2 Integrated Disabled Children s Service Accessing our services; Each of our services has different eligibility criteria, as set out below: Service Eligibility criteria Age Referral Fieldwork Residential Short Breaks Community & Home Support Occupational Therapy Direct Payments Lead Nurse, Community Nursing Community Short Breaks A diagnosed moderate to severe learning disability, or Any other condition (eg. life limiting illness) and a diagnosed moderate to severe learning disability. A diagnosed moderate to severe learning disability, or Any other condition (eg. life limiting illness) and a diagnosed moderate to severe learning disability. A diagnosed moderate to severe learning disability, or Any other condition (eg. life limiting illness) and a diagnosed moderate to severe learning disability. A diagnosed permanent and substantial physical disability, and/or A diagnosed moderate to severe learning disability. A diagnosed moderate to severe learning disability, or Any other condition (eg. life limiting illness) and a diagnosed moderate to severe learning disability. A diagnosed mild to severe learning disability and a complexity of health needs that cannot be met by a GP or school nurse. Any physical disability with or without a mild to severe learning disability and a complexity of health needs that cannot be met by a GP or school nurse. Any diagnosed disability and not already receiving any of the following services: residential stays at school or at the Light House hospice care play schemes youth services Link Care (fostering service) scheme after school clubs a Direct Payment agreed for social activities Youth Service Moderate learning disability years 5-19 years Early Help No referral needed
3 Speech & Language Therapy
4 We will accept referrals for children under 5 with no formal diagnosis. In these cases, an assessment will be undertaken by our staff to determine which agency is best placed to meet the child s needs. A review of this assessment will take place annually after a service being provided. This does not apply to Community Nursing and the Youth Service. An Early Help must be completed by a professional already known to the child or young person, for example: a school teacher; a community nurse; a health visitor; a paediatrician; an occupational therapist; a physiotherapist; a doctor (GP). The Local Offer for special needs and disabilities aims to share information about provision across education, health and social care for children and young people aged between 0 and 25. The Local Offer is for children and young people who have Special Educational Needs (SEN) or disabilities. The Local Offer is available to everyone including those without an Education, Health and Care ( EHC) plan. To access further information please see the link below; special-education-needs-disabilities/ You MUST provide evidence of diagnoses by including at least one of the following with the Early Help : 1. A psychiatric report 2. A psychologist report 3. A letter from a paediatrician 4. A completed Education Health Care Plan If we do not receive this evidence the referral will not be accepted. You may include any other relevant assessments you think will support the referral. Please choose which activity you would like your child to go to and reserve your place by calling or as soon as you can. Then send in the booking form before the closing date which is Monday 21 March But do reply as early as possible as trips soon get booked up. The number of hours we recommend to use in this holiday is shown on the booking form. Do not forget it is possible to pay for extra support if you want to! Then this is the event for you! Give them a cuddle and learn to look after them. Take part in lots of animal games and activities and then enjoy one of our special lunch boxes with all your favourite things inside. For young people under 11 and those with profound and complex needs. Cost per person, includes food! Enjoy time in the water, play water games with your friends, show off your swimming skills and splash the staff! Then enjoy a McDonalds meal of your choice! Open to all ages Cost per person

Cost per person Thursday 7 April to 4.00pm (6 hours) Trip to Gulliver s Kingdom at Matlock Are you up for going on exciting rides, like the log roll, big wheel, fire brigade ride, magic bikes, and the drop tower? Then this is the trip for you and all your family! Open to all ages. Families welcome! Cost per person Packed Lunch needed! Friday 8 April (5 hours) Ice Skating and Picnic at Nottingham Do you want to try and skate around the rink like molly? Have a go! There are lots of penguin, sleighs and other support equipment to help you. Afterwards we are off for a picnic don t forget your packed lunch! Open to all ages and abilities. Cost per person Saturday 9 April to 5.00pm (0 hours used*) Consultation Event Rosliston Forestry Park future of Fun-abil8y. In the afternoon you are free to enjoy the activities on the park with your family. Please bring a picnic lunch. *Open to all young people, but they must come with an adult over 18! Families most welcome! Transition Event We are delighted to invite you to our Transition Event for young learning disabled adults in Derby. Transition 2, Whitaker Road, Derby, DE23 6AR. Tuesday 26 April 2016 at 4.30pm until 6.30pm. You will hear directly from young learning disabled people, they will talk about their experiences in transition. You will meet people from lots of local organisations, including; Education, Health, Care, Employment, Leisure and Advocacy. Refreshments will be available for a small contribution. Parent and Carer s Page Donate and Educate This is your page to donate unwanted specialist equipment to another family and share information on accessible resources, activities and items of general interest. 7 Escape Family Entertainment Centre Autism Friendly play session for Kids with Autism, Aspergers, ADHD and their families! This takes place 3 times a year during the Easter, Summer and Christmas holidays from 9.30am 11.30am. You can check the website for dates and prices or call on Pentrich Road, Giltbrook Industrial Park, Giltbrook, Nottingham, NG16 2UZ. Creative Carers Creative Carers is run BY carers FOR carers. We support each other in a dedicated carers space (called Haven House ) and in the community. All our activities are FREE to Members (Membership is open to you if you are over 18 and caring for someone in the city). In addition, local businesses add discounts/rewards to your unique Membership Card. Monthly Relaxation / Self- Care Days Day: Last Thursday of every month. Time: 11:00am-3:00pm. What bothers you most? Is it back problems? Sleep? A lack of confidence? Eating healthily? Something else? Would you like to find ways of coping better? If so, we can support you to develop a personal self-care package. Discover the types of activities which may make a difference. In the past, carers have tried new experiences at Haven House e.g. yoga, meditation, Reiki, mindfulness, massage or just having some me time and told us it s really helped. Interested? What do you do next? Give us a call to book a time to have a chat with one of the team. Please note the Relaxation/Self Care Days get booked really fast and are by appointment only. Wednesday afternoons are dedicated to craft sessions e.g. stone painting, glass painting, clay moulding, origami, wire crafts and card making. Carers Evening Support Sessions Day: Last Wednesday every month: 30 March, 27 April Time: 6:00pm-8:00pm. In partnership with Making Space, Creative Carers are starting evening support sessions for those who struggle to come to events during the day. With a particular focus on supporting families of those with memory loss and dementia, we ll be interactively tackling a range of topics. Making Space will provide support for loved ones with memory loss and dementia in a separate space at the retreat, if needed. Address: Haven House, 31 Charnwood Street, DE1 2GU. Cooking There s an interactive cookery session every Third Thursday of the month Feb April with a specific theme.
8 Sewa Asian Session Day: First Monday of the month: 4 April. Time: 1:00pm - 3:00pm. A range of crafts sessions for Asian carers and their families with memory loss. For more information about this session contact Making Space on The session is for people suffering from memory loss and their families only. Friends for lunch Day: First Tuesday of the month: 5 April. Time: 12:00pm-2:00pm. Carers and their friends come together for chat and friendship at a monthly get together. *Carers pay for their own meals but a variety of discounted or low cost deals are available. Time: 10:00am-12:00pm. Information, peer support, chat and discussion group for parent carers of disabled children (up to the age of 30). Umbrella helps disabled children (up to the age of 30) and their families. Come and explore your own interests and pick up tips and techniques, including landscapes, from other carers. All carers welcome if you re over 18 and look after someone who lives in the city. 9 Drumming for Beginners Day: First Saturday of the month: 2 April. Time: 1:00pm - 4:00pm. No experience needed, practice developing a rhythm with an experienced tutor.
