6 Child and Family Support Program (CFSP) Provides Family Peer Support Services - community and hospital-based programs Family Support Partners have had a family member who has been hospitalized Family Support Partners complete the Parent Empowerment Program (PEP) Training and obtain a Family Peer Advocate Credential. MHA Support Partners listen to families, share information about support services, and offer the opportunity for a follow-up call after discharge.

12 Our Hospital Partners

13 Our Hospital Partners Erie County Medical Center: 162 bed inpatient psychiatric hospital which includes an adolescent unit for youth ages and a Comprehensive Psychiatric Evaluation Program (CPEP). BryLin Hospital: 88 bed inpatient acute care psychiatric hospital where care is provided for children, as young as six years of age, adolescents, and adults. The Child and Adolescent Unit has 20 beds. Buffalo Psychiatric Center: 186 bed NY State psychiatric facility for adults. BPC also has residential programs and outpatient clinics.

14 Building a Partnership Our work with the hospitals grew over time. Work over at least 10 years to build a trust. This includes a growing understanding of the role of Family Peer Support. Providers could develop this capacity internally, but this make sense given the Child and Family Support Program s expertise and community connections. ECMC and BryLin now include Family Support Partners at incident review, patient experience panels, at presentations to new employees as well as other opportunities. Youth peer mentors gave feedback on programming on Child and Adolescent unit at BryLin and several youth spoke on a panel at ECMC to talk about their patient experience. Staff from all hospital programs attended, not just inpatient unit.

23 Consent for Follow-Up Why have we experienced success? We are meeting families where they are - on the units when families are visiting. Face to face engagement by peers. Families are busy, working, often hard to stay for support group. CFSP is calling families and patients at the time that is best for them often evenings, weekends, and holidays.

24 Data Collection and Evaluation Tower Foundation grant included evaluation. CFSP working with Dr. William Wieczorek and Dr. Kelly Marczynski at Buffalo State College Center for Health and Social Research for program evaluation Two tools identified: Burden of Stress Survey to be completed with caregivers and Quality of Life Survey to be completed with adult patients/transition-age youth. Metrics to correlate activities to desired outcomes (% of individuals/families who sign consents that follow recommendations in their discharge plan, reduction of the rate of recidivism, etc.)

28 Outcomes From 2014 Increase in families attending CFSP Family Support Group and 20 Something Support Group, 46 families referred to CFSP as a result Families report that they feel better supported and are linked to community resources as a result of follow-up calls.

29 2015 Accomplishments Teen support group in community twice a month Hospital database created to track data Hired additional youth peer mentors to expand support to teens and transition-age youth Bi-monthly girls group led by youth peer mentor started at Stanley Falk Day schools (both locations) CFSP Partnering with Baker Victory Services to provide youth peer support on the residential campus, at day treatment program and day school
