1 Natinal Cuncil n Ayurvedic Educatin Educatinal Outline fr Cmpetency as an Ayurvedic Dctr INTRODUCTION This dcument serves as a guideline bth fr the develpment f the Natinal Examinatin and as a guide fr schls in preparing curriculum t meet the minimum natinal guidelines. This dcument is nt intended t be a fixed dcument but rather ne that is reviewed n an annual basis and mdified in accrdance with the input f the member schls and supprted by a majrity vte f the member schls. Any significant changes t this dcument in the future wuld be made with due regard fr the ability f the schls t adapt their curriculum accrdingly. Nte: The educatinal cmpetencies in this dcument are "in additin" t the cmpetencies nted fr the Ayurvedic Cunselr. Schls wh chse t d s, may exceed these guidelines in their training prgrams. SECTION I: SKILLS AND COMPETENCIES SECTION COUNSELING AND COMMUNICATION SKILLS NCAE Scpe f Practice Descriptin: The Dctr shuld have the necessary skills t supprt a patient in the implementatin f Ayurveda int their lives. As cunseling is a brad field with many appraches t creating behavir change, the NCAE des nt advise schls n a methdlgy but requires schls t teach ne r mre methds t their students fr the purpse f implementing Ayurveda int their lives. Related Pre-requisite r Cmplementary Knwledge: T be determined by the individual schl Testing: Nne Persnal and Family Health Histry NCAE Scpe f Practice Descriptin: The Dctr shuld have the ability t take a detailed current and past persnal and family health histry. In rder t be cmpetent, the dctr shuld have the fllwing ability / abilities: The ability t gather infrmatin by utilizing standardized frms and fllw up questins The NCAE des nt advise schls n creating frms but encurages schls t include this in their educatin Required Knwledge: All f the abve plus; Knwledge f the patients lifestyle, wrk, relatinships and daily rutines

2 Knwledge f hw the patient utilizes the five senses Knwledge f any allergy r intlerance the patient has t drugs r fd. Knwledge f what a chief cmplaint is and imprtant data abut the chief cmplaint including; Onset, Frequency, Duratin and Intensity Knwledge f what a Past Medical Histry is including; Illness, injuries, surgeries, hspitalizatins Knwledge f the rle f Family Histry f disease Knwledge f the herbs, drugs and supplements the patient is taking Testing: Nne, Cmpetency determined by the schl. Vital Signs NCAE Scpe f Practice Descriptin: The Dctr shuld have the skills t be able t effectively take basic vital signs such as bld pressure, respiratin, height, weight. In rder t be cmpetent, the Dctr shuld have the fllwing ability / abilities: Ability t take and interpret a patients measurements Ability t take and interpret a patients height and weight Ability t take and interpret a patients Western pulse Ability t take and interpret bld pressure Required Knwledge: Knwledge f the cmpnents f each examinatin Knwledge f what is nrmal and what is abnrmal fr each vital sign Testing: Theretical Testing Only. Schls will be required t prvide practical testing. Nidana NCAE Scpe f Practice Descriptin: The Dctr shuld have the skills t be able t determine the etilgical factrs (nidana) respnsible fr the dshic imbalance. In rder t be cmpetent, the dctr shuld have the fllwing ability / abilities: The ability t interpret the case histry t determine the factrs that have caused the general dshic imbalance with the patient The ability t understand hw specific behavirs may disturb specific systems f the bdy and cause disease Required Knwledge: Knwledge f the factrs that cause dshas t verflw Knwledge f the three fundamental causes f disease and hw the misuse f the senses, repressin f urges and inapprpriate daily and seasnal practices disturb the dshas.

3 Knwledge f the assciatins between inapprpriate behavirs and specific diseases. Testing: Prvided with a case histry, the student shuld be able t identify the factrs in a patient's life that are cntributing t the disease. The Dctr shuld be able t make assciatins n a multiple chice questin that link tgether a behavir and a specific cnditin in the bdy. Twenty Gunas NCAE Scpe f Practice Descriptin: The Dctr shuld be able tdetermine which gunas are predminant in the prakruti and vikriti. In rder t be cmpetent, the dctr shuld have the fllwing ability / abilities: The Dctr shuld have the ability t determine the rle and influence f the ten ppsing qualities (guruvadi gunas) in the prakruti and vikruti thrugh the data received frm the histry and examinatin f the patient. Required Knwledge: Knwledge f the 10 pairs f ppsite qualities (guruvadi gunas) assciated with the five elements (pancha maha bhutus) and the three dshas (tridsha) Ability t assciate the qualities with the symptms f vikruti Testing: Prakruti: Given the findings f a histry and exam n a patient the Dctr shuld be able t ascertain the qualities inherent in the persn. Vikruti: Given the findings f a histry and exam n a patient the dctr shuld be able t ascertain the qualities f the symptm Prakruti and Vikruti NCAE Scpe f Practice Descriptin: The Dctr shuld have the ability t assess prakruti and vikruti using Ayurvedic methdlgies f bservatin and examinatin. In rder t be cmpetent, the Dctr shuld have the fllwing abilities: Trividha Pariksha: The ability t use the fllwing three tls in diagnsis; Tuch (Sparshanam): Observatin (Darshanam) Questining (Prasnam)

