Štamfelj Ključne besede: Hertwigova epitelijska nožnica, anatomska korenina, parodoncij, izraščanje zoba Key words: Hertwig's epithelial root sheath, anatomic root, periodontium, tooth eruption Izvleček Hertwigova epitelijska nožnica je središčni organ v razvoju korenine in pretežno tudi v razvoju obešalnega aparata zoba. Na eni strani je udeležena pri nastajanju dentina, na drugi cementa, obenem pa določa velikost, obliko in število korenin. Rast korenine spremlja razvoj pozobnice in alveolne kosti. redzadnji proces je zapiranje pozobnične špranje z dlesninim pripojem. Izraščanje in obremenitev zoba ter oblikovanje koreninskega vršička so pomembni razvojni mejniki, ki zaključijo razvoj korenine in zoba. bstract Hertwig's epithelial root sheath plays a central role in the development of the root and, to a large extent, of the tooth-supporting apparatus. It is involved in the formation of dentine on one side and cementum on the other. t the same time it determines the size, shape and number of roots. Root formation is accompanied by the development of the periodontal ligament and alveolar bone. The penultimate process is closing of the periodontal space with gingival attachment. Tooth eruption, functional loading, and formation of the apex represent important final milestones in the development of the root and the tooth.

Štamfelj 49 Uvod ri razvoju zobne krone nastajajo sočasno sklenina, dentin in pulpa, pri razvoju korenine pa šest tkiv -- poleg pulpe in dentina še cement, pozobnica, alveolna kost in dlesen. ri njihovi tvorbi so udeležena štiri razvojna tkiva: zobna papila, zobna vrečica, Hertwigova epitelijska nožnica (HEN) kot naslednik skleninskega organa in ustni epitelij. Hertwigova epitelijska nožnica V razvoju zobne korenine ima središčno vlogo HEN, ki ima trojno nalogo: določi obliko, velikost in število korenin, spodbudi razvoj odontoblastov ter verjetno sodeluje v cementogenezi, s tem da sproži pojav cementoblastov in izdela prvo plast cementa (Eşian in Monea, 2011). HEN je preobražen skleninski organ. Razvije se, ko je zgrajena sklenina na zobnem vratu (Sliki 1 in 1B). Sestavljata jo samo zunanji in notranji skleninski epitelij in kot majhen celični obroček obdaja vršiček razvijajoče se korenine. Skleninska pulpa in vmesni sloj manjkata in celice notranjega skleninskega epitelija HEN se ne razvijejo v ameloblaste. HEN ločuje oba ektomezenhima -- zobno papilo in zobno vrečico -- ter deluje v dve smeri; ob zobni papili spodbudi razvoj odontoblastov, ob zobni vrečici pa sodeluje pri razvoju cementa (Zeichner-David in sod., 2003; Yamamoto in sod., 2004). Da HEN zadrži nekaj skleninotvorne sposobnosti, dokazujejo skleninski biseri, ki se razvijejo, če se HEN ne odmakne od dentinske površine v korenini. Na tistem mestu se lahko pod vplivom dentinskih sestavin ponovno pojavita skleninska pulpa in vmesni sloj, s čimer HEN postane majhen skleninski organ (Eşian in Monea, 2011). Razvoj korenine usmerjajo spodbujevalne molekule, ki si jih izmenjujeta ektoderm (HEN) in ektomezenhim (zobna papila, zobna vrečica) (Luan in sod., 2009). Usmerjevalne molekule iz zobne papile in zobne vrečice, npr. IGF1 in BM4, spodbujajo delitev celic v HEN in s tem njeno rast v apikalni smeri. Celice HEN ostanejo pri tem navidez značilne epitelijske celice. Spodbujajo razvoj odontoblastov in verjetno tudi cementoblastov ter izdelujejo nekaterim skleninskim beljakovinam podobne organske snovi (Eşian in Monea, 2011). Takšne organske snovi vsebuje preparat emdogain, ki se uporablja za spodbujanje cementogeneze pri obnovi parodoncija. Razvoj korenin stalnih zob je več kot enkrat daljši od razvoja korenin mlečnih zob, čeprav dolžina korenin stalnih zob ni toliko daljša. ri vseh zobeh se del korenin razvije, preden zobje izrastejo v ustno votlino (modra črta), preostali del pa po izraščenju (rdeča črta). Od tri do pet let po tem, ko so korenine mlečnih zob izgrajene, se začnejo resorbirati (zvezdica). Razvoj tretjega kočnika ni prikazan. Korenina