Intended Audience Managed care pharmacists. Activity Overview In the United States, obesity is characterized as this century s greatest healthcare threat. The American Medical Association and several other large organizations now classify obesity as a disease. The obesity epidemic is leading to advancements in the way obesity is treated in primary care. Behavioral therapy techniques provide a framework for discussing obesity in primary care. A number of factors either deter or stimulate the progress of weight-loss therapy. Communicating the benefits of losing weight for the management of comorbid conditions is a potent motivator for weight loss. Diet and exercise advice should be individualized to consider patient preferences, limitations, and ability. A team-based approach with participation of dieticians, nurse specialists, and obesity specialists, as well as commercial weight-loss programs, distributes the burden of management away from a single primary care clinician and provides patients with access to more treatment modalities. Obesity treatment options have increased dramatically in recent years, including new weight-loss drugs, bariatric surgery innovations, and minimally invasive intragastric balloons. The efficacy and tolerability of medications for long-term management, including orlistat, lorcaserin, phentermine-topiramate ER, and naltrexone-bupropion SR, are discussed, as well as the role of bariatric surgery and intragastric balloons. Managed care organizations are addressing changes in national requirements and are responding to the availability of new weight-loss drugs to help patients achieve better health. Statement of Educational Need Approximately one-third of American adults are obese. The high prevalence of obesity, the associated medical complications, and high healthcare costs make obesity a serious public health issue. Individuals who are extremely obese (having a body mass index [BMI] of kg/m 2 ) live an estimated 6.5 to 13.7 years less than people with a normal BMI. Obesity is a risk factor for numerous chronic diseases, including diabetes, heart disease, osteoarthritis, and cancer. In addition, obesity increases school absenteeism and decreases earning potential. Healthcare provider awareness may lead to increased screening of patients at risk, increased prescribing of medications to prevent or treat obesity, and recommendations of lifestyle changes for those who are obese. Educational Objectives Upon completion of this activity, participants should be able to: Explain the prevalence, incidence, and economic consequences of obesity. Examine current obesity treatment recommendations and discuss the role of pharmacologic options and lifestyle modifications in the management of obesity. Identify the economic impact of obesity from a long-term perspective. Demonstrate the importance of screening, lifestyle changes, and pharmacotherapy in treating and preventing obesity.
