2 Fiscal 2018 was aother excitig ad successful year for Service Program for Older People. The year bega with SPOP receivig the Commuity Gold Award from the America Psychiatric Associatio ad closed with a full ad ethusiastic audiece for the secod aual Dr. Hery Clay Mallard Memorial Lecture. It was a year filled with programmatic growth, service to the commuity, recogitio, ad outreach to ew populatios. Amog the year s achievemets were the followig: We preseted o commuity-based metal health care for older adults at a atioal meetig of the America Psychiatric Associatio, where we also received the Commuity Gold Award for excellece. We also preseted a fully-subscribed workshop at the Natioal Coucil o Behavioral Health coferece ad at a regioal meetig of the Natioal Associatio of Social Workers. SPOP was featured i the atioal joural, Psychiatry Olie. The agecy served over 2,000 older New Yorkers, ad cliic erollmet icreased by 10%. The Cliic opeed ew satellite sites i Dowtow Brookly, fuded by the Florece V. Burde Foudatio, ad i Patricia A. Caldwell Washigto Heights ad Mahatta Valley, fuded by the City Departmet for the Agig ad ThriveNYC. We created a ew office suite for our Itake Departmet. A member of the SPOP commuity was chose for a juried art exhibitio at the Maggi Peyto Gallery, hosted by Mahatta Borough Presidet Gale Brewer.

5 I SPOP: served more tha 2,000 older New Yorkers received over 5,000 iquiries ad website visits processed over 1,000 referrals from hospitals ad seior ceters completed more tha 17,000 professioal appoitmets, icludig over 4,600 home visits offered appoitmets at 14 locatios i Mahatta ad Dowtow Brookly provided bereavemet support to over 50 idividuals provided traiig, educatio, ad outreach to 500 adults I a recet cliet survey: 48% of respodets reported decreased emergecy room usage 72% reported that they ow get alog better with their families 84% reported they are ow better able to deal with a crisis 88% reported they ow deal with daily problems more effectively SPOP works to reduce uecessary emergecy room usage ad three moths of psychotherapy costs far less tha oe emergecy room visit.

SPOP s largest program is a licesed Article 31 Cliic, which offers commuity-based metal health care ad related services for adults age 55 ad older.

Based at our cetral office o the Upper West Side of Mahatta, the cliic also offers appoitmets i the home for cliets who are uable to travel ad at satellite sites located throughout Mahatta ad i Dowtow Brookly. Cliic & Commuity Services SPOP is attued to the exteral factors that affect our work particularly health care reform ad the growig ad icreasigly diverse populatio of older adults i New York City ad this year we pursued several projects desiged to meet a icreased eed for metal health care: The New York Commuity Trust ad the Altma Foudatio supported the third year of our partership with Mout Siai Health System ad its reowed Visitig Doctors Program, which provides i-home metal health care i coordiatio with medical care from the Visitig Doctors team. This year the program provided service to 71 homeboud cliets. I partership with the New York City Departmet for the Agig ad Thrive NYC, the cliic opeed sites at Red Oak ad Riverstoe Seior Ceters, brigig us to a total of six sites supported by Thrive NYC. The ThriveNYC sites are co-located at seior ceters i traditioally uderserved eighborhoods ad offer metal health care i Spaish ad Catoese. Part of our work at the six Thrive NYC parter seior ceters icludes traiig for staff as well as outreach, psychoeducatioal workshops, ad commuity programs for seior ceter members. These programs served over 650 adults i ad were effective i fosterig cliet egagemet. This year we opeed our first satellite site i Dowtow Brookly, hosted by Heights ad Hills ad supported by the Florece V. Burde Foudatio. The project icluded outreach ad commuitybuildig i Brookly to establish SPOP s presece i the borough. Our cliic site located at The Edie Widsor SAGE Ceter completed its first full year of service, fuded by the Fa Fox ad Leslie R. Samuels Foudatio. This is the coutry s first metal health ceter specifically for agig LGBT adults ad is a success story that shows the value of servig a margialized populatio withi a supportive ad safe settig. The Samuels Foudatio also supported SPOPCares, a cliic-based program that provided idividual ad group therapy this year to more tha 100 cliets with disorders relatig to caregiver distress. We awarded ie New York Times Neediest Cases grats to SPOP cliets, oe of whom was featured i The New York Times.

