Dr. Thomas Aloia is a Hepatobiliary Surgical Oncologist in the Gastrointestinal Center at the University of Texas MD Anderson Cancer Center. Dr. Aloia received his medical degree from the University of California Los Angeles Geffen School of Medicine. His postgraduate training included a general and thoracic surgery residency at Duke University Medical Center and fellowships in Surgical Oncology at MD Anderson and Hepatobiliary/Liver Transplantation at the Paul Brousse Hospital in Paris, France. He has recently completed a Certificate in Healthcare Management at the Rice University (Jones) Business School. Dr. Aloia s clinical expertise is in hepatobiliary surgery, gastrointestinal surgical oncology, minimally invasive surgery and liver transplantation surgery. He is an ad hoc reviewer for multiple medical and surgical journals and serves on the editorial board of The Journal of Oncology Practice and the Annals of Surgical Oncology. He has published multiple manuscripts related to his areas of research interests including advanced liver imaging techniques, chemotherapy-induced liver injury, and the medical economics of cancer surgery. At MD Anderson, he has focused his administrative efforts in the areas of patient safety, quality, and functional outcomes including his role as the ACS NSQIP Surgeon Champion and as the co-chair of the Division of Surgery Surgical Safety Committee. Recently, Dr. Aloia was appointed as Program Director, Department of Surgical Oncology Hepatopancreaticobiliary Surgery Fellowship Program and Associate Medical Director of the Gastrointestinal Disease Center at MD Anderson. Dirk Arnold Tumor Biology Institute Freiburg, Germany Prof Arnold is a specialist in Hematology Oncology and Palliative Care, and currently Director of the Tumor Biology Institute Freiburg. Before he took over leadership there, he was appointed as W3 Professor of the University Cancer Center Hamburg, and held positions at Universities of Halle-Wittenberg and Berlin. His major focus in clinical and translational cancer research is the development of modern treatment strategies for gastrointestinal cancers with the integration of novel drugs. Another focus of his work is optimizing treatment structures for patients with malignancies, in large scale networks for research and cancer care. Prof Arnold is a member of numerous national and international societies e.g. ASCO, ESMO (where he Chairs the Clinical Guidelines Committee), EORTC, NCI, CR UK, and has published more than 80 peerreviewed manuscripts. Vanessa Blair Whangarei Hospital and University of Auckland, New Zealand Dr Vanessa Blair, FRACS, PhD. General, Breast, Endocrine and Skin Cancer Surgeon, Whangarei Hospital, New Zealand. Lecturer in Surgery, University of Auckland. Vanessa trained in General Surgery in New Zealand (NZ) and was made a Fellow (RACS) in At Auckland University she received the Rotary Club of Auckland Prize, awarded to the most distinguished graduate in medicine. During surgical training she did a PhD in Hereditary Diffuse Gastric Cancer (HDGC), about which she has written several papers and book chapters and been involved in publication of NZ and

2 10 COSA Annual Scientific Meeting international guidelines. She is on the NZ National Tumour Board for Melanoma, convened in 2012 to formulate national standards of care. After completing a fellowship in breast, endocrine and melanoma surgery, in 2011 she returned to her home province in far north NZ to take up a consultant post at Whangarei Base Hospital, which covers a population that is 32% Maori and includes rural and remote areas. Her interests centre on her 2 young sons, the small farm her husband is developing and all things culinary. Alex Chan Department of Pharmacy at National University Dr. Alexandre Chan is a tenured Associate Professor with the Department of Pharmacy at National University of Singapore and an Associate Consultant Clinical Pharmacist at National Cancer Centre Singapore. He is also a Co- Director of the Oncology Pharmacy Residency at National University of Singapore and he is a Board Member of the Multinational Association of Cancer in Supportive Care. He is currently certified by the US Board of Pharmaceutical Specialties as Board Certified Pharmacotherapy Specialist and Board Certified Oncology Pharmacist. Dr. Chan s clinical and research portfolio includes oncology supportive care (in lymphoma and breast cancer patients), anticancer drug-drug interactions and associated toxicities, and pharmacoepidemiology. He has published over 70 full-length peerreviewed manuscripts in various medical and pharmacy journals, including Journal of Clinical Oncology, Lancet Oncology, Annals of Oncology, Pharmacoepidemiology and Drug Safety, Supportive Care in Cancer, Clinical Therapeutics and Annals of Pharmacotherapy. Dr. Chan has received numerous grants and awards for his research, and has delivered lectures at numerous regional and international oncology and pharmacy conferences. Ian Chau Royal Marsden Hospital, London; and the Institute of Cancer Research, Surrey, UK Dr Ian Chau holds the position of consultant medical oncologist within the Gastrointestinal and Lymphoma Units at Royal Marsden Hospital and honorary senior lecturer at the Institute of Cancer Research, London & Surrey, UK. He qualified from the United Medical & Dental Schools of Guy s and St. Thomas Hospitals, University of London, before undergoing medical oncology specialist training and clinical research fellowship at the Royal Marsden Hospital. His main research interests are clinical trials and translational research in gastrointestinal cancers and lymphoma. He is the principal investigator of multiple phase I, II and III clinical trials evaluating novel treatment strategies and agents in various cancer signalling pathways. He serves on the committees for the UK National Cancer Research Institute (NCRI) Colorectal Cancer and Pancreatic Cancer Clinical Study Groups as well as the American Society of Clinical Oncology (ASCO) Scientific Program Committee. He is on the trial management groups of the UK NCRI phase III gastrointestinal cancer and lymphoma trials and International Advisory Panel for the Cancer Research UK Clinical Trials Awards & Advisory Committee. He is the recipient of merit awards from ASCO Gastrointestinal Cancer Symposium, The Royal Society of Medicine Section of Oncology Clinical Prize, fellowships from the European School of Oncology and ASCO/AACR/FECS Clinical Cancer Research Workshop. He has given invited lectures in over 20 countries. He has served on international drug development advisory boards and has peer review publications in the New England Journal of Medicine, Journal of Clinical Oncology and other leading oncology / biomedical journals.

