Page 2 Page 4 Practice Overview Carrollton AGES TREATED: All adults and children ages 2+ INJURIES/ CONDITIONS YOU TREAT: Knees, shoulders, elbows, hips, ankles, feet and spine. I specialize in ACL reconstruction, hip arthoscopy, shoulder and elbow injuries and Tommy John ligament reconstruction. You do not have to be an athlete to see a sports medicine physician. Kevin Myers, M.D. Orthopedic Surgeon: Sports Medicine ABOUT DR. MYERS: I m originally from San Antonio, but lived all over the U.S. growing up as my father was a pilot in the Air Force. I graduated from Rice University in Houston and completed medical school at the University of Texas Health Science Center at San Antonio. I then went on to complete Orthopedic Surgery Residency at the Northwell Health System in New York and Sports Medicine Fellowship at the Andrews Sports Medicine & Orthopedic Center in Birmingham, Alabama. I m married with a 3 year old son and a new baby girl. My wife, Malia, is an Emergency Medicine physician. WHAT MADE YOU CHOOSE TO BECOME AN ORTHOPEDIC SURGEON? I ve always enjoyed playing and participating in a variety of sports. After sustaining some of my own sports injuries, I became fascinated to learn that a surgeon can reconstruct a patients ACL, repair their dislocating shoulder, or fix their hip labral tear with arthroscopy. In sports medicine, our goal is not to just get the patient walking again, but to get the patient back to performing dynamic and high level functional activities such as serving a tennis ball, throwing a baseball, running, cutting, and pivoting on a newly reconstructed ACL. APPOINTMENT AVAILABILITY: Typically the next day or that same week. I understand the importance of being seen promptly and getting you on the road to recovery. In sports medicine, our goal is not to just get the patient walking again, but to get the patient back to performing dynamic and high level funtional activites. WHAT IS THE BEST THING ABOUT BEING AN ORTHOPEDIC SURGEON? What I enjoy most about being an orthopedic surgeon is seeing my patients recover from their surgery and return to playing sports or get back to their desired activities. After the surgery I become more of a coach. I always say that the surgery is the easy part; it s the recovery and therapy that s the hard work, but the effort will pay off in the long run. WHAT KIND OF CONDITONS DO YOU TREAT? I treat all conditions that limit someone s activities and prevent them from doing the things they want to do, whether it be playing a certain sport, hiking, wakeboarding, skiing, or just being able to go for a walk. You don t have to be an athlete to see a sports medicine physician. In many cases, my patients are able to recover with help when more conservative alternative methods like physical therapy or injections utilized. OrthoAM: Early Morning Clinic Monday-Friday 7:00-7:30am No appointment necessary. Most insurances accepted. New patients can pre-register online at At OrthoTexas, we understand life doesn t stop because you re hurt. That s why we ve created OrthoAM orthopedic clinic in our Carrollton location. Here you can be seen by an regular orthopedic surgeon before regualr clinic hours and without an appointment North Josey Lane Carrollton, TX Phone: (972) Fax: (972)

Start Feeling Better, Faster. With Mi- Eye. Skip the MRI and get diagnosed at your first visit. Page 3 You finally made the decision to schedule an appointment with your orthopedic specialist because of a recent injury or pain in your knee or shoulder. You walk into your appointment expecting your orthopedic specialist to tell you what is causing your pain and to give you a treatment plan, but instead he asks you to schedule an MRI as soon as possible and return to go over the results. MRI s have been the standard of care used by an orthopedist to view the soft tissue around your joints so they can diagnose the problem. On average, it takes most patients two weeks to get their MRI done and schedule another appointment to go over their results. You go through all this before you finally have a diagnosis and a treatment plan. What if we told you there is a new device called Mi-Eye? Mi-Eye is the latest in alternative diagnostic technology being used by orthopedic specialists. It has the potential to be more cost effective than an MRI, allow you to be diagnosed at your very first visit, and get you feeling better up to two weeks faster. Mi-Eye being used to view the soft tissue and joints of the knee. Dr. Michael Schwartz in OrthoTexas Plano and Frisco locations is now using Mi-Eye. Mi-Eye is a disposable 2.2mm arthroscope, consisting of a minimally invasive Michael Schwartz, M.D. integrated camera and a light source, designed to allow diagnostic arthroscopes to be performed in the office. The Mi- Eye is most often used to diagnose issues in the knee and shoulder joints. So whether you have a problem with your meniscus, ACL, rotator cuff or possible arthritis, Dr. Schwartz can administer a local anesthetic and insert the Mi-Eye camera into the joint space. The camera provides a real-time, vivid image of what is causing your pain. Since Mi-Eye provides this industry leading real-time diagnosis, your treatment plan such as surgery or physical therapy can be more immediately identified, saving you time and money. Suffered a recent injury or have pain in your knee or shoulder? Book an appointment now with Dr. Michael Schwartz and ask if Mi-Eye is the right option for you to start feeling better, faster!

