1 CORRIGENDUM NOTICE 1 Outsourcing of High-end Laboratory Services for 7 Government District Hospitals of Uttar Pradesh [Bid Ref No: UPHSSP/PATHLAB/ /02] S.N. Bid Clause ref no 1. Section II, Bidding Data Sheet, ITB 15 & Section VII, Special Conditio ns of Contract SCC 6.4 Instead of d)for Payment Category 2 linked to Turn (TAT) Payment of [10%] of the claimed Gross Amount on submission of Invoice (supported with individual TAT compliance report) by the service provider which will be verified by the hospital pathologist and counter signed by the CMS / SIC / DIRECTOR of the respective hospital as per the following criteria:- Applicability of Penalty points for Termination of the Contract & Invocation of Performance Bank Guarantee:- If the service provider gets total 12 penalty points or 2 continuous months he falls in Level 4, this will lead to termination of contract. The TAT would be calculated starting from the time is taken from the patient upto successfully uploading the test result on the designated website. The website would be managed by designated personnel from UPHSSP and after the project period it shall be managed by GoUP. To be read as d)for Payment Category 2 linked to Turn (TAT) Payment of [10%] of the claimed Gross Amount on submission of Invoice (supported with individual TAT compliance report) by the service provider which will be verified by the hospital pathologist and counter signed by the CMS / SIC / DIRECTOR of the respective hospital as per the following criteria:- TAT Penalties Compliance > 95% No Penalty 5% deduction on the 85% - 95% value of the Lab Tests invoiced in that month 10% deduction on the 70% - 85% value of the Lab Tests invoiced in that month 20% deduction on the 50% - 70% value of the Lab Tests invoiced in that month. 25% deduction on the value of the Lab Tests invoiced in that month. Service provider will be asked to stop providing services after 3 < 50% warnings during the contract period with all payments and performance bank guarantee being forfeited The TAT would be calculated starting from the time is taken from the patient upto successfully uploading the test result on the designated website. Health System Strengthening Project, Training Block-2, SIHFW Campus, Block C, Indira Nagar, Lucknow U.P. India Annexure A-Updated Indicative List of high end tests (attached at Annexure 1) which are to be provided by the bidder at 7 District Hospitals. For calculation of TAT compliance, additional logistics (marginal) time shall be taken as per the following:- a. For Schedule No 5 [Shri Ram Chikitsalaya, Ayodhaya, Faizabad] 3 hours b.
