4 THE CANADIAN SOCIETY OF BREAST IMAGING ABOUT The idea of forming a group dedicated to breast imaging in Canada has been going on for some time. Professionals interested in breast imaging have been engaged in the European Society of Breast Imaging as well as the Society of Breast Imaging in the United States. Recognizing that there was a need for a place to discuss uniquely Canadian issues related to breast imaging, the CSBI was founded in October 2017 with the support of the CAR. Founding members include senior leading breast imagers from across the country including: Drs. Jean Seely (ON), Nancy Wadden (NL), Shiela Appavoo (AB), Anabel Scaranelo (ON), Yves Loisel (QC) and Paula Gordon (BC). The CSBI is a home for Canadian breast imaging professionals and patients to discuss best practices, pressures on the system and to celebrate successes. The CSBI will also be a focused vehicle to bring breast imaging stories from Canada to the rest of the world and vice versa. PURPOSE To establish a society for the improvement and dissemination of breast imaging To promote high quality breast imaging that is patient centered To improve the quality of medical education in the practice of breast imaging To foster research in all aspects of breast imaging To provide a medium for the exchange of ideas among clinicians, researchers and healthcare professionals involved with breast imaging To provide meetings for presentation and discussion of papers and the dissemination of knowledge in the area of breast imaging To establish a channel for publication of scientific reports in the field of breast imaging; and To respond to public / media inquiries concerning breast imaging. 7 PROGRAMME INFORMATION Needs Assessment The inaugural conference of the CSBI is intended to bring together practitioners from a variety of health fields to educate and collaborate on current breast imaging practices in Canada. CSBI 2018 will explore how radiologists and other imaging professionals can work to save life years and improve the quality of life through the early detection of breast cancer, make use of cutting-edge technologies to improve imaging quality, and better engage patients in the continuum of breast imaging care. Target Audience This conference is designed for general and subspecialty radiologists involved in breast imaging. It is also appropriate for health practitioners from other disciplines (family medicine, medical radiation technologists), and patients who are interested in working collaboratively to improve breast imaging care. Overall Learning Objectives At the end of the conference, participants should be able to: 1. Share the message and mandate of the Canadian Society of Breast Imaging with diverse stakeholders [CanMEDS roles: Communicator, Collaborator, Health Advocate, Leader, Professional] 2. Provide an overview of leading technology in the field of breast imaging [CanMEDS roles: Communicator, Health Advocate, Medical Expert] 3. Discuss current Canadian breast cancer screening practices and protocols [CanMEDS roles: Communicator, Medical Expert, Collaborator, Health Advocate, Professional] ACCREDITATION Canadian Radiologists This event is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance of Certification program of the Royal College of Physicians and Surgeons of Canada (RCPSC) and approved by the Canadian Association of Radiologists. The CSBI 2018 programme is approved for a maximum of 5 hours (credits are automatically calculated). Participants in the Tomosynthesis or Breast MRI Wokshops may claim a maximum of 1.5 hours (credits are automatically calculated), under Section 3, Simulation-based Activities (SIM), as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada, and approved by, the Canadian Association of Radiologists. Participants in the Radioactive Seeds Workshop may claim a maximum of 1.5 hours (credits are automatically calculated), under Section 3, Self-Assessment Program (SAP), as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada, and approved by, the Canadian Association of Radiologists. 8 American Radiologists Through an agreement between the Royal College of Physicians of Surgeons of Canada and the American Medical Association (AMA), physicians may convert Royal College MOC credits to AMA PRA Category 1 Credits TM. Information on the process to convert Royal College MOC credits to AMA credits can be found at Canadian Technologists The CSBI 2018 Conference has also been accredited by the Canadian Association of Medical Radiation Technologists (CAMRT) for Category A credits. Participants can document their learning in the RCPSC MAINPORT at PRESENTATIONS Unless otherwise indicated under individual sessions, each presentation is scheduled for 30 minutes. Speakers are allotted 25 minutes to present and 5 minutes to answer questions from the audience. Every session has been designed as an educational offering to advance practitioners professional development and the profession. PRESENTATION SUMMARY: The Tomosynthesis Mammographic Imaging Screening Trial, a National Cancer Institute-funded clinical trial under the auspices of the ECOG-ACRIN research base, which will enroll over 165,000 women in 150 sites in the US and Canada and randomize women to either tomosynthesis or digital mammography for 3-5 rounds of breast cancer screening. The goal of the study is to determine whether tomosynthesis reduces advanced breast cancers in the population being screened compared with digital mammography. Most screening programs start at 50, and screen biennially. Why? How does the Canadian Task Force on Preventative Health Care influence policy, and is it what is best for Canadian women?" 