Chandler Regional and Mercy Gilbert Medical Centers fulfill our mission and vision by investing in community outreach and educational programs, participating in advocacy efforts and establishing partnerships in the community. We are proud to share this report featuring community outreach efforts conducted during the 2017 fiscal year that provided more than 50 programs encompassing outreach events, diabetes self management classes, children s health and immunization clinics, safety education, prenatal classes, care to the underserved and underinsured, and support groups of all kinds. Our efforts reach tens of thousands of residents in East Valley communities. Dignity Health in the East Valley is truly privileged to work in this community, serving and advocating for those who lack resources and a voice. 4 The Dollars and Sense of our Community Support As one of the largest hospital providers in the East Valley, Dignity Health plays an important leadership role in health care education and outreach, providing care to those in need regardless of their ability to pay. During the fiscal year ending June 30, 2017, Chandler Regional Medical Center provided a total of $45,726,012 in community benefit. Mercy Gilbert Medical Center provided a total of $23,093,075 in community benefit. Benefits to the Poor and Broader Community As a result of Dignity Health s mission in the East Valley to deliver health care and partner with the community, 113,115 people were reached through community education and health services, education for health professionals, subsidized health services, research, donations, community building activities, community grant programs, and community benefit operations. 3 Chandler Regional Medical Center 87,770 Mercy Gilbert Medical Center 25,345 Grand Total 113,115

6 Dignity Health Community Grants The Chandler Regional and Mercy Gilbert Medical Center Community Grants Program is an important way to fulfill our mission through creative partnerships. Community of Care partners combine their resources to create a program or project that improves services in a targeted area of need. Each Community of Care grant can apply for funding to up to $100,000 based on Chandler Regional and Mercy Gilbert Medical Center initiatives, core community benefit principles, and community health needs. Communities of Care programs that received funding during the 2017 fiscal year include: Senior Community Wellness - $70,000 Neighbors Who Care About Care Valley of the Sun s YMCA (Y OPAS) Outreach Program for Ahwatukee Seniors East Valley I-Help - $70,000 Chandler Christian Community Center Lutheran Social Services Tempe Community Action Agency Partnership to Build Resilient Families - $65,000 ICAN - Positive Programs for Youth Chandler Education Foundation Valley of the Sun YMCA, Chandler/Gilbert Family YMCA Safe at Home - $60,000 East Valley Adult Resources Rebuilding Together Valley of the Sun AT Still University East Valley Perinatal Network - $42,060 Women s Health Innovations of Arizona - Lead Postpartum Support International, Arizona Chapter Pregnancy Care Center of Chandler 5

7 Community Oral Health First Teeth First - Funded by First Things First in partnership with Maricopa County Department of Public Health, Office of Oral Health. By age 4, 52% of Arizona children have experienced tooth decay. Early intervention through education, screenings, fluoride varnish application and referrals to partnering dentists can decrease early childhood caries and promote improved dental health throughout a child s lifetime. Services are provided at preschools, childcare centers, Women, Infants and Children (WIC) offices, immunization clinics and other community locations. Education promoting early childhood oral health care and establishment of a dental home at age one is offered to parents, mid-level providers, dental and medical professionals. Dignity Health Chandler Children s Dental Clinic - Affiliated Practice Dental Hygienists provide preventive dental care for underserved children through age 18. Services provided at this school-based clinic include exams, cleanings, sealants, fluoride varnish, x-rays, education and referrals for restorative and emergent care. The clinic s oral health educator provides free oral health education and oral health supplies to children, families and pregnant women in the community. The clinic and its services are sustained through grants, donations and community funders. Community Education The Community Education Department provides a variety of support groups, prenatal and safety classes free or for a nominal fee. Child Safety Classes: Free Car Seat Clinics Keeping Kids Safe Safe Sitter - Baby Sitter Training Tours: Family Birth Center Prenatal and Baby Care Classes: The Art of Breastfeeding Baby Your Baby Boot Camp for New Dads Childbirth Preparation Pregnancy and Postpartum Fit Camps Teen Childbirth Preparation Motherhood: Breastfeeding Support Group Let s Talk - Therapeutic Pregnancy and Postpartum Program Pregnancy and Postpartum Support Group TEENS 4 Teens Support Group Let s Talk - Perinatal Therapeutic Support Group An intensive therapy group for pregnant and postpartum mothers facilitated by a therapist who specializes in perinatal mood disorders. This is a closed 6-week session which requires registration and currently offered free from Dignity Health. Participants are welcome to bring a support person and their baby (baby should be nonmobile). Community Wellness Immunizations are a vital component to the health and well-being of every child. Dignity Health is the largest provider of free immunization clinics in the East Valley. The award winning immunization program provides free immunizations (vaccines provided through the State Vaccines for Children Program) for children 18 years and younger, who lack access to healthcare, are uninsured or underinsured children on AHCCCS as well as those with cultural barriers. Free clinics are held at Dignity Health and throughout the East Valley service areas. Partnerships include schools, churches, government offices, fire departments, family resource centers, Boys and Girls Clubs, WIC, Dignity Health volunteers, and Chandler library, First Teeth First. Adult Hotline Children Hotline

