Dr. Andersen received his medical degree from Washington University School of Medicine in St. Louis, Missouri in He did his residency at Oregon Health Sciences University from and then completed a fellowship in head and neck surgery/oncology at Memorial Sloan-Kettering Cancer Center in New York City from , during which time he received a $10,000 award from the American Cancer Society Clinical Fellowship and a Basic Science Award from the Manhattan Eye, Ear and Throat Hospital Resident Research Day. Currently, Dr. Andersen is involved in research on the Differential Expression of Genes in Head and Neck Squamous Carcinoma with a grant from the American Cancer Society, Oregon Division, as well as his surgical and clinical practice at Oregon Health & Science University in Head and Neck Cancer and Surgery. Cohen, MD, PhD is a Professor in the OHSU Otolaryngology Department. and is the Chief of the Otolaryngology Service at the Portland VAMC. He works in the Portland Veterans Administration Medical Center as part of the head and neck surgery Otolaryngology team. At the VA, Dr. Cohen works with OHSU residents and medical students from Otolaryngology and other OHSU departments (i.e. Internal Medicine) in this very diverse clinic. Dr. Cohen earned his medical degree at the University of Manitoba in In 1984, he completed his Ph.D. in Otolaryngology and Residency in Otolaryngology / Head & Neck Surgery at the University of Minnesota in Following residency, Dr. Cohen completed fellowship training in Head and Neck Surgical Oncology at the MD Anderson Hospital and Tumor Institute Houston Texas Dr. Cohen is Board Certified by the American Board of Otolaryngology. SPEECH PATHOLOGISTS: BETHANY ANKE, MA, CCC-SLP, Pulmonary Rehabilitation after Total Laryngectomy, employed by InHealth Technologies. Financial: Ms. Anke receives a salary from InHealth Technologies. Bethany Anke, MA, CCC-SLP, is the lead speech-language pathologist in the Department of Otolaryngology at the University of Pittsburgh Medical Center. She received a bachelor s degree in history and communications from John Carroll University and a master s degree in speech pathology from the University of Pittsburgh. Ms. Anke specializes in medical speech-language pathology with a focus in head and neck cancer rehabilitation and dysphagia. She is certified in laryngectomy voice restoration and guest lectures on the topic at local universities and colleges. Recently, Ms. Anke has been involved in a project to develop a swallow screening tool for stroke patients that will be instituted throughout the University of Pittsburgh Medical Center. Ms. Anke is certified by the American Speech-Language and Hearing Association and is licensed in the state of Pennsylvania.

Following completion of her Fellowship, she received specialized training in voice and swallowing disorders while working at the University of Iowa Hospitals and Clinics. Her clinical interests include the evaluation and treatment of organic, neurogenic, and functional voice disorders in adults and children, vocal cord dysfunction, voice and swallowing disorders secondary to head and neck cancer, and augmentative and alternative communication (AAC). She is certified in the Lee Silverman Voice Therapy program. Jana has completed in research in the areas of perceptual voice assessment, communication impairments related to ALS, and dysphagia. She is an Adjunct Faculty in the Speech and Hearing Sciences Department at Portland State University. DONNA J. GRAVILLE, PhD, CCC-SLP, Troubleshooting the TEP, Comprehensive Rehabilitation after Total Laryngectomy, employed by OHSU. Financial: Dr. Graville has no relevant financial relationships. She earned her Master s degree in Speech and Hearing Sciences at Portland State University and completed her Fellowship at the Portland V.A. Medical Center. Dr. Graville developed her expertise in voice restoration and dysphagia at OHSU s Department of Otolaryngology, Head and Neck Surgery Center. Her desire to develop a comprehensive voice and swallowing center led her to pursue a Ph.D. in Communication Disorders and Sciences at the University of Oregon. She is currently an Adjunct Professor at both University of Oregon and Portland State University. Dr. Graville s current clinical interests include voice restoration after laryngectomy and swallowing restoration after treatment for head and neck cancer. Research interests include voice and swallowing treatment outcomes after head and neck cancer. MEGAN J. HYERS, MS, CCC-SLP, Managing Laryngectomy Across Settings, employed by Providence Portland Hospital. Financial: Ms. Hyers has no relevant financial relationships. She has been practicing as a Speech-Language Pathologist for over 12 years and is the Level II clinical specialist in Head and Neck Cancer for the Providence Rehabilitation Department. She specializes in dysphagia, laryngectomy, pre and post-surgical treatment, and prosthetic speaking valves. She currently treats adults with a variety of speech, language, swallowing and cognition disorders in outpatient, inpatient, and acute rehab.

