1 Wales-wide Wales Epilepsy association cyf The project will be building on the existing work of the organisation supporting people with epilepsy and their families across Wales. They will provide outreach support in communities and within medical settings, provide training and support groups, advocacy services, telephone helpline and online support. In addition they will provide training to service providers and organisations to address stigma and promote understanding of the condition. The 370,690 grant, over three years, will fund salaries, recruitment, training, travel, marketing, monitoring & evaluation, events and overheads. Wales-wide Bridgend Street Football Wales Bridgend Carers Centre The project will build on the current provision of Street Football Wales, where they will use football and sport to tackle homelessness and social exclusion. In addition to facilitating physical activity through football, they will provide a women s specific programme, training courses, advice surgeries, volunteering, supported work placements, and refer and signpost to partners and other specialist support agencies. The project will benefit 3,800 individuals per year across Wales. The grant over a five-year period will fund salaries of project workers based in North Wales, East Wales and West Wales. The 394,643 grant will be used for project overheads, volunteer training and expenses, project delivery and management. Bridgend Carers Centre will work with young carers (5 25 years) to improve their educational attainment and employment prospects. It will deliver one to one and group sessions for young carers, with the early intervention reducing the number of young adult carers from becoming NEET (not in education, employment or training). 1
2 Awareness raising sessions will be delivered in primary and secondary schools for peers and staff through assemblies and they will be supported to develop their own young carers group. Schools that have developed internal policies and good practices around supporting for young carers will be awarded with accreditation. 96, over three years will fund salaries, training, travel expenses, marketing, overheads including management fees. Bridgend Plantlife International - The Wild Plant Conservation Charity Plantlife International will empower local people, building confidence and capacity, create new partnerships and connect with existing activities programmes to enable more people to get involved and take ownership of local green spaces in Bridgend. Specific activities will include creating a Green Network, providing training to existing volunteers, working closely with partners to support new volunteers, create and trial new wellbeing activities and support the development of local wellbeing ambassadors and green space guardians. The 247,500 grant, over four years, will fund salaries, freelance fees, recruitment, expenses, marketing and overheads, with a small amount of capital equipment. Bridgend S.T.E.E.R - The Enterprise Academy S.T.E.E.R - The Enterprise Academy in Bridgend will develop the forest nursery/school at their 37 acre Resource and Wellbeing Centre, to support children, young people and their families, who live with multiple complex needs. 9,954 will fund staff costs, forest school training, and marketing. Bridgend Wildmill Youth Club Wildmill Youth Club in Bridgend will use the grant to bring people together and build strong relationships in and across communities by taking children, young people and their families on trips and providing workshops through the school half 2
3 term holidays. 2,810 will fund workshop costs, and coach hire. Caerphilly Caerphilly Miners Centre For The Community Caerphilly Miners Centre For The Community will complete the second and final phase of refurbishment to the former Caerphilly Miners Hospital, known as The Beeches. The first floor will accommodate four bespoke rooms for community use, and the second floor will house a social enterprise centre with business starter units. The project will reduce social isolation, improvement mental health, and an increase in wellbeing, healthy lifestyles, volunteering opportunities, and return to work training and skills training for the local community. The capital grant of 99,600, over one year, will be used towards the costs of the second phase of capital works: refurbishment of the first and second floors, the installation of a hydraulic lift, and associated professional fees. Caerphilly Cardiff Cardiff Cardiff Chartist Help 4 All Mental Peer Support Bobath Children's Therapy Centre Wales British Lung Foundation Cardiff and Vale College Chartist Help 4 All Mental Peer Support in Caerphilly will use the grant to run sessions supporting people with mental health issues by providing peer mentoring and engaging social activities. 9,650 will fund office supplies, arts and craft equipment, knitting and crochet equipment, furniture and venue hire. Bobath Children s Therapy Centre Wales in Cardiff will provide therapy sessions for babies with cerebral palsy and their families. 8,997 will fund therapists and specialist staff salaries, overheads, and materials. British Lung Foundation will hold four Community Events, in Conwy and Cardiff, which will empower up to 130 people with lung conditions to better manage their conditions. 9,942 will fund venue, travel, accommodation, postage, printing, and administration costs. Cardiff and Vale College will teach resilience skills and suicide prevention to 6 volunteers who will the train the rest of the student 3