13 MAP Image Evaluation Dr. Nancy Wadden, MD, FRCPC 10:45 11:15 Salon Jarry/Joyce This presentation is a 20-minute lecture, followed by an interactive 10-minute question and answer session. PRESENTATION SUMMARY: This presentation will focus on the criteria for optimal image evaluation for CAR-MAP. The exact criteria used by clinical image reviewers in determining accreditation worthy images will be discussed and examples of optimal and suboptimal images will be presented. Each criterion will be explained and the logic for the choice of those criteria will be presented. The challenges of optimal image selection will also be examined. LEARNING OBJECTIVES At the end of this session, the participant will be able to: 1. Identify the criteria for optimal image evaluation for CAR-MAP 2. Recognize/Demonstrate the requirements for passing the CAR-MAP image evaluation 3. Consider the challenges in choosing images for evaluation Positioning Louise Miller, R.T. (R)(M), CRT (M), FSBI, FNCBC 11:15 11:45 Salon Jarry/Joyce PRESENTATION SUMMARY: Mammography Positioning is one few areas of medical imaging that does not require standardized positioning techniques. This, unfortunately, has caused much variation in image quality and subsequent evaluation throughout the US and Canada. This lecture will discuss the importance and efficacy of standardized positioning techniques that are consistent, reproducible, efficient, proficient and ergonomically sound. LEARNING OBJECTIVES At the end of this session, the participant will be able to: 1. Explain Importance of consistency and reproducibility 2. Show examples of pre-post training images 3. Apply the BIRADS lexicon for ultrasound lesion based on case-based training INTERACTIVE WORKSHOPS Tomosynthesis Workshop Moderator: Dr. Yves Loisel 13:30 15:00 Salon Kafka/Lamartine PRESENTATION SUMMARY: This simulation workshop is designed to provide individual instruction in tomosynthesis interpretation while reviewing clinical tomosynthesis data sets with accompanying standard mammography. This workshop will comprise a 30-minute didactic lecture about the theory and principles of digital breast tomosynthesis (DBT), including an overview of the advantages and challenges of the modality, and the current research climate. The lecture will be followed by 45 minutes of hands-on case review, interpretation and reporting at a DBT workstation. LEARNING OBJECTIVES At the end of the workshop, participants will be able to: 1. Identify the advantages of digital breast tomosynthesis compared with other breast imaging modalities 2. Read and interpret digital breast tomosynthesis images 3. Differentiate and define screening principles 4. Use a tomosynthesis workstation to interpret images 5. 15 To Seed or Not to Seed Moderator: Dr. Anat Kornecki 13:30 15:00 Salon Jarry/Joyce PRESENTATION SUMMARY: This interactive workshop is designed to teach the technique of breast radioactive seed localization. A 60-minute didactic session will be followed by 30 minutes of hands-on practice loading up radioactive seeds, with plenty of time for questions and discussion. Radiologists, surgeons, and technologists can also make additional arrangements after the conference to observe the technique by visiting St. Joseph s Health Centre (London, ON). TARGET AUDIENCE: radiologists, mammography and breast ultrasound technologists LEARNING OBJECTIVES At the end of the workshop, participants will be able to: 1. Recognize the advantages of a radioactive seed localized surgical procedure 2. Describe the basic steps required to begin the implementation of breast radioactive seed localization 3. Appropriate use of the BIRADS lexicon and generating a breast MRI report in a setting of staging breast cancer. 2. Case-based review the role of breast MRI in management of the neo-adjuvant setting. 