Department of Rehabilitation Medicine, School of Medicine, College of Medicine, China Medical University, Taichung, Taiwan Objective: To estimate the prevalence of rotator cuff tear in elderly patients with shoulder pain in Taiwan (rural area). Methods: We reviewed the medical records of 1077 patients (400 male and 677 female) since 2007 to 2012 who visited our hospital because of shoulder pain for over 3 months. Inclusion criteria included age over 65 y/o, no trauma history of shoulder, and no shoulder operation history. All patients received arthrogram evaluation for shoulder condition. Results: Among the 472 enrolled patients, 300 patients had shoulder pain in the right side, 170 in the left side and 2 patients in both sides. Age ranged from 65 to 96 y/o (mean 80±15). Totally 474 shoulders were assessed, and 41 % (193/474, including 75 males and 118 females) had evidence of rotator cuff tear in arthrographic findings including 92.2% of full thickness tear (178) and 7.7% of partial tear (15). Arthrographic findings in other 44 %( 208/474) of patients revealed adhesive capsulitis. Only 15% (73/474) patients revealed the normal shoulders. There were 69.6% (138/193) of patients with rotator cuff tear in right shoulder and 23.3% (45/193) in left shoulder. Conclusion: The results indicated that rotator cuff tear is an important cause of shoulder pain in elderly patients. Early accurate diagnosis and treatment can improve later socioeconomic problems and disability. The psychometric properties of the s-mhls, however, have not been examined in mid-aged and elder patients with stroke. We aimed to investigate the internal consistency reliability, convergent validity, and discriminative validity of the s-mhls in mid-aged and elder patients with stroke. Methods: Eighty-seven mid-aged and elder patients with stroke volunteered to participate in this study. Each patient was interviewed face-to-face with the s-mhls. The Public Stroke Knowledge Quiz (PSKQ) was administered as a criterion for examining the convergent validity of the s-mhls. Discriminative validity of the s-mhls was determined with age and education level as independent grouping variables. Results: The s-mhls demonstrated good internal consistency reliability (Cronbach s alpha=0.82) and high correlation with the PSKQ (Pearson s r=0.62). Satisfactory discriminative validity of the s-mhls was supported by significant differences in s-mhls scores between different age groups (i.e., middle-aged and elderly) and education level groups (i.e., below junior high and above senior high). The s-mhls, however, showed a notable ceiling effect (24% of the patients), indicating that the s-mhls cannot differentiate between individuals with high health literacy. Conclusion: The results of this study suggest that the internal consistency reliability, convergent validity, and discriminative validity of the s-mhls are adequate for assessing health literacy of mid-aged and elder patients with stroke for research purposes. However, for clinical usage, the s-mhls is needed to improve its internal consistency reliability for individual comparison and ceiling effect. Reduced muscle mass and declined muscle strength due to aging not only poses a great impact on independence of functional activity in elderly but is the major cause of fall. To effectively and precisely evaluate the muscle strength in the elderly could make a great help in designing their rehabilitative programs later. However, little research was studied the test-retest reliability of muscle strength with handheld dynamometer in aboriginal elderly in Taiwan. The purpose of the study was to investigate the test-retest reliability of muscle strength with hand-held dynamometer in bilateral upper and lower extremities of community-dwelling aboriginal elderly women. Method: A convenience sample of 31 aboriginal elderly women from Hsiu-lin township, Hualien County were recruited in two phases of the study. In the first phase, sixteen elderly Taiwanese aboriginal women were collected to calculate the inter-rater test-retest reliability of muscle strength. The first therapist using hand-held dynamometer with Break method measured twice the strength of bilateral upper-extremity and lower-extremity muscle groups, with interleaving 30 seconds between each muscle testing. After another 30-minute rest period, the second therapist performed the same muscletesting procedure as the first one. In the second phase for intrarater test-retest reliability, another 15 subjects were recruited and conducted the same procedure as the first phase by the first therapist, but one week apart from the two measurement sessions. Results: In the first phase, the inter-rater intraclass correlation coefficients (ICCs) were ranged from to 0.987, with standard error of testing ranged from Kg and coefficient of variation from 3.04 to 9.43%. In the second phase, the intra-rater ICCs were ranged from to 0.977, with standard