3 3 Dawn Farm Ypsilanti 6633 Stony Creek Road Ypsilanti, MI Kana Enomoto, Deputy Assistant Secretary Substance Abuse and Mental Health Services Administration 5600 Fishes Lance Rockville, MD October 1, 2016 Dear Ms. Enomoto: Attached is a grant proposal to be submitted for the Substance Abuse and Mental Health Services Administrations, which I have prepared on behalf of Dawn Farm. The proposal details a need for an additional dorm building in order to provide more residential treatments for drug abuse users. This will enable Dawn Farm to take in even more patients and recovery rates will be faster. Statistics and studies have shown that long-term recovery methods are more effective than the usual short-term rehabilitation methods that most rehab centers practice. Many rehab centers usually provide a 30-day treatment process, which has shown to not decrease the rate of relapse for drug abuse patients. Long-term recovery methods of up to 3 months provide a more effective treatment for many patients and most choose not to use drugs after the treatment. Dawn Farm provides this recovery method by teaching patients how to farm, cook, and most of all, cooperate with one another in order to build a harmonious community. The proposed dorm building would be established adjacent to the main building of Dawn Farm and would be approximately 3000 square feet. Included in this building are two bathrooms, a kitchen, and 15 rooms. This allows for community building, as patients will cook, clean, and sleep under the same roof together. This plan fits well into the goals of the Substance Abuse and Mental Health Services Administrations. It helps change the lives of many drug abuse users and helps them find a place in a community where they will feel comfortable in. Based on many research and evidence, long-term rehabilitation methods, especially including farming, will truly help decrease the relapse of drug abuse patients. 4 4 U.S. Addiction Treatment System Rehabilitation centers for drug abuse patients are not always effective in treatment and many drug abuse patients resort to using drugs again after treatment (Fig A). Many addiction treatment specialists with little formal education or training in addiction may still be hired by rehabilitation centers. Out of all 50 states in the U.S., 14 states require only a high school diploma or a GED to become an addiction counselor and 10 states require only an associate s degree. The most shocking of all is that 20 states do not necessitate any sort of degree, certification, or license in order to be an addiction counselor. With such little requirements, many drug rehab centers barely help drug abuse patients or change their lives according to what many rehab centers claim to perform. The National Center on Addiction and Substance Abuse at Columbia University reports, Contrary to the 30-day stint typical of inpatient rehab, people with serious substance abuse disorders commonly require care for months or even years. The short-term fix mentality partiality explains why so many people go back to their old habits (National Center on Addiction and Substance Abuse). This study also yielded statistics, which revealed that only 1 in 10 drug abuse patients receive any form of treatment. The treatments the few patients receive have no evidence based care. In the end, many patients do not receive the necessary care in order to stop abusing the use of drugs.

6 6 Dawn Farm Services At Dawn Farm, drug rehabilitation patients receive a long-term treatment setting. Unlike most rehabilitation centers, Dawn Farm offers residential stays of up to 3-6 months. It provides two different residential settings, both of which are located in Michigan. The first setting is located at a working farm of 64 acres, allowing for a very therapeutic experience that a rural community has to offer (Figure A). The farm plants over 2000 useful plants and shrubs with the help of all the staff and patients (Figure B). The other facility is located in Michigan s downtown, Ann Arbor (Figure C). Offering the same services, the facility at Ann Arbor provides a more city experience where patients find it easier to obtain employment opportunities after treatment. Dawn Farm provides effective treatment because the employed staff members are all very capable with 8 out of 9 clinical staff members completing a master s degree in social work or counseling. Staff members without a master s degree have decades of experience and carries the highest level of addiction counselor certification. At any facility of Dawn Farm, residents receive daily education and recreation activities, group therapy, individual therapy, and case management by skilled professionals. Dawn Farm allows patients to build healthy relationships and practice
7 7 cooperation. Dawn Farm also helps patients discover their place in communities after treatment with a strong aftercare program, ensuring that patients will receive employment and housing upon leaving Dawn Farm. 10 10 Dawn Farm Residential Housing and Living Costs Although the costs of staying at Dawn Farm are among the least expensive in the United States, there is still a daily rate that must be covered by all residents (Fig. D). Dawn Farm strives to decrease their cost rates and maintain lower fees and this is shown through surveys performed by Addiction Center. Well-known inpatient centers often cost up to $20,000 for a 30-day program, not including the medication that goes along with the treatment. At Dawn Farm, patients pay $12,600 for a 30-day residential treatment. Dawn Farm provides many different forms of treatment, but each comes with a price. The longer the treatment the better the outcome as supported by the success stories provided by Dawn Farm s main webpage. However, many patients are lacking employment and come from low-income families so it is extremely hard for many patients to be able to pay the amount Dawn Farm needs in order to treat them properly. There are outpatient treatments, but studies have shown that treatment outcomes improve with longer periods of care and careful monitoring (National Institute on Drug Abuse). Allowing drug abuse patients to become residents at Dawn Farm benefits both the patient and the community. More funding will enable drug abuse patients to discover their role in the community and learn how to work alongside others without the use of drugs.
