3 Volumn 10 - Dec 2014 Message From Editor Greetings from Sarawak Heart Foundation! The second half of 2014 for Sarawak Heart Foundation has been inundated with a number of activities. First on the list of activities was the health talk for staff on 6 July 2014 at Hilton Kuching. This was followed by Hari Raya Visit led by none other than our Trustee YABhg Datin Patinggi Dato Hajjah Jamilah Anu, wife of our Right Honourable Chief Minister of Sarawak to the SGH Heart Centre on 23 July 2014 to spread Hari Raya joys to the patients there. YABhg Datin Patinggi also led the Foundation s delegation to spread Yuletide cheers to the patients at SGH Heart Centre in December One of the highlights for the 2nd half of 2014 will definitely be the handover of SGH Heart Centre s Dormitory by the Foundation on 23 September 2014 by the Governor of Sarawak and also the Foundation s Chairman, Tuan Yang Terutama Tun Pehin Sri Haji Abdul Taib Mahmud. The project was taken up jointly with Plaza Merdeka Holding Sdn Bhd and with the dormitory coming online, relatives of patients at the SGH Heart Centre putting up at the ward or rest areas will be a thing of the past. At the same event, the Foundation also honours its founding and first Chairperson, the late YBhg Datuk Amar Laila Taib for her vast contributions through the Foundation with a portrait which is located prominently at the entrance of SGH Heart Centre. The other highlights is none other than the Foundation s signature event, the Walk-a-Mile held in celebration of World Heart Day 2014 with the theme of Heart-Healthy Environments. The event this year was again organized jointly with Sarawak General Hospital Heart Centre on 28 September 2014 at Kuching South City Hall. Our Guest-of-Honour, Tuan Yang Terutama Tun Pehin Sri Haji Abdul Taib Mahmud flagged off the participants for our signature Walk-a-Mile. For the 1st half of 2015, the Foundation will tentatively organized Heart Week in Sibu tentatively on March 2015 followed by Heart Week in Miri on April I would like to take this opportunity to wish you all a Happy New Year 2015 and for those who will be celebrating the Chinese New Year, GONG XI FA CHAI!! 5 Volumn 10 - Dec 2014 HANDING-OVER SGH HEART CENTRE ON 23 SEPTEMBER 2014 His Excellency The Governor of Sarawak cum Chairman of Sarawak Heart Foundation Tun Pehin Sri Haji Abdul Taib Mahmud handed-over the following items to SGH Heart Centre:- (a) Minimally invasive heart surgery equipment SGH Heart Centre is the regional referral Centre for heart and lung surgeries. Heart surgery is conventionally performed through an incision down the middle of the chest. In recent years, heart surgery has advanced significantly worldwide and it is now possible for heart surgery to be performed through small incisions with the aid of special instruments and equipment. Minimal access heart surgery has the advantage of less pain, lower complication rates, and faster recovery for the patients. The Sarawak Heart Foundation had collaborated with SABATI for the fund-raising dinner which was held on 8 March 2014 for the setting up of a minimally invasive heart surgery at SGH Heart Centre. The equipment costs approximately RM1.5 million. (b) Oxygen concentrator There are numerous cases where children (mostly newborns) with critical heart lesions who need urgent surgery is airlifted on board commercial airplane for surgery procedures in Kuala Lumpur. These children requires oxygen supply while on board the plane. Current new airplanes are not wellequipped to provide the necessary oxygen supply thereby, making it almost impossible to transport the children. With the oxygen concentrator (a portable oxygen supply machine which is certified for use in commercial aircraft), this will benefit between children with various forms of heart disease from Sarawak which are airlifted to Institut Jantung Negara, KL each year for surgery. 3

6 Volumn 10 - Dec 2014 (c) Mural projects by Faculty of Business and Design, Swinburne University, Sarawak Campus SGH has collaborated with Swinburne University, Sarawak Campus Faculty of Business and Design, for the Mural Project to create a conducive environment for the rehabilitation of heart patients. (d) SGH Heart Centre s Dormitory The SGH Heart Centre at Kota Samarahan was officially opened on 1 January 2011 and since its opening, it has seen an increase number of heart patients from all over Sarawak. Whilst SGH Heart Centre is now more conducive for the rehabilitation of heart patients, unfortunately there is lack of accommodation facilities within the vicinity of SGH Heart Centre for the relatives of the heart patients who are there to tend to the patients. Most of the relatives of the patients are from the not so well-off families and thus, are not able to put up at the local hotels or inns located in the city for long period of time. This results in some of them putting up at the ward or rest areas at SGH Heart Centre. SHF had taken up this project jointly with Plaza Merdeka Holdings Sdn. Bhd to cater for this need. (e) Portrait of the late YBhg Datuk Amar Laila Taib The late YBhg Datuk Amar Laila Taib was the first Chairman of SHF in The late YBhg Datuk Amar had contributed her very best efforts and energy as a volunteer. The portrait of the late YBhg Datuk Amar Laila Taib, the first Chairman of Sarawak Heart Foundation is to commemorate her unselfish efforts rendered throughout the years. 4