4 Ashtavidha Pariksha: The ability t apprpriately use the fllwing eight tls in diagnsis; Pulse Assessment (Nadi Pariksha) Urine Assessment (Mutra Pariksha) Stl Assessment (Mala pariksha) Tngue Assessment (Jihva Pariksha) Speech and Vice Assessment ( Shabda Pariksha). Skin Assessment (Sparsha Pariksha). Assessment f Eyes (Drika Pariksha) General Appearance f external features - Face, Nse (Akriti pariksha) Dashavidha Pariksha : The ability t assess the patient utilizing the criteria: Dushyam- Structural and functinal abnrmalities f the bdy Desham -Gegraphical nature f the place where patient lives Balam -Physical strength Kalam- The seasn and climatic cnditins Analam -The digestive system f the patient Prakriti- Cnstitutin Vayas -Age f the patient Satvam- Psychlgical nature f the patient Sathmyam- General and persnal habits f the patient (ie. smking) Aharam- Nature f the patients diet and sensry intake (e.g. vegetarian r nn-vegetarian) Required Knwledge: Knwledge f hw t interpret the findings leading t the crrect cnclusin f prakruti and vikruti. Testing: Presented with findings, the Dctr shuld be able t interpret the findings and cme t a prper diagnsis f prakruti and vikruti. Schls t be respnsible fr practical testing. Subdshas NCAE Scpe f Practice Descriptin: The Dctr shuld be able t shw skill and cmpetency in determining which subdshas are affected by a disease. In rder t be cmpetent, the dctr shuld be able t: Assess a symptm and determine the subdsha(s) respnsible Required Knwledge: Knwledge f the lcatins and functins f each subdsha

5 Knwledge f the signs and symptms f disturbance t each subdsha Testing: Presented with a symptm, the Dctr shuld be able t determine the subdsha that is mst clsely assciated with the cnditin. Dhatus / Upadhatus NCAE Scpe f Practice Descriptin: The Dctr shuld be able t assess each dhatu / upadhatu and determine if it is affected by disease. In rder t be cmpetent, the dctr shuld be able t: Assess each dhatu and upadhatu Assess symptms and determine the dhatus and upadhatus invlved in the disease Required Knwledge: Knwledge f the seven dhatus and their related upadhatus Knwledge f hw dhatus are frmed Knwledge f hw t assess each dhatu and upadhatu Knwledge f the signs and symptms that ccur when a dhatu is infiltrated by a dsha Knwledge f healthy dhatus Testing: Presented with a symptm, the dctr shuld be able t determine the dhatu that is invlved in the cnditin. Srtamsi NCAE Scpe f Practice Descriptin: The Dctr shuld be able t assess each srta and determine which srta is invlved in the presenting symptm. In rder t be cmpetent, the dctr shuld be able t: Assess each srta fr imbalance Determine which srta is invlved in the presenting cnditin Required Knwledge Knwledge f each srta Knwledge f hw the dhatus make up a srta Knwledge f hw t assess the health f a srta Knwledge f the signs and symptms when a dsha infiltrates a dhatu within a srta Knwledge f the signs f a healthy srta Testing: Presented with a symptm, the dctr shuld be able t determine which srta has been affected. Mala - Dhatu

6 NCAE Scpe f Practice Descriptin: The Dctr shuld be able t assess the state f the malas and dhatu malas. In rder t be cmpetent, the dctr shuld have the fllwing knwledge r skill: Ability t assess the grss malas and dhatu malas Ability t recgnize the signs and symptms f nrmal and vitiated mala Related Pre-requisite r Cmplementary Knwledge: Knwledge f what cnstitutes the nrmal and abnrmal quantity and qualities f malas Testing: Presented with the results f an assessment f a mala, the dctrs shuld be able t identify the dsha that has vitiated the mala. Agni - Ama NCAE Scpe f Practice Descriptin: The Dctr shuld be able t assess the clinical state f agni, and ama in the patient thrugh questining and bservatin. In rder t be cmpetent, the dctr shuld have the fllwing knwledge r skill: Ability t determine the state f agni by questining the client abut appetite, digestin and eliminatin Ability t recgnize the signs and symptms f ama in the bdy by bservatin f the tngue, stls and bdy dr Required Knwledge: Knwledge f the thirteen majr agnis Knwledge f the functin f agni Knwledge f the symptms and signs f the fur states f agni Knwledge f hw ama is frmed Testing: Given the findings based n a histry (questining) and exam (bservatin), the Dctr shuld be able t determine the state f agni and ama. Samprapti (Dctr) NCAE Scpe f Practice Descriptin: The Dctr shuld be able t shw skill and cmpetency in determining the current stage f samprapti by bservatin (darshana), tuch (sparshana) and questining (prashna). In rder t be cmpetent, the Dctr shuld be able t assess the fllwing: The Dctr shuld be able t evaluate the patient and determine the stage f pathlgy the patient is experiencing. Required Knwledge

