4 Quick History of System of Care System of Care is a national initiative (see map) that began over 25 years ago Provided a definition, framework and philosophy Intended to guide the field in reforming systems and services

7 What is a System of Care? What it is It is a philosophy; a process and practice for building a more collaborative and effective service structure for families who are experiencing the most mental health needs What it s not It is not a program

9 Who are we? Represent Public Mental Health, Juvenile Justice/Courts, the Department of Human Services, Schools and many community partners who have committed to this initiative. Brief explanation of Public Mental Health

10 What are we seeing? What else we know about Saginaw County: % of youth involved with Saginaw County Juvenile Court that appear to have significant mental health concerns % of youth beginning treatment with SCCMHA that have moderate to severe problems in school 75% 80%

12 Needs Assessment Revealed Need for System of Care in Saginaw We discovered that families were not being served well across systems and we had data to back this up We had real-life examples of the systems not working for families We had an opportunity for a start-up grant from the State of Michigan to begin the reformation work

13 SOC TIMELINE: st System of Care Community Established MDCH Block Grant for SOC Start up and Exploration Applied for Substance Abuse and Mental Health Services Admin. Awarded Cooperative Agreement with SAMHSA Grant

14 What is the overall vision? We believe that every child deserves the opportunity to live a fulfilling life, especially children with serious emotional or behavioral challenges. We are committed to their success through these principles: Use of family driven, youth guided practice Respect for the unique background and culture of each family A collaborative, unified team of community partners A provision of supports and services that have proven results

15 How have we been achieving our vision? 1. By changing how we do business and how we serve families with mental health challenges 2. By challenging the status quo 3. By changing policies and procedures that pose barriers to successful treatment 4. By including youth and families in all areas of planning and implementation of this initiative 5. By educating our community

16 Saginaw SOC Identified Population Children and youth, ages 6 to 17 with serious emotional disturbances who are involved in the child welfare, special education, and/or juvenile justice systems and are at risk for out-of-home placement, psychiatric hospitalization, expulsion from school, or court involvement

(Mobile Urgent Treatment Team) Co-located Mental Health, JJ, and DHS workers between agencies Community Education Initiative Collaborative arrangement with Family Court/ Juvenile Justice/ Detention for shared mental health screening on all youth entering JJ system Evidence Based Practice Implementation

25 Open Table Return on Investment [ROI] Investment Created Volunteer 1,700+ hours $35,000+ Goods & Services $9,000 Capital $1,000 - $3,000 (micro loans) Total: $40,000-$45,000+ Return on Investment Using a return on investment model, the City of Phoenix Human Services Department analyzed the economic impact of six completed Tables in the city. Results indicate that for every dollar Open Table, government programs and other program partners invested in the families, the families benefited $7.44. Also, as some Brothers/Sisters complete educational programs that began while on their Tables and earn higher levels of income, this financial impact could The Open Table, Inc., 2015 increase substantially.

Total church membership in 2012=145,691, ,000+ congregations (Hartford Institute for Religious Research) 91.7% of congregations participate in social service programs (only 14.3% use outside funding support). 46 Bridging Divides: From a Table Member We wanted to work with a youth aging out of the system. Our first table was with a boy who had been in foster care and was coming out of prison. We carried many preconceived notions of what people in prison would be like. When we met him, we were so surprised at how open and honest he was with us. He had made bad choices, served his time, and was ready to start again. He was motivated. We had to learn to see him as a new man, not a man with a history. He s a welder. He is engaged to a great girl. He doesn t take any social services. He has embraced us as extended family. Table Member, Iowa

47 Open Table Members and Their Networks Represent Enormous Renewable Capital With ten people on a table, we collectively know almost everyone in the congregation, and what resources they are. The congregation members always offer their skills for whatever contribution is needed; and we also use third parties, people not in the congregation, whom we knew from our backgrounds.