3. Case-based review of breast MRI guided biopsy using commercially available biopsy software. TARGET AUDIENCE: This workshop is designed for practicing radiologists with experience in day to day breast radiology. Prior experience in breast MRI is not a pre-requisite. Increased breast density lowers the sensitivity of mammography for the detection of breast cancer and is also associated with an increased risk of breast cancer. Controversy exists regarding whether or not increased breast density alone (without other risk factors) warrants supplemental screening with ultrasound and/or MRI. LEARNING OBJECTIVES At the end of this session, the participant will be able to: 1. Describe how breast density is assessed 2. Summarize the evidence that supports increased breast density as a risk factor for breast cancer 3. 17 SPEAKER BIOS MANISHA BAHL, MD, MPH Harvard Medical School, Boston, Massachusetts, USA Dr. Bahl is a breast imaging radiologist at MGH and the Director of the MGH Breast Imaging Fellowship Program. She received her medical degree from UC San Francisco and completed residency and fellowship at Duke University Medical Center. She is a current recipient of the RSNA Research Scholar Grant and the ESSCO-MGH Breast Cancer Research Grant to investigate the application of machine learning algorithms to risk-stratify patients with ductal carcinoma in situ. PAULA GORDON, OBC, MD, FRCPC, FSBI University of British Columbia Dr. Paula Gordon is a Clinical Professor in the Department of Radiology at UBC. She is a fellow of the SBI, and on their Board of Directors. She is Medical Director of the Sadie Diamond Breast Program at BC Women's Hospital. Some of important contributions to the breast imaging literature are: her 1995 paper in Cancer, which was the first to show that ultrasound can find cancers missed on mammography in dense breasts; and her 2015 paper in AJR, showing that 98% of breast radiologists have annual mammograms starting at age 40, and recommend the same to their patients, friends and family. She also volunteers as a Director of the Board of the Canucks for Kids Fund. She has received a Killam Teaching Prize from the University of British Columbia, a Queen Elizabeth Diamond Jubilee Medal, and was invested in the Order of British Columbia. In 2014 she was named one of Canada's 100 Most Powerful Women by the Women's Executive Network. And in 2016, she was made an honorary member of the UBC Medical Alumni. ANAT KORNECKI, MD Dr. Anat Kornecki is an Associate Professor of Medical Imaging, Head of the Division of Breast Imaging at the Western University and the Regional Breast Imaging Lead at the South West Regional Cancer Program. Her main area of research is breast imaging. Dr. Kornecki had supervised several educational initiatives including the Breast Imaging Fellowship Programme and resident training. She is involved in organising CME events and workshops in Breast Imaging and presented in several forums, nationally and internationally. April 25, 2018 Page 17
18 SUPRIYA KULKARNI, MD, FRCPC Dr.Supriya Kulkarni is a staff radiologist at the Joint Department of Medical Imaging (JDMI) at UHN, Mount Sinai Hospital and the New Women's college hospital, Toronto, Ontario. She is an Assistant Professor in the Department of Medical Imaging, University of Toronto. She has over 16years of experience in the subspecialty practice in Breast Imaging at JDMI. Dr. Kulkarni is responsible for all educational initiatives in the division of breast imaging (JDMI) including the Breast Imaging Fellowship Programme (University of Toronto) and resident training. She is involved in organising CME events & workshops in Breast Imaging and is invited as a guest speaker in several forums nationally & internationally. She is a recipient of several teaching awards including the prestigious Wightman-Berris Academy individual postgraduate teaching award ( ). She is also the recipient of the 2017 Award for Outstanding cancer education, (global) Princess Margaret Cancer Centre and is on the steering committee of the Gattuso Rapid Diagnostic Clinic at the Princess Margaret Cancer Centre. She has special interest in all aspects of Breast Imaging and particularly in Digital Breast Tomosynthesis; Breast MRI & MRI guided intervention. She is on the advisory committee of Cancer care Ontario for the Provincial High-risk Breast MRI screening (OBSP) and is the QMP facility lead, Chair of the Working Group for OBSP Screening of Transgender People and member of the Overarching Policy Steering Committee for inclusion of Trans People in Organized Cancer Screening. 19 TRISH MACNEILL Dense Breasts Canada Trisha found out she had breast cancer by fluke. She was getting a bone scan for a work injury and it revealed soft tissue uptake in her right breast. Due to her breast density, both mammogram and ultrasound failed to find her breast cancer. Fortunately, diagnostic MRIs are an option in her province when first degree relatives have breast cancer and it revealed her cancer. She has a Masters in Public Administration concentrated in economic development and has extensive experience working with all levels of government and their ministers' offices. LOUISE MILLER, R.T. (R)(M), CRT (M), FSBI, FNCBC Director of Education - Mammography Educators, San Diego, CA Louise Miller is Founder of the Mammography Practicum at the School of Medicine, University of California, San Diego. Her courses have trained for over 40,000 radiologic technologists in the US, Canada and over 20 foreign countries. Louise has provided consultation and invited lectureships for noted institutions such as Harvard Medical School, the Mayo Clinic, UCLA and Stanford University and has worked extensively with renowned breast imaging specialists including Drs. Làszló Tabar, Edward Sickles and Daniel Kopans. She has received several prestigious national awards that recognize her outstanding dedication, commitment, achievement and overall impact on breast cancer detection worldwide. April 25, 2018 Page 19
