Cone beam technology produces highly detailed three-dimensional imaging volume data using a cylinder 40 mm in diameter and 30 mm high. Three-dimensional views axial, coronal and sagittal allow observation of the designated part. It is possible to reslice the image at any angle for optimum viewing. Radiograph Procedure The regional imaging area, cylindrical in shape, is positioned relative to the region of interest by moving the patient chair laterally, longitudinally, and vertically. A small cone-shaped X-ray beam irradiates the image intensifier (I. I.) with a CCD camera for approximately 18 seconds while the C-arm makes one 360 rotation around the region of interest. The accurate raw images, corresponding to the angle of the C-arm, are transferred to the computer connected to the 3D Accuitomo and digitalized. After necessary corrections, a reconstruction algorithm generates high-resolution, three-dimensional images, displayed on the computer monitor. Superhigh Resolution The 3D Accuitomo s resolution is greater than 2 line pairs per mm (MTF10%). The highly detailed images have a voxel size of mm per side. The slice width and pitch can be set to anywhere between to 2 mm. Low X-Radiation Dosage Exposure time is approximately 18 seconds, and the amount of X-radiation is similar to that of a conventional panoramic film exposure. As for the CTDIw value of X-radiation dosage for the head and neck, the X-radiation is one-seventh * the amount compared to a conventional medical CT. Isotropy of the Voxel The voxel is isotropic in all three planes, so that there are no image distortions due to slice pitch or helical pitch. Compact The 3D Accuitomo requires only about 1.6 times ** the space of a dental panoramic X-ray unit (1,620 mm x 1,200 mm). * The CTDIw value of the 3D Accuitomo is compared according to IEC with the recommended X-ray loading factors and CTDIw value of diagnosis reference level for facial and parallel sinus indicated in the ICRP Pub. 87 Annex A. ** In comparison with our Veraviewepocs dental panoramic X-ray unit. 5 The Newest Technology for Acquiring Three-Dimensional Diagnostic Imaging Information Three-dimensional imaging allows for optimal examination of the hard tissue of the dento-maxillofacial region. Generate highly detailed, three-dimensional images with only one x-ray exposure of approximately 18 seconds. Use the dedicated software, i-dixel, to freely set slice angles. Develop the most accurate diagnosis for implants, apical lesions, temporomandibular joints, impaction, etc. The basic technology is provided by NUBIC (Nihon University Business, Research and Intellectual Property Center). Developed in collaboration with Nihon University School of Dentistry and J. Morita Mfg. Corp. Axial Sagittal Coronal 5

Easy-To-Use Image Processing Software on Your Desk With various image processing functions, i-dixel can easily display information, including three-dimensional and two-dimensional data. It also works in a network environment. XYZ Window Move the cursor to display all three planes for any point. Z cursor at point B Z cursor at point A Image after moving the Z cursor from point A to point B (axial image is changed). Setting the angle for a new slice. Reslicing Set a new angle to create any number of new slices. After Reslicing Other Major Features Display histogram Display of the patient direction Distance and angle measurement Density measurement Distance measurement for the space of the slice Surface rendering Intensity modulation (gamma modulation etc.) Filter (Spatial frequency filter etc.) DICOM export (DICOM 3.0 storage class) Slice Window Display a list of three-dimensional views axial, coronal, and sagittal with 1 mm pitch. 7

Simple, Accurate Positioning The three positioning beams and an LCD make positioning easy. Scout images enable even more accurate positioning. Easy Positioning First set each patient s initial position using the three positioning beams and record the position. Next, line up the region of interest in the LCD. The chair automatically moves into the optimum position. X Cursor Z Cursor Y Cursor Three-dimensional image with the region of interest located at the center. * The circles in the images are inserted for illustrative purposes to indicate the center of the region of interest. The actual images do not feature a circle. Scout Images For even more accurate positioning, prepare scout images. After positioning, take two still X-ray images of coronal and sagittal views to confirm that the position is adequate. If not, adjust the image on the computer by placing the cursor at the center of the region of interest in the two scout images. * Taking scout images increases X-radiation dosage by approximately 5 10%. 9
Before Dental Implant Surgery High-resolution images show the detailed structure of the bone trabeculae and the condition of the mucous membrane of the maxillary sinus. Observation of Dental Implant Reslicing allows operators to observe at a vertical plane to the implant. Impaction of Lower Right Third Molar It is easy to observe the three-dimensional relations between the mandibular canal and the impacted lower right third molar. 10
a c d e b Relative Positions of the Mandibular Canal and the Impacted Third Molar a. Pre-treatment panoramic image. 19-year-old female. The root of the third molar interferes with the mandibular canal. b. to j. Images by 3D Accuitomo. c, d, and e. The sagittal images were observed from the buccal to the lingual side, but it was difficult to determine the exact relationship between the mandibular canal and the tooth root. f. to j. The coronal images were observed from the distal to the mesial side, revealing that the mandibular canal passed between the distal root and the mesial root on the lingual side of the third molar. The distal root was in close proximity to the canal (i). f g h i j 11
a b c d e f g h Alveolar Bone Loss a. Pre-treatment radiograph periapical intraoral image. 64-year-old male. Probing revealed a deep palatal pocket on the upper left first molar. b. to k. Images by 3D Accuitomo c. Sagittal image of the two buccal roots does not show bone loss. d. Sagittal image of the center of the tooth shows bone loss at the bifurcation of the mesial root and the palatal root. e. Sagittal image of the palatal root shows bone loss in the vertical direction on the mesial side of the palatal root. f. to k. Continuous axial images from the cervical to apex region. Image (f) shows bone resorption from the mesial aspect of the palatal root cervically. Image (g) is 1 2 mm closer to the apex than image (f). It shows two-walled bone resorption localized on the palatal root. Images (h) and (i) are 1 2 mm closer to the apex and demonstrate three-walled bone loss. Images (j) and (k) are an additional 1 2 mm closer to the apex and show bone loss localized on the mesial side. i j k Clinical data on pages 11 and 12 are quoted from Dental Magazine No. 111, SPRING 2004 Clinical Report by Dr. Mitsuhiro Tsukiboshi, Aichi prefecture, Japan 12
Clinical Examples Temporomandibular Joint Sharp images display the head of the condyle. Apical Lesion Images show the apical lesion at the buccal root of the upper left first molar. Root Bulla The radicular cyst at upper left second premolar is shown, with bone loss on the bottom of the maxillary sinus. Clinical data on pages 10 and 13 are provided by the Department of Radiology, Nihon University School of Dentistry, and the Department of Oral Radiology, Matsumoto Dental University School of Dentistry. 13
Thinking ahead. Focused on life. In 1916, Junichi Morita started to import products of the leading dental equipment manufactures into Japan where demands for modern dentistry were growing.
