Full-time and part-time employees enrolled in Healthy Rewards Employee Incentive Program receive Wellness points for participation. 2 Program Descriptions There are many ways to earn points toward the Wellness portion of the Healthy Rewards incentive program. This calendar lists all programs and activities that can be attended by employees (as well as spouses and additional adults covered by a Saint Francis Healthcare System insurance plan) to earn Wellness points. For a complete listing of all the ways that Wellness points can be earned, please refer to your Healthy Rewards Goals Packet that was given to you at your annual Wellness appointment. Assessment Clinic (for enrollment/re-enrollment in Healthy Rewards) 25 Wellness points* Wednesday, December 5 (Saint Sebastian & Saint Martha Conference Rooms) 7:00-10:00 am Thursday, December 27 (Wellness Office) 3:00-4:00 pm All Assessment clinics will be first-come-first served! No appointment is necessary to participate. Complete your biometric assessment (consisting of height, weight, BP, and optional fitness testing) for your Annual Healthy Rewards appointment. Assessment must be completed at least two days prior to your Annual Healthy Rewards review appointment. BEEF (Body weight exercise that is Easy, Effective, and Fast) - 25 Wellness or Fitness points* Tuesday, December 4 (Fitness Plus Athletic Republic (second floor of Fitness Plus) Tuesday, December 18 (Fitness Plus Athletic Republic (second floor of Fitness Plus) 8-8:20am or by appointment (call x5970 to schedule a different day/time) This program will review specific guidelines and recommendations for quick and easy bodyweight resistance training that can be done at home, at work, or on the go! A 30-day fitness log will be provided at the end of class. *Completion of the 30-day exercise log is required in order to receive 25 FITNESS points. Fit or Fat?- 15 Wellness points Tuesday, December 18 (Located in the Wellness Office) 7:00-8:00 am (takes about 10 minutes) BODY FAT PERCENTAGE: Learn what your percent body fat is. We will be measuring percent body fat using skinfold calipers, the most widely used method of estimating percent body fat in the fitness field. Percent body fat in healthy percent fat range for your age and gender is worth 50 points in the Body Weight category. Establish a baseline for the coming year! ***NEW*** STEP TEST: Come let us measure your cardiovascular endurance! The Step Test is a 3-minute fitness test that will reveal your personal aerobic capacity. You can use this measurement to establish a baseline for Healthy Rewards goals and possibly earn 50 points in the Fitness category on your next annual visit.

3 Food of the Month- 10 Wellness points Thursday, December 13 (Located in the Tower Lobby) 11:30-12:15 pm Come catch the cooking demo on wheels and take a taste of an exciting new recipe! Healthy Bites Luncheon 20 Wellness points Thursday, December 6 (Saint Sebastian & Saint Martha Conference Rooms) 12:10-12:40 pm First Thursday of each month Various health professionals lead this monthly program discussing a variety of health-related topics. It is also open to the public. Feel free to bring your lunch to eat while you listen. Heart & Health Screening Wednesday, December 5 (Saint Martha Conference Room) 7:00-10:00 am First Wednesday of each month Cholesterol ($30) and blood glucose ($7) test 15 Wellness points Results in minutes!! Blood pressure checks are free of charge; cholesterol and blood glucose tests require a small fee (and a hour fast, water is encouraged). Jingle Bell Run **VOLUNTEERS NEEDED!! (15 Wellness points) Saturday, December 8 (Outside Fitness Plus) 9:00 am Jingle across the finish line to a toasty hot chocolate and kick off the holiday season at the annual Fitness Plus Jingle Bell Run/Walk. Registration will begin at 7:45 am, and the race will promptly begin at 9 am. The adult registration fee is $30 in advance and $40 the day of the race. The 5K and one-mile participants will receive a long-sleeved cotton shirt. All proceeds benefit the Arthritis Foundation. Participants are encouraged to dress in their best holiday-themed attire. Grab your reindeer antlers and Santa hats, because judges will be scouting the crowd for the best dressed. Registration forms are available at Fitness Plus or you can register online. ** To volunteer contact Sandy at x5312 or sduncan@sfmc.net. Race begins at 9 AM, volunteers needed 8:30-10:00 am. Salad Week 5 Wellness points Sunday, December 9 Saturday, December 15 Choose a day this week for your department to have a Salad Luncheon. You and your colleagues organize a potluck of your favorite salad fixings (lettuce, carrots, cucumbers, tomatoes, nuts, cheese, beans, beets, avocado, etc ). Send Wellness a list of all of those that participated. 5 Wellness points per participant.