Page 4 Physical & Rehabilitation Therapy: Why It Matters Anyone who has experienced an orthopedic injury knows the frustration of being limited or completely unable to participate in their daily activities or sports. After an injury, many patients experience pain, swelling, muscle weakness, decreased range of motion, and lack of coordination. In some cases, an orthopedic injury to the muscle, bone, ligament, or joint can be healed with conservative treatments like medication, injections, and/or rehabilitation therapy. Some patients with more severe orthopedic injuries may require surgery followed by rehabilitation therapy (Physical and Occupational Therapy). At OrthoTexas, one of the many options we offer our patients is direct convenient access to our rehabilitation clinics under the same roof as your OrthoTexas physician. At OrthoTexas, your therapist and OrthoTexas physician are able to work side by side to establish your treatment plan. This collaborative approach helps you return to your activities, minimize the likelihood of re-injury, and decrease potential frustration caused by unmet functional expectations. Our therapists use evidence based treatment to manage injuries using techniques such as: electrical stimulation, neuromuscular re-education, ultrasound, cold lasers, decompression, soft tissue cupping, dry needling, Graston Technique, Kinesio taping, manual therapy, muscle energy techniques, and joint mobilizations, just to name a few. Your therapist can then combine those treatments with the necessary stretching and strengthening to minimize the amount and duration of pain, improve joint movement, and reduce swelling and scarring. This provides the most beneficial environment and pattern for healing to take place. Physical or Occupational Therapy will enhance your outcome and shorten the downtime due to an injury or surgery, and ultimately get you back to your daily activities and sports you enjoy! MARK LINANE, PT, MS ALLEN & MCKINNEY CLINIC DIRECTOR MICHAEL HOUSE, M.D. WHAT IT MEANS TO BE A TEAM PHYSICIAN Michael House, M.D. was raised in Denton, Texas and graduated from Denton High School. After completing his undergraduate studies at Texas A&M University, Dr. House went to Texas Tech University for his medical degree. From Texas Tech, Dr. House went to the University of Alabama at Birmingham completing his orthopedic residency in Dr. House then returned to Denton and opened his orthopedic practice. Shortly after, Dr. House became the official team physician for the University of North Texas and Denton ISD High Schools: Braswell, Denton and Ryan High School. Dr. House decided to become the team physician at UNT and these three Denton high schools because of his love for sports and taking care of high-level athletes. For 18 years, Dr. House has been involved in all aspects of the athletic seasons: from preseason evaluations, being available during practices and games, to offseason care. Dr. House s objective when treating any athlete is to make sure they are able to play to their best of their abilities. It is also his passion to treat injured athletes and get them back to the game in the safest timeframe. Dr. House is committed to caring for the athletes in his community.

