2 ENGLISH AIM OF THE CARER PEER SUPPORT PROGRAM The Health Condition Support Grant Program (HCSG) is an initiative by the Health Minister of Victoria to support people living with a chronic health condition and their carers to achieve a healthy and manageable lifestyle within the community through sharing stories and information, validation and support. People living with dementia in the community and their carers from culturally and linguistically diverse communities engaged in the Carer Peer Support Group program. Carers and care recipients engaged in health literacy education sessions that included an array of topics delivered by professionals, as well as activities that foster social inclusion, participation in social and physical activities that promote health and wellbeing. This toolkit provides peer support groups with tips to develop group facilitation skills in order to manage support groups and facilitate activities. The toolkit is simplified taking into consideration the target groups predominantly elderly, language and literacy, access to resources, and the translation into nine community languages. Alzheimer s Australia Vic is keen to make the journey of people living with dementia and their carers in the community as positive and supportive as possible. 3 CROATIAN CILJ PROGRAMA UZAJAMNE PODRŠKE ZA NJEGOVATELJE Program dotacija za osobe sa specifičnim zdravstvenim potrebama (The Health Condition Support Grant Program - HCSG) je inicijativa Vikorijskog ministra za zdravstvo, koja kroz razmjenu iskustava i informacija, potvrđivanje i podršku, osigurava pomoć osobama koje žive sa kroničnim oboljenjem i njihovim njegovateljima, kako bi im pomogla živjeti zdravim i samostalnijim životom unutar zajednice. Uključuje osobe sa demencijom iz kulturalno i jezično raznolikih zajednica koje žive unutar zajednice i njihove njegovatelje, koji na pozitivan način sudjeluju u Programu grupa za uzajamnu pomoć za njegovatelje. 4 ENGLISH 1. THE ROLE OF THE FACILITATOR To facilitate is to enable and enhance the ability of individuals. To provide assistance to a group to enable it to achieve its objectives. A facilitator helps to support the group s harmony and potential. A group facilitator enhances each participant s sense of confidence and inclusion to participate in meaningful activities. To be able to facilitate a group, a facilitator needs to have qualities such as: Flexibility Patience Creativity Fairness Respect for difference A facilitator needs to know: How the group dynamics works and the ability to manage differences Individual strengths and qualities of each member Local services that might provide information or assistance to the group or individuals There might be one or two facilitators sharing the task of running a peer support group. It is important to keep track of the activities and the wellbeing of participants in the group and share the tasks equally. A facilitator needs to foster a sense of wellbeing and inclusion among participants in the group in order to maintain interest and achieve the purpose of joining the peer support group. The facilitator s role is to find a time and space for meetings that are: Accessible (public transport, car pool, location proximity) and building that is equipped for people with special needs (disabled access) OH& Safety: floor space, temperature (heating/cooling), seating, resources Frequency and length of meetings Well equipped: tables, chairs, good lighting, kitchen, toilets, etc Ensure that the space has public liability insurance It is important that the facilitator arrives before the group to the meeting location in order to set up the space and have it ready for the participants when they arrive. 4
5 CROATIAN 1. ULOGA FACILITATORA/VODITELJA Facilitacija je podrška pojedincima kako bi oni mogli ostvariti svoj potencijal. To je pružanje podrške grupi kako bi ona mogla ostvariti svoje ciljeve. Voditelj pomaže uspostaviti harmoniju i ostvariti potencijal grupe. Voditelj njeguje osjećaj samouvjerenosti u svakog pojedinca unutar grupe i njegovo uključivanje u svrsishodne aktivnosti. Da bi mogao formirati i voditi grupu, voditelj mora posjedovati sljedeća iskustva i osobine: fleksibilnost strpljenje kreativnost osjećaj za pravednost poštivanje razlika Voditelji mora poznavati: grupnu dinamiku i mora biti u stanju uskladiti razlike među sudionicima jače strane svakog pojedinca i kvalitete svakog od članova mjesne službe koje mogu pružiti informacije ili pomoć grupama ili pojedincima. Grupa za uzajamnu podršku može imati jednog ili dva voditelja koji će dijeliti poslove njene organizacije. Potrebno je voditi evidenciju o aktivnostima i zdravstvenom stanju sudionika i ravnopravno dijeliti zadatke. Voditelj mora stvoriti osjećaj boljitka i inkluzije među članovima grupe, kako bi mogao održati zainteresiranost i opravdati svrhu uključivanja u grupu za uzajamnu podršku. Uloga voditelja je osigurati da je vrijeme i mjesto održavanja sastanaka: pristupačno (javni prijevoz, parkiralište, blizina mjestu stanovanja) i da je objekt prilagođen osobama s posebnim potrebama (pristup za onesposobljene) ima pokrivene adekvatne sigurnosne aspekte: adekvatan prostor, temperaturu (grijanje/ hlađenje), mjesta za sjedenje i relevantne sadržaje, prilagođen učestalosti i trajanju sastanaka ima dobru opremljenost: stolove, stolice, adekvatnu rasvjetu, kuhinju, zahode, itd ima tekuću policu osiguranja Važno je da voditelj bude na mjestu održavanja sastanaka prije drugih članova grupe da bi pripremio prostor prije dolaska sudionika. 5
6 ENGLISH 2. SKILLS NEEDED TO FACILITATE A GROUP For a Peer Support group to succeed, a facilitator needs to be well prepared and organised. A facilitator keeps a record of the activities and timetables that the group participates in. Some of the important skills that a facilitator needs to have are: Collaborative problem solving skills Listening skills Preparation and organisation skills Keep the group on track with group goals Knowing what techniques are needed at what time It is important for the facilitator to: Know how the group s dynamics work Know the individuals participating in the group and their personalities Have the ability to be proactive to minimise conflict Include every person in the discussion for plans and activities Reach a consensus and compromise on decision making Listening skills: A facilitator needs to have good listening skills. Good listening requires the worker to frequently remain silent in order to hear what is being said. The facilitator identifies the needs of individuals and the themes emerging from the group discussions. Speaking skills: It is important that the facilitator shows respect and consideration when speaking to individuals and the group. A facilitator encourages all individuals to participate in the group discussion. A facilitator s voice is clear and the words are articulate. Speaking skills are important for effective communication techniques, conflict management, building and maintaining relationships. A very important skill for the facilitator is the ability to learn from the group members, and to encourage individuals to share their skills and knowledge in a positive way. 6
7 CROATIAN 2. ISKUSTVA I OSOBINE POTREBNE ZA VOĐENJE GRUPE Da bi grupa za uzajamnu podršku mogla uspjeti, voditelj mora biti dobro pripremljen i organiziran. Voditelj održava evidenciju o rasporedu i aktivnostima u kojima grupa sudjeluje. Neka od važnih iskustava i osobina koje voditelj mora imati su: sposobnost zajedničkog rješavanja sporova sposobnost saslušavanja drugih osoba sposobnost pripreme i organizacije doslijedno slijeđenje zajedničkih ciljeva grupe znati koje tehnike koristiti u danim situacijama Važno je da voditelj: poznaje dinamiku grupe poznaje osobe koje sudjeluju u grupi i njihovu osobnost posjeduje sposobnost aktivnog angažmana u suzbijanju konflikta uključuje sve sudionike u diskusiju o planovima i aktivnostima u procesu donošenja odluka otvara put ka kompromisu i dolazi do konsenzusa Sposobnost saslušavanja: voditelj mora biti osoba koja može saslušati druge osobe. Voditelj ohrabruje sve pojedince u sudjelovanju u grupnim diskusijama. Glas voditelja je jasan i riječi su mu razumljive. Sposobnost govorništva važna je da bi se postigla djelotvorna komunikacija, suzbili mogući sukobi i izgradili i održali dobri međuljudski odnosi. Vrlo važna osobina koju voditelj treba imati je i sposobnost učenja od drugih članova grupe kako bi na pozitivan način mogao podijeliti njihova iskustva i njihovo znanje. 7

