2 As an organization, Lahey Health embraced the American Hospital Association s call to action for all hospitals to be leaders in creating a culture of health and for healthcare organizations to serve as role models for their patients. We This brochure provides details about our 2014 Live Better wellness program. Your privacy is important to us. That is why the wellness program will continue to be completely confidential. No individual health data will be shared with Lahey Health. recognize that in order to be a leading organization in creating such a culture, we are dependent upon the choices that each one of us makes every day for our own personal health and wellbeing. We support each other along the way with our collective resources. This is how we all will continue our journey to Live Better. Complete details of the Lahey Health benefit plans are included in the official plan documents. If there is any difference between the information presented in this newsletter and the official plan documents, the plan documents will govern. 3 Dear Colleague, We are pleased to introduce the next phase of our voluntary Live Better wellness program. Our program is open to all Lahey Health colleagues enrollment in a Lahey Health medical plan is not required. Harvard Pilgrim Health Care will continue to administer our program and individual results will remain completely confidential. All colleagues are encouraged to continue participating in the program or participate for the first time this year. Colleagues enrolled in a Lahey Health medical plan may again earn up to a $600 incentive earning applied to their 2015 annual Lahey Health medical plan premium $400 when all three steps below are completed by a colleague, and $200 when all three steps are completed by a spouse who is covered under a Lahey Health medical plan. Step 1: Participate in a biometric screening session. Step 2: Complete the online health questionnaire (HQ). Step 3: Meet three or more biometric health measures. Get started now. 4 Our Journey Continues The Live Better wellness program is not only a way to save my family $600 on my health insurance premiums this year this program has changed my life and the lives of my family members. When I went for my biometric screening in 2012, I did it solely to save money on my health insurance. I had no intention of changing my habits, and I didn t think that I was unhealthy. All of that changed the moment I saw the numbers. My BMI was 44, and my blood pressure was completely out of control. It was very tough to see those numbers. My husband was in an even more dire situation, with a higher BMI and high blood pressure. We were both ticking time bombs. With a 4-month-old daughter, we realized that we needed to do something, or we would face the very real possibility of not seeing her grow up. My husband and I joined the Lahey Hospital & Medical Center medical weight loss program in January With the help of the team there, my husband and I have lost more than 200 pounds combined. As of my 2013 biometric screening, my blood pressure was in the normal range, and my BMI was 34! My husband and I both have a ways to go to reach our weight loss goals, but we are well on our way and determined to make it there. This program has had a very personal impact on me and my family. Not only has the change impacted us, but I can see how it is building a foundation of healthy lifestyle habits for our daughter. Amy Zaganjori Applications Support Specialist Patient Financial Services One Colleague s Story In the fall of 2013, we again made our voluntary wellness program available to all Lahey Health colleagues. If you are one of the thousands of colleagues and spouses across Lahey Health who took these steps toward making your personal health a priority, congratulations! Our program focused on health awareness, which included a free biometric screening and an online health questionnaire. All participants received a personalized health profile highlighting certain aspects of their current state of health and certain potential health risks. We are pleased to report that 87 percent of colleagues and spouses who participated met three or more of the health measures! Colleagues who are covered under a Lahey Health medical plan and who completed the screening, questionnaire and met three health measures earned a $400 incentive earning toward their annual Lahey Health medical plan premium in Additionally, if a spouse covered under a Lahey Health medical plan completed the screening and the questionnaire, colleagues earned an additional $200 incentive earning toward their 2014 annual Lahey Health medical plan premium. 4 Live Better: The Lahey Health Wellness Program Winter 2014
5 Continuing to Help You Live Better As many of us are aware of our health status and health risks, we need to continue to take action and lead a healthy (or healthier) lifestyle. Colleagues will again be offered the opportunity to earn an incentive earning toward their annual Lahey Health medical premiums. In addition, if a spouse is covered under our medical plan, colleagues will have an opportunity to earn an extra incentive earning if their spouse participates in step 1, step 2 and NOW step 3 of our program. How to earn the 2015 wellness incentive To earn the 2015 wellness incentive earning, colleagues and spouses will now need to complete the same three steps. Step 1: Know your numbers. Our onsite biometric screenings this fall will continue to look at certain key health measures for colleagues and spouses. Step 2: Take the health questionnaire (HQ). Colleagues and spouses who take the HQ will instantly receive a personalized health profile. Step 3: Meet three or more 2014 health measures. Health measures may be key indicators for wellbeing. More details about these measures are on the following page. Colleagues: Complete all three steps, which include meeting three or more 2014 health measures. Spouses: Complete all three steps, which include meeting three or more 2014 health measures. 6 Health Measures Updated for 2014! The updated 2014 health measures outlined below will mean different things for different colleagues. Some of us already meet the measures today, so efforts in 2014 will be focused on maintaining good habits. Others might need to consider making certain changes in nutrition or physical activity. Wellness is personal how you embrace this opportunity to Live Better is completely up to you. These health measures were updated for 2014 using the same commonly used biometrics assessed in 2013, established by our clinical leaders and approved by Lahey Health leadership. Updates take into account recent national clinical changes and what we learned about the collective Lahey Health colleague community from previous screenings (remember, we do not have access to individual data). The collective data showed these are important risk areas for our colleagues. Biometrics Health Measures (Measured During Fall 2014 Biometric Screening) Body Mass Index (BMI) BMI range: 19 through 25, or If you participated in the fall 2013 biometric screening and: Your BMI was greater than 25, up to and including 26, in 2014 have a BMI between 19 and 25; or Your BMI was greater than 26, up to and including 31, in 