2 MARY & DICK ALLEN DIABETES CENTER The has two locations in Orange County, one at the Mike & Lori Gray Campus Hoag Health Center-Newport Beach, adjacent to Hoag Hospital s Newport Beach campus and a second at Hoag Health Center Irvine - Woodbridge. Specialized programs focus on meeting the challenges of diabetes care by providing comprehensive services and support that reflect the diversity in the community. The Allen Diabetes Center also offers educational programs and support services designed specifically to meet the needs of patients with diabetes and their families. All professional educational programs of the are made possible by philanthropic support from our generous donor community. PROGRAM OVERVIEW The presents its annual conference on diabetes. This knowledgebased, one-day conference will focus on comprehensive and innovative clinical and psychosocial approaches for management of prediabetes and diabetes, emerging technologies in insulin delivery and glucose monitoring, successful transitions of care for emerging adults and inpatients, and cardiovascular disease management in diabetes. WHO SHOULD ATTEND This is a knowledge-based activity designed for diabetes educators, physicians, registered nurses, registered dietitians, pharmacists, nurse practitioners, physician assistants, and other health care providers interested in staying up to date on current practices of care for their patients with diabetes and other related conditions.

5 PROGRAM OBJECTIVES Describe the unique features and ideal patient populations for continuous glucose monitoring (CGM) devices. Identify examples of wireless glucose meters and smartphone applications that can help enhance diabetes management. Identify practical strategies for addressing disengaged patients and promoting more effective self-management. Identify primary stakeholders and their roles in a successful transition process for emerging adults with type 1 diabetes. Discuss clinical and psychosocial needs of emerging adults with type 1 diabetes around their transition to adult care Discuss adult treatment options and blood glucose goals based upon transition of care from the hospital Discuss interventions to modify risk factors for prevention of type 2 diabetes Explain effective ways to proactively screen, assess, follow-up, and evaluate patients with prediabetes Explain the relationship between diabetes and cardiovascular disease, and approaches to modify risk of cardiovascular disease DESIRED LEARNING OUTCOMES: As a result of the program, participants will consider implementing strategies to motivate disengaged patients in diabetes self-care, identify emerging technologies in diabetes management, consider important factors affecting transition of care in both emerging adults and those moving from the hospital to an outpatient setting, discuss multidisciplinary approaches to management of prediabetes and type 2 diabetes, and assess cardiovascular risk in type 2 diabetes.