4 body. 9,600 will fund a resilience skills and training workshop. Cardiff Cardiff Community Housing Association Cardiff Community Housing Association will run two Christmas events to bring people together and to consult with tenants to find out how the introduction of Universal Credit and Welfare Reform has impacted on their lives, so that the Association can improve their services. 1,700 will fund food and drink, a photographer, decorations, and general supplies. Cardiff Cruse Bereavement Care Cardiff and The Vale Branch Cruse Bereavement Care Cardiff and The Vale Branch will use the grant to run a comprehensive training programme for new and existing volunteers to allow them to offer bereavement support face-to-face, by telephone and via . 8,405 will fund training, refreshments, management and administration, and branch support costs. Cardiff Hybu Hwyl C.I.C. Hybu Hwyl C.I.C. in Cardiff will use the grant to set up a series of children s activity clubs, for primary school children in the local area. 9,992 will fund staff, room hire, staff training, and equipment. Cardiff Oasis Cardiff Oasis Cardiff will work with refugees and asylum seekers to develop skills in order for them to engage with the wider community. There will be opportunities to participate in catering qualifications, weekly cookery classes, volunteer for pop up events whilst also offering opportunities to improve English language skills, employability and confidence. The project will benefit people in the Cardiff area. The 482,246 grant, over five years, will fund staff salaries, freelance fees, training, equipment, marketing, travel and overheads. Cardiff St John Lloyd R C Primary School St John Lloyd RC Primary School in Cardiff will bring together parents and children from four schools to create lanterns for a parade event. 4,700 will fund artists costs for 4
5 workshops, craft materials, and refreshments. Cardiff Steps4 Change Ltd Steps4 Change Ltd in Cardiff will provide careers advice and mentoring for BAME young people from low income families. 10,000 will fund admin and publicity, travel, workshop trainer, refreshments, volunteer, and insurance. Carmarthenshire Carmarthenshire Carmarthen Breakthro' Project Coleg Elidyr Camphill Communities Carmarthen Breakthro' Project will use the grant to create two safe, fun and relaxing sensory rooms for children and young adults with additional needs. 10,000 will fund building works. Coleg Elidyr will use capital funding to modernise and extend the existing community education hub in Rhandirmwyn, Carmarthenshire. The project will improve the standard of learning for young adults with additional learning needs and the wider disability community. It will revitalise the services on offer, ensuring support is tailored to beneficiaries' needs to have the greatest impact on their lives. The project will enable the college to provide a Centre of Excellence for fellow professionals across Wales and beyond, allowing professionals the opportunity to come together to observe lessons, develop new approaches and share best practice. The 700,000 funding, over two years, will pay for capital work and professional fees. Carmarthenshire Carmarthenshire Fresh Start Community and Youth Society Llanstephan Memorial Hall Fresh Start Community and Youth Society in Carmarthenshire will encourage people to become active in the community, through a range of educational, family and intergenerational activities. 9,800 will fund session worker and materials, publicity, tools, protective clothing and equipment, insurance, and venue hire. Llanstephan Memorial Hall in Carmarthenshire will carry out some refurbishment work to their local village hall, to provide better facilities for user 5
6 groups. 9,685 will fund replacement windows, woodwork, and scaffolding. Carmarthenshire Ceredigion Morfa Family Centre HAUL - Arts in Health Morfa Family Centre in Carmarthenshire will offer support, help and advice for expectant/new parents and carers within their community. 10,000 will fund staff costs, running costs, and resources. HAUL - Arts in Health in Ceredigion will use arts as a vehicle to promote and maintain good health and well-being for those experiencing ill mental health. 6,000 will fund art materials, workshop leader costs, venue hire, and promotion. Ceredigion Therapies4Services Therapies4Services in Ceredigion set up a mentoring service for active soldiers, veterans and bereaved military families in order to help aid their mental wellbeing. 10,000 will fund rent, bills, furniture, and marketing. Conwy Cruse Bereavement Care Ltd North Wales Area Cruse Bereavement Care Ltd North Wales Area will recruit and train volunteers to provide support and advice to bereaved people. 9,360 will fund room hire, training costs, office equipment, access ramp, and promotional materials. Denbighshire The Jolly Dancers The Jolly Dancers in Denbighshire will run exercise and wellbeing classes with people in their area, most of whom are disabled. 6,287 will fund venue hire, equipment, an exercise coach, and refreshments. Denbighshire Tourette Syndrome (UK) Association Tourette Syndrome (UK) Association will hold a weekend full of activities and events for children and families who live with Tourette Syndrome. 7,500 will fund activities, accommodation, staff, travel and visiting experts. Flintshire 1125 C.I.C C.I.C. in Flint will deliver creative workshops to combat feelings of isolation and reducing the risk of anti-social behaviour for young people living in emergency accommodation or when they have been excluded from school. 9,995 will fund youth workers, materials for making jewellery, T- Shirt printing, graffiti, workshop, celebration event and refreshments. 6