7 Volumn 10 - Dec 2014 Scientific Rationale for Exercise Therapy in Cardiac Rehabilitation by Dr. Leong Be Kim Head & Rehabilitation Physician, Department of Rehabilitation Medicine Introduction Ischemic symptoms occur as a result of imbalance between myocardial oxygen supply and demand. Study has shown hemodynamic abnormalities were seen in nearly all patients with coronary artery disease during ischemia whereas ECG and symptomatic evidence of ischemia occurred in only 50% and 30% of patients respectively. Therefore, regular hemodynamic monitoring during circuit exercise training is a safety measure to detect any abnormal responses produced by myocardial ischemia. Hemodynamic Response Monitoring Myocardial oxygen consumption can be reliably estimated by the product of heart rate and systolic blood pressure. This is called the rate-pressure product (RPP), i.e. double product. Regular exercise training lowers heart rate and blood pressure responses during submaximal exercise. This can lead to higher functional capacity level and patients will experience fewer symptoms during routine and leisure time activities. 5 Heart Rate Monitor

8 Volumn 8 - Dec 2013 Benefit of Exercise Therapy in Cardiac Rehabilitation Four basic pathologic factors that affect myocardial oxygen supply in coronary artery disease (CAD) patients as following: 1. Coronary artery stenosis with endothelial dysfunction 2. Microvascular dysfunction 3. Abnormalities of the autonomic nervous system 4. Abnormalities of coagulation and fibrinolytic system Several trials demonstrated a combination of aggressive risk factor modification and exercise training is associated with a slowing of progression and sometimes regression of CAD. Study revealed those expending < 1000 kcal per week experienced the greatest amount of disease progression, whereas subjects expanding > 1500 kcal per week demonstrated the slowest rate of disease progression. Regression of CAD was observed in patients training at an average energy expenditure of 2,200 kcal per week. Atherosclerosis in Coronary Artery Disease Patients Normal coronary arteries dilate in response to intravascular acetylcholine, a paradoxical vasoconstriction is observed in patients with CAD or chronic heart failure. Endothelial dysfunction is thought to be result from decreased production of nitric oxide within vascular smooth muscle cells. Exercise training partially normalise coronary endothelial dysfunction and improve myocardial blood flow. Patients with previous myocardial infarction and those with chronic heart failure often have abnormalities of autonomic function which manifest as an elevated resting heart rate and an attenuated peak exercise HR response. Patients with reduced heart rate variability (variation in R-R interval) have a fivefold increase in mortality. Chronic exercise training partially reverses autonomic dysfunction in patients with cardiovascular disease. Endothelial Dysfunction in CAD or Heart Failure Patients 6

9 Volumn 10 - Dec 2014 Autonomic abnormalities in CAD Patients - Increase in Sympathetic, Decrease in Parasympathetic Activities Abnormalities in the concentrations of hemostatic elements have been identified as cardiovascular risk factors. Studies demonstrate an inverse relationship between hematologic parameters and physical activity parameters. Therefore, long term exercise training reduces erythrocyte rigidity, platelet aggregration and adherence which reduce cardiovascular events. Significant Increase in Platelet Aggregation during Myocardial Infarction Conclusion Exercise therapy in stable cardiovascular patients improve mortality or morbidity outcome by ameliorating pathological factors contributing to myocardial ischemia. 7

10 Volumn 8 - Dec 2013 World Heart Day 2014 on 28 September 2014 at Kuching South City Hall, Kuching, Sarawak The World Heart Day was celebrated on 28 September 2014 at the Kuching South City Hall, Kuching, Sarawak. This event was organized jointly by the Sarawak Heart Foundation, SGH Heart Centre, Nestle Products Sdn Bhd and Legosan Sdn Bhd with Yang Amat Berbahagia Datin Patinggi Dato Hajjah Jamilah bt. Haji Anu as the organizing Chairperson for the World Heart Day The activities for the day included launching of the children s Physical Activity Exercise books, Walk-A-Mile, free health screening, zumba and chinese folk dances by famous dance groups and games. His Excellency, the Governor of Sarawak, Tun Pehin Sri Haji Abdul Taib Mahmud was the Guest-of-Honor of the World Heart Day There were a total of 2000 participants registered for the walk and many people from nearby neighbourhood came to participate also. 8
