Welcome & Agenda Overview Victoria welcomed the group and introductions were made at approximately 12:05 pm. II. Hiring Update The Hiring update was presented by Phill Ginter. He informed the group that HealthlinkNY did receive LIFT grant and we are in the process of hiring three additional staff members one for the LIFT grant and two will be as Population Health Coordinators for PHIP work in Hudson Valley (one for Southern portion and the one for Northern region) and each would be assigned to a workgroup one to Social Determinants of Health Workgroup and one to the Behavioral Health Workgroup. Phill informed the group that the goal is to provide candidates with an update by the week of April 10 and will keep group posted. Additionally, for the second round of interviews, we d like to extend offers to Steering Committee to participate in PHIP interviews and if you are interested in being a part of this process, please let Victoria

2 Steering Committee Meeting Minutes Page 2 of 5 know if you can join interviews. In person is preferred, but you can participate on the phone as well. Erin stated she may be able to join, but must leave by 4 pm. III. Social Determinants of Health Workgroup Update The next item on the agenda was the SDOH Workgroup Update. Victoria relayed to the group that March 17 Blueprint for Health Equity event was postponed because of the snow storm and prior closings affected many of the attendees. Also, stated by Victoria, was that thus far 195 participants have gone through the Blueprint for Health Equity. Then Victoria informed the group that we will be hosting the following Blueprint events for the remainder of the year: Sullivan rescheduling due to postponement Rockland April 27 Ulster September 26 Putnam October 17 Orange Date not yet determined Joint NYMC & WCC Student Event Date not yet determined The next event is scheduled for Rockland County with Cheryl Hunter-Grant will be our M.C. along with Erika Bernabei and Ron Figueroa as our speakers. RECAP will be performing the poverty simulation. Additionally, Victoria informed the group that we are also looking to partner more with organizations to assist with budgeting and the Social Determinants of Health Workgroup has been communicating about hosting webinars and some other follow up items to the Blueprint event. Stacy questioned Ulster s representation and Victoria shared that we ve had some participation from Ulster and will be hosting a Blueprint in Ulster on 9/26/17. IV. LHD PA Collaborative Group Update Victoria informed the group that the LHD PA Collaborative Workgroup has met and we will be sharing information on this group and the activities, etc. as they are developed. V. Behavioral Health Workgroup Update The next item discussed was Behavioral Health Workgroup Update and Victoria informed the group that she had conducted the first Mental Health First Aid Training along with Jim Conklin at SALT. Two additional trainings are currently scheduled for the Rockland County Library System and we will continue offering Mental Health First Aid trainings to agencies and groups who are interested. The goal for the Behavioral Health Workgroup is to offer a minimum of six trainings. Next, Victoria discussed the Mental Health Toolkit and stated that initially the group had identified stigma and we have decided to take on workplace wellness to bring in the mental health component. She also relayed to the group that a survey was created to inventory what organizations were currently engaging in a form of workplace wellness. So far 88 surveys have been completed across both the Southern Tier and Mid-Hudson region. Additionally, one hour long interviews will begin in April 2017 with

3 Steering Committee Meeting Minutes Page 3 of 5 approximately 45 to take place in the Mid-Hudson region and the piloting of the Mental Health Toolkit will begin in July 2017 at approximately 16 sites. Questions arose from the group as to who the proper person in the organization would be to complete the survey and Phill Ginter explained that it should be the person in charge of workplace wellness in the company or the equivalent who could be the Human Resources Manager. Victoria also took this time to relay to the group that if they are interested in participating in the Mental Health Toolkit interviews they should let us know. Next, Victoria relayed to the group that HealthlinkNY Community Network will be participating in the Sullivan County Wellness Training on April 7 th. At this event, we will be introducing the Mental Health Toolkit information to the attendees to continue building on our list of potential participants for the interviews and pilot sites. VI. Workgroup Goals for 2017 Victoria shared the most recent draft of the Steering Committee Workgroup Goals based on previous conversations for further review. VII. Membership Review Steering Committee Goals were presented to the group based on what we had previously talked about - priority areas and membership gaps. Some of the group presented revisions and feedback to Victoria and changes were made to the matrix during the meeting. Others handed their revisions in at the end of the so they could be incorporated into the matrix and the remaining members decided to continue reviewing the matrix to suggest changes at the next meeting. Phill then presented the group with another option for the meeting format to this group (same format used at the most recent Southern Tier meeting). Hosting a local convening group in a convenient location in each region (North and South of the Hudson Valley) and coming together virtually for Steering Committee Meeting. The thought is to engage more stakeholders by having an in-person format. Group agrees that in person meetings are more engaging. Caren Fairweather stated that her Ellenville office could be available to accommodate the group. A few other possibilities given were: Health Alliance, Kingston in Ulster County Ulster County Department of Health, Golden Hill Lane Kingston, NY Also mentioned was that Joseph Todora, Sullivan County Commissioner Division of Health & Family Services, has sophisticated video conferencing capabilities and may be another possibility. VIII. Calendar of Topics Victoria shared a Calendar of Health Topics and informed the group that it was previously asked of everyone to share their organization s topics to build and share with the stakeholders to take some of the burden off the stakeholders.

The group agreed to focus on the following areas: Behavioral health Chronic disease Social determinants The group agreed to highlight the following items each month: January - Cervical Health Awareness, No Name Calling Week, Mental Wellness February - American Heart, African American Health, National Eating Disorders March - Women s History Month, National Nutrition April - County Health Rankings (highlighting improvements), Alcohol Awareness, National Parks & Recreation Day, National Public Health Week May - Mental Health, Lyme Disease Awareness, Hepatitis Awareness, Screen Free Week, Lyme Disease Awareness June - Men s Health Week, HIV Testing Day, June-teenth, National Cancer Survivor Day, Safety July - National Minority Mental Health Awareness, Outdoor Safety, World Hepatitis August - World Breastfeeding, National Immunization September - Suicide Prevention, National Childhood Obesity Awareness, Hispanic Heritage October - Health Literacy Month, Substance Use Awareness, Mental Health Day, MHFA Training Day November / Great American Smoke-Out, Diabetes Awareness, Hunger and Homeless Awareness Week, Veterans Day December/ World AIDS Day, Seasonal/mental health/suicide prevention After full review and discussion of the Calendar of Health Topics, Victoria informed the group that this is a good starting point and this list may change as the Committee reviews and changes the focus areas of the Steering Committee. IX Membership Structure Victoria reviewed the Membership Structure with the group and explained that the list is derived from attendance of the last three Steering Committee meetings. The goal is to reach out to those that no longer come and ask for representation from their organization and determine whether this is a gap we need to fill and this is a collaborative effort.

5 Steering Committee Meeting Minutes Page 5 of 5 Victoria asked the group to review the Membership Matrix and provide revisions to us. Some revisions were made during this meeting. Victoria also explained that the next meeting is the third Friday of May (May 19) and Ulster cannot make it because of mandatory training in the morning. Group voted to have meetings for the remainder of year be held in the afternoon from 2 pm to 4 pm instead of mornings. The meeting wrapped up and was adjourned at approximately 2 pm.