8 Building Blocks for Children Offers vision and hearing screening services to help medically prepare under-served children for school. This is a portable program targeting children in the East Valley community and serves children up to age 18 years. The clinics, located in areas of greatest need, are accessible to those least likely to receive vision/hearing screening from mainstream health care. Program Coordinator René Moorehead Chronic Disease Self Management Education Chronic Disease Self-Management Education is a six-week workshop presented by certified Lay Leaders utilizing Stanford research based curriculum on self-management of chronic diseases and diabetes. This program significantly assists our communities in reaching the 2020 goal of increasing the quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and injury, improve health, and enhance quality of life. Through strategic partnerships we are able to expand capacity building opportunities as well as offer the workshops at no cost to the individual and those involved in their care. 9 Center for Diabetes Management Diabetes is a widespread health issue that affects more and more people every day. It is the 7th leading cause of death in Arizona. Center for Diabetes Management offers education and assistance to individuals with Type 1 and Type 2 diabetes, gestational diabetes, and those with pre-diabetes. Family education sessions are available for children with diabetes and pre-diabetes and their family members. Additional services include insulin initiations, insulin pump therapy, continuous glucose monitoring, and community outreach programs. The goal of the Center is to empower patients with diabetes and their family members to manage their disease as part of an overall healthy lifestyle and preserve their quality of life. Healthy Families Through this program, clinical social workers screen new mothers to determine if they are at high risk for child abuse. For those mothers who are at risk, resources and referrals are provided to them prior to discharge. Chandler Regional Medical Center was the first hospital in the East Valley to offer Healthy Families through a partnership with Southwest Human Development, and the same program is now offered at Mercy Gilbert Medical Center. Primary services offered to mothers are introduction to community services and assistance with issues that contribute to stressful parenting. Diabetes A Sweet Life, diabetes outreach and education, offered 6 times per year Monthly Insulin Pumpers Support Group 8

10 Lactation Program The lactation program is staffed by RN/ICBLC s (International Board Certified Lactation Consultants) who provide the following services to mothers and their babies: Promote, support and protect breastfeeding for the mother who intends to breastfeed. Help new mothers establish breastfeeding with their newborns: - With healthy, term babies. - With babies who are ill or growing in the Special Care nursery. - Help special care babies learn to breastfeed. Create care plans for babies with special needs. Help mothers to provide milk for their babies when situations are not optimal. Offer phone consults for concerns after discharge. Support employees who are breastfeeding by providing a space and time to pump their breast milk while at work. Offer Breastfeeding Support groups to the mothers who deliver here as well as those who deliver in the community; we provide free Breastfeeding Support Groups for all new moms in the community seeking guidance with breastfeeding. Consultation to other care givers who give care to nursing mothers. 9