In rder t be cmpetent, the Dctr shuld be able t assess the fllwing: The Dctr shuld be able t assess the state f prana, tejas and jas thrugh bservatin and questining (Prasna). 8 NCAE Scpe f Practice Descriptin: The Dctr shuld be able t shw skill and cmpetency in the understanding f the majr diseases affecting each system f the bdy. The Dctr shuld be able t shw knwledge f the majr diagnstic and labratry tests used in Western Medicine. In rder t be cmpetent, the Dctr shuld be able t d the fllwing: Be able t read the results f a typical CBC, UA and Bld Chemistry; understand nrmal and abnrmal and what each test means. Be able t interpret reprts frm the fllwing tests: X-ray Diagnstic Ultrasund MRI Endscpy CT Scan Clnscpy Bne Scan Laparscpy Required Knwledge: Knwledge f what each test is and what it tests fr. Knwledge f when the tests are indicated Knwledge f what a nrmal and abnrmal test means Understanding f the cncept f false negatives and false psitives Knwledge f the terminlgy necessary t read the reprts Testing: Prvided with a disease, the Dctr shuld be able t identify the basic tests utilized t bth diagnse and mnitr the cnditin. Research NCAE Scpe f Practice Descriptin: The Dctr shuld be capable f reviewing literature and writing their wn reviews f medical and Ayurvedic literature. In rder t be cmpetent, the Dctr shuld be able t d the fllwing: Write a cherent review f literature Required knwledge: Knwledge f different types f research designs (Blind, Duble Blind, Cntrlled, etc) Knwledge f hw t scan the pub-med database Knwledge f hw t read the basic results f research Knwledge f hw t prperly write citatins Testing: T be managed by the schls. SECTION 2: RECOMMENDATIONS, TREATMENT AND OTHER INTERVENTIONS

10 Lifestyle NCAE Scpe f Practice Descriptin. The Dctr shuld be cmpetent in recmmending practices with due regard as t daily, seasnal and stage f life f the patient. In rder t be cmpetent, the Dctr shuld be able t: Recmmend daily rutines Prper ral hygiene (Tngue scrapping, Swishing, Brushing, Flssing) Taking care f Eyes (Eye wash) Self Abhyanga Neti Supprt the patient t make life style changes t create harmny Adjusting bed times and wake up times Times t take fd Instruct the patient in adjusting lifestyle in accrdance with the seasns Required Knwledge: Knwledge f each f the practices and hw t perfrm, with indicatins, cntraindicatins Knwledge f the effect f apprpriate physical, devtinal, spiritual practices n dshas and gunas Knwledge f hw each practice affects the dshas Knwledge, when apprpriate, f hw t adjust each practice per the prakruti / vikruti f the patient Knwledge f prper daily rutine fr each dsha Knwledge f hw the seasns might impact ne's rutine Knwledge f evening rutines(ratricharya) Testing: Academic and Clinical; Academic is directly related t the knwledge abve, Clinical is given a snap sht f a patient's lifestyle, the Dctr shuld be able t identify imprvements t be made in the patient's lifestyle in accrdance with the patient s prakruti and vikruti. Sensry Therapy NCAE Scpe f Practice Descriptin. The Dctr shuld be cmpetent t prescribe the apprpriate use f therapies fr each f the five senses. In rder t be cmpetent, the Dctr shuld be able t: Prescribe apprpriate arma therapy in harmny with the patients prakruti / vikruti Prescribe apprpriate tuch therapy in harmny with the patients prakruti / vikruti Prescribe apprpriate sund therapy in harmny with the patients prakruti / vikruti Prescribe apprpriate visual therapy in harmny with the patients prakruti / vikruti (Fr dietary therapy see the sectin fr fd and diet) Required Knwledge: The Dctr shuld be able t identify excessive utilizatin, deficient utilizatin and the wrng (mis) f each f the five senses Knwledge f the prper use f each sense

