1 American Red Cross/WV Chapter Disaster - The mission of the American Red Cross is to prevent and alleviate human suffering in the face of emergencies by mobilizing the power of volunteers and the generosity of donors. The basic needs we provide such as food, clothing and shelter help put struggling families on the path to recovery. ARC will meet immediate emergency disaster needs for food, clothing, shelter and medications. 300 adults will be served; 160 children will be served; and 135 cases will be addressed. American Red Cross/WV Chapter Senior Backpack - Under the Senior Preparedness and Disaster Kit Program, seniors receive the independence and support that helps them stay in their homes. The project provides vulnerable seniors in Cabell, Lincoln, Mason and Wayne counties and Lawrence County, Ohio, with vital emergency backpacks and senior preparedness literature. Seniors will learn how to prevent and be prepared for a disaster; seniors will able to age in place by being safe and healthy in their homes; the community will be better able to collaborate to meet needs in a disaster Big Brothers Big Sisters Community and School-based mentoring - Programs make meaningful, monitored matches between volunteers and children on a one-to-one basis in a Community Based Program and a School-Based Mentoring Program with an After-School component. : Children facing adversity are matched with a positive, caring adult in a professionally supported mentoring relationship. They will improve behavior in the areas of confidence, competence, and caring. Children will show improvement in school-related performance, avoidance of risky behaviors and socio-emotional competency Boys and Girls Club of Huntington Educational Support Program improving the graduation rate through academic support (Power Hour) which includes homework help, technology, and support programs. : More students will participate in Power Hour, and will achieve high or improved grade point averages in school. The organization will build stronger communication between school and the club through the use of county computer programs and scheduled communication. Cabell County Special Olympics - dedicated to providing individuals with intellectual disabilities a sports and training competition in a variety of olympic-type sports. :
2 Athletes will become more physically fit; will improve sports skills; will improve their knowledge of sport(s) rules. Volunteers will increase acceptance and awareness of students with disabilities. The number of athletes who are overweight will decrease. Children s Place - Children s Place is a child-care center which serves approximately 70 children annually. It seeks to provide a stable environment and create bonds with nurturing and quality adults so that the children will develop appropriate social skills to grow up resilient, healthy, caring, and responsible. Students will develop appropriate social skills for their age; will secure attachments with caring adults; and, students will develop intrinsic motivation. Children s Place Garden Project The Garden Project will allow parents living in poverty to take part in cooking classes and take home the food required to make the recipes for their families. This program is not meant to reduce the existing weight of Children s Place students, but instead to provide fresh fruit/vegetables and educational classes for the families of Children s Place to develop life-long healthy eating habits that will prevent obesity. : Children have access to fresh fruits and vegetables at home which increases their exposure and development of an educated palate for healthy foods; parents get free access to education on how to create healthy meals; parents get free access to recipes that create healthy meals; low income families receive additional food support that allow them to create healthy meals at home where children learn to eat healthy food and create an educated palate Christian Associates/Cridlin Food and Clothing Pantry - Christian Associates/Cridlin Food and Clothing Pantry, Inc. is a non-profit collaborative effort of thirteen downtown Huntington churches working together for over 40 years with the purpose of joining eligible people with numerous resources to provide nutritional food, quality clothing and basic needs essentials to at risk individuals and families in Cabell and Wayne Counties. Cridlin Food and Clothing Pantry will provide food to 100 families per week, and 50 families/individuals per week will receive paper items and toiletries. Community Mission Outreach The purpose of the CMO is to help needy families in the area feed and clothe their families and improve their well being by providing nutritional food and adequate clothing. Over 11,000 individuals and families will receive food and clothing during 2013.
3 Coalition for the Homeless - The Coalition for the Homeless serves those experiencing homelessness in the Cabell-Wayne area. The Coalition's day shelter, Harmony House, provides homeless health services, offered through a partnership with Valley Health Systems and their ongoing McKinney-Vento Health Care for the Homeless funding. CHCH will provide 300 medical exams, 100 dental exams, 100 eye exams and glasses, and 16 nights of temporary, emergency shelter annually. CHCH will also fill 700 prescriptions; provide 550 sets of hygiene supplies to individuals/families; and provide 180 street showers. 350 transportation trips to medical and vision appointments also will be provided Communities in Schools - CIS uses a holistic approach to address the needs of students, academically and socially by helping ensure: A one-on-one relationship with a caring adult A safe place to learn and grow A healthy start and a healthy future A marketable skill to use upon graduation A chance to give back to peers and community : Site Coordinators (high school Graduation Coaches) demonstrate competencies in the CIS framework, data collections system, and school-based implementation; students demonstrate improved coping skills via measurable behavior, i.e., attendance, behavior and course performance/ completion; students exhibit understanding of the importance of utilizing supportive networks Developmental Therapy - Developmental Therapy Center (DTC) provides access to evidencedbased, occupational, physical and speech/ language therapy for infants, children & adults in Cabell, Wayne, Mason, Lincoln (WV) and Lawrence (OH). Children referred to DTC for screening for delays/deficits are screened free of charge. Qualifying patients with a deficit will receive an evaluation. All qualifying patients will have an individualized plan of care tailored to their needs. Patients will achieve the goals in their treatment plan, while at least 15% will make progress toward meeting their goals with the support of their family. Patients, family members and caregivers will have access to monthly support groups to share concerns, ask questions, and discuss issues in a forum led by a professional. All services are provided on a sliding fee scale for those who qualify. Ebenezer Medical Outreach - This program serves uninsured individuals between the ages of 19 and 64 who live in the Cabell, Wayne, Mason, and Lincoln counties in West Virginia and Lawrence County, Ohio. : 100 patients will receive 3-4 medical visits per year and, 100 patients receive needed medications.