error of testing ranged from Kg and coefficient of variation from 5.48 to 9.31%. Conclusion: Both intra-rater and inter-rater ICCs were high in bilateral upper-extremity and lower-extremity muscle groups with hand-held dynamometer testing in the elderly Taiwanese aboriginal women. Further investigations would be warranted for a larger sample with different sex or health status to obtain more information about the intra-rater and inter-rater reliability of hand-held dynamometer measurements. Division of Geriatric Research, Population Health Science Institute, National Health Research Institute, Taiwan Objectives: Polypharmacy is often confronted by older people with multiple medical diseases. We wonder if polypharmacy is associated with depression, a geriatric syndrome for early detection to prevent disability. However, few studies have examined the association between polypharmacy and depression. To compare the association between polypharmacy and depression in older people with chronic diseases. Methods: The elderly outpatients were recruited for a 2-year comprehensive geriatric assessment (CGA) study in a medical center. Positive depression was diagnosed by the Geriatric Depression Scale 15-item version (GDS-15) score 10 or active depression disorder under treatment. Polypharmacy was defined as use of 9 or more medications. The association of polypharmacy with depression was evaluated by chi-square test and multivariate logistic regression. Results: A total of 189 aged 65 and over outpatients enrolled into the study. There were 19 patients (10.1%) with GDS 10 and 13 patients (6.9 %) with active depressive disorder. Polypharmacy was identified in 26 (13.8%) patients. The odds ratio (OR) of positive depression for the patients with medications 9 was 8.43 (95% confidence interval [CI], ; P <0.01) and medications from 5 to 8 was 2.36 (95% CI, p=0.093), comparing the patients with medications < 5. Multivariate logistic regression analysis revealed that the overall OR for patients using 9 or more drugs was 5.27 (95% CI, , P < 0.01) after adjustment of frailty and economic status. Conclusions: Use of 9 or more medications is associated with depression among outpatient older people with chronic diseases. Whether polypharmacy is a predictor of depression or not needsfurther study with big sample size. 8

9 A05 TREND OF LIFE EXPECTANCY OF PATIENTS FOR RADICAL PROSTATECTOMY OF PROSTATE CANCER IN TAIWAN 2012 UPDATE Chih-Cheng Lu, Wen-Chou Fan, Tse-Chou Cheng Chi Mei Medical Center, Liouying, Taiwan Objective: Life expectancy of the patients receiving radical prostatectomy in localized prostate cancer is 10 years or more, based on the clinical consensus of prostate cancer diagnosis and management by Taiwan Cooperation Oncology Group (TCOG, 2010). This study is to review the trend of life expectancy of male population in Taiwan region for the reference in clinical practice. Materials and methods: From 1991 to 2012, life expectancy reports by the service of the Department of Statistics, Ministry of the Interior (MOI) of Taiwan were reviewed. The printed practice guidelines for prostate cancer by TCOG and National Comprehensive Cancer Network (NCCN) were reviewed. Investigate the trend of life expectancy of the male population in Taiwan region which includes Taipei city, Kaohsiung city but no Fukien special areas. Results: Average 5-year survival rate of prostate cancer is around 77% mentioned by TCOG. When the patient s life expectancy is less than 5 years, no further workup or treatment is suggested except for some high risky patients by NCCN. Radical prostatectomy is considered when the patients with a life expectancy of 10 years or more and no serious co-morbid conditions that would contraindicate an elective surgery by NCCN in From 1991 to 2012, the male life expectancy at birth in Taiwan was from 71.8 up to 76.4 years. The report of life expectancy more than 10 years from MOI showed at the age of 71 years in 1991, 72 years from 1992 to 1995, 74 years in 1996, 75 years from 1997 to 1999, 76 years from 2001 to 2005, 77 years in 2006 to In 2012, the male life expectancy at the age of 77 and 78 years were and 9.62 years, respectively. Conclusions: In this limited updating study, 77 years of age is the upper limit for the patients with localized prostate cancer considering to undergo radical prostatectomy in Taiwan region. Further research with longer follow-up is needed to clarify the relationship. A total of 4745 elderly Taiwanese comprised the major analytic cohort. SRH was categorized into good, fair and poor health. Cox regression methods were used to examine the association between SRH and mortality. Results: During the follow-up periods (median: 10.1 years), 1644 deaths occurred. Participants with poor SRH were at higher risk of mortality [relative risk (RR), % confidence interval (CI), )] than those in good SRH. This association was strongest among the young-old (ages 50-59) than older-old individuals. The association was statistical significant in men (RR, 1.53, 95% CI, ) only, but not in women (RR, 1.19, 95% CI, ). Conclusion: Low SRH was associated with increased risk of mortality in older adults. The associations were stronger in men and young-old adults. The physicians should consider a more detailed consultation and support for these groups. 9