Given a case histry, the Dctr shuld be able t identify the actins that are causing disharmny thrugh each f the senses and be able t recmmend psitive changes. Herbs NCAE Scpe f Practice Descriptin: Ayurvedic Dctrs shuld be cmpetent t recmmend predesigned r custm herbal frmulas fr all purpses including the treatment f disease and able t frmulate their wn. In rder t be cmpetent, the Dctr shuld be able t: Recmmend r prescribe the apprpriate herbs t balance agni, eliminate ama and supprt the malas while prtecting and building jas Recmmend r prescribe herbs and design frmulas fr the treatment f disease Required Knwledge: Understand the cncepts f rasa, virya, vipaka and prabhava Knwledge f the classificatin f herbs and their energetics based n taste (rasa), ptency (virya), pst-digestive effect (vipaka), guna (quality) and prabhava (unique actin). Knwledge f the actins f herbs (karma). The Dctr shuld knw the cmmn actins f an herb. When Western terminlgy clsely matches the Sanskrit terminlgy, the Western terms shuld be knwn. When a unique actin is described in Sanskrit that has n simple translatin int English, then the Sanskrit terminlgy fr that actin shuld be knwn. Knwledge f the effect an herb has n the agni, ama dshas and mala. 12 NCAE Scpe f Practice Descriptin: The Dctr is able t recmmend pacificatin (shamana) therapies t balance the dshas and eliminate ama. In rder t be cmpetent, the Dctr shuld be able t: Utilize thse tls allwable within their scpe f practice t gently cleanse the bdy, reduce ama and alleviate the dshas in accrdance with the principles f shamana chikitsa. Required Knwledge: Knwledge f the seven traditinal frms f palliative therapy; Deepana: Herbs that increasing agni Pachana: Herbs that digest ama Kshuudhanigraha: Cntrl ver diet including the use f fasting Nigraha: Cntrl ver water and il intake Vyama: Use f exercise Aatpa seva: Frms f creating heat withut directly increasing agni such as sunbathing. Maruta seva: Ways t cl the bdy t prtect pitta dsha such as mn walks. Knwledge f indicatins and cntraindicatins fr palliatin therapy Knwledge f hw t design a palliatin prgram t crrect an imbalance in each dsha Knwledge f hw t determine if palliatin therapy is advisable The principles f shamana chikitsa Testing: Clinical, given a case scenari, a Dctr shuld be able t determine if palliatin is indicated. Panchakarma NCAE Scpe f Practice Descriptin: The Dctr shall be cmpetent t recmmend and versee panchakarma prgrams as well as individual treatments / bdy therapies. In rder t be cmpetent, the dctr shuld be able t: Design a panchakarma prgram that meets the needs f the patient Be able t adjust the prgram accrding t the respnse f the patient Supervise the administratin f the prcedures Perfrm the prcedures Required Knwledge: Knwledge f each f the fllwing therapies: Abhyanga Anuvasana basti Shirdhara Niruha basti Svedana Virechana Nasya Vamana Knwledge f the indicatins and cntraindicatins fr panchakarma and each therapy nted abve

Tnificatin (Brimhana) NCAE Scpe f Practice Descriptin: The Ayurvedic Dctr is cmpetent t prescribe therapies fr general tnificatin (brimhana) f the bdy and mind. In rder t be cmpetent, the Dctr shuld be able t: Determine when tnificatin is indicated Utilize all therapies within their scpe f practice t achieve the gal f tnificatin Required Knwledge: Knwledge f the indicatins and cntraindicatins fr tnificatin therapy Knwledge f hw t design a tnificatin prgram in accrdance with the dshic vikruti. Knwledge f hw t manage a tnificatin prgram and make adjustments t the prgram in accrdance with the patient s prgress Knwledge f hw t assess the state f agni, ama and jas Knwledge f hw t assess the dhatus and the signs f depletin Testing: Academic and clinical. Given the findings n a histry and exam, the Dctr shuld be able t determine if tnificatin therapy is indicated and be able t identify which therapies are tnifying. Rasayana and Vajikarana Therapy. NCAE Scpe f Practice Descriptin: The Dctr is cmpetent t recmmend, implement and versee pst, panchakarma rejuvenatin (pashchatkarma) prgrams and prgrams specific t building reprductive health. In rder t be cmpetent, the Dctr shuld be able t; Identify which patients are in need f vajikarma therapy Apply Vajikarana therapies when indicated. Apply Rasayana Therapy fllwing panchakarma Required Knwledge: See the sectin n Panchakarma fr paschatkarma Knwledge f the use f reprductive tnics and stimulants

The Dctr is able t recmmend basic diet (ahara) and lifestyle, herbs and ther treatment within their scpe f practice t manage the prenatal, and pst natal mther. In rder t be cmpetent, the Dctr is be able t: Prperly supprt the mther frm the time she prepares fr cnceptin thugh pst-partum care, utilizing the tls within their Ayurvedic scpe f practice.
