2 PSAP Paralysis Start Exercise During the exercise it will be important to remember and comply with the following statements: It is important NOT to jump ahead in the exercise scenario. In other words don t focus on what might happen, focus on the Common Operating Picture that is presented during the exercise to make informed decisions. In any exercise a number of assumptions and artificialities may be necessary to complete play in the time allotted. During this exercise, the following apply: 1. The scenario is plausible, and events occur as they are presented. 2. There is no hidden agenda, nor any trick questions. Exercise Basics: 1. This exercise contains a total of 11 scenario and inject pages. 2. The exercise is self-paced and it should take approximately three hours to complete. However, based on the time spent on each scenario and inject the exercise may be completed in under three hours or it may take much longer than three hours to complete the exercise. 3. There is a scheduled 15 minute break approximately half way through the exercise. 4. Thoroughly review all scenario information and injects as they are presented during the exercise. 3 PSAP Paralysis Scenario # 001 Image of Karst For the past several days the level of incoming calls to the Holmes County Public Safety Access Point (PSAP) has been below normal. Day 1 Friday At 2:45PM the local forecast office of the National Weather Service issues a Severe Thunderstorm Watch for most of Northwest Florida including Holmes County. Throughout the day anticipation of tonight s Holmes County High School vs Freeport High School football game has been building. Students, parents, teachers and local football fans are expecting an exciting game between the Blue Devils and the Bulldogs. At 8:17PM a motorist driving south on Waukesha St notices a large hole on the west side of the street just north of the Bonifay Fire / Police Station. The motorist makes a mental note of the hazard but decides not to call anyone. Day 1 Friday 8:37PM Over the next 20 minutes a very large sinkhole has opened consuming all of Waukesha St and a large portion of the parking area just north of the Bonifay Fire / Police Station. In addition to Waukesha St collapsing into the sinkhole, a utility pole supporting electrical and telephone lines and the city s water and sewage main have all been devoured by the sinkhole. 4 PSAP Paralysis Inject # 002 Image of Phone Suddenly the lights in the PSAP flicker as the Bonifay Fire / Police Station loses electrical power and the building s auxiliary generator comes on line. The PSAP s on-duty Communications Officer is in the middle of receiving information from a 911 caller regarding a house fire located in Westville when the phone line goes dead. Following standard procedures the Communications Officer tries to re-establish contact with the 911 caller but notices the phone does not have a dial tone. Describe in detail the immediate actions that will need to be taken by the Communications Officer at this point. (Note: In other words describe what actions will be taken regarding losing communications with the 911 caller and the PSAP totally losing telephone communications.) 5 PSAP Paralysis Inject # 003 Image of Football Game Day 1 Friday 8:43PM As the Holmes County High School vs Freeport High School football game begins its 4th Quarter a fast moving thunderstorm with wind gusts up to 58 MPH starts to move over the football stadium. As football players and fans take cover from the storm a brilliant flash of lightning strikes the crowd injuring 14 individuals. Within moments numerous individuals are calling 911 to report the incident. With the Holmes County PSAP temporarily un-able to answer incoming 911 calls, describe in detail how first responders will be notified of the incident at the football stadium. 6 PSAP Paralysis Inject # 004 Image of Evacuation Day 1 Friday 8:51PM As the sinkhole continues to grow, the threat of the Bonifay Fire / Police Station collapsing into the sinkhole s crater forces the building to be evacuated. As the building is being evacuated the auxiliary generator is shut down due to safety reasons. Identify the location that will be immediately used to temporarily house the Holmes County PSAP. Identify the primary, secondary and tertiary individuals by name along with their contact information (primary and secondary phone numbers) that will need to be contacted to gain access to the identified location. Once contact is made with the appropriate individual how long will it take to gain access to the identified location? (Note: Remember it is now almost 9:00PM on a Friday.) 7 PSAP Paralysis Inject # 005 Image of Staff The normal Friday night shift at the Holmes County PSAP calls for a single Communications Officer to be on duty. The on-duty Communications Officer is almost instantly overwhelmed as the current series of events begins to occur. Is there a written set of guidelines and / or procedures for requesting additional PSAP staffing support after normal business hours? If the above answer is yes. Describe in detail the processes for requesting additional personnel to staff the PSAP. Yes No If the above answer is no. Briefly explain why there is not a written set of guidelines and / or procedures then describe in detail the process for requesting additional PSAP staff. 8 PSAP Paralysis Inject # 006 Image of Notification Upon the PSAP losing telephone communications and having to evacuate the building will the on-duty Communications Officer need to make contact with any or all of the county s emergency response agencies (Sheriff s Office, Fire Departments, EMS, Police Department, etc.)? If the above answer is yes. Describe in detail the process for notifying each agency. Yes No Prepare a prioritized list of emergency response agencies that will need to be contacted and list two means for contacting each agency. If the above answer is no. If the above answer is yes. Prepare a list of all other agencies, organizations and / or companies that will need to be contacted and state the reason for making contact with the identified entity. 10 PSAP Paralysis Inject # 008 Image of Relocation Day 1 Friday 9:43PM Contact has been made with all of the appropriate agencies, organizations and companies, each being notified of the current operational status of the Holmes County PSAP. The attention of the PSAP s on-duty Communications Officer now turns to the relocation of the PSAP to the identified alternate location. Is there a written set of guidelines and / or procedures regarding the relocation of the PSAP to an alternate location? If the above answer is yes. Describe in detail the process for relocating the PSAP to the alternate location. (Note: In describing the process identify the amount of time it will take to accomplish each action.) Yes No If the above answer is no. Briefly explain why a written PSAP relocation plan does not exist then describe in detail the process for relocating the PSAP to the alternate location. 11 PSAP Paralysis Inject # 009 The PSAP has been relocated to the alternate location. Image of Resources Due to the speedy evacuation of the Bonifay Fire / Police Station and the hazard of re-entering the building due to the threat of the station collapsing into the sinkhole none of the equipment, supplies, procedural documents, etc. currently housed at the PSAP are available. Is a back-up cache of equipment, supplies, documents, etc. available? Yes No If the above answer is yes. Identify the location where the cache is stored and itemize all of the material assets that are contained in the cache. Prepare an itemized list of any additional material resources that need to be added to the cache so that the PSAP can relocate with a minimum of interruptions. If the above answer is no. Briefly explain why the needed cache of resources is not available. Prepare an itemized list of materials resources that will be needed to efficiently and effectively operate the PSAP at the alternate location. 12 PSAP Paralysis Inject # 010 Image of Mobile 911 Center Based on current information and the fact that the Bonifay Fire / Police Station will remain unusable until the sinkhole is stabilized will there be a need for additional communications equipment and / or assets via the Statewide Mutual Aid Agreement (SMAA)? If the above answer is yes. Describe in detail how the needed equipment will be accessed via the SMAA. Yes No Identify all potential communications equipment and / or assets that will be needed in the days and possibly weeks ahead. 13 PSAP Paralysis Inject # 011 Image of Demobilization Day 22 Wednesday 11:27AM Over the past several weeks the Florida Department of Transportation has stabilized the sinkhole and repaired Waukesha St. The Holmes County Building Inspector has inspected the Bonifay Fire / Police Station and issued a certificate of occupancy indicating the structure is safe to relocate the PSAP back into the building. Prepare an Incident Action Plan for relocating the PSAP back to the Bonifay Fire / Police Station. In developing the IAP list the objectives; identify all resources that will be needed to accomplish each listed objective; and list the expected timeline to complete each identified objective.