3 COSA Invited International Speakers 11 Harvey Jay Cohen Duke University School of Medicine, North Carolina, USA Harvey Jay Cohen, MD, is Walter Kempner Professor, Director, Center for the Study of Aging and Human Development, Chair Emeritus, Department of Medicine at Duke University Medical Center. Dr. Cohen co-chairs the Cancer in the Elderly Committee for The Alliance for Clinical Trials in Oncology and is PI of the Partnership for Anemia: Clinical and Translational Trials in the Elderly (PACTTE). He is past President of the American Geriatrics Society, the Gerontologic Society of America and the International Society of Geriatric Oncology. He has published extensively with more than 300 articles and book chapters, with special emphasis on geriatric assessment, biologic basis of functional decline, and cancer and hematologic problems in the elderly. He is co-editor of Geriatric Medicine, 4th Edition; Practical Geriatric Oncology; and author of the book Taking Care After 50. He has received the BJ Kennedy award from the American Society for Clinical Oncology, and the Paul Calabresi Award from the International Society of Geriatric Oncology. Patti Ganz David Geffen School of Medicine; UCLA; and Jonsson Comprehensive Cancer Center, California, USA Professor of Medicine at David Geffen School of Medicine; Professor of Health Services, Jonathan and Karin Fielding School of Public Health at UCLA; Director, Cancer Prevention & Control Research, Jonsson Comprehensive Cancer Center, Los Angeles, CA, USA Professor Patricia A. Ganz, M.D., a medical oncologist, has been a member of the faculty of the UCLA School of Medicine since 1978 and the UCLA School of Public Health since Since 1993 she has been the Director of Cancer Prevention and Control Research at the Jonsson Comprehensive Cancer Center. In 1999 she was awarded an American Cancer Society Clinical Research Professorship for Enhancing Patient Outcomes across the Cancer Control Continuum, and in 1999 and 2000 received the Susan G. Komen Foundation Professor of Survivorship Award. Dr. Ganz was elected to the Institute of Medicine, U.S. National Academy of Sciences, in She served on the National Cancer Institute Board of Scientific Advisors from and on the American Society of Clinical Oncology (ASCO) Board of Directors from A pioneer in the assessment of quality of life in cancer patients and survivors, Professor Ganz is active in clinical trials research with the National Surgical Adjuvant Breast and Bowel Project (NSABP). She has focused much of her clinical and research efforts in the areas of breast cancer and its prevention, and was a member of the NCI Progress Review Group on Breast Cancer. At the Jonsson Comprehensive Cancer Center, she leads the scientific program focused on Patients and Survivors. In 1997 she established the UCLA Family Cancer Registry and Genetic Evaluation Program, which serves patients and survivors, as well as those at high risk for familial/ hereditary cancers. Her other major areas of research include cancer survivorship and late effects of cancer treatment, cancer in the elderly, and quality of care for cancer patients. She served on the Institute of Medicine (IOM) Committee responsible for the 2005 report From Cancer Patient to Survivor: Lost in Transition, and on the 2008 IOM Committee for the report Cancer Care for the Whole Patient: Meeting Psychosocial Health Needs. Professor Ganz is an Associate Editor for the Journal of the National Cancer Institute, and CA-A Journal for Clinician, and is on the Editorial Board of the Journal of Cancer Survivorship. She was a founding member of the National Coalition for Cancer Survivorship (NCCS) in 1986, and has directed the UCLA-LIVESTRONG Survivorship Center of Excellence at the Jonsson Comprehensive Cancer Center since 2006.

