This was due to the recent increase in Vancomycin-Resistant Enterococcus infections in the hospital. Patient charts were reviewed to assess the need for oral vancomycin therapy and whether appropriate diagnosis of C. difficile was confirmed. The MUE results were presented to the Pharmacy and Therapeutics Committee ( P&T) Emergency Room Pharmacist Conducted research on the importance of an emergency room pharmacist for clinical interventions and to ensure drug review prior to patient administration. Acted as the emergency room pharmacist for 4 weeks and prospectively documented all mediation interventions made and assessed the acceptance by other emergency room staff. This project was presented at SERC in April Medication Use Evaluation (MUE) Performed a medication use review on patients on long acting beta agonists (LABAs). This was due to the recent FDA warning on the increased risk for asthma exacerbations and death for patients on LABAs monotherapy. Assessed patient charts to see if they were prescribed a corticosteroid in conjunction with their LABA and also assessed their adherence to therapy. Presented result to P&T.
4 E. Guirguis page Process Improvement Project Evaluated the medication distribution process of the inpatient pharmacy in regards to missing doses requests from the patient floors. Conducted data collection of the primary reasons why missing dose requests were initiated and verified if they were true missing doses or avoidable requests. Implement recommendations for reduction of avoidable requests and compared the number of requests pre and post implementation.
