1 Non-Product Specific Workshop Series: DO YOU SEE WHAT I SEE? Visualizing Disorders During VFSS and FEES - By Dr Joseph Murray JANUARY 2019: 2 Days Workshop (Plus 2 Satellite Workshops in 1 day, available for Registration by Speech Therapists) Day One: (12 January 2019, Saturday, ) This session will focus on improving the accuracy and reliability of a practicing clinician s interpretation of instrumental swallowing assessments. The course will include a review of the popular assessment techniques with particular focus on the videofluoroscopic and Fiberoptic Endoscopic Evaluation of Swallowing (FEES) assessments of swallowing. During this session the clinician will review tape recorded segments of swallowing studies and will be able to provide anonymous responses to the presenter s stimulus questions by using hand held keypads that communicate with the presenter s computer. This technique allows candid responses from the audience members and allows the attendee to compare their answers to other attendees and to expert opinion that has been rendered in the past. This session is intended to calibrate co-workers who use the videofluoroscopic instrument in their institution by introducing a common set of operational definitions and shared lexicon. The attendee will list 2 perceptual biases that can affect the interpretation of the videofluoroscopic swallow assessment (VFSS) and Fiberoptic Endoscopic Evaluation of Swallowing The attendee will be able to compare and contrast reliability and accuracy. The attendee will describe methods for reducing bias when interpreting instrumental swallow assessments. Day Two: (13 January 2019, Sunday, ) The program is designed to provide a didactic review of signs and symptoms of dysphagia as viewed with the laryngoscope during the performance of the Fiberoptic Endoscopic Evaluation of Swallowing. Additionally, a hands-on session will train the attendees in the appropriate physical manipulation and placement of the endoscope on live subjects. (Endoscopes will be provided during hands-on sessions). Didactic Session: The attendee will develop understanding and skill in distinguishing normal and abnormal findings. The attendee will be able to identify detailed anatomy of the nasal cavity, nasopharynx, hypopharynx and laryngopharynx. The attendee will demonstrate understanding of the application of the procedure for both assessment and treatment with differing patient populations. The attendee will learn the indications and contraindications for safe administration of the procedure. Hands-on Session: The attendee will learn the appropriate physical manipulation of the instrumentation. The attendee will demonstrate the ability to modify the protocol based on inferences drawn from spontaneous and elicited signs and symptoms of dysphagia Delivery method for teaching will include classical lecture techniques, demonstration through videotape and handson application combining visual evidence and connected learning, collaboration through discussion of case-studies and final integration of remaining questions through discussion and review.
2 VISITING SPEAKER: Dr. Joseph Murray is Chief of the Audiology Speech Pathology Service for the VA Ann Arbor Healthcare System. His research and publications are in the area of assessment, treatment and management of patients with dysphagia. His book Manual of Dysphagia Assessment in Adults, is being readied for e-publication. He has provided numerous workshops, seminars and invited lectures in North America, South America, Europe and Australia. 4 SATELLITE WORKSHOPS: WORKSHOP A: (3 hours) The Effect of Tracheostomy Tubes on Swallow Function Date: 14 January 2019 Time: 9am to 12 noon Venue: GSBE Global Academy This session will help the clinician understand the basics of tracheostomy tube types and function with special emphasis on the effect of tubes on swallowing function. The learner will view illustrated examples of tracheostomy tube placement and the effect of the tubes on swallow function. Guidelines for treating patients with tracheostomy will be reviewed in detail and case studies will be presented. The attendee will demonstrate skill in distinguishing different types of tracheostomy tubes The attendee will learn the effect of Passy-Muir valves on swallow function The attendee will demonstrate understanding of guidelines for the assessment and treatment of patients with trachestomy WORKSHOP B: (3 hours) Aspiration Pneumonia and Dysphagia Date: 14 January 2019 Time: 3pm to 6pm Venue: GSBE Global Academy This session will focus on the emerging research and changing practices in the management of dysphagia as it relates to aspiration pneumonia. This session will include a review of the contribution of oral biofilms to aspiration pneumonia and systemic health. The genesis and treatments for aspiration pneumonia will be reviewed. Additionally the attendee will be challenged to question conventional wisdom which held that controlling aspiration via NPO orders or enteral feeding were a reliable prophylaxis for aspiration pneumonia. The attendee will understand the association between aspiration pneumonia and dysphagia The attendee will be able to list the contributions of cytokines to the development of oral colonization of pathogens. The attendee will describe methods for reducing the risk of aspiration pneumonia in institutionalized elders.