4 12 COSA Annual Scientific Meeting Lee Jones Duke Cancer Institute, North Carolina, USA I am Associate Professor and Scientific Director of the Duke Center for Cancer Survivorship in the Department of Radiation Oncology within the Duke Cancer Institute. I was born in Stoke-on-Trent, England and completed my Bachelor s of Science (Hons) in Sport and Exercise Science at the University of Brighton (Eastbourne, East Sussex, England). Following a year of employment as a lifestyle consultant, I moved to Canada to complete my Masters of Science in Kinesiology at Lakehead University (Thunder Bay, Ontario, Canada). From there I moved to Edmonton, Alberta to complete my PhD in Exercise Oncology research at the University of Alberta. I also completed my Postdoctoral Fellowship in Exercise Oncology research at the University of Alberta. I joined DUMC in January 2005 in the Program of Cancer Prevention, Detection, and Control. In July 2009, I was made Director of Cancer Survivorship Research and the Duke Exercise-Oncology Program within the Duke University Health System and Duke Cancer Institute. I live in Durham with my wife, Nicole. I am an avid exerciser and have completed the New York Marathon twice on both occasions to raise money for the Tug McGraw Foundation that supports brain tumor research. Janine Overcash James Cancer Center; Solove Research Institute; and Ohio State University, Ohio, USA Janine Overcash is currently the Director of Nursing Research at the James Cancer Center and Solove Research Institute and Clinical Associate Professor at The Ohio State University, College of Nursing. Dr. Overcash is a geriatric nurse practitioner specializing in the care of the older cancer patient. She has assisted in the design and management of one of the first geriatric oncology programs located at the H. Lee Moffitt Cancer Center and Research Institute in Tampa, Florida. Dr. Overcash has authored over 35 peer reviewed journal articles in the area of geriatric assessment. A book entitled, The Older Cancer Patient: A Guide for Nurses and Related Professionals by Janine Overcash and Lodovico Balducci highlights principles of care of the older person with cancer and received book of the year award by the American Journal of Nursing. Dr. Overcash has completed a post doctorate with the John A. Hartford Building Academic Geriatric Nursing Capacity Program. Dr. Overcash participated in the Geriatric Nurse Educational Consortium sponsored by the American Academy of Colleges of Nursing (AACN) and the John A. Hartford Foundation which instructed over 500 faculty from all over the United States. Dr. Overcash is currently interested in maintaining functional status of older women undergoing chemotherapy. Other research interests include understanding falls, performance status and independence in older cancer patients. Bas Wijnhoven Erasmus MC Rotterdam, The Netherlands Bas Wijnhoven went to medical school in Utrecht the Netherlands. After obtaining his MD in 1997 he started a PhD program on the molecular and clinical aspects of oesophageal cancer at the Departments of Surgery and Pathology, Erasmus MC in Rotterdam. In 2001 he received his PhD with honour. In the year 2000 he started his general surgical training in the city of Delft and subsequently Rotterdam and registered as general surgeon in From 2006 till 2008 he worked as a researcher and surgeon at the Department of Surgery, Flinders Medical Center, Adelaide, Australia. Within these two years, he enjoyed a training program in upper gastrointestinal and hepatobiliairy surgery. Since 2008 he is appointed as staff surgeon at the Erasmus MC Rotterdam and is program leader on oesophagogastric surgery. His clinical interests are benign and malignant diseases of the upper gastrointestinal tract. Research is focused on studies on clinical outcome after surgery for cancer and the molecular biology of Barrett s oesophagus and oesophageal adenocarcinoma. In close

5 COSA Invited International Speakers 13 collaboration with other departments within the Erasmus MC, preclinical and clinical studies on neoadjuvant treatment for oesophageal and gastric cardia cancer are another area of interest. Dr. Wijnhoven has a keen interest in medical publishing: he served as editor assistant for the British Journal of Surgery and is editorin-chief for Digestive Surgery. He has authored and co-authored over 80 articles in national and international peer-reviewed journals. Starting November 2012 he is Program Director of Surgical Training at the Erasmus MC and responsible for general surgical training in the Rotterdam area. Rebecca Wong Princess Margaret Hospital, Toronto, Canada After graduating from Sheffield University and completing her radiation oncology residency at Queens University, Dr. Rebecca Wong joined the Department of Radiation at the University of Toronto, first with the Odette Cancer Center, and since 2001 with the Princess Margaret Cancer. She is Director of the clinical trials program at her institution, and full professor in the department of radiation oncology. Her clinical and research interests are in gastrointestinal malignancies and palliative radiotherapy. Through her role as the Co-chair for the esophagogastric disease oriented group and chair for the symptom control committee at National Cancer Institute of Canada Clinical Trials Group, she strives to help shape the Canadian research directions in these clinical areas. She is also active in evidence based guidelines development through her leadership roles in the Cancer Care Ontario Practice Guideline Initiative. She is the author of over 100 peer reviewed publications, and 8 book chapters.