20 Etta Pisano, MD American College of Radiology Dr. Etta Pisano is Professor in Residence of Radiology at Harvard Medical School and Chief Science Officer for the Center for Research and Innovation of The American College of Radiology. Previously, she served at the Medical University of South Carolina as Dean of the College of Medicine and at the University of North Carolina as Director of the UNC Biomedical Research Imaging Center, and Director of the N.C. Translational and Clinical Sciences Institute. ANABEL SCARANELO, MD, PhD, FSBI University of Toronto, Toronto, ON Dr. Scaranelo has an appointment as Assistant Professor of Radiology in the Medical Imaging Department at the University of Toronto. She is a board-certified staff radiologist in the Division of Breast Imaging at the Joint Department of Medical Imaging and works at Sinai Health System, Women's College Hospital and Princess Margaret Cancer Centre (UHN) in Toronto. She has authored numerous articles, books, book chapters, clinical guidelines and has lectured on topics in breast imaging. Dr. Scaranelo is member of European Society of Breast Imaging since its inception, a founding member of the Canadian Society of Breast Imaging and a Fellow of the Society of Breast Imaging (FSBI) in the USA. Her clinical interests include ultrasound-guided interventional procedures, digital mammography, and dynamic-contrast enhanced breast MRI. Her research interests focus on breast computeraided detection systems, non-contrast enhanced MRI techniques, and quality metrics in breast imaging JEAN SEELY, MDCM, FRCPC University of Ottawa Dr Jean Seely is Head of the Breast Imaging Section at the Ottawa Hospital. She is Associate Professor of Medicine at the University of Ottawa. She is Regional Breast Imaging Lead for the Ontario Breast Screening Program in the Champlain region. Jean is an Executive Board member of the Canadian Association of Radiologists Breast Imaging Working group and the CAR Mammography Accreditation Program. She is committed to reduce the mortality and morbidity of breast cancer and providing high quality breast imaging nationally and internationally. She is Chair of the newly formed Canadian Society of Breast Imaging. April 25, 2018 Page 20
21 ISABELLE TROP, MD MPH After an undergraduate degree in Molecular Biology, Isabelle Trop pursued her medical studies at McGill University in Montreal. She then completed a residency in diagnostic radiology at the university of Montreal and followed with a Women's Imaging fellowship at the Beth Israel Deaconess Medical Centre in Boston, under the supervision of Janet Baum and Sughra Raza. Through this fellowship training and her first years of practice back in Montreal, she completed a Masters in Public Health at Harvard. Back in Montreal, her professional practice became breast-only in 2006 and Dr Trop was named head of Breast Imaging at the Centre Hospitalier de l'université de Montréal, a position she still holds. She earned the title of Junior Clinical Scientist from the Quebec Health Research Foundation (FRQS) from 2011 to 2015, after which her professional interests shifted towards teaching. For the past 3 years, she was Assistant director of the radiology residency program at the university of Montreal and was named Program Director in July She was in charge of the Breast Imaging fellowship program of the University of Montreal until this nomination. Dr Trop serves on several committees pertaining to breast care nationally and she is a regular consultant to governmental scientific advisors. Last but not least, she accepted in the fall of 2016 to serve as an Associate Editor, Breast imaging, for the journal Radiology. NANCY WADDEN, MD, FRCPC Eastern Health, Newfoundland and Labrador Nancy Wadden is the Medical Director of the Breast Screening Program for Newfoundland and Labrador and Clinical Associate Professor in the Faculty of Medicine at Memorial University of Newfoundland. Dr. Wadden is the chair of the Mammography Accreditation Program Staff Working Group and a member of the Standards in Breast Imaging in Canada for the CAR. She continues to be involved with The Canadian Society of Breast Imaging, The Canadian Breast Cancer Screening Initiative and The Working Group of the Canadian Mammography Quality Standards.
