Accurate diagnosis, risk stratification, and optimal use and duration of appropriate antibiotics are the cornerstones for its management. Procalcitonin (PCT) is a useful marker for bacterial infection used to guide antibiotic therapy in lower respiratory tract infections (LRTI) including communityacquired pneumonia (CAP). In the light of inherent limitations of culture-guided management, nonspecific radiologic findings, prohibitive cost of antibiotics and presence of other co-morbidities that mimic infections, PCT may be a promising tool for antibiotic therapy decision-making algorithms. However, it is expensive and available locally in very few health facilities. Its routine use for clinical decision making whether to start, continue or discontinue antibiotics in the local clinical setting needs further discussion.