Rachel King earned her Master of Science in Speech-Language Pathology at the University of Arizona in She interned at the Southern Arizona VA Healthcare System and completed her fellowship at the Portland VA Medical Center. Her clinical focus is in evaluation and treatment of swallowing disorders associated with a wide variety of medical etiologies, including head and neck cancer. She regularly completes instrumental evaluations of swallowing, including Modified Barium Swallow (MBS) studies and Fiberoptic Endoscopic Evaluation of Swallowing (FEES). Rachel is experienced in methods of alaryngeal communication, including the tracheoesophageal puncture. She also regularly supervises interns and fellows and guest lectures at Portland State University (PSU). Her research interests include clinical evaluation of swallowing disorders as well as diet and swallowing outcomes after chemoradiation therapy for head and neck cancer. ANDREW D. PALMER, MS, CCC-SLP, Introduction to TE Voice Restoration, Achieving Proficient Alaryngeal Speech, employed by OHSU. Financial: Mr. Palmer has no relevant financial relationships. He interned at the Voice Center at the University of California San Francisco and completed his fellowship at OHSU. His clinical focus is in the area of head and neck cancer rehabilitation and dysphagia. He regularly teaches graduate classes in adult dysphagia, is an Adjunct Faculty at Portland State University, and has taught Fiberoptic Endoscopic Evaluation of Swallowing (FEES) courses nationally. He is also interested in alaryngeal speech rehabilitation and is part of the faculty of the International Association of Laryngectomees (IAL) Voice Institute each year. Research interests include swallowing, voice and quality of life outcomes after surgery. EMILY POOLE, MA, CCC-SLP, Pulmonary Rehabilitation after Total Laryngectomy, employed by InHealth Technologies. Financial: Ms. Poole receives a salary from InHealth Technologies. Non-financial: Ms. Poole has no relevant non-financial relationships. Emily Poole MA, CCC-SLP joined InHealth Technologies in 2011 as a Clinical Specialist. With nearly ten years of clinical experience, she has joined InHealth from Rush University Medical Center in Chicago where she was the Senior Speech Language Pathologist and Associate Faculty member in their Communication Disorders program. She has specialized in adult acquired populations with an emphasis on voice restoration, dysphasia and head and neck cancer populations in all levels of care. Ms. Poole also has extensive experience in clinical training and professional education.
4 MELODY SHELDON, MA, CCC-SLP, Managing Laryngectomy Across Settings, employed by Southwest Rehabilitation. Financial: Ms. Sheldon has no relevant financial relationships. Melody Sheldon earned her undergraduate degrees in Speech Pathology and Audiology and Spanish, and graduate degree in Speech Pathology at Western Washington University in Bellingham, Washington. She interned at the Seattle Veterans Administration Medical Center under the supervision of Dr. Robert Miller in After completing her clinical fellowship, Melody worked part time in a private practice and in the school system in Santa Cruz, California specializing in the assessment of bilingual children. She moved to Coos Bay with her husband and children in 1989 and set up a private practice with emphasis in both the pediatric and adult populations. Her clinical focus is in the area of head and neck cancer rehabilitation and dysphagia. She also works with autistic population as well as oral motor impairments in children. She works closely with the physicians in the area providing services such as videostroboscopy, modified barium swallows, fiberendoscopic evaluations of the swallow, custom tracheostomy tube fittings, and sizing and fitting of the tracheoesophageal puncture prosthesis by Blom-Singer and Provox. HEATHER STEBECK, MA, RSLP, Managing Laryngectomy Across Settings, employed by Vancouver Island Health Authority. Financial: Ms. Sheldon has no relevant financial relationships. Heather Stebeck is a speech-language pathologist who has more than 20 years of experience working with adults in acute care hospitals in Victoria, BC. For the last seven years her work has been focused on the medical and surgical head and neck cancer population across the Vancouver Island Health Region. Heather is the primary provider of preoperative, post-operative and outpatient swallowing and voice restoration care to the laryngectomy population in the region. She is certified by the BC College of Speech and Hearing Professionals and holds advanced competencies in Voice Restoration (Voice Prostheses); Talking Tracheostomy Tubes or Speaking Valves Swallowing and/or Voice Restoration; and, Videofluoroscopic Assessment of Swallowing Disorders in Adults. KENDRA STEVENS, MS, CCC-SLP, Managing Laryngectomy Across Settings, employed by Sky Lakes Medical Center. Financial: Ms. Stevens has no relevant financial relationships. Non-financial: Ms. Stevens has no relevant non-financial relationships. Kendra Stevens is a Speech Language Pathologist in Klamath Falls, Oregon. She received her Masters of Science at the University of Wyoming in She moved to Klamath Falls in 1999 and has worked in nearly every setting an SLP can work in a small town: including general education/school settings, classrooms for severely disabled children, early childhood, home health, nursing homes, transitional care, outpatient rehab, and acute hospital settings. Kendra began working with Laryngectomee patients in 2007 replacing TEPs and electrolarynx training. She is now the rehab manager for Sky Lakes Medical Center, as well as maintaining an acute hospital caseload and working with some of our more complex Head/Neck cancer outpatients.

Pete grew up in Scappoose, Oregon, attended the University of Oregon and graduated with an economics degree at Portland State College. He served in the US Navy and is a Vietnam Service veteran. He had a long career with Boise Cascade Corporation in the pulp and paper industry. Pete's laryngectomy was in June 2010 at OHSU. He is active in his community with a Salem Rotary Club, and the Oregon New Voice Club here in Portland. Leisure activities include fishing, boating, camping, hunting and similar outdoor adventuring.