SUPPORT GROUP TECHNIQUES Establishing a group Set the group s purpose Identify individual s needs: emotional, social, physical Decide size of the group Place and time of meetings: day time, night time, weekend Group guidelines clarified : rights and responsibilities, punctuality, staying focused on the task, confidentiality, commitment, equal participation Number of facilitators needed: one, two or more Resources Funding grant application, fundraising or collaboration with another organisation Catering (refreshments, tea, coffee, biscuits) Equipment: tables, chairs, whiteboard, computer, stationery, art and craft tools, audio and DVD, books, etc Access to information from Alzheimer s Australia Vic and other relevant organisations Volunteers Group facilitation styles It is important for a group facilitator to maintain a balance between TASK (what the group needs to do) and the PROCESS (the way the group functions taking into account the needs of the individual members). Here are some of the styles to facilitate a group: Autocratic The main concentration is on the task of the group Facilitator determines the pace and direction of the group Facilitator takes credit for the effectiveness of the group Democratic Group decides together on the approaches, goals, guidelines and pace of meetings Facilitator models and encourages group to self-appraise its progress Facilitator draws on the resources and skills of the individual group members Permissive The group has a good time The group may not always achieve its goals The facilitator gives very little direction and information 8

Voditelj se koristi informacijama i iskustvom drugih članova grupe. Permisivni Grupa se dobro zabavlja. Grupa ne mora uvijek ostvariti svoje ciljeve. Voditelj daje vrlo malo smjernica i informacija. 9