He has umerous medical complicatios ad was discouraged, depressed, ad isolated at the start of treatmet. Over the course of treatmet, Mr. R has begu to see a future for himself. He has take more resposibility for his health ad is ow egaged i physical therapy with a goal to icrease his idepedece. He has also started to sped more time with family members ad frieds ad takes pleasure i his social iteractios. He ackowledges that his health remais fragile, but he has regaied a sese of hope ad optimism about ways to ejoy life. 8 PROS/Persoalized Recovery Orieted Services SPOP is home to New York State s oly Persoalized Recovery Orieted Services/PROS program exclusively for older adults, which provides full-day rehabilitatio support for adults with severe metal illess. Our PROS populatio geerally has a log history of ipatiet psychiatric care ad the life skills traiig they receive through our program is cetral to their ability to live i the commuity. This year we served 78 participats. Durig we implemeted a full schedule of special evets, icludig a Fall Festival, where we welcomed a group of corporate voluteers from JPMorga Chase, holiday parties, a creative arts showcase, ad a Sprig Flig. Other highlights of the year icluded: We offered 80 groups each week o topics developed by our participats, icludig Meal Plaig & Cookig; How to Ope a Bak Accout; Computer Skills; Usig the Library; Commuicatio; ad Adaptig to Agig. We lauched a comprehesive smokig cessatio program for participats ad staff. Oe member of the PROS commuity was selected for a competitive juried art show hosted by Mahatta Borough Presidet Gale Brewer at the Maggi Peyto Gallery. We added a secod creative arts therapist positio to our staff. We welcomed studet iters from Columbia Uiversity School of Nursig, Columbia Uiversity School Occupatioal Therapy, Sophie Davis School of Biomedical Educatio ad Silberma School of Social Work at Huter College. I have bee a participat i PROS for three years. The excellet help that I have received at SPOP has improved my metal health to the poit that I will soo graduate from PROS. I kow that I will miss the PROS staff, my peers, the delicious meals, ad the program s warm ad respectful atmosphere. But I kow that I ca come back to SPOP, my sweet home ay time that I may eed help.

9 Bereavemet Support Traiig & Educatio Mr. E, age 78, came to the SPOP bereavemet program after the death of his mother. He was iitially reluctat to commit to the full program ad ofte expressed ager ad impatiece durig group sessios. With the support of the facilitators he gradually became a more active participat, ad others i the group appreciated his humor ad compassio. At the coclusio of the program he erolled for a secod te-week group ad also bega idividual therapy through our cliic. SPOP provides peer-led Bereavemet Support free of charge for adults of all ages. We use a model of peer-led groups ad all of our voluteers receive extesive traiig ad supervisio. The program offers te-week groups for spousal or o-spousal loss, idividual support as eeded, voluteer opportuities, ad social gatherigs. The groups have bee helped adults foster ew social coectios, ad may participats cotiue for a secod te-week sessio ad meet iformally outside of SPOP. This year the program received over 160 iquiries ad referrals; we completed 49 itake iterviews ad referred 20 idividuals to the Cliic or other programs. We coducted six te-week spousal loss groups ad thee te-week groups for o-spousal loss, with a total of 56 participats. Five idividuals received idividual support from a traied program voluteer The program also icluded a two-day traiig for prospective voluteers i October ad Bereavemet Support Celebratio i the sprig for program participats. Traiig & Educatio With the support of Mahatta Borough Presidet Gale Brewer, SPOP offers traiig to other orgaizatios at the itersectio of agig services, health, ad metal health. This service is over ad above the ogoig traiig that SPOP offers through the Cliic ad other programs.