4 Girl Scouts Black Diamond Council - In an effort to ensure that girls healthily reach their potential, Girl Scouts of Black Diamond Council is implementing the Healthy Habits program. The Healthy Habits program will help ensure that participants increase their physical activity and make smart food choices. : Girls will demonstrate their improved knowledge of healthy food choices; girls will be able to explain a variety of ways to stay physically active, and why it is important. Girl Scouts Kentucky s Wilderness Road Live Healthy Lead Healthy program Encourages girls to get active, eat better, improve their self-esteem, and create a path toward a healthier life. Girls will be exposed to activities that increase physical activities, and will have increased knowledge and improved positive attitudes toward healthy food choices. Girls will strengthen their commitment against using tobacco. Goodwill Family Service provides therapeutic counseling services to individuals, children and families. Individuals will exhibit coping skills, and knowledge and understanding of those skills. Working age individuals will be better equipped to retain or obtain employment as a result of counseling. Goodwill Life Skills assists consumers in understanding the impact of their personal life on their work life, as well as the skills needed to be successful on the job. Life Skills participants will exhibit an increased understanding of appropriate employment skills and strategies; consumers will exhibit an increased understanding of appropriate financial decisions; and, participants will demonstrate an increased understanding of basic life skills that promote independence and employment The Healing Place - is an evidenced-based, long-term, residential program using a social model of recovery through peer mentoring and personal accountability without pharmaceutical intervention.
5 33 persons will receive the opportunity for free substance abuse recovery. These individuals also will gain skills through the Recovery Dynamics curricula which is a 6-month period which helps them understand the physical, mental and spiritual nature of alcohol and drug addiction, and provides a systematic application of the principles of the 12 steps of AA to their daily lives. 5 individuals from the criminal justice system will participate in the Recovery Dynamics curricula as an alternative to incarceration. 100% of individuals will give back to THPH community by taking responsibility for a job to maintain the program Huntington City Mission - The Huntington City Mission (HCM) provides the only Community Feeding Program within a 50 mile radius of the Tri-State area of Huntington, WV. The targeted population for the program is the homeless of the area and low-income community residents. With United Way funding, approximately 280 duplicated clients and/or community residents will receive assistance daily with food/meals; and, HCM will serve 280 meals per day for 72 days. Kiwanis Child Care to provide affordable, quality day care to children of parents who work or attend school. Kiwanis provides child care services to 70 children daily and as a result 62 families are able to secure, retain and maintain employment or educational endeavors. Children with special needs are identified and receive Special Needs Services. Children are assessed and receive developmental opportunities from Early Childhood Education trained staff in age appropriate and adequately equipped classrooms in order to reach developmental milestones Mason County Homeless Shelter - provides food baskets, hygiene products and cleaning supplies to people who are homeless or in danger of becoming homeless in addition to providing a safe place for people to live. 380 individuals and families will receive food baskets, and 60 families will receive cleaning supplies or hygiene products. New Baptist Church reducing food insecurity by providing clients enough food for 3 meals for 5 days for each person in the household. New Baptist Church food pantry will reduce food insecurity by providing food to 125 individuals quarterly; and by providing 1875 meals each quarter.
6 Prestera Center Renaissance Place Helps addicted women break the cycle of their addiction. This program also targets the children by providing resources to assure their age appropriate development, as well as prevention resources to assure that they do not turn to substance abuse in their lives. All children residing at the Renaissance Pinecrest campus with their mothers for at least 6 months achieve developmental milestones on schedule or demonstrate effective prevention, coping, and safety skills (depending on their age) and, these children will receive age appropriate Child Development Specialist services at least 4 days a week. River Valley Child Development Program TEACH The goals of TEACH are to provide financial assistance to early childhood teachers who need to earn college degrees in early childhood education; increase wages once coursework has been completed; and reduce turnover in childcare staffing. Early childhood providers will understand the importance of earning a higher level of education and its relation to quality child care; early childhood providers will apply to college and will apply for FAFSA and T.E.A.C.H.; will increase their education level; will receive an increase in wages; and, will have high employment retention rates Tri-State Literacy Council - Tri-State Literacy Council (TSLC) serves adults (in Cabell, Wayne, Boyd, and Lawrence Counties) who would like to learn to read or learn to read better. Students with 30+ instructional hours will increase literacy level by.5 grade level or 3 points on the CASAS Life Skills Assessment scoring scale; students with 12+ instructional hours quarterly will report progress toward, or accomplishment of, a personal/professional goal; students with school-aged children will report an increased awareness of literacy enriching family activities. Unlimited Future - Unlimited Future, Inc. (UFI) is a business incubator and resource center for the Tri- State area, providing critical support services and education that entrepreneurs need to be successful. - Unemployed/underemployed participants will learn the importance of positive workplace behavior (soft skills), gain these skills and increase their likelihood of maintaining long-term employment; unemployed/underemployed participants will learn the affect of soft skills on work promotions and everyday life through class training and mentorship; unemployed/underemployed participants will gain long-term employment within local small businesses.
7 YMCA Childhood Obesity Program This program will provide planned activities for the YMCA s afterschool program, teaching children the importance of everyday exercise and how to make healthy eating decisions. Children will learn how to be active; will learn exercises to decrease risk of obesity and related effects; and will learn about healthy eating