Secondly, it intends to develop a conceptual framework encompassing service quality, patient satisfaction and recommendation. Finally, the relationship between the three variables is validated by applying structural equation modelling (SEM). Methods: Data was collected from those in the elderly health examination program in a branch of Taipei City Hospital in April 2013 through a structured questionnaire based on the SERVQUAL model. The technique of SEM was adopted to analyze sample data and test hypotheses. Results: Service quality is found to have a significant direct effect on patient satisfaction. So is patient satisfaction on recommendation. However, neither direct nor indirect effect of service quality on recommendation has reached a significant level. Conclusions: The direct effect of service quality on elderly health checkup examinees indicates that improving quality of healthcare might be a solution to increase senior users satisfaction. Results of the research presents policy implications in terms of elderly care and healthcare quality. There were several study that confirm the using of white blood cell and ratio neutrophil lymphocyte count as a parameter of systemic inflammations in critically ill patients. The aim of this study is to determine the relation between mortality with age, white blood cell, lymphocyte, netrophil, ratio neutrofil limfosit in geriatric patients with SIRS. Methods: This is a analytics observational retrospective study and data was taken from medical record. The study population included patients aged > 60 years hospitalised in Sanglah Hospital between Juni 2011 to April 2012 and fulfill criteria for SIRS (temperature > 38 C or < 36 C, heart rate >90 beats/ minute, respiratory rate >20 beath/ minute or PC02 <32, WBC > 12x109/L or < 2x109/L). All statistical analyses were conducted with SPSS version 21. Results: A total 126 patients ( aged from years, with a mean 72,06 years) were followed. The patients is 75 (59,5%) male and 51 (40,5%) female. The neutrophil count is range from 0,6-57,9 x 103 /ul, the lymphocyte count is range from 0,2-11,6 x 103 /ul. Ratio neutrophil and lymphocyte range from 0,52-78,5. There was no significant correlation between white blood cell, lymphocyte, ratio neutrophil lymphocyte and length of stay based on bivariate analysis. Using multivariate linier analysis, we found that age and neutrophil can be used as weak predictor to length of stay (adjusted R= 0,02) Conclusions: There s no significant correlation between white blood cell, lymphocyte and ratio neutrophil lymphocyte geriatric patient with SIRS. However, screening for frail elderly and preventing their functional decline are also very important. We tried to increase function evaluation in the elderly annual health examination and use case management method to improve the function of examinees thereafter. Methods: This is a quasi-experimental controlled trial. Four hundred and four adults aged 70 and older who joined in the 2013 annual elderly health examination at Taipei city hospital Yangming branch and Canadian Study of Health and Aging Clinical Frailty Scale (CSHA- CFS) Chinese In-Person Interview Version scored 3-6 were enrolled. One hundred and thirty two participants who agreed to receive functional examination joined in the experimental group. Interventions: Experiment group received CSHA- CFS, Fried Frailty Index(FFI), timed up and go test(tug), Short-form Physical Performance Battery (SPPB) examination, exercise instruction and nutrition education at 0, 3rd and 12th month and routine annual elderly health examination at 0 and 12th month. Case manager reminded the experimental group to keep exercise and nutrition supplement by telephone interview at 6th and 9th month. Control group receive CSHA- CFS and routine annual elderly health examination at 0 and 12th month. Both groups received telephone interview to collect adverse event data at 3rd, 6th, 9th and 12th month. Results: The baseline characteristics including gender, age, CSHA- CFA, difficult in walking 800 meters, difficult in climbing 10 stairs, number of comorbidity... were similar for the two groups.112 (84.5%) persons of experimental group and 267 (98.2%) persons of control group completed the study. CSHA- CFS of the experimental group were more likely to improve (OR=9.50, 95% CI= ) and less likely to deteriorate (OR=0.04, 95% CI= ), one year after intervention respectively. Within experimental group, FFI improvement percentage was greater than deterioration percentage (46.4% vs. 2.4%, p<0.0001), five meters