CMAC212 Session 2 Five Shu Points Chinese Medicine
Session 2 - Objectives o Understand the value & clinical relevance of point groupings. o Gain an in-depth understanding of the five shu points including their actions and clinical applications. Endeavour College of Natural Health 2
Point Classification o Why are points grouped into different classifications? _who, what, where, when, why_? (Vitellozzi, 2006). Endeavour College of Natural Health 3
Point Classification o Many point classifications happened historically and reflect the practice of acupuncture at that time. o Point groupings provide important clinical clues as to a points function and use. Endeavour College of Natural Health 4
The Five Shu Points
Session 2 You need to have read the discussion about shu points pages in A Manual of Acupuncture by Deadman & Al-Khafaji, Endeavour College of Natural Health 6
The Five Shu Points What are they? What do they do? How and when do we use them? Endeavour College of Natural Health 7
The Five Shu Points The five shu points are the most commonly used acupuncture points. Having a thorough knowledge of them and their application hugely informs the development of a good point prescription. Endeavour College of Natural Health 8
Self-directed learning o You need to memorise every shu point on every channel including which element they belong to. For example Liver 2 is the ying spring & fire point on the Liver channel. o You will need this information for the rest of your acupuncture career & the quicker you learn these point categories the easier life will be o There is a point table included as a handout under week 1 on LMS. Put it somewhere you will see it everyday. Endeavour College of Natural Health 9
Qi-gong_brocart-old-3 (No-Stress, 2013). Historical Context The Five Shu points were first mentioned in the Ling-Shu (Spiritual Pivot), a section of the Huang Di Nei Jing (Yellow Emperor s Inner Classic), created between the Warring States period ( BCE) and the early Han period (206 BCE 220 CE) If you are interested in reading the original discussion see the Ling Shu translation by Wu Jing-Nuan (2004. pp.8-13) Scroll One The roots of the acupuncture points. Endeavour College of Natural Health 10
The Five Shu Points o The Five Shu points are extremely important and make up a large portion of clinical practice. o They are situated below the elbows and knees where the Qi is more dynamic and therefore able to affect change within the body more rapidly. Endeavour College of Natural Health 11
The Five Shu Points o The Five Shu describe a flow of Qi within the body which is different to the normal flow of Qi through the 12 main meridians. o It describes a flow of qi from Heaven into Human rather than the circulation of qi through the normal meridian circuit. (Think of the image of a person standing on the earth with arms outstretched). Endeavour College of Natural Health 12
Krka Waterfall Croatia Nature Park (fjaka, 2015) Five Shu Points Jing-Well Ying-Spring Shu-Stream Jing-River He-Sea Endeavour College of Natural Health 13
Lets explore how the Qi flows through these points. Endeavour College of Natural Health 14
5 shu on arm (adapted from Deadman & Al-Khafaji, 2007). The flow of Qi through the Meridian The flow of Qi can be likened to a river, emerging through a well, starting to flow at the spring and gathering in width and depth until it enters inwards at the Sea. Endeavour College of Natural Health 15
Jing-Well & Ying-Spring o The flow of Qi emerges from the tips of the fingers or toes or the bottom of the feet. (This may be through the points at the end or start of the meridians). o These points are called Jing-Well and the Qi is described as bubbling. o The Qi then flows towards the Ying-Spring points which are located between the webbing of the fingers and toes or at the same level. o These are the points where the Qi is described as gliding. Endeavour College of Natural Health 16
Shu-Stream o Shu-Stream are the points where the qi pours through or flourishes. o On yin channels the Shu-Stream points are also the Yuan Source points. Endeavour College of Natural Health 17
Jing-River & He-Sea o The Qi continues to flow from the extremities inwards and the next Five Shu is the Jing-River. Jing-River points are located proximal to the ankles or wrists. o Whereas before the qi may have been described as trickling the qi now flows. o The He-Sea represents the uniting of the rivers into the Sea and this is where the Qi goes inwards. o He-Sea points are located around the elbows and knees. Endeavour College of Natural Health 18
Class Discussion o How does the flow of Qi through the Five Shu points differ from the normally considered flow of Qi through the meridians? o The flow of qi through the five shu points is conceptualised as flowing from the extremities to the centre rather than following the normal numbered circuit. (See Deadman & Al-Khafaji, 2007, p.30 for a detailed discussion). Endeavour College of Natural Health 19
The Five Shu Points & their Functions
