14 Wittier Street Healt Center Wittier Street Healt Center is a private, nonprofit, independently licensed community ealt center located in Lower Roxbury. Wittier is accredited by te Joint Commission and recognized by te NCQA as a Level 3 (igest level) Patient Centered Medical Home. We offer 40 ealt care programs and wellness services designed to meet te primary ealt care, beavioral ealt, and social needs of our community. Our mission is to provide ig quality, reliable and accessible primary ealt care and support services to promote wellness and to eliminate ealt and social disparities. Our vision is to serve as te premier leader in urban ealt care to diverse populations. We serve 30,000 clients in Roxbury and te surrounding neigboroods of Jamaica Plain, Dorcester, Mattapan, Roslindale, Mission Hill and Greater Boston. We ave an etnically and racially diverse patient population, wit 42% identifying as black or African-American, 40% as Hispanic/Latino, 6% as wite, and 12% as oter. Over a tird of our patients (35%) are uninsured, despite recent ealt care policy reforms. 81% of our patients live in public ousing and 53% live below 200% of te poverty line. Approximately 35% of Wittier s patients are uninsured. We serve patients from diverse backgrounds including refugees and new immigrants. Te individuals and families we serve face a disproportionate risk of major diseases and ealt complications, including diabetes, cancer, cardiovascular disease, astma, obesity, depression, and HIV/AIDS. Tey also bear te burden of neigborood violence and limited access to fres foods, safe green spaces, employment and educational opportunities and affordable ousing. 14
15 In response, our primary care delivery model is patient-centered, teambased, compreensive, continuous, and community-responsive. We strive to offer every service tat a patient migt need for teir ealt and social needs, wic are intertwined. For example, a patient wit diabetes and eye complications, wo cannot drive to er doctor s appointment, will be transported via our patient suttle van to te ealt center, were se can see er provider on te tird floor, speak wit a nutritionist on te fourt floor, and be referred to te optalmologist on te first floor. Troug our focus on combining ealt wit wellness and social supports, we are elping to close te gap in ealt care delivery, and to reduce te ig ospitalization and mortality rates in our community. Togeter wit our supporters and our partners in te community, we are moving closer and closer to acieving equity in ealt care.
