Community College PTA program since 1993 and has worked in acute care, SNF and home health. Carla has been an APTA/OPTA member since 1991, having served as the Eastern District Secretary from , and as the Oklahoma PTA (Affiliate) delegate to the House of Delegates in 1995 & She was appointed to the PT Advisory Committee to the OK Medical Board from , chaired the PT Advisory Committee from During that time, Carla served as the Oklahoma alternate delegate and delegate to the Federation of State Boards of PT (FSBPT) in 1998 & She served on the FSBPT Subject Matter Expert Committee for the Physical Therapist Assistant Job Analysis from and the OPTA Nominations Committee from Carla has been an active member of the Eastern District Continuing Education Sub-Committee since Attributes and experiences that I bring to the table for the role of PTA on the PT Advisory Committee are 5 years experience on the PT Advisory Committee. She has worked for two different home health agencies, the most recent being Encompass Home Health for the past 6 years. Kelli received Physical Therapist Assistant of the year in 2013 from the Oklahoma Association for Home Care and Hospice. In 2014, she received her Advanced Proficiency in Geriatrics through the APTA. Kelli also became LSVT Big certified in She has served as the president of the PTA SIG for the past two years. At Encompass Home Health, Kelli is a certified preceptor and has trained employees in Oklahoma, Kansas, Florida and Arizona. This year Kelli was awarded Preceptor of the year for the Encompass Tulsa office. Candidate question: What attributes and experiences do you bring to the table to fulfill this position? Having served as certified preceptor for Encompass, I have a vast knowledge of the procedures and policies that are required in our profession. I have a good understanding of ethical values with implementing policies at work with new hires. I have performed numerous chart audits and assist new hires with adhering to these procedures and policies with proper documentation, patient care, and overall characteristics in this profession.
