3 FSA Accomplishments for Fiscal Year Served 2334 unduplicated community residents, and 7,713 hours of face to face contacts. FSA spent 345 hours in the community working together on shared goals and by collaborating and coordinating to provide better quality and accessible care throughout all service areas. FSA gave back to the profession by providing 448 hours of supervision and mentoring to 8 Clinical Interns who were able to sucessfully complete internships through our clinical programs. FSA instituted fully paperless charts - Added Human Resource position to support and develop staff FSA completed reaccreditation through Council on Accreditation for 4 more years

4 EADLINE Screening, Assessment, and Support Services (SASS) Program performed 1,858 emergency psychiatric assessments on children/ adolescents in Kane and Kendall Counties. This is 134 more assessments than FY17. Of those assessments 1,093 were delfections back to the community and 764 were hospitalizations. Mental Health Juvenile Justice Program Served 35 youth ages 8-18 who were involved or at risk of being involved with the juvenile justice system and struggling with mental health sysmptoms from Kane Couty. Secured the grant for Family Support Program Formerly known as Individual Care Grant served 34 client. FSP program serves youth under 21 years of age with the highest need for long term and intensive theraputic services. There were 396 additional youth transferred into the SASS program from other agencies in the iscal year. To meet the higher volume of calls, an additional shift and crisis worker were added to help cover day-time crisis services. A fourth SASS therapist was added to assist with follow-up care. The SASS program worked diligently this last year to shorten the gap between when services are done being rendered and when the chart is formally closed. The year began with 68% being closed within 30 days and ended with 96% being closed in 30 days!

5 School Based Mental Health Program (SBMH) SBMH program uses Wraparound model with high- idelity of family voice + family choice to feel in control of their treatment and future. Lorem ipsum School Based Team serviced 480 unduplicated client families with 6,880 hours of direct service. Components of the SBMH Program Intensive community-based family therapy with in-school support to the student in 19 schools from U-46 and D-300 Individual and Family Therapy are provided through the year for continuity of care. Building a collaborative Wrap Around Team for each client to build natural supports and utilize combined strengths. Wraparound Resource Advisory Team (WRAT) Our SBMH staff are supported by several organizations through a group called the Wraparound Resource Advisory Team (WRAT). They provide resources + expertise to support client families. Some of the organizations involved are: Kane County Health Department Kane County Of ice of Regional Education Community Crisis Center Elgin Police Department Aunt Martha s District 300 District U-46

6 Therapeutic Mentoring Program This program, formally known as Family Support Services changed its name to Therapeutic Mentoring Program. Theraputic Mentors assisted clients in developing new skills such as improved communication and social skills, healthy habits to improve mood stability, and symptom management with 1370 hours of direct service. The change in name re lects our dedication to providing therapeutic interventions to assist clients meet their recovery goals. Referral process and form were modi ied. These changes make community support services accessible, allow us to be more skills oriented, and helps with the coordination and recommendation of services. Therapeutic Mentors collaborated and increased their consultation with Therapy Services, School-Based Mental Health Services, and Mental Health Juvenile Justice Services. Therapeutic Mentors began implementing models such as, Wellness Recovery Action Plans into their treatment to assist with maintaining clients stabilized in the community.

7 Therapy Service Program The program s goal is to restore optimal functioning through family-centered treatment. Added Tele-Psychiatry to our service array. Added a Play Therapy Room that promotes different types of theraputic play. The Outpatient Therapy Program served 710 unduplicated clients and their families. and provided 7,788 hours of services The team worked to improve outcomes by increasing clinical knowledge and was trained in Dialectical Behavior Therapy. On Satisfaction surveys, clients report that they would highly recommend our services 9.6 out of 10 times.