7 Flintshire Amy and Friends Amy and Friends in North Wales will fund weekly sessions for siblings of children who have a variety of illnesses to combat feelings of sadness, isolation, guilt and loneliness. 10,000 will fund sessional workers, rent for two clubs and activity costs. Gwynedd Siop Griffiths Cyf The project will extend Siop Griffiths into a community facility, to house a tourism business and bunkhouse-style accommodation, creating 16 training placements and 4 apprenticeships for young people in hospitality, catering and tourism as well as volunteering opportunities. Local activities will be delivered to the community utilising the café and digital centre which already form part of the building. The project will develop skills amongst young people, create employment opportunities, increase social interaction and reduce feelings of isolation. The 413,976 grant, over three years, will fund the capital works, staff salary, recruitment, travel and expenses, marketing and promotion, monitoring and evaluation and professional fees. Isle of Anglesey Isle of Anglesey Merthyr Tydfil Anglesey Childcare Providers Ltd Holyhead Community Events Group Ysgol y Graig Primary School Anglesey Childcare Providers Ltd will teach their children the importance of healthy eating and exercise, in order to help build confidence and wellbeing. 8,191 will fund equipment for outdoors, playroom and kitchen, and craft plus workshop costs. Holyhead Community Events Group on Anglesey will bring people together through a series of free community-wide events. 10,000 will fund an outdoor cinema, raft race, Christmas 2019 event, and a volunteer awards ceremony. Ysgol y Graig Primary School in Merthyr Tydfil will provide a programme of activities for the local community to help address the issues they face. 9,700 will fund sessional workers, materials and hire costs, transport costs, and publicity. 7
8 Neath Port Talbot Resolven Building Blocks Resolven Building Blocks in Neath Port Talbot will run workshops for young people and children with disabilities who are experiencing emotional issues, while providing advice, information and support. 8,060 will fund salary, staff training, transport, recruitment, workshops, Neath Port Talbot Skewen Women's Institute marketing and translation. Skewen Women's Institute in Neath Port Talbot will bring members together to learn how to nurture their mental health, through a programme of activities. 6,900 will fund hall hire for clubs/workshops, tutors, workshops, and visits and events. Neath Port Talbot Ysgol Cwm Brombil Ysgol Cwm Brombil in Neath Port Talbot will run intergenerational sessions for four months, inviting pupils, their families and the local community into their new school to share thoughts on past, present and future education and experience the facilities available. 9,920 will fund staff costs, refreshments, resources and bilingual translation. Pembrokeshire Pembrokeshire Milford Community Society North Pembrokeshire and Cardiganshire Group Riding for the Disabled Association Milford Community Society in Pembrokeshire will use the grant to provide a series of workshops designed to engage NEETs in employment skills. 9,900 will fund tutor fees, materials, insurance costs, equipment, and publicity materials North Pembrokeshire and Cardiganshire Group Riding for the Disabled Association provide weekly riding sessions for people living with a variety of disabilities, including autism, downs syndrome, and cerebral palsy. 2,400 will fund riding sessions. Pembrokeshire PAT Ponies CIC PAT Ponies CIC in Milford Haven will use the grant to offer Getting Back to Nature retreat days which will allow individuals experiencing difficulties such as mental health disorders to meet and work with each other on their set goals and aims for personal growth and overall wellness. This grant for 10,000 will fund 8
9 fencing, a disabled toilet, staff costs, and psychotherapist fees. Pembrokeshire Powys Powys Powys Powys Tenby Church In Wales VC Primary School Cruse Bereavement Care Powys Llangorse Common Community Toilets Llangunllo Community Hall Management Committee The Radnorshire Wildlife Trust Limited Tenby Church In Wales VC Primary School in Pembrokeshire will work with pupils, families and other community members, on a series of decorative canvasses, murals, and mosaics for the new school building. 10,000 will fund a family engagement and support worker, project and murals tutors, marketing, and resources. Cruse Bereavement Care Powys will recruit and train volunteers to provide support and advice to bereaved people. 8,904 will fund trainer, venue, utilities, refreshments, PR, volunteer and staff costs. Llangorse Common Community Toilets in Powys will rebuild the toilets at the site to provide an improved facility for users. 10,000 will fund building work. Llangunllo Community Hall Management Committee in Powys will improve the kitchen facilities in the local village hall for users. 4,863 will fund kitchen equipment, painting and redecorating, and installation costs. The Radnorshire Wildlife Trust Limited in Llandrindod Wells will help local people of all ages to restore their well-being, reconnect with nature and take part in a series of creative workshops to design new nature reserve information resources. 9,950 will fund staff costs, layout and design resources, publication of resources, staff travel, and minibus hire. Rhondda Cynon Taf Affinity Choir Affinity Choir in Rhondda Cynon Taf will plan, prepare and perform at an International Women s Day event in Cardiff. 9,820 will fund fees, venue costs, BSL interpreter, IT equipment, uniform, publicity, and refreshments. Rhondda Cynon Taf Hearing Dogs for Deaf People Hearing Dogs for Deaf People will establish two LinkUp sessions for a year, which bring deaf people together, build strong relationships between deaf and hearing 9