Page 5 INJURIES/ CONDITIONS YOU TREAT: Knee ligament injuries (ACL, PCL, MCL/LCL), complex revision ACL tears, knee arthritis, meniscus tears, shoulder rotator cuff tears, complex revision rotator cuff tears, shoulder instability, throwing injuries, shoulder arthritis, hip arthritis, pediatric and adult fractures. INJURIES/ CONDITONS YOU DO NOT TREAT: Hip arthroscopy, pediatric deformity/ congenital malformations and spine surgery. APPOINTMENT AVAILABILITY: Same day or next day. ABOUT DR. DEAR: I m from a small town in Texas most people have never heard of Perryton. I m proud of my roots and loved growing up in a small, tight knit community. My father is the county auditor and my mom recently retired from working as our church secretary. I owe a lot to how they raised me and the values they instilled in me. Other than God and family, I think the most important thing in life is treating people with respect and trying to walk in as many people s shoes as possible. WHAT MADE YOU CHOOSE TO BE- COME AN ORTHOPEDIC SURGEON? I always knew I wanted to somehow integrate sports into my career. Like most kids in Texas, I didn t know what to do with myself if I wasn t playing and competing, so it seemed only natural to later find a career that could incorporate that interest. By the time I finished college at Texas A&M, I had narrowed my interests to sports management/marketing and the medical field. I worked as an A&M Athletic Department intern and shadowed orthopedic surgeons at the same time. About midway through my sophomore year, I officially made the decision to pursue medicine, and the rest was history. My main objective, above all others, is to get patients better, with or without surgery. I have a fantastic, beautiful wife, Erin, who I ve known since 5th grade. She is the rock of our family, and I can t believe how lucky I am to have her at my side. We have two wonderful boys, and our life is about to get even crazier with new born twins no time for hobbies anytime soon! Cuyler Dear, M.D. Orthopedic Surgeon: Sports Medicine DO YOU HAVE ANY ADDITIONAL QUALIFICATIONS? After my five-year orthopedic surgery residency program, I had the option of going straight into practice or going through additional training. I chose to spend an extra fellowship year training specifically in sports medicine. It was honestly the best year of my entire medical training. I was able to hone my arthroscopy skills and learn the cutting edge of knee and shoulder surgery. I also got to live 3 miles from the sunny Southern California beaches, which wasn t too bad, either. WHAT KIND OF CONDITONS DO YOU TREAT? Sports medicine is a combination of conditions that include: knee and shoulder arthroscopy, trauma surgery, fracture care, and even adult reconstruction (knee, hip, shoulder replacement) as the athlete ages. I enjoy performing ACL reconstructions, stabilizing shoulders with multiple dislocations, fixing rotator cuff tears, and replacing arthritic joints. I also have a special passion for fracture treatment. My residency at a level 1 trauma center in San Antonio gave me fantastic trauma training, so you ll often see me up late fixing fractures at our local hospitals.

Page 6 Practice Overview Allen & McKinney AGES TREATED: All ages. The youngest patient I have treated is 2 years old and the oldest is 96 years old. INJURIES/ CONDITIONS YOU TREAT: All musculoskeletal injuries, back pain, sports injuries, sprains, strains, fractures, overuse injuries and arthritis. INJURIES/ CONDITONS YOU DO NOT TREAT: Complicated traumatic fractures and complicated spine. Brent Bridwell, M.D. Family Medicine: Sports Medicine ABOUT DR. BRIDWELL: I am originally from the east coast and moved to Texas with my family in I am board certified in family medicine with a fellowship in sports medicine. I specialize in non-surgical management of musculoskeletal injuries, I do not practice family medicine and I primarily focus on sports medicine. I spend most of my time utilizing conservative options to treat various orthopedic conditions. These conditions range from musculoskeletal pain, fractures, arthritis, and other injuries. WHY DID YOU CHOOSE TO DO A FELLOWSHIP IN SPORTS MEDICINE? I enjoy getting to know my patients and helping them get back to their daily lives after an injury. I enjoy treating everything from fractures, sports injuries to arthritis in kids to adults. Despite an orthopedic surgeon s ability and qualification to treat an injury or painful joint, some patients are still apprehensive, overwhelmed, and even sometimes opposed to the idea of surgery as a solution. I am here to offer those conservative alternative treatment methods outside of surgery to meet these patients best interests. DO YOU TREAT FOR PAIN MANAGEMENT? I treat patients who suffer from muscle, bone and joint pain. Although I do treat for pain management, I specialize in acute pain rather than chronic pain. APPOINTMENT AVAILABILITY: Accepting new patients, same day or next day availibilty. DO YOU OFFER CONSERVATIVE TREATMENTS FOR YOUR PATIENTS? Yes, my focus is on conservative treatment first. Some of the non-operative treatment options I provide include medications, therapy, injections, and other conservative modalities. Before discussing any treatment plan, I make certain that my patient can benefit from a more conservative non-operative method(s) rather than going through with a surgery they may not need. Dr. Bridwell and his family. Often, pain keeps people from enjoying their daily activities, exercise routines, playing their favorite sports, or from participating in other active hobbies. I enjoy being able to help people improve their quality of life and lead a healthier and active lifestyle. WHAT ARE THE BENEFITS OF DOING INJECTIONS VIA ULTRA SOUND? Having been trained in the use of ultrasound and knowing its benefits, I feel it is a beneficial tool in helping aid efficacy of injections. An ultrasound injection is an is performed under direct ultrasound visualization. Not only does this increase the precision and accuracy of an injection, but it allows me to inject areas of the body that would be difficult to reach without visualization. Ultrasound allows real time imaging during the procedure, is quick and convenient, and can be done in the office without sedation.
