1 REPUBLIC OF SOUTH SUDAN MINISTRY OF HEALTH Weekly Update n Ebla Virus Disease (EVD) Preparedness fr Suth Sudan Update # 5 Date: 1 Octber 2018
2 1. Highlights WHO has elevated the risk assessment f EVD spread in the regin frm High t very High. This implies that Suth Sudan and ther three neighbring cuntries (Uganda, Rwanda and Burundi) have t develp and test peratinal readiness fr a ptential EVD respnse. Fur additinal screening sites i.e. Nimule River Prt and in Yei River state, Yei Airprt, Kaya, and SSRC ffices have started entry screening f travelers. One suspected EVD death alert frm New Site in Juba was investigated and it tested negative n PCR fr Ebla, Marburg, Rift Valley Fever, Crimean Cng Hemrrhagic Fever and Ssuga viruses. 2. Ebla Situatin update frm Nrth Kivu f Demcratic Republic f Cng 2.1. Latest updates The EVD utbreak is still nging, and nine health znes in the tw prvinces are affected: Nrth- Kivu Prvince: Beni, Butemb, Oicha, Mabalak, Kalunguta and Musienene. Ituri Prvince: Mandima, Kmanda and Tchmia. Tw cases have been cnfirmed in Tchmia, almst 200 km (125 miles) away frm the nearest ther knwn cases in the Ituri prvince. Tchimia is nearer t Uganda brder alng lake Albert. Three cntacts f the abve- mentined cases have been identified in Sebag Landing site, in Kikuube District n the Ugandan side; they are Cnglese Refugees wh went t visit ne f the cases while she was still alive. As f 30 th September 2018, A ttal f 159 cases f EVD have been reprted, f which 127 are cnfirmed and 32 are prbable. Of the 127 cnfirmed, 72 died, and 45 were cured. Cumulative deaths are 104. Over 5306 cntacts have been line listed since the start f the Outbreak, f which 75-97% are being fllwed up daily in the past week. Since the beginning f the utbreak n 1 st August, 2018, 12,940 peple have been vaccinated, including 3,547 Healthcare wrkers. 3. Public Health Preparedness and Readiness 3.1. Crdinatin Tw natinal taskfrce meetings chaired by the Hn. Minister f Health and c- chaired by the Incident Manager were held n 25 th and 27 th September 2018 at the PHEOC in Juba. The majr themes f discussin included activating the technical wrking grups n safe and dignified burials and Vaccines, therapeutics, and research wrking grups. The partner mapping alng the different thematic areas has been fast- tracked fr better crdinatin and implementatin f the EVD preparedness plan. All the thematic pillars including Case Management and Infectin Preventin and Cntrl, Epidemilgy and Surveillance, brder pints and prts f entry, Labratry, Risk Cmmunicatin
3 and Cmmunity Engagement and Persnnel Safety and Security, held their weekly meetings n the scheduled days Resurce mbilizatin On 25 September 2018, the WHO and Health Cluster briefed the Inter Cluster Wrking Grup (ICWG) n the prgress f the Ebla preparedness activities and the slicited the supprt f the cmmittee twards facilitating humanitarian access, lgistical supprt fr Ebla preparedness, resurce mbilizatin, and supprt fr rapid deplyment f investigatin teams and transprtatin f suspect case specimens. The budget required fr implementatin f the EVD preparedness peratinal plan ttals t USD 3,665,387. A Dnr pledge meeting t slicit fr mre resurces is scheduled fr 1 Octber During the week, WHO wrked with the ther UN agencies n a jint United Natins prpsal t supprt Ebla preparedness activities fr the next fur mnths. The prpsal fcuses n six pririty areas including prt health screening; risk cmmunicatin; surveillance and labratry; case management and infectin preventin and cntrl; vaccines, therapeutics, and research; and crdinatin and leadership. 3.3 Surveillance and Labratry On 26 Sep 2018, the Public Health Emergency Operatins Center (PHEOC) received a call reprting a suspect Ebla death frm New site, Nrthern Bari payam, in Juba. The rapid respnse team was quickly dispatched t cnduct an epidemilgical investigatin and btained an ral swab and cardiac bld fr labratry testing. The deceased was a 25- year- ld Ethipian businessman. His illness started n 24th September 2018 with malaria like symptms that included fever, headache, back pain, and flu- like symptms including cugh. He received treatment fr malaria and typhid frm a private clinic n the same day. Having imprved n the treatment he received, n 26 September 2018, he gt up well and pened his shp. Hwever, sn thereafter, he started vmiting bld, cllapsed and died as he was being rushed t a nearby clinic. The samples sent t the Uganda Virus Research Institute (UVRI) n 27th September 2018 and the labratry results released n 28th September 2018 shwed that the samples were negative fr Ebla, Marburg, CCHF, RVF and Ssuga viruses by PCR. 3.4 Prt Health and Screening Currently entry traveler screening fr Ebla expsure and symptms is nging at eight f the 39 mapped pints f entry. These include: Juba Internatinal Airprt, Nimule grund crssing and River Prt, Yambi Airprt, Sakure, Gangura brder areas, Yei Airstrip, and Wau Internatinal Airprt. Fur new entry pints, Nimule River The Prt health staff facilitating the screening f travellers at JIA
