3 One year, One organization, One lasting impact Every year, Catholic Charities dedicates its time and resources to help those in need. But 2016 was truly a noteworthy year. As one multifaceted organization, we were able to help thousands of our community s most at-risk members, from youth to seniors. Here we highlight three programs that impacted Catholics and non-catholics of all ages one of the things that makes Catholic Charities so unique. CREATING LASTING RESULTS Among 493 nonprofit human services, people gave a median score of +47 for willingness to recommend the service. Catholic Charities Net Promoter Scores are consistently above average HUMAN CATHOLIC CHARITIES OF BUFFALO

4 Feeding All Ages Catholic Charities operates eight food pantry and outreach centers in Erie, Cattaraugus and Allegany counties, assisting individuals who qualify based on income. Our staff and volunteers also assess clients situations for underlying factors that might help improve their income in the long-term and refer them to other services or resources as needed. To help improve clients health, we offer Cooking Matters classes that teach them to shop for healthier food on a limited budget. In 2016, the total number of bags of food received by clients, at 43,599 bags, reflected in a 28% increase over bags received in HUMAN +85 OF 564 PANTRY CLIENTS SURVEYED NET PROMOTER SCORE willingness to recommend the service COOKING MATTERS CLASSES Nearly all 354 Cooking Matters class participants reported they had learned how to shop for healthy food on a budget. Classes were held in 13 locations, including at our food pantries and at five community sites. TARGET: OUTCOME: 97 % 99 % CLIENTS RESPONDING AFFIRMATIVELY When your food runs low, they ll help, and they are very respectful. RANDY HOLIDAY DISTRIBUTIONS A total of 4,009 families received special food packages at Easter, Thanksgiving and Christmas, and one pantry s distributions for Passover, Rosh Hashanah and Hanukkah. Of our pantry clients, 405 homebound seniors received gift bags at Christmas and 203 infants through teens received gifts, prepared by the Ladies of Charity volunteer organization affiliated with Catholic Charities. 405 GIFT BAGS FOR SENIORS 4,009 FAMILIES RECEIVED FOOD PACKAGES 203 GIFTS FOR INFANTS TO TEENS

5 Changing Lives, One Family at a Time Multisystemic Therapy (MST) is an intensive family and community-based treatment that addresses delinquent, at-risk youth behaviors in Erie, Niagara, Cattaraugus and Allegany counties. By recognizing that youth are surrounded by a network of connected systems family, peer groups, school and their neighborhood MST therapists are available 24/7 to work directly in the home with families, tending to all factors related to the youth s problematic behavior across these systems. The ultimate goal is to help youth remain in their home, attend school or work and stay out of trouble with the law, putting them back on track for lifelong success. +47 HUMAN +78 OF 170 MST CLIENTS SURVEYED NET PROMOTER SCORE willingness to recommend the service MST helped me a lot with my mom. I took my mom for granted before She s my best friend. RACHAELE NOTED OUTCOMES In 2016, 228 cases completed treatment, achieving these results: OUTCOMES AT DISCHARGE TARGET U.S. AVG C.C. AVG Percent of youth living at home 90% 89.8% 93.4% Percent of youth in school and/or working 90% 85.8% 86% Percent of youth with no new arrests 90% 88.4% 92.5% Also, of those due for follow up at six months post discharge in 2016, 89 families were successfully contacted and reported 87% youth at home, and at school and/or working, and 85% not re-arrested. Of those due for one-year follow up, 66 were successfully contacted and reported 89% youth at home, 83% at school and/or working and 89% not re-arrested.

6 Aging in Place Gracefully Giving hope and guidance to seniors in need is one way Catholic Charities strengthens our communities. Project HOPE (Helping Older People Excel) is designed to help seniors, age 60+, who have a mental health diagnosis and may be at risk in their current situation, age in place successfully. We teach these clients to navigate the system with the help of programs currently available to them, then discharge them successfully to their newly found support system or transfer them to a higher level of care if their needs change. +47 HUMAN +63 OF 32 PROJECT HOPE CLIENTS SURVEYED NET PROMOTER SCORE willingness to recommend the service NOTED OUTCOMES Using accepted mental health assessments as the measure, the target for at least half of all clients to report improved mental conditioning and functioning was achieved: TARGET: 61 % OUTCOME: 82 % OF CLIENTS OF CLIENTS FACE-TO-FACE VISITS Project HOPE met its target to increase by 3% the number of face-to-face visits it provided, achieving a 7% increase in face-to-face visits OUTCOME: TARGET: 2016 OUTCOME: 2,073 2,135 2,275 VISITS VISITS VISITS 125 clients; 369 people impacted They have been my angels.
