2 Lewis County Suicide Prevention Coalition Mission: The Coalition will support and empower individuals and families in our communities by providing awareness and knowledge about suicide prevention. PA Goal 2.3: Prevent Suicides Among Youth and Adults Location/reach: Lewis County PA Agenda Focus #2: Prevent Substance Abuse and other Mental Emotional Behavioral Disorders The CHIP Project is an expansion of a newly formed suicide prevention coalition in the county.

5 2014 Project Problem: The suicide mortality rate in Lewis County is 3 times higher than the NYS rate. Who is affected? Primarily Adult males; and, Community as a whole AIM Statement #1: By January 2014, Lewis County will establish a suicide prevention coalition and build its infrastructure to: Provide skill-building and training opportunities for Coalition stakeholders Develop calendar of community training opportunities Develop committees to focus work of the Coalition

6 AIM Statement #1 Continued: Initial Focus of the Coalition is to engage the following stakeholders in the community to reach its vision of preventing suicide in Lewis County: human service providers, mental health professionals, veteran s services, law enforcement, physicians, emergency responders, hospice, youth, clergy, funeral home directors, bartenders, hairdressers, school administrators, and school counselors during Outcome Measures: 75% of Lewis County residents will be aware of the Suicide Prevention Coalition Increase Suicide Prevention Coalition membership by 20% each year. Process measures: # of stakeholders engaged and trained Training calendar established Committees activated

7 AIM Statement #2 By December, 2017, reduce the age-adjusted suicide mortality rate by 10% to 14.4 per 100,000 (Lewis County Baseline: 16.0 per 100,000) by: Providing outreach on suicide prevention to community Through outreach campaigns, education messages, social media use by coalition stakeholders Utilization of screening and assessment tools at the local hospital s inpatient and outpatient programs Assessment of individuals in the local hospital environments through consistent use of SBIRT or C-SSRS

8 AIM Statement #2, Continued Initial Focus of the Coalition for AIM #2 is to: provide community awareness activities to reach Lewis County residents and engage them in efforts to identify and assist family and friends who may be at risk for suicide. And, to implement risk assessment at all outpatient and inpatient settings of the local hospital. Target Populations: Community members, hospital staff and providers Outcome Measures: 100 % of local hospital patients are screened Process Measures: Community awareness activities are conducted Awareness campaign implemented annually Meetings with hospital leadership are held to facilitate implementation of the patient screening process.

Focus Area 1: Mental Health Promotion Prevention Agenda Focus Areas* Focus Area 2: Prevent Substance Abuse and other Mental Emotional Behavioral Disorders Collaborate through a prevention partnership to integrate, implement and coordinate suicide prevention initiatives. Advocate for increasing MEB disorder screening in primary care settings of adolescents and adults at risk. Promote screening and early intervention (such as SBIRT), which improves the likelihood that the person will receive evaluation and treatment. Collaborate to increase knowledge among the general public, health care providers and school personnel of the warning signs for suicide and how to connect individuals to assistance and care. Focus Area 3: Strengthen Collaboration/Infrastructure

10 Lewis County Suicide Prevention Coalition Good News To Date: Coalition has successfully engaged the targeted stakeholders Active Committees are addressing community outreach Local hospital is engaged in preparing for screening implementation Four funding sources have been secured to support work of the Coalition Challenges To Date: Screening implementation may be slower than anticipated Moving stakeholders from training phase to implementation has resulted in limited active participation 10