se razvija v času, ko zob izrašča proti ustni votlini. Sile in rastni pritiski, ki razvijajoči se zob premikajo, ne razobličijo mehkih tkiv nastajajoče korenine, ker jih varujejo okolna tkiva, tako da se dedni nadzor oblikovanja korenine, ki ga izvajajo celice HEN, v polnosti izrazi. Eden od kazalnikov normalnega razvoja zoba je pravilen potek skleninsko-cementnega stika, ki je za vsak zob značilen, ima pa nekaj skupnih lastnosti, med katerimi je ta, da je skleninsko-cementni stik aproksimalno izbočen v okluzalni smeri, bukalno in palatinalno pa proti apeksu. Le takšen skleninsko-cementni stik omogoča pravilno oblikovanje medzobne papile in marginalnega parodoncija. Variabilnost skleninsko-cementnega stika je majhna in je vidna le na mikroskopski ravni. Dedni nadzor razvoja pulpo-dentinskega sklopa kot jedra zoba v kroni in korenini ne poteka po popolnoma enakih poteh, kar se kaže pri nekaterih razvojnih napakah, kot je dentinska displazija II, kjer krona in korenina nista enako močno prizadeti. Ko je zgrajena prva plast dentina v korenini, HEN začne propadati, njene celice se nekoliko odmaknejo od dentinske površine in nadomestijo jih cementoblasti. Medtem se HEN zaradi celičnih delitev apikalneje podaljša in na tem mestu sproži razvoj odontoblastov, nato pa začne propadati. To postopno odmiranje in podaljševanje HEN v apikalni smeri se nadaljuje do konca razvoja korenine. HEN tako ostaja ves čas razvoja korenine enako dolga, približno 0,1 mm, manjša se le njen premer. ri tem bazalna lamina pod notranjim skleninskim epitelijem HEN na vsakem mestu zavzema mesto bodoče dentinskocementne meje (Luan in sod., 2009). Življenjska doba HEN oz. trajanje razvoja korenine je pri stalnih zobeh 5--8 let, od tega približno 2/3 časa

Razvoj korenin pri mlečnih zobeh je krajši, vendar ne sorazmerno z dolžino korenin. osebnost pri mlečnih zobeh je resorpcija korenin, ki se začne od 3 do 5 let po zaključenem razvoju. V nekaj letih mlečni zobje izpadejo. Lahko bi se reklo, da je nenavadno, da v teku evolucije organizem ni iznašel manj potratne menjave mlečnih zob s stalnimi. Na robu apikalnega, razvijajočega se dela korenine v dejavnostnem smislu potekajo bistvene lastnosti zgodnjega razvoja, to so proliferacija celic (rezultat je podaljšanje HEN), morfodiferenciacija (HEN zasede mesto bodoče cementno-dentinske meje in s tem določi obliko ter velikost in število korenin) in histodiferenciacija (razvoj odontoblastov in cementoblastov). Čeprav je HEN mikroskopsko majhen organ, je trdoživo tkivo s širokim biološkim potencialom. To dokazujejo primeri poškodovanih zob, pri katerih je HEN kljub okvari ohranila sposobnost usmerjanja razvoja korenine (Sliki 2 in 2 B). HEN in njeni ostanki v pozobnici (Malassezovi epitelijski ostanki) predstavljajo potencialno zalogo progenitorskih celic. Značilne oblikovne lastnosti razvijajoče se korenine na rentgenski sliki so: kratka korenina, tanke dentinske stene in obsežna pulpina votlina ter široka, lijakasta apikalna odprtina. Normalno potekajoč razvoj odebeli dentinske stene, zoži pulpino votlino in oblikuje vršiček korenine. Oblikovne lastnosti koreninskega vršička ali apeksa so še posebej pomembne pri kliničnem delu in njegov razvoj označujemo s posebnim izrazom -- apeksogeneza. Koreninsko deblo večkoreninskih zob nastaja do razcepišča enako kakor enojna korenina. Na ravni razcepišča pa HEN požene vodoravne epitelijske jezike -- dva za dvokoreninski zob in tri za trikoreninski zob. Epitelijski jeziki rastejo drug proti drugemu in ko se na sredini zrastejo, sprožijo razvoj odontoblastov, ki ustvarijo dno pulpinega prekata. Epitelijski jeziki razdelijo prvotno HEN na dve oz. tri manjše HEN, za vsako korenino po eno. Razmeroma pogosto na tej stopnji razvoja korenine HEN ne požene podaljškov in njen premer se proti apeksu ne zmanjšuje, kar vodi v pojav taurodontne korenine. Korenina je nekoliko krajša kakor na desnem prvem stalnem sekalcu, pulpina votlina je v celoti mineralizirana (puščica). Tvorba zobnega