4 SideStepping Diabetes 25 Wellness or Blood Glucose points* Wednesday, December 19 (Saint Martha Conference Room) 8:15-9:00 am Pre-registration is required! Call x5316 With early detection and lifestyle change, you can reduce your risk of developing diabetes. Come learn how! This class is offered once per month; it will be the same information each month. *This class may be taken for partial credit under the Blood Glucose portion of the Healthy Rewards goals if you have glucose/hgba1c levels outside the recommended range (the 30-day challenge handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion. TLC (Therapeutic Lifestyle Change) 25 Wellness or Blood Lipids points* Wednesday, December 19 (Saint Martha Conference Room) 9:30-10:15 am Does your heart need a little TLC? It may if you have high cholesterol or other risk factors for heart disease. TLC helps you take small steps that can result in big changes by lowering your risk for heart disease. This class is offered once per month; it is the same information each month. *This class may be taken for partial credit under the Blood Lipids (cholesterol) portion of the Healthy Rewards goals if you have cholesterol levels outside the recommended ranges (the 30-day challenge handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion. Walk Till You Drop 25 Blood Glucose points Wednesday, December 19 (Wellness Office) Appointments will be available between 9:00 and 9:30 a.m. Participation is limited! Pre-registration is required! Call x5316 Have you ever been told that you are Pre-diabetic or Diabetic? Have you heard that exercise can help you get better control over your blood sugars? If so, come join us on the Fitness Plus track on Thursday September 20 to walk till you drop - your blood sugar, that is! To participate, you must commit to: * Eating a meal about minutes before your walking session * Avoiding sugary snacks or beverages after your meal * Walking at least 20 minutes at a BRISK pace during your appointment * Having Wellness staff check your blood sugar by finger-stick before and after your walk Weight-wise 25 Wellness OR Body Weight points* Wednesday, December 19 (Saint Martha Conference Room) 7:30-8:15 am This is an introductory class covering the basics of nutrition and weight loss. Learn how to set small weight loss goals, monitor your progress, track your eating and more! *This class may be taken for partial credit under the Body Weight portion of the Healthy Rewards goals if your initial BMI is greater than 25 (the 2-week food log handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion.

5 5 Wellness points (maximum 60 points per year) Every Wednesday (Elevators outside Tuscan Sun) 11:15 am-11:45 am This is a weekly weigh-in for anyone trying to lose or maintain weight. You will be provided with a new health tip each week to help keep you on track. Wellness Walk 5 Wellness points OR 5 Fitness points (maximum of 25 points per year) Tuesday, December 11 (Fitness Plus track meet by the Wellness Office) 11:30 am-1:00 pm (walk as long as you like) Take part of your lunch break to burn off some calories (and stress!). You do not need to be a Fitness Plus member to participate in these Wellness Walks. Walk 4 laps (0.5 mile) and earn 5 Wellness Points. Walk 8 laps (1 mile) and earn 5 Fitness points.

6 Fitness Plus Saint Francis Family Week Our gift to you! Check in to Fitness Plus for five or more of these special events to earn 15 Fitness points for Healthy Rewards! All events led by Fitness Plus & Wellness staff. Event #1: Jingle Bell Run Saturday, December 8 (Outside Fitness Plus) 9:00 am Registration will begin at 7:45 am, and the race will promptly begin at 9 am. The adult registration fee is $30 in advance and $40 the day of the race. The 5K and one-mile participants will receive a long-sleeved cotton shirt. Event #2: Circuit Workout Monday, December 10 (Fitness Plus gym) 7:00-8:00 pm (15-20 minute workout. Check in by 7:40pm) Get a complete workout with a sequence of back to back strength building exercises. The routine is made for you, you just do it! Complete the minute circuit. Event #3: Yoga Mini-Class Tuesday, December 11 (Fitness Plus Studio 2) 7:30-7:50 am This will be a great intro to yoga, offering techniques aimed at integrating mind, body, and sprit for improved flexibility, muscle strength, stress relief, and so much more. Event #4: Wellness Walk Tuesday, December 11 (Fitness Plus track meet by the Wellness Office) 11:30 am-1 pm (walk as long as you like) Take part of your lunch break to burn off some calories (and stress!). Event #5: 12 Laps of Christmas Wednesday, December 12 (Fitness Plus track meet by the Wellness Office) 6:00-8:00 pm Celebrate the gift of good health by walking 12 laps (1.5 miles) around the track! Event #6: Food of the Month Thursday, December 13 (Fitness Plus Lobby) 12:30-1:00 pm Come catch the cooking demo on wheels and take a taste of an exciting new recipe!

7 Event #7: Kickboxing Thursday, December 13 (Multipurpose Room) 4:45-5:15 pm Come get your aggression out with Ryan. Be prepared to sweat! Event #8: Stress Break Dodgeball Friday, December 14 (Fitness Plus Multipurpose Room 12:00 pm-1 pm (drop in anytime!) Bring your department over or just yourself and throw balls at each other for stress relief! Event #9: Yoga Saturday, December 15 (Fitness Plus Studio 2) 9:00-9:45 am This will be a great intro to yoga, offering techniques aimed at integrating mind, body, and sprit for improved flexibility, muscle strength, stress relief, and so much more.
