2 Statements & Search Criteria Mission Multiple Sclerosis Center of Southwest Florida (MS Center) is a local organization whose mission is to improve the quality of life for those in Southwest Florida who are affected by Multiple Sclerosis. Background The organization was incorporated in 2000, as MS Home. The founders were Debbie Heil and Lisa Luthringer, who themselves have MS. The organization has since been renamed the Multiple Sclerosis Center of Southwest Florida. The Board of Directors developed a strategic plan in 2014, which included plans to hire an Executive Director. The organization hired Kay Jasso as Executive Director in January The MS Center provides access to therapeutic and wellness services through qualified contracted providers. Clients can receive services, not typically covered by insurance, that help them to manage the symptoms of the disease. Services such as exercise classes for balance and strength, art classes and support groups to reduce isolation, and massage therapy to reduce pain. The MS Center covers the cost of the services, requiring only a small co-pay from the client. The center also provides mental health counseling and educational seminars. The center provides a place where participants know that they can go for support and resources. CEO/Executive Director Statement Since coming onboard a few months ago, I have spoken with the clients and people in the community about the MS Center. Current clients believe the Center is a lifeline, and are incredibly grateful for all that we do. The Center is the only MS organization providing direct services to people with MS in Southwest Florida. We are funded only by individual donations, local fundraising events, and foundation grants. Fundraising activities by the National MS Society do not support the Center s programs. Unfortunately, many people don't the know about the Center and its programs. I have also found that the Center's services and programs are underutilized because of hours and location. The Center has the potential to serve many more people with MS. There are many unmet needs, especially the needs of younger patients, families, and men. We are currently conducting surveys in order to gain information that will help us improve our program planning. I believe that the organization has a strong supportive board of directors, and many loyal supporters that will help the organization remain vital and serve the needs of the community for many years to come. 3 Programs Programs Therapeutic and Wellness Programs Description Long-Term Success MS Center provides access to therapeutic and wellness services through qualified contracted providers. Clients can receive services, not typically covered by insurance, that help them to manage the symptoms of the disease. Services such as acupuncture, massage therapy, mobility training, aqua therapy, yoga, mental health counseling and personal training are provided. MS Center covers the cost of the services, requiring only a small co-pay from the client. The Center also provides educational seminars, support groups, resources and information. Plans are to increase the number and types of contracted providers, to include additional mental health counselors, physical therapists, and social workers. The Center also plans to expand locations and hours of services to open up our programs to working adults, families, caregivers, and young people. The Center will provide more activities including field trips to help our clients who are often isolated and sedentary. Ideally the Center will have ongoing activities and a day program where people can stay for longer periods. The organization's long term plans includes increasing the number of volunteers and staff members to include an intake counselor, a development assistant, a program director, and an activities coordinator. 4 Impact Goals What are your organization's long-term strategic goals? The expected outcomes of reaching these goals are: Patients will have increased mobility and reduced pain Clients will have reduced isolation. Clients will have reduced stress, depression, and improved coping skills Families and caregivers of MS patients will have increased support and education Clients will have more information about the effects and benefits of improved nutrition More people suffering from MS in Collier County will have access to services and information to help them manage their disease. Strategies What are the major strategies you will do to accomplish your goals? To increase patients mobility and reduce their pain. Continue to provide access and financial assistance for alternative therapies such as acupuncture and massage therapy. Provide access and financial assistance for physical therapy and occupational therapy Research benefits of other alternative therapies such as hyperbaric therapy. To reduce Isolation Increase individual MS patients connection to each other and to the community. Increase activities at the MS Center: therapeutic, educational, and social. Improve the website to include more information,links to resources, and an online application Provide community outings Improve access to transportation Provide information and guidance about free transportation Provide information and assistance in obtaining hand controls in their own vehicles To reduce stress, depression, and improve coping skills Increase access to counseling and social services. Provide connection to resources about available counseling and social services. Add facilitated support groups that include young adults,, the newly diagnosed, and men. Provide financial assistance to those with limited incomes. To provide support to the families and caregivers of MS patients. Add support groups for families Add educational resources and lectures on caregiving Add respite program for caregivers To improve knowledge of the effects and benefits of improved nutrition Increase educational resources and lectures on nutrition Provide access to individualized nutritional counseling To increase access to medication and diagnostic testing To increase access and awareness of free medication programs and free medication To provide assistance with ADLs Add homemaker/companion services To provide services to more people with MS in Collier County Financial assistance with selected services. 5 Computer training Move the center to a location that would be more accessible to a greater percentage of MS patients. Outreach to local MS support groups, and to the national MS organizations. Capabilities What are your organization s capabilities for doing this? The MS Center was established in 2000 to serve those affected by MS in Southwest Florida. It is the only organization, aside from medical institutions, that is providing direct services to MS patients. The services are provided by contracted qualified professionals. Currently the Center has 14 contracted providers. The MS Center staff and board members are actively making community connections in SWFL and across the state to gather more resources for its clients. The MS Center provides most of its services at low or no cost to the client. Monthly educational lectures are also provided at no charge. Indicators What indicators do you use to measure progress? The MS Center recently conducted a general survey with its clients to determine what additional or change in servcies are needed. This general survey will be conducted annually. The MS Center will implement a new evaluation tool in the summer of 2015 to measure the effectiveness of services for individual clients. The evaluation will be conducted in an annual client interview. Individual client records will be used to record types of services utilized and the client's evaluation of services. The number of new client registrations will continue to be tracked. Increased website traffic, Facebook likes, and responses to online s will be tracked to evaluate communication efforts. Progress What have and haven t you accomplished so far? We provided direct services to 93 MS clients in Services provided included acupuncture therapy, massage therapy, aquatherapy, equestrian therapy, mental health counseling, and a support group. Most were offered at low or no cost to the client. We conducted 8 educational workshops in 2014, with over 150 attendees. We reached over 200 people with MS through and mail. 5

8 Management Executive Director/CEO Executive Director Mrs Kay Jasso Term Start Jan Experience Kay Jasso is a nonprofit professional, who has over 10 years of experience specializing in relationship building, marketing, grant writing, and board governance. Kay began her nonprofit career at the Collier County Community Foundation where she held the position of Program Manager. She worked closely with the local nonprofit organizations and with the grant committee. After six years, Kay moved on to Healthy Start of Southwest Florida where she served as Operations Manager. In that role, she wore many different hats ranging from event management to grant writing. Kay was most recently on staff at Goodwill Industries of Southwest Florida as a grant writer. There, Kay s role was to provide grant research, grant writing, program budget preparation, and to help develop community collaborations. Kay also has a long history of volunteerism. She has served on several local nonprofit boards, including President of Music for Minors, Secretary of the Gulf Coast Society of Fundraising Professionals, and Secretary of Miracle Limbs Courage in Motion. She also served as a panel member for four years on the Collier County Foster Care Review Board. Before coming to Florida in 2002, Kay was a successful professional in the consumer products industry in Chicago. Kay held management positions at the Quaker Oats Company and at Unilever/ Best Foods.
