We appreciate this opportunity to share with you our accomplishments, successes and celebrations. Crossroads is in its 42nd year of providing services to the communities of Western Iowa. This past year has been one of change, growth and new endeavors. Through it all, our clients and their families continue to be the pivotal reason of why we do what we do. Crossroads has the awesome responsibility of currently providing care and services to more than 600 people in our programs. In addition to the clients we serve, our staff also provides guidance and communication to parents, caretakers and guardians. The amount of responsibility we assume on a daily basis can be overwhelming, but we certainly do not do it alone. In this Annual Report, we will focus on the impact Crossroads makes in the lives of individuals with intellectual and developmental disabilities, mental illness and brain injuries through collaborations, contributions and community engagement. Our finances are strong and our programs are solid, thanks in part to the support of the community, monetary contributions from our supporters, and partnerships from local businesses. You ll meet Doug, Hunter and Christina program clients who have been successful in achieving independence and self-sufficiency. They are shining examples of what can happen when someone is given the opportunity to actively seek out opportunities for an enhanced quality of life. Success is measured differently for each client we serve. One individual may be striving towards living independently in a Crossroads residential group home, while another might be working with our staff to acquire work skills in an effort The nearly 400 people who work at Crossroads are relentless in their efforts to teach and encourage our clients to attain independence and to reach their full potential at home, at work, and in their communities. We have much to be proud of in regards to program growth including new homes that were opened, the expansion of our volunteer opportunities, and an increase of clients placed in jobs in the community. Many thanks to our Board of Directors for their ongoing guidance and expertise in directing Crossroads along the path to success. Our gratitude is extended to all of the Crossroads staff as well, for their undying dedication and love they have for our clients and for the mission. 3 L.I.N.K.S. Living Skills, Integration, Networking, Kinship, and Self-advocacy Day Services Meet Doug Crossroads welcomed Doug into its L.I.N.K.S program in Council Bluffs in He had difficulties with mobility and was hard of hearing. His new surroundings caused him to be irritable and anxious. Doug struggled to adjust. DOUG Every day Crossroads staff would greet Doug and surround him with support and positive attention. They would patiently work with him on using his cane and encourage him to use his hearing aids. As the months went on, Doug became more social and trusting. You can now find Doug cracking jokes, socializing with his peers, and willingly accepting help from the staff. He now looks forward coming to L.I.N.K.S every morning, and we look forward to seeing his smile! 4 What We Do Everyone who comes to Crossroads does so for different reasons. Perhaps you ve reached a point in your life that you would like your own place but need the support to begin the process. Or you re the parent of an adult child who s been struggling to find a job in the community and needs guidance with job training and employment opportunities. Maybe working and living independently isn t in your path, but you could use a place to go during the day that would allow for structured activities and socialization. At Crossroads of Western Iowa we believe every person deserves to experience a full abundance of life. We help accomplish this by offering programs and services that integrate adults with disabilities into the community. We have given hope and opportunity to thousands of people and their families over the past 42 years, and we look forward to providing assistance to thousands more in the years to come! 19% increase of L.I.N.K.S clients served compared to last year. 167 individuals were provided community integration opportunities through our L.I.N.K.S program. 5
4 Residential Living Introducing Hunter Upon graduating from high school, Hunter entered into Crossroads Residential Living program. He moved into a house with two other roommates, embarking on a new phase in his life of independence. It was a challenging adjustment for Hunter. 7NEW HOMES OPENED SINCE LAST YEAR CROSSROADS RESIDENTIAL LIVING 252 SERVICES PROGRAM PROVIDED ASSISTANCE FOR INDEPENDENT DAILY LIVING TO CLIENTS CHRISTINA 93% satisfied Supported Employment This is Christina Christina works a part-time job, attends L.I.N.K.S during the week, and lives in her own apartment. She s living a life that she set out to achieve since first coming to Crossroads more than a decade ago. In 2004, Christina enrolled in our Residential Living program in Onawa, Iowa. She shared a house with several other program clients and together, they learned about household responsibilities and budgeting expenses. She participated in group activities and community outings. She worked with staff on things like social skills, personal hygiene and effective communicating. Everything Christina learned and was exposed to at Crossroads prepared her for obtaining a job in the community. In 2006, she began working for a preschool center. For ten years, she gained real work experience in the field of child care. Most recently, due to budget cuts, Christina was laid off from her job. She took immediate action to search for a new one. She embarked on a new career path working in the food industry with a local restaurant. individuals receiving 282 employment services. people received job training and vocational placement services in our Supported Employment program. When he first moved in, Hunter spent the majority of time in his bedroom. He struggled with social anxiety and communicating. With the help of Crossroads staff and his roommates, Hunter slowly became more comfortable in his new environment. He started to trust the staff and rely on his roommates for help. As the months passed, Hunter started helping with cooking dinner and other household chores. He even began participating in field trips and errands that took him away from the house. Today, Hunter is much more open to new and different things. He s come a long way since he first came to Crossroads and continues to tackle his challenges head on! 6 HUNTER 70program clients placed in new jobs. 7
