14 Examples of Activities Post-Training and Action Planning Jefferson County, KS Public Health Dayton - Montgomery County, OH Established contact with KS state diabetes program manager. Formed the Jefferson County Diabetes Coalition. Launched media campaign, including editorial in local newspaper and PSAs on local radio stations. Conducted a diabetes focus group to provide data to the Ohio Department of Health sstate diabetes plan. Reviewed local diabetes data (compiled by PH- D&MC) and prioritized at risk populations and geographical areas of greatest need. Contacted community members with type 1 or type 2 diabetes to talk about community needs and answer questions for the coalition. Developed coalition Facebook page ( County-Diabetes-Coalition/ ) Developed an informational brochure featuring diabetes-related resources and referrals.
15 Examples of Activities Post-Training and Action Planning Fulton County, GA Established the Diabetes Action Coalition. Participated in State of Georgia s 5-year Diabetes Strategic Planning and the Diabetes Awareness Forum. Hosted Georgia Diabetes Coalition and Georgia Medical Care Foundation s first Coffee and Conversation Diabetes Forum; planning five additional similar opportunities throughout Georgia. Took the lead on planning and implementing the Division of Health Promotion Advisory Council using the Diabetes Today: Planning for Coalition Action. Obtained verbal agreement from Emory Univ. Div. of Endocrinology, Metabolism, and Lipids to provide training/certificates for community members with no medical background who can assist with diabetes education. Established relationship with Kennesaw State University to assist in development/summarization of assessment tools. Met with 14 city mayors within Fulton County, all of whom gave commitments of support to the Fulton County Diabetes Initiative.