11 11 Patients without funding can also look for help from insurance companies. Insurance funds can be from companies like Medicaid, Medicare, State-financed health insurance, or Military insurance (Addiction Center). In 2008, the Affordable Care Act and Mental Health Parity and Addiction Equity Act require insurance providers to offer coverage for behavioral health conditions, including mental health and substance abuse treatments. This means that insurance policies must cover substance abuse treatments to the same extent that they cover other medical conditions (Lauren Brande, M.A.). However, insurance companies only generally partially cover inpatient residential treatment (Brande). One of Dawn Farm s most effective treatments is the residential treatment and it is the optimal choice for most drug abuse patients.
12 12 Insufficient Housing The farming facility at Dawn Farm has only 36 beds and the beds are separated by genders. Dawn Farm receives more male patients than females one so the wait for males is usually longer than it is for females. However, there is space for flexibility and at times the female rooms are converted to male rooms. Some patients also apply for Dawn Farm unfunded and it is part of Dawn Farm s policy to also admit patients without any funding for their treatment. Dawn Farm usually dedicates 15% to 20% of their beds to these unfunded patients, but the demand is frequently high so the wait becomes even longer. Patients may need to wait up to 1-3 months before they can receive housing at Dawn Farm. The wait list has been increasing over the years and whenever Dawn Farm is able to, it admits more residences in, however, it is becoming harder for patients without funds to enroll. The longer the wait, the higher the possibility for patients to relapse back to their old drug abuse habits because they are not obtaining proper treatment for their problems. The housing in the downtown location has even less rooms. With a total of 13 rooms, they do not provide a lot of housing for many patients in need of it. Additional rooms and beds would greatly improve the long wait list and provide more help and care for patients.
13 13 Dawn Farm has 64 acres of land and instead of adding on to their housing facility, another dorm building could be built to include more patients without hindering the land used for farming (Fig E). A camp-style lodge would be optimal in terms of price and space. It could contain 15 rooms, allowing up to 30 people to live there. A camp-style lodge would still provide the same feeling of community and nature. 14 14 Benefits of Residential Treatment in Dawn Farm Dawn Farm provides a unique setting for treatment of drug abuse clients. Dawn Farm utilizes a method quite different from other drug rehab centers. It provides a farm setting where drug abuse clients can discover a sense of bonding and community through the cooperated work of farming, cooking, and other housekeeping chores. This residential style treatment improves the relapse rate of drug abuse and changes many clients lives. Dawn Farm not only provides detoxification and treatment for drug abuse, but also aids clients in the search for employment and housing upon leaving Dawn Farm. It ensures that patients receive the help they truly need and aids in their search for the place they belong to in a much bigger community. Dawn Farm is a reliable center for drug abuse patients and is high in demand as the wait to apply may take up to 3 months. Due to their lack of housing and rooms for more patients, Dawn Farm has to accommodate by placing patients in a wait list. Patients in need of attention and care may receive treatment later than is optimal. Dawn Farm has also decreased much of its fees, however, the costs of housing and other amenities create a struggle for more patients to choose, or even be able to, stay at Dawn Farm where their recovery is almost guaranteed.

19 19 Summary Drug abuse is a serious issue and although there are many rehabilitation centers in the U.S., statistics have shown that many patients relapse into using drugs again even after treatment. Rehabilitation centers strive to change the lives of drug rehab patients and hope that they will stop using drugs after treatment. However, due to the lenient requirements necessary in order to become a drug abuse counselor, most treatments fail to effectively aid these patients into leading better lives. Dawn Farm is a very special rehabilitation center that truly focuses on helping their patients recover and live a fruitful life. Located in a natural setting surrounded by farmland and forestry, Dawn Farm provides the optimal environment necessary to treat drug abuse patients. They also provide lessons on farming and the plant harvests are turned into food for meals by the patients. Dawn Farm provides a community where they are able to form bonds and cooperate with one another. More patients should be admitted to a center like Dawn Farm because it has shown to change many drug abusers lives. Dawn Farm will truly benefit from another dorm building. Costs and funding will continue to be a problem, but with such a hardworking community; more and more people will notice and help out.