5 We cannot do what we do without you! During the 2016/2017 fiscal year, Crossroads celebrated numerous successes in the areas of fund development, quality improvement and agency growth. There s a lot of dedicated individuals working at Crossroads. Whether you re one of our direct support professionals, or work in our administrative office, everyone plays a pivotal role in moving our mission forward. As organizations often do, we also experienced a few setbacks mainly in the area of employee turnover. Caring for adults with disabilities can be a challenging career field, and finding the right people to carry out this role can also be a challenge. But these types of setbacks only make us stronger, motivating us to create a work environment that allows for development opportunities and competitive benefits for our employees. 100% satisfaction of our services to funders and businesses. Contributions People get involved in Crossroads for a variety of reasons. Whether they donate their time, goods or money, Crossroads is very appreciative of the various levels of support we do receive. We have supporters who donate their clothes and household goods to our Share the Ware store in Missouri Valley. Those donations help provide our program clients with job tasks allowing them to experience real work. Last year we collected more than $75,000 in monetary support for Crossroads. Whether it was from taking part in our two golf tournaments, grant requests, or an outright donation, most of the money we raised went towards our Fresh Start Fund. This fund was established to assist program clients in need of necessities like work uniforms, bedroom furniture or transportation fees people are currently employed with Crossroads. Crossroads currently provides services in 6 counties. Community Engagement For over four decades we ve made it our mission to empower people through opportunities that lead towards independence and self-sufficiency. It s always a good thing when an individual or a local business wants to show their support for Crossroads. But we find it s just as important for Crossroads to give back to the community as well. Whether it s our employees taking part in the Special Olympics Polar Plunge, or serving as a sponsor in a local chamber event, Crossroads understands the importance of giving back to the communities who have been so good to us. We have numerous Crossroads employees who serve on local planning committees, volunteer for other nonprofits, and hold community service positions. We are members of the Iowa Association of Community Providers, Nonprofit Association of the Midlands and the Human Services Advisory Council. We are CARF accredited and active members in multiple Chambers of Commerce. $76K raised from donations, fundraising and special requests. Collaborations We ve all heard the proverb, It takes a village to raise a child. That is very true in the world of nonprofit work. In order to be successful, it takes the hands of many people to make it happen. We ve mentioned the impact that volunteers have on our mission, but what about partnerships and collaborations with area businesses and other nonprofits? Those, too, are important in projecting our mission forward. The businesses that partner with Crossroads do so for various reasons including providing job and volunteer opportunities to our clients, serving as a sponsor for a fundraiser or special event, or collaborations with other nonprofits on awareness and disability rights. Thank you to each and every one of you who support Crossroads in some form or another. Like we ve said before, we couldn t do what we do without you! Often times, we find that the easiest thing people can do to support Crossroads is donating their time. Volunteers play a pivotal role in helping us integrate our clients into the community and keeping our mission moving forward. 6 This merger allows us to enlarge the geographical footprint of Crossroads and provide more opportunities for people with disabilities. We re inspired by the mission, vision and values of Crossroads and look forward to being part of the future growth of the organization. In order to be successful in carrying out our mission, we are always seeking innovative ways to expand our service area in an effort to provide programs and support to a larger number of people. Other $166,345 Total $14,639,305 Expenditures Program Services $12,029,454 Support Services $2,468, Most recently, Crossroads has merged with two other nonprofit organizations Cherokee Works Services located in Cherokee, Iowa, and CASS Incorporated in Atlantic, Iowa. Both organizations mirror Crossroads in that they offer programs and services for adults with intellectual and developmental disabilities including employment, residential and adult dayhab. The combined organization will utilize the Crossroads of Western Iowa name, mission, and programming structure. Today, Crossroads employs nearly 400 people and is providing services to more than 600 individuals. Our service area still spreads across western Iowa but has expanded from Council Bluffs to over 200 miles north to Cherokee, Iowa. We have an exciting 2017/2018 fiscal year already in the works including building a high-performance workforce, new efforts to engage the community in our mission, and a continued focus on quality improvement. Crossroads is excited for the opportunities that continue to come our way.
