18 The fact is.. Postoperation: 50-70% patients experience severe pain 20-40% experience moderate pain following surgery

20 Reasons for ineffective analgesia (1): Pain is merely a symptom, not harmful Fear of potential for addiction Concerns about opioidrelated side effects:- resp. depression, nausea/vomiting, pruritus Do have adverse effects No evidence Can be avoided by - titration - individualization

34 Clinical Indicators of Respiratory Depression Sedation score 0 = none 1 = mild, occasionally drowsy, easy to arouse 2 = moderate, constantly or frequently drowsy, easy to arouse 3 = severe, somnolent, difficult to arouse S = normally asleep, easy to arouse Respiratory rate < 8/min => respiratory depression but unreliable Oxygen saturation may also unreliable, esp. receiving supplemental oxygen

Sedation ไม ใช score< 2? รายงานแพทย งดให ยาจนกว า ใช ไม ใช RR > 8 / min? รายงานแพทย คอย 5 นาท ไม ใช ให ยา 1 มล. ใช BP normal? ใช ได ยา < 3 คร งใน 15 นาท ใช อาย <70 ป? ใช ปวดมาก? Pain score > 7? ใช ให ยา 2 มล. ไม ใช ไม ใช ไม ใช รายงานแพทย รายงานแพทย ไม ใช ให ยา 0.5 มล. sedation score < 2 และ RR > 8 คร ง/นาท (อาจต องเตร ยมให ยา naloxone คร งละ 0.1 mg I.V.) ปวดมาก?
