14 ASSIGNMENTS: You are responsible for all reading assignments in course texts and handouts in addition to the content of the lecture and laboratory sessions themselves. Since information in each assignment will be used during the class sessions/laboratory for discussion/practice, it is essential that materials be carefully studied prior to class or laboratory. Students are required to be partners/ patients to their fellow classmates to facilitate the learning process. This assignment will occur throughout the entire semester. Students will be required to perform sterilization duties which include: gowns, instruments and infection control. COMPETENCY EVALUATION Technique & Skill Evaluations: Each of the objectives will have a graded attempt skills test. During graded attempts there will be no feedback, only after the evaluation is completed. Each evaluation must be completed on the scheduled date (announced by instructor), and graded according to the given criteria. Weighted grade criteria check sheets are used to evaluate skills. The Competency grade is to be determined as follows: First Attempt: (Total points earned/ Total points available) x 100 = % correct (Remediation will be required if not passed) Second Attempt: (Total points earned/ Total points available) x 89 = % correct (Remediation will be required if not passed) Third Attempt and Beyond: (Total points earned/ Total points available) x 79 = % correct (Remediation will be required until passed or until the required date at the end of the semester) Students who receive a failing grade for competencies will be required to remediate after each attempt. The student must make an appointment with a full-time preclinical instructor for remediation at the instructor s availability. All competencies must be completed and passed in order to progress in the class and the Dental Hygiene program. All competencies must be completed and passed by the student by the last day of regular class (November 30, 2016). If not, the student will be dismissed from the class and the Dental Hygiene program with a grade of F for this specific course. The student will still be required to take the written paper final exam. Any changes to this policy will be made by the lead instructor/ program director. Tests/Competency Evaluations: All make-up tests (lecture and competencies) will be given on the 1 st day the student returns to school when absent barring major illness. A documented excused reason must be given (see program handbook). Five points will be deducted each day a test is taken late, unless it is approved by instructor. The student is responsible for making arrangements with the teacher on the 1 st returned day. If a student is physically tardy by 5 minutes by instructor cell phone (your body must be in the classroom/lab) on the day an exam is given (written and/or clinical competency) there will be an automatic 5 point deduction off the final score of the exam/competency. Any student caught cheating on any test or competency evaluation will receive a 0 for a grade, an F for the course and dismissed from the program. See program handbook. Competency Grading: Students will be graded by one instructor on each attempt. If the student fails that specific competency, a different instructor will grade on the next attempt. Instructors may repeat grading depending on the number of attempts made by the students.

15 Written Test Policy: All written paper tests (classroom) are typically taken with a scantron. Only answers filled in on the scantron will be accepted. If you did not completely fill it out, had one answer on the paper test and another on the scantron, skipped questions, etc.; be advised that ONLY the answer on the scantron will be accepted. All written paper tests in the classroom must be filled out. Answers on the paper test will only be accepted if the scantron sheet was destroyed (mechanical failure) by the grading machine. Scantron sheets that are bent, torn or incorrect form (purchased from an off brand company) will not be accepted by the instructor. ADA STATEMENT Any student with a documented disability (e.g. physical, learning, psychiatric, vision, hearing, etc.) who needs to arrange reasonable accommodations must contact the Disability Services Office at the Respective College at the beginning of each semester. Faculty is authorized to provide only the accommodations requested by the Disability Support Services Office. The ADA counselor for HCC is located on the 1stfloor (The Center). TITLE IX EDUCATION AMENDMENT Title IX of the Education Amendments of 1972 requires that institutions have policies and procedures that protect students rights with regard to sex/gender discrimination. Information regarding these rights are on the HCC website under Students-Anti-discrimination. Students who are pregnant and require accommodations should contact any of the ADA Counselors for assistance. It is important that every student understands and conforms to respectful behavior while at HCC. Sexual misconduct is not condoned and will be addressed promptly. Know your rights and how to avoid these difficult situations by logging in from your HCC student account, go to Go to the button at the top right that says Login and click. Enter your student number. ACADEMIC HONESTY STATEMENT All students are expected to fulfill the rights and responsibilities outlined in the Houston Community College Student Handbook. COURSE REQUIREMENTS: The student is required to demonstrate basic dental hygiene instrumentation skills as evidenced by a 75% or higher accuracy on learning assessments, present the principles and procedures required for comprehensive dental hygiene patient care that are based on scientific evidence, assimilate the knowledge of theory and apply it into clinical examination and instrumentation. Demonstrate professional communication styles that will be used to help discussion and inform patients, as well as work as a team member. Develop critical thinking and problem solving skills which will help combine theory into practice in all pre-clinical and clinical settings. Professional behavior will be required at all times.

16 CLASS/ LAB/ CLINIC/ POLICY S REQUIREMENTS: Lab/Clinic: Students are allowed in lab or clinic with direct faculty supervision when practicing on other students. Open lab days (Friday) can be used to practice on typodonts without faculty supervision. Students are to be dressed in scrubs/lab coat at all times (no exceptions). Students are required to be partners with fellow classmates for practice and competency evaluations. Professional Policy: Students are expected to participate fully in all lab and clinical activities and to model professional behavior at all times. All students are considered mature enough to seek faculty assistance and to monitor their own progress in meeting course requirements. Attendance Policy: Students are expected to attend all regularly scheduled classes and clinic. Students may need to arrive several minutes early to prepare for clinic and class. Expect to stay after class/lab to complete sterilization and other tasks. These tasks will be assigned on a rotation schedule. These duties are mandatory. Failure to do so will be addressed in the Professionalism grade. In order to become proficient in these skills, it is necessary that you attend all scheduled class and laboratory sessions. Students who are absent from class/lab (unexcused) for a total of three (3 times) will receive two (2) points off the final course tabulated grade. Only excused absences are accepted with proper documentation. For example: doctor s excuse, jury duty etc. REQUIRED HIPAA TRAINING: Students are required to complete HIPAA training (House Bill 300/Texas State Legislature) regarding privacy of patient records offered through the Texas Dental Hygiene Educators Association. The training (online) must be completed by November 30, 2016 (last day of regular instruction). The discounted cost of the course is paid by the student. If not completed by designated date, two (2) points will be deducted off the total written test average. Student code is TDHEAS

17 NOTICE: Students who repeat a course three or more times face significant tuition/fee increases at HCC and other Texas public colleges and universities. Please ask your instructor/counselor about opportunities for tutoring/other assistance prior to considering course withdrawal, or if you are not receiving passing grades. HCC Grading Scale A = points per semester hour B = 89-80: 3 points per semester hour C = 79-70: 2 points per semester hour D = 69-60: 1 point per semester hour 59 and below = F 0 points per semester hour IP (In Progress) 0 points per semester hour W(Withdrawn) 0 points per semester hour I (Incomplete) 0 points per semester hour AUD (Audit) 0 points per semester hour IP (In Progress) is given only in certain developmental courses. The student must reenroll to receive credit. COM (Completed) is given in non-credit and continuing education courses. To compute grade point average (GPA), divide the total grade points by the total number of semester hours attempted. The grades "IP," "COM" and "I" do not affect GPA. For Health Science programs, see the Program/Discipline Requirements section for specific grading requirements.