4 Prt (supprted by SCI and ARC) and in Yei River state, Yei Airprt, Kaya, and SSRC ffices screening sites (supprted by IOM) were fficially pened. WHO and Save the Children riented the immigratin, custms and brder plice fficers n EVD n Mnday 24th September during their general mrning parade. Wrld Visin Suth Sudan deplyed a team t set up screening pints in Ez and Nyaka. CUAMM is planning t setup a screening site at Maridi Airstrip. The electrnic EWARS platfrm is being used t capture, aggregate, and analyze pints f entry screening data with the table belw, summarizing the number f travelers screened in week 38 at the active pints f entry. WHO and WVI mentred health wrkers in 10 health care facilities in Yambi n infectin preventin and cntrl fr Ebla preventin. Pint f entry training fr MH and partners is scheduled fr 3 rd Octber Risk cmmunicatin, cmmunity engagement and scial mbilizatin The risk cmmunicatin, cmmunity engagement and scial mbilizatin activities cntinue t be implemented t empwer and enhance public awareness n Ebla preventin. Radi talk shws cntinue t be aired with supprt frm UNICEF at the natinal and sub- natinal level in English, Arabic, Zande and ther lcal dialects. Radi jingles with messages n Ebla are running n 13 radi statins in English and 3 lcal languages.
5 Ebla preventin IEC materials have been develped in English and have been translated frm English t Arabic. Translatin int ther native languages is planned. The use f the tll- free line is increasing with an average f ver 200 calls received daily seeking fr infrmatin n EVD and als reprting suspected Ebla alert cases. As part f the nging cmmunity engagement and sensitizatin in the high- risk states, WVI supprted the distributin f 469 Ebla brchures in Yambi. Wrld Visin Internatinal (WVI) supprted ne mass crss brder awareness sessin n 26th September 2018 in Nabiapai brder pst, lcated at the Suth Sudan, DR Cng brder. At least 10,086 peple were reached with infrmatin n Ebla preventin in Yambi and Nzara cunty thrugh cmmunity engagement with husehlds, cmmunity health wrkers, and mther- t- mther supprt grups. WHO is als wrking with UNOCHA and ther partners n infrmatin disseminatin strategy t amplify the preparedness activities and enhance public awareness Lgistics and Persnnel deplyment Three Internatinal cnsultants have been deplyed by WHO t supprt the taskfrce t enhance capacities fr infectin preventin and cntrl, surveillance and verall crdinatin f Ebla preparedness activities Vaccines and Therapeutics The vaccines, therapeutics, and research wrking grup has been activated and during the week, the team finalized the Ebla vaccine preparedness plan. The Vaccine and Therapeutics TWG has reached ut t WHO Vaccine team in Geneva t prvide guidance with the develpment f prtcl fr cmpassinate use f the Ebla vaccine as well as the develpment f the necessary in- cuntry capacity (Human and cld chain). WHO Reginal Office fr Africa has initiated an integrated apprach fr the 4 pririty cuntries neighbring DRC (Suth Sudan, Rwanda, Burundi and Uganda) t prepare fr the Ebla vaccine use. This will ensure that all the fur cuntries are at the same level f supprt given the limited resurces Safety and Security The Gvernment is crdinating with the relevant sectrs t imprve access t high- risk lcatins that are currently inaccessible due t insecurity Challenges/Gaps. The critical preparedness gaps currently entail: The absence f a designated islatin facility in Juba r any f the high- risk states is f great cncern due t the current categrizatin f Suth Sudan as Very High Risk The slw pace f Ebla preparedness activities due t limited resurces available in- cuntry Recmmendatins and pririty fllw up actins
6 WHO t engage different actrs (WFP, UNICEF and ther partners) t fast- track the establishment f islatin facilities. WHO t cntinue t advcate fr additinal resurces frm funding agencies t supprt the EVD preparedness and readiness activities.