10 people, and work to address the difficulties that arise from being deaf in a hearing world. 10,000 will fund communication support, listening equipment, travel expenses, accommodation, and printed materials. Rhondda Cynon Taf Swansea Tonyrefail & District Community Council Gendros Community Choir Tonyrefail & District Community Council in Rhondda Cynon Taf will develop their Christmas event by hosting a second day of activities especially for children with disabilities, which will allow them to fully participate. 10,000 will fund venue hire, various child entertainers, and sundries. Gendros Community Choir in Swansea will run choir practices and performances, building stronger intergenerational links with their local school, and improve individuals confidence and wellbeing. 4,700 will fund a choir master, hall hire, portable P.A. system, equipment, uniforms, and transport. Swansea Gorseinon Institute Gorseinon Institute in Swansea will improve their space, enabling them to cook meals for those using the foodbank, begin healthy eating and wellbeing sessions, and improving facilities for the various other groups that use the space. 8,000 will fund a new kitchen and fitting costs. Swansea Swansea Mayals Primary School PBC Foundation (UK) Limited Mayals Primary School in Swansea will provide training for school staff to deliver therapeutic intervention sessions, to help children with complex social-emotional needs. 10,000 will fund staff training, supply cover, resources, sessional fees, and evaluation costs. PBC Foundation (UK) Limited who support people with liver disease, will run series of self-management workshops throughout Wales for people with the condition and their families, providing information and a place to socialise. 7,444 will fund travel costs, invitations, materials, venue hire and accommodation. 10
11 Swansea Pontarddulais and District Community Car Scheme Ltd Pontarddulais and District Community Car Scheme Ltd in Swansea will purchase a new disability car to ensure older people and people with disabilities continue to receive a good service. 10,000 will part-fund an Swansea Swansea The Vale of Glamorgan Swansea Women s Aid The Friends of the City of Swansea Botanical Complex Croeso Llantwit accessible vehicle. Swansea Women s Aid will expand the current DAISE project to incorporate a triage worker at their One Stop Shop, further focus on one to one appointments and introduce further opportunities for mindfulness and counselling. The project activities will be delivered at their main base in Swansea Centre and throughout surrounding areas in existing family centres. The project aims to engage with at least 1,200 women suffering from Domestic Abuse or Sexual Abuse in Swansea. The 482,109 grant, over two years, will cover staffing costs and other related revenue delivery costs. Friends of the City of Swansea Botanical Complex will strengthen the relationships between the volunteers and their referrals from the Brain Injury Unit, Social Services and students with learning difficulties. 3,072 will fund entrance fees, guides, refreshments and travel. Croeso Llantwit in Llantwit Major will develop a community-led grassroots response to supporting the resettlement of refugees in the town. 5,695 will fund volunteer, training and conference costs, administration, publicity and events, and interpretation costs. Torfaen Able Radio Able Radio in Pontypool will develop a volunteer group to carry out ongoing grounds maintenance at their site, which is a converted school that hosts three charities for people with learning disabilities and mental health issues. 5,200 will fund an accessibility ramp, equipment and a secure storage unit. 11
12 Torfaen Llanderfel Court Community Group Llanderfel Court Community Association in Torfaen will organise a series of events and trips which have been requested by their community. 4,935 will fund a pool table, cues, dart board, bingo machine, travel, entrance fees, meals, printer, and stationery Torfaen Torfaen Parents of Play Information Torfaen Pontnewynydd Friends for Life CommunityHub costs. Parents of Play Information Torfaen will support families with complex needs through provision of workshops where parents can build networks and find out information. The group also provide one-to-one support for children so they can experience activities. 9,500 will fund volunteer expenses, activities, three forums, and training costs. Pontnewynydd Friends for Life Community Hub in Torfaen will use the grant to organise a programme of activities for their members many of whom are older and disabled and experience isolation. The project will help build strong relationships within this community and across communities. 4,635 will fund hall hire, support worker, volunteer expenses, transport, trips and activities. Torfaen The Hwb Torfaen The Hwb Torfaen will create a recording studio which will be used by the disadvantaged young people in their area to record music they have created enabling them to gain new skills. 9,996 will fund IT and recording equipment, recording software, and CD printing costs. Wrexham Wrexham St. Marys Catholic Primary School Wrexham Glyndwr Students' Union Limited St. Marys Catholic Primary School in Wrexham will transform the outdoor area into a natural environment for the school and community to use in and out of school hours. 8,895 will fund gardening equipment and provision for a Youth Offenders team. Wrexham Glyndwr Students Union Limited will run a suicide prevention training programme for students and staff at the University. 9,600 will fund training for six volunteers. 12