ACTIVITIES TO ENGAGE CARERS AND CARE RECIPIENTS In order to maintain the interest of individuals to participate in the peer support group, it is important for individuals to feel the benefit for joining the group. The facilitator identifies the needs of individuals and the motivation for them to engage in group meetings and activities. It is recommended to have a variety of activities in order to achieve a balance and satisfy the different needs of individuals. When deciding on an activity, the facilitator needs to consider the ability of individuals to participate. Have a checklist: What are the risks associated with this activity? Are there any OH&S issues involved? Would the majority of the group be able to participate? Would some individuals be disadvantaged? Is the activity affordable? Is the level of this activity easy or difficult? Is the activity enjoyable or boring? How will this activity enhance/improve the wellbeing of the participant? Examples of activities: Physical Yoga, TaiChi, stretching, swimming, walking, dancing, chair-based exercise, gardening Social Story-telling, book club, visiting local markets, movie club, knitting, cooking healthy recipes, art and craft, singing (choir), playing cards (bridge), or cultural board games (backgammon, Qigong, etc.) Social media Learning to use the computer, skype, facebook, twitter, using an ipad, iphone and Google. Alzheimer s Australia resources We Can, We Can, We Can and Relate, Motivate, Appreciate both Montessori based books and DVD Alzheimer s Australia Vic has developed these resources to enhance the quality of life and wellbeing of people living with dementia and their carers. These resources are available to download from the website or to order via the main office. A list of resources is provided in the last section of this booklet. 10

AKTIVNOSTI ZA UKLJUČIVANJE NJEGOVATELJA I PRIMATELJA NJEGE Da bi se zadržao interes pojedinaca za sudjelovanje u grupi za uzajamnu podršku, potrebno je da članovi osjećaju korist uključivanja u grupu. Voditelj utvrđuje potrebe pojedinih članova i njihovu motivaciju da se uključe u sastanke i aktivnosti grupe. Savjetuju se različite aktivnosti kako bi se ostvarila ravnoteža i zadovoljile različite potrebe pojedinaca. Kod odlučivanja o vrsti aktivnosti, voditelj mora uzeti u obzir sposobnost svih pojedinaca da sudjeluju u aktivnosti. Sastavite sljedeći popis: Koji rizici su vezani za ovu aktivnost? Jesu li preispitana pitanja sigurnosti? Može li većina članova grupe sudjelovati u dotičnoj aktivnosti? Da li bi pojedine osobe bile uskraćene? Je li aktivnost cijenom pristupačna? Je li aktivnosti lako ili teško provediva? Je li aktivnost zanimljiva ili dosadna? Kako će ova aktivnost poboljšati/unaprijediti zdravstveno stanje i kvalitetu života sudionika? Primjeri mogućih aktivnosti: tjelesne joga, tai-chi, vježbe rastezanja, plivanje, šetnja, ples, vježbe u stolici, rad u vrtu društvene čitanje priča, klub knjige, posjeta mjesnim tržnicama, klub ljubitelja filma, pletenje, priprema zdravih recepata, umjetničke sekcije i kućna radinost, pjevanje (zbor), kartanje (bridž) ili kulturološki specifične igre sa pločom (triktrak, Qigong, itd ) društveni mediji obuka u korištenju računala, skype, facebook, twitter, korištenje IPad-a, IPhone-a, i Google-a. 12 ENGLISH 5. SUSTAINABILITY AND COLLABORATION WITH SERVICE PROVIDERS A peer support group needs funding and resources in order to come together and enjoy the social support and activities. A facilitator needs to bridge relationships with many organisations and service providers in order to receive support and looks for opportunities to generate sustainable resources and activities for the participants. Support can be in the form of: In-kind which means that an organisation can provide a service for free. Example: photocopying, or tea, coffee and biscuits, or free space for meetings, etc Partnerships: this means that groups can collaborate on activities with another community organisation for a specific project. The community organisation will subsidise the cost of activities or part of the activities. Funding: applying for a community grant from local council or other sources for community activities. If the group cannot write the application, local community organisations or local council can assist. Fundraising: the group can sell items at school fetes or community events, or to sell raffle tickets and ask for donations. Many health and community organisations, as well as local council would be able to provide a professional person to deliver a workshop or an activity session free of charge. Information available in the list of resources in the last section of this booklet. It is recommended that the facilitator consults with the group to gather ideas about generating resources and support. There are many leadership roles for individuals for example, different people may organise social activities, manage finances, run the activities for the group and organise guest speakers However, it is important that participants do not feel burdened or pressured that fundraising is the sole purpose of getting together as a group. In order to sustain the facilitation of the group, a facilitator needs to have support. The role of a facilitator is a challenging one as dealing with a diverse group there may be people with high needs. Therefore, a facilitator needs support and opportunities to de-brief with a trusted colleague after every group meeting. Do not to assume a facilitator won t need it. 12

ODRŽIVOST I SURADNJA SA DRUGIM DAVATELJIMA USLUGA Grupa za uzajamnu podršku trebat će novčana sredstva i potrebne sadržaje kako bi se mogla sastajati i uživati u društvenoj podršci i aktivnostima. Voditelj grupe mora uspostaviti vezu sa mnogim organizacijama i pružateljima usluga kako bi dobio potrebnu pomoć, te aktivno tražiti mogućnosti generiranja održivih resursa i aktivnosti za sudionike. Pomoć može biti u sljedećem obliku: u naravi - što znači da organizacija može pružiti besplatnu uslugu.