walk speed increased from 1.0±0.2 m/sec to 1.2±0.2 m/ sec (p<0.0001), grip strength increased from 22.3±7.1kg to 24.8±6.7 kg (p<0.0001), SPPB increased from 10.0±1.6 to 11.0±1.2 (p<0.0001), TUG decreased from 10.9±2.9 second to 9.5±2.1 second (p<0.0001). However, there was no statistical difference of composite endpoint including hospitalization, emergency department visit and falls between two groups though the incidence of composite endpoint was higher in the control group. Conclusions: Case management after functional evaluation in the annual elderly health examination may improve function for adults above 70 years old. We suggests to add FFI and TUG in the annual elderly examination. Acknowledgements: The study was supported by Department of Health, Taipei City Government. A10 FALLS PREVENTION STRATEGY IN INDONESIAN ELDERLY POPULATION Vera, Wiharja A Maranatha Christian University, Bandung Indonesia Objectives: Falls in elderly is associated with decreased functional status and increased cost of medical treatment. Thus, in developing countries with restricted health budget, falls in elderly should be prevented. Methods: We conducted a survey study on falls risk screening among 125 Indonesian elderly. The aim of the study was to identify which falls risks commonly found in Indonesian elderly population to find appropriate falls prevention strategy. Results: 82,3% sample were still independent (IADL Lawton- Brody), but only 52% sample did routine physical exercise. 30,4% sample had falls during the last three months. 19,2% sample had orthostatic hypotension. 27,2% sample had weakness of lower extremities. 18,4% sample had instability. 59,2% spent more than 10 seconds for timed up and go test. Thus routine physical exercise to strengthen lower extremities should be endorsed to prevent falls in this population. Conclusions: In Indonesian elderly population with limited resources and funds, falls prevention strategy include simple falls risk screening in the primary care setting and routine exercise of lower extremities. Further study is required to quantify the effect of this strategy in preventing falls among Indonesian elderly population. Keywords: falls, prevention, elderly 11

Department of Orthopedics, National Taiwan University Hospital, Taipei, Taiwan Objectives: We aim to determine the feasibility and effectiveness of World Health Organization Fracture Risk Assessment Tool (WHO FRAX) based osteoporosis screening and referral program among geriatric health examination (GHE) recipients. Methods: Our target populations were 3000 GHE recipients at the National Taiwan University Hospital (NTUH) in Trained coordinators explained the studies and distributed informed consents as well as FRAX based questionnaires at the stage I of GHE. Those who were willing to participate into the study returned the consent forms and questionnaires at stage II of the GHE. Answers were entered by study coordinators into the Taiwanese FRAX calculator to obtain 10- year predicted fracture risks for participants on sites. Those who scored 20% for major osteoporotic fracture or 3% for hip fracture were considered as high risks. They were invited to be referred for osteoporosis assessments and managements by study coordinators or stage II GHE geriatricians. Exploratory data analysis was applied to determine the feasibility and effectiveness of proposed program. Results: Overall, 1319 (44%) of GHE recipients completed the study questionnaires, and 913 (69%) were screened as high risk. However, only 182 (20%) were successfully referred for further assessments, 65 (40%) received clinical diagnosis of osteoporosis (defined as having fragility fracture or bone mineral density (BMD) T score -2.5), 38 (58%) were prescribed anti-osteoporosis medications. Roughly 40% (n=16) of medications were reimbursed by National Health Insurance (NHI). Conclusion: Most GHE recipients were reluctant to receive FRAX based screening when they perceived the program is a clinical study. Even screened as high risks, only minority were willing to be referred for further managements because of limited reimbursements on BMD and anti-osteoporosis medications from NHI. Both patient campaigns and policy reforms were urgently needed to improve osteoporosis cares in Taiwan. A12 THE PILOT STUDY OF TAILORED-MADE LOW INCOME PEOPLE MANAGEMENT Ming-Yu Chen, Pi-Shan Hsn, Cheng-bo Hou, Hung- Chang Hung, Meng-Chih Lee Taiwan, R.O.C. Abstract Introduction: The Number of low income status people in Taiwan is increasing. Most of them don t go to the hospital due to poor condition when they are sick. Also they don t have good health habit that die to health cognition is not enough. This is a vicious cycle. The aim of the pilot study tries to set the integrated care management system for them. Purpose / Methods: Taichung Hospital started the pilot study since 2012.The steps are as follows: 1. Multidisciplinary team: Doctors, Case managers, Social workers, Nutritionists all together to set the care plan. 2. Set up a integrated connecting system. We set a system to connect the social Affairs, Health Bureau, National health Insurance and the hospital. 