Welcome & Agenda Overview Phillip Ginter welcomed the group, introductions were made and the meeting opened at approximately 2:05 pm. II. ALICE Project Presentation The next item on the agenda was the presentation of the ALICE (Asset Limited Income Constrained Employed). Phill introduced Mary Shaheen, Vice-President United Way of New York State, to the group to share insights that came about through the ALICE Report. Mary shared an in-depth ALICE report with the group that she stated was intended to shed a light on those that are employed, but still cannot afford to make ends meet and struggle to pay their own bills. The ALICE Report was discussed in detail with the group and full report is available by visiting the United Way website

2 Steering Committee Meeting Minutes Page 2 of 5 III. Social Determinants of Health Workgroup Update The next item discussed on the agenda was the Social Determinants of Health Workgroup Update. Hisieni Sacasa gave an overview on the workgroup where she relayed that the workgroup performed their final Blueprint for Health Equity event of She reported that the workgroup has had a total of seven events in 2018 with 2 in Sullivan; 1 in Putnam; 1 in Orange; 1 in Rockland, 1 in Westchester and 1 in Ulster. Over 500 attendees have participated with over 135 organizations in attendance. At our last Steering Committee Meeting, we discussed structural racism as a focus at the Blueprint events and the workgroup has begun having that discussion. Other ideas from the workgroup include: Sending out videos before the event Giving participants structural terms Adding a racial lens to the poverty simulation Hisieni also informed the group of a conversation with Missouri Community Action Network, the group that developed the poverty simulation, and they like our before and after presentations to the poverty simulation and are looking into how they can assist us with bringing in other elements. IV. Behavioral Health Workgroup Update Next on the agenda, Rachel Romaine shared the update on the Behavioral Health Workgroup as follows: Anti-Stigma Campaign o Focusing on an anti-stigma messaging campaign had been an interest of the workgroup prior to the creation of the Toolkit and Mental Health First Aid trainings, so we have returned to it. o Workgroup has agreed on using an already existing campaign and disseminating it throughout the seven counties o Currently compiling a list of campaigns for the workgroup to vote on at the next meeting Some of the campaigns have a general message that can be targeted towards a population, so we will also be deciding on what population to focus on. Orange County Department of Mental Health has been using the #BeThe1To campaign and targeting it towards middle-aged Caucasian men, so this campaign already has traction in the region. Mental Health Toolkit Next, Rachel shared an update on the Mental Health Toolkit as follows: o Through the Orange Wellness Coordinator Training, we have had 10 businesses commit to using the toolkit once it has been launched, 3 of which we have calls scheduled to begin the implementation process.

Hope Presbyterian The Enlarged City School District of Middletown Amscan, Inc. New Hope Community Kolmar Mount Saint Mary College o Plan on developing a Mental Health Lunch & Learn series to offer with the Toolkit Series would include approximately one-hour trainings on: Mental Health Awareness Stress Management in the Workplace Developing a Self-Care Plan The group requested that we also look at postpartum depression gap. Rachel will look at the gaps from the surveys. o Since the Behavioral Health Workgroup has expressed interest in focusing on the Anti-Stigma Campaign, the focus on the Mental Health Toolkit will largely be moved to the Workplace Wellness Workgroup. Mental Health First Aid Next Rachel gave an update on the Mental Health First Aid Training that included the following: Assisted with arranging a Youth Mental Health First Aid training at Valley Cottage Library in Rockland County Led by Isabel Burk, The Health Network, and Dr. Katherine Roberts, the Teacher s College at Columbia. Funded by Project AWARE (Advancing Wellness and Resiliency Education) o We held two Adult Mental Health First Aid trainings one with Lower Hudson Valley Perinatal Network, co-instructed by Alice Joselow from Ossining Communities That Care, and one with Greenburgh Public Library in Westchester SBIRT Trainings Next, Rachel informed the group that Victoria and Phill held their first SBIRT Training on November 16 th in partnership with Montefiore PPS at ADAC in Orange County.

LHD Collaborative Workgroup Update Next on the agenda Hisieni provided the Committee with an update on the Local Health Department Collaborative Workgroup. She informed the Committee that this workgroup is working on a region-wide community health assessment that could be broken down by county and part of that is a survey. In the survey, we are looking at incorporating the Health in All Policies and this workgroup wants to go with a vendor and we are looking at Sienna College. This workgroup is also trying to coordinate with hospitals and had a meeting on 11/13 and the hospitals seem interested in collaborating as hospitals can use the information to inform their plans. A second meeting is being planned. Also explained to the Committee was that the LHD Collaborative Workgroup is considering the Hanlon Method which is a data driven method used to pick priorities for each of our counties. VI. Workplace Wellness Workgroup Next, Rachel gave an update on the new Workplace Wellness Workgroup. She informed the Committee that this workgroup is a new workgroup will focus on improving the health of the workforce in the region and provided the Committee with the following highlights: Consists of representatives from organizations that provide free programs throughout the region to promote health & wellness in the workplace. First meeting held on Nov. 9 th to form goals for the workgroup moving forward o Goals that the group agreed upon: o 1. Collaborate in delivering free wellness programs, such as Smoking Cessation, Water Friendly Worksites, Breastfeeding Friendly Policies, Mental Wellness, Heart Health, etc. to worksites across the seven counties. o 2. Host Wellness Coordinator Trainings in the seven counties of the HV. Essentially, this is a how to create a workplace wellness program without a budget training. It consists of a full day of presentations on how to create a wellness committee, what free resources are available to them through the local organizations, and how improving the health of the workforce has been successful for local businesses. We have written into our workplan to have at least one training per quarter. Westchester and Rockland are interested in holding trainings in the First Quarter Orange County is planning on hosting one in Newburgh in the Second Quarter, that this workgroup will sponsor Dutchess County potentially a fourth o 3. Create regional resources to promote health & wellness in the workplace Workplace Wellness Resource Toolkit Reaching out to programs across the region to submit a description of their organization, description of their programs offered to

5 Steering Committee Meeting Minutes Page 5 of 5 workplaces, and their contact information going to put it together in a toolkit to publish on our website and hand out at our trainings o 4. Improve the health of the workforce by promoting healthy eating options on college campuses in the region Preventative care for employees in the region Ensure that the employees you are hiring out of the regional college aren t already unhealthy. Make them aware of healthy choices. The colleges employees are eating those foods too, so you want them healthy. Rachel informed the group that if they know someone who would be a good fit for this workgroup, they should contact her accordingly. Bonnie Reyna expressed that employers have reached out to her for an employee satisfaction survey and inquired as to whether we were familiar with any. Phill recommended that Bonnie review the stay interview portion of the Toolkit and if interested, HealthlinkNY can set up a survey monkey and provide aggregate results. In addition, HealthlinkNY will keep an eye out for other surveys and report back to Bonnie. A few others in the group were also interested in receiving information on the Workplace Wellness Workgroup and the flyer will be sent to the group along with the Meeting Minutes. VII. Year 4 The next item on the agenda was the Year 4 Work plan and Phill presented a brief overview. o Update Phill informed the group that earlier in the month a draft of the Year 4 Workplan was submitted to New York Department of Health. The plan was revised and resubmitted this morning to the Program Manager and we are anticipating contracts will be ready by mid-december Once we have confirmation from the State, we will share the same with the Steering Committee. o Continued planning discussion Phill informed the group that we are finalizing the progress report for the first three years and will distribute to partners and possibly coordinate some face to face meetings to make shareholders aware of what s been going on with the PHIP. VIII. Adjourn The meeting wrapped up and was adjourned at approximately 3:10 pm.