2014 have at least a one-point drop from your fall 2013 biometric screening result; or Your BMI was greater than 31, in 2014 have at least a two-point drop from your fall 2013 biometric screening result. Blood Pressure If you are under age 60: Systolic (top number) less than 140 AND Diastolic (bottom number) less than 90 Non-fasting Cholesterol If you are age 60 or over: Systolic (top number) less than 150 AND Diastolic (bottom number) less than 90 By having your cholesterol taken at the screening, you meet this health measure. You are encouraged to speak with your physician about this result. Non-fasting Glucose (blood sugar) less than 126 Glucose (Blood Sugar) Nicotine No nicotine trace from oral swab test For participants who may have difficulty meeting these health measures, for medical reasons, alternatives are available under certain circumstances. Please contact Harvard Pilgrim at for additional information. What do the measures mean to you? If you and/or your spouse participated in our biometric screenings last year, compare your results to these health measures. If you and/or your spouse are already meeting at least three out of the five, congratulations! Continue to stay on track for similar results this year to meet step 3. If you and/or your spouse have room to improve, now is the time to learn how you can positively work to improve your health by making healthy lifestyle changes. We know success does not necessarily happen overnight and that achieving some of these health measures will require significant lifestyle changes for some colleagues and their spouses. That is why the health measures you choose to focus on are up to you. Live Better Mid-year Biometric Screening Check-in As part of Lahey Health s commitment to help you Live Better, Harvard Pilgrim Health Care will be administering mid-year biometric screening check-ins this March. Curious about how you are doing on the Lahey Health s Live Better wellness biometrics? 8 Upcoming Wellness Seminar and Challenge We are continuing to offer you a variety of wellness seminars designed to help you with exercise and fitness, diet and nutrition, stress management and nicotine use. Reduce Your Stress, Live Happier and Healthier Seminar Living off of coffee? Not enough sleep? Need time to relax? Attend this seminar to learn how prolonged stress affects the body and may cause high blood pressure, high cholesterol, depression and insomnia. Also learn how to balance work and home life, including solid easy-to-follow nutrition tips which may help you to live a happier life. Look for dates, times and locations soon. Fresh Fruit Challenge 21 days, are you in? Take part in our voluntary fresh fruit challenge a fun new way to bring colleagues together and eat healthier. This online challenge, offered through Your Wellness Account at HarvardPilgrim. org/laheyhealth, works quite simply. There s an app too, so participating is even easier with a smart phone! All you need to do is eat enough fruit to meet your personal recommended goals, and log your daily intake in Your Wellness Account. You may even earn tickets to be entered into a raffle for gift cards to popular local retailers where you can purchase items that may help promote your healthy efforts. You have access to numerous wellness resources, including daily check-ins, healthy highlights, learning modules and much more throughout the year, day or night. Your 9 Wellness Account Learn Your Wellness Account offers so many ways to learn more about good health. When you log in, you will have access to: Learning modules engaging 10 to 15 minute sessions on timely health topics such as nutrition, exercise, diabetes and heart health. Within these modules, link directly to helpful tools and resources. Under Healthy Highlights and Wellness Activities, you ll find many quick ways to learn more while tracking your own path to Live Better. Reliable Health Resource in the online Wellness Center from an A to Z Multimedia Encyclopedia, to wellness calculators and a symptom navigator, you ll find the right e-tool to learn what you want to know. Earn The more you log in, the more opportunities you have to earn activity tickets toward Lahey Health s quarterly raffles. Whether you complete a quick daily check-in, a learning module, participate in a wellness activity, a lifetrack challenge or a Lahey Health group challenge (coming later in 2014), you re on a healthy path to Live Better. Winners of quarterly raffles can choose from one of the following gift cards with which you may purchase items that may help promote your healthy efforts: Dick s Sporting Goods, L.L. 10 Questions and Answers We know you may have questions about Live Better and what the health measures mean to you. Here are some answers to get started. What if I did not participate, or my spouse did not participate, in 2013? If you or your spouse did not participate in our wellness program this past fall, you and your spouse still have the opportunity to earn a wellness incentive toward your 2015 annual Lahey Health medical premiums. Here is how: 1. Participate in a 2014 onsite biometric screening and complete the confidential online health questionnaire. Later this year, you will receive information with dates, times and locations. Be sure to complete both of these steps by the deadline. 2. Meet three or more out of the five health measures. To meet the BMI health measure, you will need to be in the range of 19 through 25 since you do not have baseline data from Get ready by taking advantage of Lahey Health wellness resources during the year, whether it is an onsite fitness center, Weight Watchers program or attending a Harvard Pilgrim seminar. Is there any personalized help available? Starting a big or small lifestyle change can be difficult. Even with all of the great resources available to us, sometimes we need a little extra motivation to take the first steps to Live Better. That s why Lahey Health colleagues and their covered family members enrolled in our medical plans have access to a health coach through Harvard Pilgrim at no cost. This can help you get that boost of support and motivation that you need. Interested? 11 How else can I use my health questionnaire? Completing your confidential, online health questionnaire gives you a handy personalized wellness profile. Do not keep the information to yourself! Be sure to bring a copy to your next doctor s appointment so you can review it with your primary care provider or with a specialist you see for your care. Your results can be a great conversation starter about your health and lifestyle, so that you can work with your provider to make a plan to stay healthy or make improvements. What if I cannot find my HQ results? If you do not have your HQ results and are a Harvard Pilgrim member, don t worry. They remain in your secure HPHConnect account until you take another HQ. Here is how to retrieve your results: 1. Visit HarvardPilgrim.org/LaheyHealth. 2. Look to the left and you ll see HPHConnect. Click to log in with your username and password. 3. Click on Health Questionnaire. 4. Access your results. While in HPHConnect, you will also find an array of health and wellness resources all designed to support your efforts to Live Better.