3. Case management specialists follow up the conditions in the community. Results: Initial Results are as follows: Total cases are 247 in 77 household. Their health behavior rate increase 46.1%;complete preventive medical rate from 11% to 51%;Their compliance for chronic disease increase to 147%;Durg compliance from 21% to 128%;HbA1C Execution rate from 31% to 91.4%;Hypertention control rate from 18% to 68.9%;Satisfation 93.4%;Their Quality for Life (SF-12) increase has statistics significance. Conclusions: Total care for the low social-economic people is very important. Integrated system between National Health Insurance, Social affairs, Health Bureau and local Leadership is also the key factor. Risk factor survey, chronic disease management, home visit can see the improvement. Comments: Integrated team care management combined with health management cloud is very important. It can improve these people from Bio-psy-social-spirit-family aspects. 12

Health Sciences University of Mongolia Background: Mongolia is one of the most influenced countries by non-communicable diseases among developing countries. Non-communicable diseases, including cardiovascular diseases, cancers, respiratory diseases, diabetes, and injuries have become the major causes of morbidity and mortality in Mongolia. Goal: To assess the prevention of non-communicable diseases from a health professional s standpoint and to aid it s further development. Methods: We conducted 17 focus group discussions (FGD) from March to April 2013 in primary and secondary health care settings, of which six in urban and eleven in rural areas. We identified themes concerning the current situation of NCD related health services from the perspective of health professionals, and insights into institutional and professional experiences related to management, implementation and coordination of the newly implemented MCA-Mongolia NCD prevention and control project. Results: As a result of the health project, NCD related knowledge and skills of health professionals have improved through progressive training, and development of guidelines and manuals. During the project, availability of equipment supplies has improved. Accordingly, medical equipment and laboratory reagents needed for early detection of NCDs were provided to primary and secondary health care settings, despite some challenges in the implementation of the project. As result of the project implementation, increased public awareness on NCDs, and attitude change were considered as the biggest changes. Conclusion: The health project was considered as successful to provide knowledge on the best practice in NCD prevention. Keywords: health project, non-communicable diseases, preventive activity, focus group discussion A14 FACTORS ASSOCIATED WITH THE UPTAKE OF INFLUENZA VACCINATION AMONG ELDERLY IN TAIWAN - ANALYSIS OF THE 2009 NATIONAL HEALTH INTERVIEW SURVEY Shih-Chun Lin, Kun-Wei Tsai, Malcolm Koo Dalin Tzu Chi Hospital, Buddhist Tzu Chi Medical Foundation, Chiayi, Taiwan Objective: To investigate the factors associated with the uptake of influenza vaccination among elderly in Taiwan Methods: We conducted a secondary analysis of the data from the Taiwan 2009 National Health Interview Survey. Individuals aged 65 years and above without cognitive impairment were included in the analyses. Logistic regression analyses were conducted using the variable whether the respondent had received influenza vaccination in the past year as the independent variable. Results: Of the 2603 respondents, 1405 (54.0%) had received influenza vaccination in the past year. Results from the multivariate logistic regression analysis indicated 10 factors were significantly associated with the uptake of influenza vaccination. The factors included age of 75 years or above (odds ratio [OR]=1.71, p<0.001), living with a spouse (OR=1.34, p<0.001), had diagnosed with at least one chronic disease (OR=1.34, p=0.001), had made outpatient visits in the past month (OR=1.39, p<0.001), had done exercise in the past two weeks (OR=1.33, p=0.001), not a current smoker (OR=1.69, p<0.001), living in rural townships (OR=1.42, p<0.001), no religious beliefs (OR=1.29, p=0.023), awareness of the free annual senior health examination service (OR=1.55, p<0.001), and had participated in community activities (OR=1.42, p=0.001). Conclusion: This secondary data analysis of a representative sample of Taiwan population demonstrated that a number of factors were significantly and independently associated with the update of influenza vaccination in elderly Taiwanese. Our findings could be served as a basis for planning public health intervention to target resources to those groups with a low uptake of influenza vaccination. 13