cementa O razvoju zobnega cementa je znanega bistveno manj kakor o razvoju dentina in sklenine. o klasični domnevi se cementoblasti razvijejo iz nediferenciranih ektomezenhimskih celic zobne vrečice, ki se dotaknejo površine dentina, od katere se je HEN predtem odmaknila in začela propadati (Hai in sod., 2010). Razvoj cementoblastov naj bi spodbudile beljakovine v še nemineralizirani površinski plasti dentina korenine ali skleninskim beljakovinam podobne snovi, ki jih izdelajo celice HEN (Kim in sod., 2009). Nekateri avtorji sklepajo, da se celice HEN verjetno preobrazijo v cementoblastom podobne celice in izdelajo prvo plast cementa (Tummers in sod., 2007; Fleischmannova in sod., 2010). Organski matriks ima pri cementu v glavnem enako sestavo kakor pri dentinu. omembna razlika je dvojno poreklo kolagenskih vlaken: cementoblasti izdelajo t. i. notranja kolagenska vlakna, pozobnični fibroblasti pa zunanja ali Sharpeyjeva vlakna. Notranja kolagenska vlakna potekajo vzporedno s površino korenine, Sharpeyjeva vlakna pa pod kotom. Tudi mineralizacija cementa v osnovi poteka podobno kakor pri dentinu. Cementoblasti

5 Tvorba zobnih tkiv v korenini 52 pripravijo organski matriks za mineralizacijo (dozorevanje kolagena, odstranjevanje organskih snovi, posebno zaviralcev mineralizacije, in vode) in dovajajo mineralizacijske ione. Za prvo plast cementa, ki se dotika dentina, imajo ojedrevalno vlogo verjetno kristali dentina, poznejši ojedrevalci so organske molekule v cementnem matriksu, npr. osteopontin. Najprej se začnejo mineralizirati Sharpeyjeva vlakna, nato notranja kolagenska vlakna in nazadnje medvlakenska osnovna snov, zato meja med cementom in precementom ni ravna. Cementoblasti skupaj s pozobničnimi fibroblasti izdelajo različne vrste cementa, ki se razlikujejo glede prisotnosti cementocitov in vrste kolagenskih vlaken. ri počasnem nastajanju cementa izločajo cementoblasti snovi samo na koreninski strani; cementoblasti se lahko odmikajo in ne zaostajajo v cementu, zato nastaja necelični cement, ki ima veliko Sharpeyjevih vlaken in služi pričvrstitvi zoba v alveolo ter prenašanju griznih sil. To je prvi cement, ki se naloži na korenino. ri hitri cementogenezi izločajo cementoblasti snovi na več straneh hkrati, tako da so kmalu ujeti v cement in se spreminjajo v cementocite. Nastaja celični cement, ki ima različno število cementocitov, notranja vlakna in manj zunanjih kolagenskih vlaken kakor necelični cement. Hitro nastajajoči cement služi predvsem vzpostavljanju pravilne širine pozobnične špranje pri razjedah in premikih zoba. V cementocite spremenjene cementoblaste nadomestijo novi cementoblasti, ki se razvijejo iz progenitorskih celic v embrionalnih ostankih v pozobnici ali izvirajo iz kostnega mozga. Na posameznih mestih vratne sklenine lahko obstajajo tudi tanke luske cementa, ki nima ne cementocitov ne kolagenskih vlaken. Izdelajo ga verjetno cementoblastom podobne celice, če zakrneli skleninski organ ne varuje sklenine; njegova vloga ni poznana. Cement in dentin korenine nastajata sočasno, pri tem se organska matriksa dentina in cementa sproti zraščata, mineralizacija pa povezavo dokončno utrdi. Hkrati se cement s Sharpeyjevimi vlakni povezuje s pozobnico, ki se na drugi strani s kolagenskimi vlakni zrašča z alveolno kostjo. (Sliki 3 in 3 B). Razvoj pozobnice in zobne pulpe redhodnica pozobnice je zobna vrečica ali zobni folikel, ki ob apeksu nastajajoče korenine sočasno s tvorbo cementa in alveolne kosti dozoreva v pozobnico (ElNesr in very, 2002). Bistvo te spremembe je v zamenjavi nezrelih kolagenskih vlaken, ki obkrožajo zobni zametek, z zrelimi kolagenskimi vlakni, zraslimi s cementom in alveolno kostjo. Fibroblasti pozobnice z izdelavo kolagenskih molekul vlakna, ki so vpeta v cementu in kosti, a so na začetku kratka, daljšajo proti sredini pozobnične špranje. Tu se vlakna s cementne in kostne strani razvejijo in med seboj povežejo. Izraščenje zoba v ustno votlino, njegova obremenitev in dokončno oblikovanje apeksa