Class Activity o As we discuss the shu points consider using coloured pens to locate each of these points. o Pay attention to their functions and consider how their 5 shu grouping and element may contribute to their super powers. o These are the points you will be using most frequently in practice so you NEED to have a thorough understanding of them. Endeavour College of Natural Health 21
Jing Well Points Yin channels = Wood Yang channels = Metal o Used to revive consciousness o Able to strongly clear heat (they are often bled for this purpose). o Treats diseases at the upper reaches of the channel, (a distal approach to treatment). Endeavour College of Natural Health 22
Classical Use Jing Well Points o Treats fullness below the heart 68 th difficult issue from the Nan Ching (1986, p.577). o When the disease is at the zang, needle the jing-well point chapter 44 of the Ling Shu (2004, p.157). o Needle in Winter - classical indication from chapter 2 of the Ling Shu (2004, p.13). 26 Ying Spring Points Yin channels = Fire Yang channels = Water These points clear heat in the body. Endeavour College of Natural Health 26
Class Discussion Why are Ying Spring points good at clearing heat? o They are either fire points (Yin channel for draining) or water points (yang channel for cooling). o They are also located distally and are easily able to clear pathogenic qi from the body. o REMEMBER: Fire points are used to drain fire and clear heat, not to tonify yang qi. Endeavour College of Natural Health 27
Ying Spring Points o Ying-Spring points of the yin channels can treat disorders of the zang (as well as the shu-stream) (Deadman & Al-Khafaji, 2007, pp ). o If the complexion changes in colour, needle the yingspring point - chapter 44 of the Ling Shu (2004, p.157). o Treat diseases of the yang channels especially full pathogenic factors such as stagnation and heat (Deadman & Al- Khafaji, 2007, pp ). o Needle in Spring time - chapter 2 of the Ling Shu (2004, p.13). 49 Chinese Medical Diagnostic Method Refresher
How do we diagnose in Chinese Medicine? Observation Shen, Demeanour, Movements, Channels, Tongue, Skin etc. Interrogation Asking the Right Questions, Identifying Symptoms. Palpation Pulse, Channels, Points, Abdomen, Skin. Listening and Smelling Listening to the breathing, coughing, voice etc. What does a patients breath or body odour smell like? Endeavour College of Natural Health 50
How might our diagnostic method impact on our point selection? Observation Observation may lead us to a specific point category. For example a patient with a disturbed Shen may lead us to consider a Sun Si Mao ghost point such as PC 8. Crazy man deserves a crazy edit (Cajandig, 2015). It may also lead to the employment of a specific approach e.g. a patient with a red face and lots of heat rising may lead you to a point prescription that utilises the idea of needle below to treat above for example KI 1. Endeavour College of Natural Health 51
Szerelmi gyikossagok (Kommunikalco, 2013). How might our diagnostic method impact on our point selection? Interrogation Identifying a particular symptom may lead us to an empirical point e.g. PC6 for nausea. It may also lead us to a internal pattern made up of multiple symptoms and we may apply a Zang Fu approach to treatment. Endeavour College of Natural Health 52
EricEisenmann_Anma_Massage_0489 (Eismane, 2014). How might our diagnostic method impact on our point selection? Palpation Palpation helps us to identify channel involvement and a Jing Luo approach may become the crux of our treatment strategy. Palpation of points along a specific channel may further help us refine our point prescription. Endeavour College of Natural Health 53
How might our diagnostic method impact on our point selection? Hearing and Smelling A quiet voice reflects lung deficiency and might lead us to a constitutional approach to treatment addressing the ben (root). Any point suggestions? Relics of a bygone age sounds you ll probably never hear again (Townsend, 2013). Smell might lead you to pathology based approach using point indications. For example a stinky patient may have a systemic damp heat problem so your point selection may include points that focus on clearing this pathology. Smell my shoe (Moss, 2014). Endeavour College of Natural Health 54
How might our diagnostic method impact on our point selection? o Any other ideas????? Endeavour College of Natural Health 55
Pre-Readings Next week we are doing 5 element treatments. Some people find this tricky to get there head around. You need to read 2 sources that discuss this topic before next week and bring any questions along. (Not the Worsley schools entry/exit approach as this will be discussed later). Here are some possible sources: Chapter 9 Five Element treatments in Ross,J. Acupuncture Point Combinations: The Key to Clinical Success Philadelphia. U.S.A. Churchill Livingstone. The Nan-Jing this is the book where 5 element treatments are first mentioned. Meridian therapists use five element theory to dictate their point selection, so you may find some discussion of this theory in scholarly articles that reference Japanese acupuncture. Happy Hunting!! Endeavour College of Natural Health 56
Assessment Checkpoint Have you brought along the case study you plan to use for your assignment next week so we can ensure you are on the right track? Endeavour College of Natural Health 57
