3 October 17th, 2014 Re: NOTICE OF PUBLIC HEARING on Duplicate Addresses: Tuesday, October 28th, :00 p.m. Proposed New Address: Dear As Mayor of Hainesville, I have been in receipt of inquiries and complaints regarding duplicate addresses on the west end of town. The Village Board has been exploring the duplicate address issue involving the west end of Hainesville, Round Lake Park and Round Lake. In review of the history of the situation, apparently in August of 1990 addresses were changed to accommodate the new 911 system coming to the Round Lake Area. The new addresses in Hainesville came fully into effect in 1993, and over the years since, residents have experienced various problems with mail being improperly addressed (not the post office's fault), deliveries from other services such as food, UPS, FEDEX etc. and most alarming is periodic confusion with 911 calls. After speaking with the Police Chief, Fire Chief, and Postmaster we feel the best solution is to revert back to the old addresses. Technology has greatly advanced over the years and what may have been considered obstacles back then no longer exist. Although the Village is not required by law to hold a Public Hearing on this issue, the Board of Trustees and I feel it is one of the best ways to communicate on this issue to the residents affected, and also provide an opportunity for comments and questions. After the Public Hearing is closed, the Village Board would formally vote on the change of addresses at an upcoming board meeting. We are recommending that the change of address go into effect February 1, 2015 so as to provide adequate notice for residents to make the necessary address change notifications. This timing would also provide an opportunity to include a notification in your holiday greetings to family and friends. In closing, I wish to express that this change is being pursued to provide immediate response to the right address within the 911 system and improve your quality of life by eliminating mistakes in deliveries to the wrong residence. Respectfully, Mayor Linda Soto

4 December 9, 2014 Re: New address 96 N. Forest Glen Drive - effective February 1st, 2015 Dear Mr. & Mrs.: This letter is official notification that the Hainesville Village Board voted at the November 11th Regular Board Meeting to change the addresses as provided by the Greater Round Lake Fire District to eliminate duplicate addressing and the complications that this has caused. This issue was presented and discussed at a Public Hearing held on Tuesday, October 28th, 2014 at 7:00 p.m. We are in receipt of acknowledgement of the Ordinance requesting the address change that was sent to the United States Post Office. PLEASE NOTE THAT THIS ADDRESS CHANGE TAKES EFFECT STARTING FEBRUARY st. Property owners will need to change their deed through the Lake County Recorder of Deeds at for a fee of $ Your driver's license can be updated with the new address for a fee of $5.00. Credit card companies and any other billing accounts and bank accounts should be notified of the change of address. You may choose to use up your current checks simply by crossing out the old address and writing in the new one; you should confirm that this is acceptable to your financial institution. Also, any current subscriptions need to be updated to reflect the address change. The holiday season is a great time to send notice to your family and friends of the address change that will go into effect in the New Year starting February 1st, House and/or mailbox numbers will need to be changed at that time also. In order to help offset the cost of these items the Village Board voted to provide a flat stipend to those residents affected. Upon completion and return of the enclosed form by January 30, 2015, property owners of owner-occupied single family homes will receive an amount of $ Property owners of tenant-occupied, rented single family homes will receive an amount of $ Tenants of tenant-occupied rented single family homes will receive a flat amount of $ Tenants of rental apartments will receive a flat amount of $ Apartment building owners will receive $20.00 per apartment unit. On or after February 1, 2015 it is highly recommended that you fill out a "Change of Address Card" listing your old address and changing it to the new address. You may do this in person at your local post office for free or online at usps.com for $1.05.

5 Enclosed is a form that you will need to complete and have notarized and submit to the Hainesville Village Hall no later than January 30th, The Village Hall is open Monday, Tuesday and Thursday 9:00 a.m. - 5:00 p.m. Friday 9:00 - Noon. We are closed on Wednesday. You may also return it by mail in the enclosed envelope or place in the drive-up drop box on the north side of the Village Hall. Stipend checks will be mailed the week of February 2nd, We are sympathetic to the minor inconvenience this recent action causes, but overall the elimination of duplicate addressing and the mare serious complications that have stemmed from that duplication will be eliminated and therefore improve the quality of life and public safety in your neighborhood. Respectfully, Mayor Linda Soto and Board of Trustees