Ob obremenitvi po izraščenju zoba se število in debelina kolagenskih vlaken v pozobnici zvečata, vlakna se tudi dokončno usmerijo in zberejo v štiri skupine: zobno-dlesnina in medzobna vlakna, kostno-dlesnina ter zobnoalveolna vlakna, ki so najštevilčnejša. Drobna kolagenska vlakna, oksitalanska vlakna in intersticijski prostori pozobnice se izoblikujejo, ko je zob obremenjen. Fibroblastom, najštevilčnejšim celicam pozobnice, se verjetno po izraščenju zoba v ustno votlino pridružijo vnetnoimunske celice in tkivni bazofilci. rogenitorske celice ob žilah so ostanek nediferenciranih ektomezenhimskih celic, prihajajo pa tudi iz mozgovnih prostorov. Iz njih se poleg cementoblastov razvijejo fibroblasti in osteoblasti. Osteoklasti nastanejo iz krvnih monocitov (Huang in sod., 2009). Žilje in živčevje pozobnice se razvijata sočasno z žiljem in živčevjem pulpe. ri tem se avtonomno in somatično živčevje razvijata ločeno, verjetno trigemično oživčenje prehiteva simpatično (Eşian in Monea, 2011). Žile in živci se pojavijo ob zobnem zametku v dobi brsta. V dobi čepice sta dva kapilarna pleteža -- v zobni vrečici in zobni papili; v zobno vrečico vstopijo živčna vlakna. V dobi zvona vrastejo arterije in vene v zobno vrečico in zobno papilo, v zadnjo tudi živčna vlakna. Kapilarna mreža v pulpi je najobsežnejša v kroni, ko se razvijejo odontoblasti in na začetku dentinogeneze. Limfno žilje zobne pulpe in pozobnice se verjetno razvije po izraščenju zoba v ustno votlino ali najpozneje, ko je zgrajena koreninska konica. S pojavom pododontoblastičnega pleteža po dograditvi koreninske konice je živčevje v pulpi dokončno razvito (Moe in sod., 2008; Zmijewska in sod., 2003) (zato šele takrat postane ocena stanja zobne pulpe z vitalitetnim preskusom zanesljiva). V skleninskem organu ni ne žil ne živcev in celice se prehranjujejo z difuzijo iz zobne papile in zobne vrečice (orto in sod., 2009). reobražanje zobne vrečice v pozobnico in zobne papile v pulpo (v korenini ima le enocelično plast odontoblastov, brez s celicami bogate in revne plasti) se postopno širi v apikalno smer in se zaključi, ko je 1--3 leta po izraščenju zoba v ustno votlino zgrajena koreninska konica. Ob izraščanju v ustno votlino imajo zobje zgrajene od pol do dve tretjini korenine. ozobnica mlečnih zob in stalnih kočnikov je takrat v grobem razvita po vsej dolžini zgrajene korenine, pri stalnih nadomestnih zobeh pa bistveno manj in šele čez nekaj časa dohiti pozobnico drugih zob. Ta "zamuda" je klinično pomembna, saj obremenitev stalnih nadomestnih zob takoj po izraščenju hitro lahko preseže dovoljeni prag in povzroči poškodbe. Razvoj alveolne kosti in dlesni Kostna votlina, v kateri je zobni zametek zgodnjega razvoja, še ni alveolna kost. Ta nastaja skupaj s korenino, razvojem cementa in pozobnice. Izdelajo jo osteoblasti, ki se razvijejo iz nediferenciranih ektomezenhimskih celic zobne vrečice (ElNesr in very, 2002; Fleischmannova in sod., 2010). Nekatere tkivne lastnosti dlesni, npr. poroženevanje, se začnejo pojavljati že veliko pred izraščenjem zoba. Tudi zasnovi epitelijskega in vezivnega pripoja dlesni na zob sta prisotni, že preden zob izraste. natomsko se dlesen oblikuje, ko zob izrašča v ustno votlino, vzporedno dozorevata tudi epitelijski in vezivni pripoj, od katerih vsak zavzema približno polovico biološke širine pripoja (Slika 4). ripoj je rezultat usklajene rasti epitelija, pozobnice, cementa in alveolne kosti. ri vzpostavljanju normalnega pripoja dlesni po operaciji parodoncija ali pri vsadkih je kritičen dejavnik počasna rast cementa. Epitelij zato zasede preveč površine na zobu/vsadku in ne dovoli razvoja vezivnega pripoja. Zaključek Ob razvoju korenine nastaja korenina v ožjem pomenu besede ali anatomska korenina, ki jo sestavljajo dentin, cement in pulpa, obenem pa nastaja obešalni aparat s cementom, pozobnico, alveolno kostjo in dlesnijo. ri razvoju korenine v širšem pomenu besede zato izstopa medsebojna usklajenost razvoja posameznih tkiv.
