2 Chief of Audiology (July 2006 to January 2018) 1) Responsible for all oversight of audiology clinical division 2) Monitor wait lists, no show rate, clinical productivity and clinical assignments. 3) Develop plans for new hires, schedules and reassignment of clinical duties. 4) Evaluate need for new/additional equipment to ensure state of art clinical services could be provided at Shands/UF audiology centers. 5) Work closely with audiologists to market hearing devices. 6) Work with Shands Management Team to verify compliance of clinical operations with Joint Commission standards. 7) Collaborate with Shands Clinical Coordinator to ensure compliance of employee files, proper orientation of new employees and employee competencies. 8) Collaborate with Shands Clinical Coordinator to ensure proper CPT codes, diagnosis codes and Joint Commission standards are included in each patient record. 9) Moderates regular division meetings 11) Generates and evaluates clinical schedules 12) Generates and maintains audiology doctorate student clinical schedules 13) Provides clinical services in area of pediatric and otolaryngology clinics. (Responsible for 30% clinical time; however cover 60% due to personal preference). 14) Meets with Chair of Department to discuss overall clinical operation. 15) Oversees front desk operation as it relates to clinical operation. 16) Schedule and maintain weekend call for Universal Newborn Hearing screenings on Weekends and holidays at Shands Hospital. 17)Maintain leave calendar to ensure adequate clinical coverage in all audiology staffed clinics. 18) Ensure all audiology equipment is in good working order and proper maintanence and Calibrations are complete. 19) Provide orientation to all incoming audiology doctorate students regarding requirements needed to be a student clinician. 20) Provide clinical services in area of pediatric and otolaryngology clinics (~60%clinical time) Clinical Audiologist During my time as a full time clinical audiologist, I had the highest clinical productivity, based on clinical revenue, in The Department of Communicative Disorders from July 1995 to July Clinical Experience UF Otolaryngology Clinics (1995-Present) Provide extensive audiological evaluations including full diagnostic audiograms, diagnostic auditory brainstem response testing, acoustic reflex testing, and visual reinforcement audiometry. Treat approximately patients daily during these clinics. Work closely with

3 Otologists including Dr. Patrick Antonelli, Dr. John Harwick and Dr. William Collins to collaborate treatment when re/habilitative services are needed. Infant/Pediatric Hearing Program (1995 to Present) Involved in initiation of Universal Hearing Screening program at Shands Hospital. Began program with a grant providing funding in Completed 500 infants via IRB study- Part of Statewide committee consisting of audiologists in the State of Florida to discuss protocols and the write up of the bill to Submit to be passed for all infants in state of Florida to receive newborn hearing screenings. Provided hearing healthcare to ages 1-21 years of age. Diagnostic evaluations included conventional auditory brainstem response testing, distortion product otoacoustic emissions, behavioral and visual reinforcement audiometry and tympanometric testing. Habilitative services included choosing/fitting amplification, assistive listening devices and communicating with parents and educational system in regards to needs of child. Craniofacial Clinic (1995- Present) Part of a nationally known group of specialists working with children with craniofacial anomalies. Performed diagnostic audiological evaluations and hearing devices when needed.
